    The Six Dimensions of Wellness and Cognition in Aging Adults

    Objective: Examine how wellness in six dimensions (occupational, social, intellectual, physical, emotional, and spiritual) protects cognition in aging adults. Background: cognitive impairment increases with age. Baby boomers represent a significant percent of the population at risk for cognitive impairment. Cognitive impairment has a negative impact on nursing resources, health care finances, patient mortality, and quality of life. Wellness and prevention is one focus of Institute of Medicine’s vision for the future of nursing. Method: Literature was retrieved from Cumulative Index to Nursing and Allied Health Literature and MEDLINE. Research that examined the affect of wellness in each of the six dimensions on cognition in older adults was included. Results: One or more of the following may protect cognition in aging: midlife occupation complexity, marriage, social networks, formal education, intellectual activities, physical activity, healthy nutrition, motivational ability, purpose in life, and spirituality. Conclusion: Wellness in one or more of the six dimensions may protect cognition in aging. The cognitive protective benefits may increase when wellness in more than one dimension is demonstrated. High wellness in one dimension may protect cognition by compensating for low wellness in another dimension. The interconnectedness of each of the dimensions signifies the importance of evaluating older adults holistically. Wellness throughout the life span may result in improved cognition in aging. Application: Future research is needed to examine the relationship between the six dimensions of wellness and cognition, and to determine if one dimension of wellness is a significant predictor of cognitive health in aging adults

    Evidence for the validity of the Children’s Attraction to Physical Activity Questionnaire (CAPA) with young children

    Attraction to physical activity is important to an individual’s intrinsic motivation to engage in play, games and sports. While there are instruments designed to measure attraction to physical activity in middle childhood years, the lack of authentic measures in young children has impeded research in this area. In this study we sought to address the validity of a scale to tap young children’s attraction to physical activity. Evidence for validity was based on internal consistency, content analysis, and factor structure. Australian school children (180 boys and 154 girls) from school year two, aged six to eight years, were individually administered a modified version of the Children’s Attraction to Physical Activity Scale (CAPA; [1, 2]). The results indicated that internal consistency was acceptable for most of the subscales when negative statements were excluded from the analyses. Factor analysis revealed that the liking of games and sports, liking of physical exertion and exercise, and the importance of exercise subscales were more robust. Second order factor analysis indicated that the overall construct of attraction to physical activity was viable in this age group. With some modifications, the scale appears to provide an acceptable approach to the measurement of attraction to physical activity in young children

    Scaling functional status within the interRAI suite of assessment instruments

    Abstract Background As one ages, physical, cognitive, and clinical problems accumulate and the pattern of loss follows a distinct progression. The first areas requiring outside support are the Instrumental Activities of Daily Living and over time there is a need for support in performing the Activities of Daily Living. Two new functional hierarchies are presented, an IADL hierarchical capacity scale and a combination scale integrating both IADL and ADL hierarchies. Methods A secondary analyses of data from a cross-national sample of community residing persons was conducted using 762,023 interRAI assessments. The development of the new IADL Hierarchy and a new IADL-ADL combined scale proceeded through a series of interrelated steps first examining individual IADL and ADL item scores among persons receiving home care and those living independently without services. A factor analysis demonstrated the overall continuity across the IADL-ADL continuum. Evidence of the validity of the scales was explored with associative analyses of factors such as a cross-country distributional analysis for persons in home care programs, a count of functional problems across the categories of the hierarchy, an assessment of the hours of informal and formal care received each week by persons in the different categories of the hierarchy, and finally, evaluation of the relationship between cognitive status and the hierarchical IADL-ADL assignments. Results Using items from interRAI’s suite of assessment instruments, two new functional scales were developed, the interRAI IADL Hierarchy Scale and the interRAI IADL-ADL Functional Hierarchy Scale. The IADL Hierarchy Scale consisted of 5 items, meal preparation, housework, shopping, finances and medications. The interRAI IADL-ADL Functional Hierarchy Scale was created through an amalgamation of the ADL Hierarchy (developed previously) and IADL Hierarchy Scales. These scales cover the spectrum of IADL and ADL challenges faced by persons in the community. Conclusions An integrated IADL and ADL functional assessment tool is valuable. The loss in these areas follows a general hierarchical pattern and with the interRAI IADL-ADL Functional Hierarchy Scale, this progression can be reliably and validly assessed.     Parents’ Perceptions of the Philly Goat Project’s All Abilities RAMble: A Qualitative Study of Animal-Assisted Intervention for Intellectual and Developmental Disorders

    Individuals with intellectual and developmental disabilities (IDD) are a growing population. Considering the wide diversity in IDD and the financial burden of traditional treatment modalities, Animal-Assisted Interventions (AAI) has emerged as an innovative and non-traditional treatment for individuals with a range of disabilities, including individuals with IDD. To the authors’ knowledge, the present study was one of the first to explore a goat-assisted therapy experience for children with IDD. This study explored the experiences of 23 children with the All Abilities RAMble – a goat-assisted therapeutic activity offered by the Philly Goat Project (PGP). Key themes in our study included the RAMble activating joy, RAMble as a place to practice skills, RAMble as a unique service providing transferable skills, RAMble as a personalized and child-directed program, and RAMble providing a sense of community. This research provides valuable insight into the child’s experience with goat-assisted therapy, which can inform future goat-assisted therapy interventions for children with IDD. The results suggest there are several positive impacts from the All Abilities RAMble, further supported by the literature on AAI and IDD. Additionally, the emergence of social skill(s) and communication skill development, transferability of acquired skills, and sense of community prompted provided strength in this exploratory inquiry of this specific form of AAI. Goat-assisted therapeutic activities, such as the PGP All Abilities RAMble, is an opportunity to practice skills and develop relationships that are often under-examined in this specific population. This study demonstrates the need for more empirical research on goat-specific AAI with children with IDD, and their families. Future research recommendations include exploration of siblings and parents’ experience at the RAMble and further inquiry into what contributes to outdoor or nature-based programming beneficial to people with disabilities

    Some observations from acoustic emissions recorded during a yield test of a bearing raceway compressed into plasticity using a rolling element are presented. The general objective of the study is to establish whether there is enough evidence of the onset of sub-surface plasticity in the acoustic emissions signature. It is discussed here how acoustic emissions monitoring during compression could indicate the onset of subsurface plasticity as a precursor to damage propagation to the surface. Some comparisons are drawn between the acoustic emissions activity levels and time-frequency response during elastic deformation and at yield loads

    The Coronavirus-2019 (COVID-19) pandemic has shifted research and healthcare system priorities, stimulating literature on implementation and evaluation of telerehabilitation for a variety of patient populations. While there is substantial literature on individual telerehabilitation, evidence about group telerehabilitation remains limited despite its increasing use by rehabilitation providers. Therefore, the purpose of this manuscript is to describe our expert team’s consensus on practice considerations for adapting in-person group rehabilitation to group telerehabilitation to provide rapid guidance during a pandemic and create a foundation for sustainability of group telerehabilitation beyond the pandemic’s end. &nbsp

    Some general properties of the renormalized stress-energy tensor for static quantum states on (n+1)-dimensional spherically symmetric black holes

    We study the renormalized stress-energy tensor (RSET) for static quantum states on (n+1)-dimensional, static, spherically symmetric black holes. By solving the conservation equations, we are able to write the stress-energy tensor in terms of a single unknown function of the radial co-ordinate, plus two arbitrary constants. Conditions for the stress-energy tensor to be regular at event horizons (including the extremal and ``ultra-extremal'' cases) are then derived using generalized Kruskal-like co-ordinates.
