Development of a universal short patient satisfaction questionnaire on the basis of SERVQUAL : Psychometric analyses with data of diabetes and stroke patients from six different European countries
Objective A short questionnaire which can be applied for assessing patient satisfaction in different contexts and different countries is to be developed. Methods Six items addressing tangibles, reliability, responsiveness, assurance, empathy, and communication were analysed. The first five items stem from SERVQUAL (SERVice QUALity), the last stems from the discussion about SERVQUAL. The analyses were performed with data from 12 surveys conducted in six different countries (England, Finland, Germany, Greece, the Netherlands, Spain) covering two different conditions (type 2 diabetes, stroke). Sample sizes for included participants are 247 in England, 160 in Finland, 231 in Germany, 152 in Greece, 316 in the Netherlands and 96 in Spain for the diabetes surveys; and 101 in England, 139 in Finland, 107 in Germany, 58 in Greece, 185 in the Netherlands, and 92 in Spain for the stroke surveys. The items were tested by (1) bivariate correlations between the items and an item addressing 'general satisfaction', (2) multivariate regression analyses with 'general satisfaction' as criterion and the items as predictors, and (3) bivariate correlations between sum scores and 'general satisfaction'. Results The correlations with 'general satisfaction' are 0.48 for tangibles, 0.56 for reliability, 0.58 for responsiveness, 0.47 for assurance, 0.53 for empathy, and 0.56 for communication. In the multivariate regression analysis, the regression coefficient for assurance is significantly negative while all other regression coefficients are significantly positive. In a multivariate regression analysis without the item 'assurance' all regression coefficients are positive. The correlation between the sum score and 'general satisfaction' is 0.608 for all six items and 0.618 for the finally remaining five items. The country specific results are similar. Conclusions The five items which remain after removing 'assurance', i.e. the SERVQUAL-MOD-5, constitute a short patient satisfaction index which can usefully be applied for different medical conditions and in different countries.Peer reviewe
The impact of accessibility and service quality on the frequency of patient visits to the primary diabetes care provider : results from a cross-sectional survey performed in six European countries
BackgroundVisits to the primary diabetes care provider play a central role in diabetes care. Therefore, patients should attend their primary diabetes care providers whenever a visit is necessary. Parameters that might affect whether this condition is fulfilled include accessibility (in terms of travel distance and travel time to the practice), as well as aspects of service quality (for example in-practice waiting time and quality of the provider's communication with the patient). The relationships of these variables with the frequency of visits to the primary diabetes care provider are investigated.MethodsThe investigation is performed with questionnaire data of 1086 type 2 diabetes patients from study regions in England (213), Finland (135), Germany (218), Greece (153), the Netherlands (296) and Spain (71). Data were collected between October 2011 and March 2012. Data were analysed using log-linear Poisson regression models with self-reported numbers of visits in a year to the primary diabetes care provider as the criterion variable. Predictor variables of the core model were: country; gender; age; education; stage of diabetes; heart problems; previous stroke; problems with lower extremities; problems with sight; kidney problems; travel distance and travel time; in-practice waiting time; and quality of communication. To test region-specific characteristics, the interaction between the latter four predictor variables and study region was also investigated.ResultsWhen study regions are merged, travel distance and in-practice waiting time have a negative effect, travel time no effect and quality of communication a positive effect on visit frequency (with the latter effect being by far largest). When region specific effects are considered, there are strong interaction effects shown for travel distance, in-practice waiting time and quality of communication. For travel distance, as well as for in-practice waiting time, there are region-specific effects in opposite directions. For quality of communication, there are only differences in the strength with which visit frequency increases with this variable.ConclusionsThe impact of quality of communication on visit frequency is the largest and is stable across all study regions. Hence, increasing quality of communication seems to be the best approach for increasing visit frequency.Peer reviewe
The impact of travel distance, travel time and waiting time on health-related quality of life of diabetes patients: An investigation in six European countries
Physicians' attitudes and views on pharmaceutical innovation and their impact on prescribing behaviour: determinants and implications for pharmaceutical policy
Introduction and Objectives: New pharmaceutical technology is an important determinant of pharmaceutical expenditure growth; at the same time, new drugs have significantly contributed to improved patient outcomes and have substituted other types of health care. The diffusion of new pharmaceutical technology affects pharmaceutical expenditure growth as well as patients’ outcomes and access to new medications. Hence, the study of new drug diffusion is of importance for pharmaceutical policy. The present study aims to explore factors influencing new drug adoption by prescribing physicians in the Greek health care context. Material and Methods: A systematised literature review was performed in order to identify factors affecting new drug adoption and inform the design of the primary research. The latter employed a mixed methods approach and used a theoretical framework which was developed based primarily on the Diffusion of Innovation Theory and complemented by two frameworks on the implementation of health services innovations and evidence-based interventions. The qualitative study focused on the adoption of new antidiabetic medicines that are used in the management of Type 2 Diabetes Mellitus (T2DM), whereas the quantitative study focused on adoption of a new class of antidiabetic medications, the Sodium-glucose co-transporter 2 inhibitors (SGLT-2 inhibitors). In both the qualitative and quantitative study, the samples comprised of physicians involved in the management of T2DM in adults. In the qualitative study, semi-structured interviews were performed. Eligible participants were identified using generic purposive sampling. Interviews were audio-recorded, transcribed verbatim and analysed using framework analysis. The results of the analysis and the findings of the literature review informed the development of the closed questionnaire that was used in the quantitative study. The questionnaire was developed in electronic format. Convenience sampling was employed. In specific, recruitment of participants was performed via four medical Scientific Societies which forwarded to their members’ e-mail addresses an invitation letter and the link to the electronic questionnaire. Identification of participants in the survey was not possible as questionnaire completion was anonymous. Results: 10 physicians were interviewed in the qualitative study. The mean interview duration was 44 minutes. The following main themes were derived: 1) Perceptions on pharmaceutical innovation, 2) Use of information sources and the new drug adoption process, 3) Factors influencing prescribing of new medicines for T2DM, and 4) Attitudes towards recently introduced pharmaceutical policy measures aimed at influencing prescribing behaviour. The qualitative study revealed that new drug adoption is not an instantaneous decision, rather it is a process. During this process, prescribers combined information on newer antidiabetic drugs from a variety of information sources; the perceived credibility of each source was important in the process of information-seeking. Framework analysis also brought forward a wide range of factors influencing new drug adoption; the attributes of the new drug, patient-related factors, external influences and prescriber needs and characteristics affected the adoption decision. The new drug’s perceived relative advantage was a critical factor for its adoption by participants in the qualitative study. The relative importance of this attribute was also dependent on its compatibility with the prescriber’s needs, values and practices. Other factors that affected adoption of newer antidiabetic drugs included the drug’s price compared to available pharmacotherapies (mostly in terms of its impact on the level of patient cost-sharing), patient clinical and socioeconomic characteristics as well as preferences, and prescribers’ personal characteristics. The final sample in the quantitative study consisted of 259 physicians. Mean age was 49.3 years (SD=9.1 years). 56.4% of participants were male. 81.9% of participants systematically prescribed SGLT2 inhibitors, whereas 13.9% replied that they prescribed SGLT2 inhibitors tentatively. 3.9% did not prescribe this class of drugs, although they were familiar with them. The results of the statistical analysis of responses to the questionnaire survey support the qualitative research findings on the importance of the perceived attributes of the new drugs and prescriber characteristics on new drug adoption. In both logistic and linear regression analyses perceived relative advantage of SGLT2 inhibitors had a significant separate effect on the stage of adoption and the (self-reported) percentage of T2DM patients being prescribed SGLT2 inhibitors. In addition, prescribers reporting lower perceived complexity of this class of drugs had higher odds of systematically prescribing SGLT2 inhibitors. Other factors that had a significant separate effect on adoption of SGLT2 inhibitors were related to prescriber characteristics, namely: the number of years of working experience (as approximated by number of years since completing their specialisation), participation in conferences on diabetes care as an author of a scientific presentation, the degree to which participants perceived that it was easy for them to stay up-to-date with developments regarding newer antidiabetic drugs and, finally, having participated in a phase III or IV study of SGLT2 inhibitors. Conclusions: Understanding the process and factors influencing new drug adoption is important for the development of pharmaceutical policy measures that will promote rational diffusion and use of new drugs. The results of the present study contribute to the development of such policies in Greece, by identifying determinants of new drug adoption in the management of T2DM and bringing forward issues that need to be addressed such as the importance of ensuring access by prescribers to high-quality and credible information on newly marketed drugs, considering aspects of value that are important to patients and physicians when appraising new drugs and the need for deliberate planning and implementation of a comprehensive policy for managing pharmaceutical innovation, part of which are measures aiming at changing prescribing behaviour. The present study is the first study to explore new drug adoption in Greece, employing a mixed methods approach. Future research could investigate prescribing of other new drug classes and/or use of new health technologies, thus contributing to better understanding of health technology adoption in the Greek context.Εισαγωγή: Η είσοδος της νέας φαρμακευτικής τεχνολογίας αποτελεί διαχρονικά σημαντικό προσδιοριστικό παράγοντα της αύξησης της δαπάνης για φάρμακα. Παράλληλα, υπάρχουν ισχυρές ενδείξεις ότι η φαρμακευτική καινοτομία έχει συμβάλλει σημαντικά στη βελτίωση των εκβάσεων των ασθενών και στη μείωση της χρησιμοποίησης άλλων μορφών περίθαλψης. Η μελέτη της διάχυσης της νέας φαρμακευτικής τεχνολογίας έχει υψηλή χρησιμότητα για τη φαρμακευτική πολιτική καθώς ο τρόπος που η νέα τεχνολογία θα διαχυθεί και θα ενταχθεί στην κλινική πρακτική επηρεάζει την εξέλιξη της δαπάνης (των ασφαλιστικών οργανισμών αλλά και των ασθενών) για φάρμακα, αλλά και τις εκβάσεις υγείας και την πρόσβαση των ασθενών σε νέες θεραπείες. Η παρούσα διδακτορική διατριβή αποσκοπεί στη διερεύνηση των παραγόντων που επηρεάζουν τη λήψη απόφασης από την πλευρά των συνταγογράφων γιατρών σχετικά με τη συνταγογράφηση ενός νέου, πρωτότυπου φαρμάκου στο περιβάλλον του συστήματος υγείας της Ελλάδας. Υλικό και Μέθοδος: Στο πρώτο στάδιο εκπόνησης της διατριβής, πραγματοποιήθηκε ανασκόπηση της διεθνούς βιβλιογραφίας, προκειμένου να διερευνηθεί η υπάρχουσα γνώση αναφορικά με την υιοθέτηση (ένταξη στη συνταγογραφία) των νέων φαρμάκων από συνταγογράφους γιατρούς και, επιπροσθέτως, για να υποστηρίξει το σχεδιασμό της πρωτογενούς έρευνας. Η πρωτογενής έρευνα πραγματοποιήθηκε με τη χρήση «μεικτής» μεθόδου. Το θεωρητικό πλαίσιο της έρευνας διαμορφώθηκε έχοντας ως βάση τη θεωρία της Διάχυσης της Καινοτομίας και συμπληρώθηκε στη συνέχεια με στοιχεία από δύο υποδείγματα που αφορούν στην υιοθέτηση νέων υπηρεσιών και παρεμβάσεων στον υγειονομικό τομέα. Τα φάρμακα τα οποία επελέγησαν ως αντικείμενο της ποιοτικής έρευνας ήταν οι νεότεροι αντιδιαβητικοί παράγοντες, οι οποίοι χρησιμοποιούνται στη διαχείριση του Σακχαρώδη Διαβήτη τύπου 2 (ΣΔτ2), ενώ η ποσοτική έρευνα εστίασε στην ένταξη στη συνταγογραφία μίας νέας κατηγορίας αντιδιαβητικών φαρμάκων, των αναστολέων του υπότυπου 2 του συμμεταφορέα νατρίου-γλυκόζης (SGLT2 αναστολέων). Το δείγμα της ποιοτικής και ποσοτικής έρευνας αποτέλεσαν γιατροί διαφόρων ειδικοτήτων, οι οποίοι παρακολουθούν ενήλικες ασθενείς με ΣΔτ2. Στην περίπτωση της ποιοτικής έρευνας το μεθοδολογικό εργαλείο που επελέγη ήταν η σε βάθος συνέντευξη έρευνας με τη χρήση Οδηγού Συνέντευξης. Η επιλογή των συμμετεχόντων έγινε με τη χρήση σκόπιμης δειγματοληψίας. Κατόπιν ενημέρωσης και συγκατάθεσης των συμμετεχόντων, οι συνεντεύξεις έρευνας ηχογραφήθηκαν με τη χρήση ψηφιακού καταγραφέα φωνής, ψευδωνυμοποιήθηκαν και απομαγνητοφωνήθηκαν. Για την ανάλυση του ερευνητικού υλικού πραγματοποιήθηκε θεματική ανάλυση με τη χρήση της μεθόδου Framework. Στη βάση των αποτελεσμάτων της ποιοτικής έρευνας και της ανασκόπησης της βιβλιογραφίας αναπτύχθηκε ερωτηματολόγιο, το οποίο χρησιμοποιήθηκε για τη συλλογή δεδομένων στο πλαίσιο της ποσοτικής έρευνας σε δείγμα συνταγογράφων γιατρών. Εργαλείο της έρευνας αποτέλεσε ερωτηματολόγιο κλειστού τύπου το οποίο ήταν διαθέσιμο ηλεκτρονικά. Η μέθοδος δειγματοληψίας που επιλέχθηκε στην ποσοτική έρευνα ήταν η δειγματοληψία ευκολίας. Η ενημέρωση και πρόσκληση των γιατρών να συμπληρώσουν το ερωτηματολόγιο πραγματοποιήθηκε από τέσσερις Επιστημονικές Εταιρείες, οι οποίες προώθησαν στις διευθύνσεις ηλεκτρονικής αλληλογραφίας των μελών τους ενημερωτικό σημείωμα καθώς επίσης το σύνδεσμο που οδηγούσε στο ηλεκτρονικό ερωτηματολόγιο. Δεν υπήρχε δυνατότητα ταυτοποίησης των συμμετεχόντων στην έρευνα. Αποτελέσματα: Πραγματοποιήθηκαν 10 συνεντεύξεις έρευνας με γιατρούς οι οποίοι παρακολουθούσαν ενήλικες ασθενείς με ΣΔτ2. Η μέση διάρκεια των συνεντεύξεων ήταν 44 λεπτά. Από τη θεματική ανάλυση προέκυψαν οι ακόλουθες θεματικές κατηγορίες: α) Αντιλήψεις των συμμετεχόντων σχετικά με τη φαρμακευτική καινοτομία, β) Πηγές ενημέρωσης που χρησιμοποιούν οι γιατροί σχετικά με τα νέα φάρμακα και η διαδικασία ένταξης ενός νέου αντιδιαβητικού παράγοντα στη συνταγογραφία, γ) Παράγοντες που επηρεάζουν την απόφαση των γιατρών να συνταγογραφήσουν ένα νέο αντιδιαβητικό παράγοντα, δ) Απόψεις των συνταγογράφων γιατρών σχετικά με τα μέτρα επηρεασμού της συνταγογράφησης που εφαρμόστηκαν στην Ελλάδα. Όπως προέκυψε από την ποιοτική έρευνα, η ένταξη ενός νέου πρωτότυπου φαρμάκου στη συνταγογραφία δεν είναι αποτέλεσμα μιας στιγμιαίας απόφασης αλλά μιας διαδικασίας. Οι συμμετέχοντες γιατροί συνδύαζαν πληροφορίες για τα νεότερα αντιδιαβητικά φάρμακα από ένα εύρος πηγών καθ’ όλη τη διαδικασία λήψης απόφασης για την ένταξη στη συνταγογραφία τους ενός νέου αντιδιαβητικού φαρμάκου. Στο πλαίσιο των συνεντεύξεων έρευνας αναδείχθηκε επίσης η διάσταση της αξιοπιστίας που αποδίδουν οι συμμετέχοντες σε κάθε πηγή πληροφόρησης. Η ποιοτική έρευνα ανέδειξε επίσης ένα σημαντικό εύρος παραγόντων που επηρεάζουν την απόφαση των συμμετεχόντων γιατρών να εντάξουν στη συνταγογραφία ή να απορρίψουν ένα νέο αντιδιαβητικό φάρμακο. Οι παράγοντες που αναφέρθηκαν από τους ίδιους τους συμμετέχοντες ήταν τα χαρακτηριστικά του φαρμάκου, τα χαρακτηριστικά και οι προτιμήσεις των ασθενών τους και οι επιρροές του εξωτερικού περιβάλλοντος, ωστόσο, πρόσθετοι παράγοντες αναδύθηκαν από την ανάλυση και, ειδικότερα, τα χαρακτηριστικά του συνταγογράφου γιατρού και του εργασιακού του περιβάλλοντος. Το αντιληπτό συγκριτικό πλεονέκτημα του φαρμάκου ήταν κρίσιμος παράγοντας για την υιοθέτηση των νεότερων αντιδιαβητικών παραγόντων μεταξύ των συμμετεχόντων στην ποιοτική έρευνα. Ωστόσο, η βαρύτητα του συγκριτικού αυτού πλεονεκτήματος στη λήψη της απόφασης για τη συνταγογράφηση ή όχι του νέου φαρμάκου ήταν συνάρτηση και της συμβατότητάς του με τις ανάγκες, αξίες και πρακτικές του συνταγογράφου γιατρού. Στην ποιοτική έρευνα, η σχετική τιμή του νέου φαρμάκου έπαιζε ρόλο στην απόφαση των γιατρών αναφορικά με τη συνταγογράφηση του, καθώς διαμόρφωνε το ποσοστό συμμετοχής των ασθενών αλλά και λόγω του ανώτατου ορίου μηνιαίας δαπάνης συνταγογράφησης του γιατρού που όριζε το θεσμικό πλαίσιο αναφορικά με τα φάρμακα που καλύπτει η κοινωνική ασφάλιση. Αν και αποτελούσε σημαντικό προβληματισμό, οι επιπτώσεις της επιλογής των νεότερων φαρμάκων στην επίδοση του γιατρού όσον αφορά στην επίτευξη των στόχων συνταγογράφησης είχε μικρότερη βαρύτητα σε σχέση με τα άλλα χαρακτηριστικά του φαρμάκου. Ένα επίσης σημαντικό εύρημα της θεματικής ανάλυσης ήταν η επίδραση παραγόντων σχετικών με τον εκάστοτε ασθενή (κοινωνικοοικονομικά χαρακτηριστικά, προτιμήσεις), στη λήψη απόφασης σχετικά με τη συνταγογράφηση των νεότερων φαρμάκων. Το τελικό δείγμα της ποσοτικής έρευνας αποτελείται από 259 άτομα. Η μέση ηλικία των συμμετεχόντων ήταν 49,3 έτη (SD=9,1 έτη). Το 56,4% των συμμετεχόντων ήταν άντρες. Όπως προέκυψε από τις απαντήσεις των συμμετεχόντων, σε ποσοστό 81,9% συνταγογραφούσαν συστηματικά αναστολείς SGLT2, ενώ το 13,9% τους συνταγογραφούσε δοκιμαστικά και το 3,9% ανέφερε ότι γνώριζε τη συγκεκριμένη κατηγορία φαρμάκων αλλά δεν τους συνταγογραφούσε. Τα αποτελέσματα της έρευνας ερωτηματολογίου επιβεβαιώνουν τα ευρήματα της ποιοτικής έρευνας αναφορικά με τον κρίσιμο ρόλο των αντιληπτών χαρακτηριστικών των νέων αντιδιαβητικών παραγόντων στην απόφαση για τη συνταγογράφησή τους. Από την ανάλυση των δεδομένων της ποσοτικής έρευνας, προέκυψε πως τόσο το στάδιο υιοθέτησής τους όσο και η διείσδυση των SGLT2 αναστολέων στη συνταγογραφία των γιατρών επηρεάζονται από το αντιληπτό συγκριτικό πλεονέκτημα των SGLT2 αναστολέων έναντι εναλλακτικών θεραπειών. Υψηλότερο αντιληπτό συγκριτικό πλεονέκτημα των SGLT2 αναστολέων σχετιζόταν με τη συστηματική συνταγογράφησή τους και με υψηλότερο ποσοστό ασθενών οι οποίοι λάμβαναν το φάρμακο. Επίσης, οι γιατροί που θεωρούσαν λιγότερο δύσκολο να κατανοήσουν τον τρόπο δράσης και να χρησιμοποιήσουν την κατηγορία αυτή φαρμάκων αυτά ήταν πιο πιθανό να τους συνταγογραφούν συστηματικά. Οι λοιποί παράγοντες που βρέθηκαν να επηρεάζουν ανεξάρτητα την υιοθέτηση των νέων φαρμάκων στην πολυπαραγοντική ανάλυση αφορούσαν σε χαρακτηριστικά των συνταγογράφων γιατρών και συγκεκριμένα ήταν: ο αριθμός των ετών εργασιακής εμπειρίας του γιατρού, η συμμετοχή σε συνέδρια σχετικά με το Σακχαρώδη Διαβήτη ως συγγραφέας ανακοινώσεων, ο βαθμός στον οποίο ο γιατρός εκτιμούσε ότι ήταν εύκολο να παρακολουθεί τις εξελίξεις για τα νεότερα φάρμακα για το ΣΔτ2 και η συμμετοχή σε μελέτες φάσης ΙΙΙ και IV της υπό μελέτη κατηγορίας φαρμάκων. Συμπεράσματα: Η κατανόηση των παραγόντων που επηρεάζουν την υιοθέτηση των νέων τεχνολογιών υγείας είναι σημαντική για το σχεδιασμό πολιτικών που θα προωθούν την ορθολογική διάχυση της φαρμακευτικής καινοτομίας. Τα ευρήματα της παρούσας έρευνας αναφορικά με την υιοθέτηση (ένταξη στη συνταγογραφία) των νεότερων φαρμάκων για τη διαχείριση του ΣΔτ2 συμβάλλουν προς αυτή την κατεύθυνση αναδεικνύοντας ζητήματα όπως η ποιότητα και η αξιοπιστία της πληροφορίας σχετικά με τα νέα φάρμακα στην οποία έχουν πρόσβαση οι συνταγογράφοι γιατροί, η συνεκτίμηση διαστάσεων που έχουν αξία για τους γιατρούς και τους ασθενείς κατά τη διαδικασία αξιολόγησης των νέων θεραπειών, και η αναγκαιότητα μιας συνολικής πολιτικής για τη διαχείριση της φαρμακευτικής καινοτομίας, μέρος της οποίας θα αποτελούν τα μέτρα επηρεασμού της συνταγογράφησης. Η παρούσα έρευνα αποτελεί την πρώτη προσπάθεια διερεύνησης της υιοθέτησης της νέας φαρμακευτικής τεχνολογίας στην Ελλάδα με τη χρήση μεικτής προσέγγισης. Μελλοντικές ερευνητικές προσπάθειες θα μπορούσαν να διερευνήσουν σε βάθος την υιοθέτηση άλλων κατηγοριών φαρμάκων ή και τεχνολογιών υγείας, με σκοπό την πληρέστερη κατανόηση του φαινομένου στο περιβάλλον της Ελλάδας
Vaccination against HPV virus: a systematic review of economic evaluation studies for developed countries
Introduction: During the last years, a significant number economic
evaluations of HPV vaccination has been published. Given that
cost-effectiveness constitutes an essential part of decision-making with
regards to the reimbursement of a health technology, the purpose of this
study is to provide a supportive tool to decision-makers regarding the
economic efficiency of the introduction of HPV vaccination to national
immunization programs.
Areas covered: The PubMed database was searched in order to identify
cost-effectiveness studies for HPV vaccination. A total of 42 articles
were finally retrieved. All retrieved cost-effectiveness ratios (ICERs)
were converted in the same currency unit (I25132/QALY, whereas the mean ICER of
vaccination against oncogenic HPV 16,18 types is estimated at
I$38,253/QALY.
Expert commentary: HPV vaccination would be a cost-effective
intervention in the setting of high-income countries and could reduce
the incidence of HPV-related diseases
The role of Health Technology Assessment in pharmaceutical policy decision-making in Greece. Findings from a qualitative study
This study aimed to investigate stakeholders' beliefs regarding Health
Technology Assessment (HTA) in pharmaceutical policy in Greece. A
qualitative research using semi-structured interviews was performed.
Interviewees were selected using purposive sampling amongst
pharmaceutical market various stakeholders. According to stakeholders,
there is an urgent need for more extensive use of HTA. The establishment
of an HTA organization is a demand of wide consensus amongst
participants. Alpha change in the current culture towards greater
transparency was seen as a prerequisite for the successful establishment
of Eta tau Alpha. Stakeholders agreed that the introduction of HTA
should be part of a wider pharmaceuticals' reimbursement system reform
HEALTH TECHNOLOGY ASSESSMENT OF MEDICINES IN GREECE: PHARMACEUTICAL INDUSTRY EXECUTIVES' VIEWS
Objectives: The aim of this study was to investigate originator
pharmaceutical companies’ practices in relation to health technology
assessment (HTA) and the views and perceptions of their executives on
the importance of HTA in pricing and reimbursement of medicines in
Greece.
Methods: A qualitative study was performed, using individual
semi-structured interviews based on an interview schedule with
open-ended questions. The target population was market access
departments’ executives of originator pharmaceutical companies. Our
target sample consisted of sixteen executives, of whom ten agreed to
participate. Saturation point was reached after eight interviews. Data
were audio recorded, transcribed verbatim, and analyzed using content
analysis.
Results: Participants considered HTA as a very important complementary
tool for decision making in health policy, particularly in the field of
pharmaceuticals and medical devices. They believed that, in Greece, HTA
could be institutionalized for the reimbursement mechanism of medicines
under certain conditions relating to current health policy-making
attitudes and conditions pertaining in the country. They considered that
there are many constraints which must be overcome as well as
opportunities to be exploited.
Conclusions: Decisions in pharmaceutical policy should be scientifically
substantiated and HTA should be institutionalized primarily for
reimbursement decisions. Development of guidelines for conducting
pharmaco-economic evaluation, change in health policy goals, recording
of cost and epidemiological data, and broader participation of all
stakeholders in HTA decision-making processes are suggested as
prerequisites for a successful implementation of HTA in Greece
Cost-Effectiveness of Apixaban vs. Other New Oral Anticoagulants for the Prevention of Stroke: An Analysis on Patients with Non-Valvular Atrial Fibrillation in the Greek Healthcare Setting
Background and Objectives Three new oral anticoagulants (NOACs) are
currently approved for stroke prevention and systemic embolism in
patients with non-valvular atrial fibrillation (NVAF). The objective of
this analysis was to assess the cost effectiveness of apixaban against
other NOACs for the prevention of stroke in patients with NVAF in
Greece.
Methods A Markov model that evaluated clinical events, quality-adjusted
life expectancy, and costs for patients treated with apixaban or other
NOACs formed the basis of the analysis. Clinical events were modeled for
a lifetime horizon, based on clinical efficacy data from an indirect
comparison, using the ARISTOTLE, ROCKET-AF, and RE-LY clinical trials.
Resource use associated with patient monitoring was elicited via a panel
of experts (cardiologists and internists). Cost calculations reflect the
local clinical setting and followed a third-party payer perspective
(Euros, discounted at 3 %).
Results Apixaban was projected to reduce the occurrence of clinical
events and increase quality-adjusted life expectancy and incremental
costs of treatment compared with other NOACs. Taking into account costs
of medications, patient monitoring, and management of events, the
incremental cost-effectiveness ratios for apixaban 5 mg twice daily vs.
dabigatran 110 mg twice daily, dabigatran 150 mg twice daily, and
rivaroxaban 20 mg once daily were estimated at
(sic)9907/quality-adjusted life-year (QALY), (sic)13,727/QALY, and
(sic)6936/QALY gained, respectively. Extensive sensitivity analyses
indicated that results were robust over a wide range of inputs.
Conclusions Based on the results of this analysis, apixaban can be a
cost-effective alternative to other NOACs for the prevention of stroke
in patients with NVAF in Greece
Cost-Effectiveness of Varenicline Versus Bupropion, Nicotine-Replacement Therapy, and Unaided Cessation in Greece
Background: Varenicline was designed to relieve symptoms of nicotine
withdrawal, including cigarette craving, and to block the reinforcing
effects of continued nicotine use. The cost-effectiveness of varenicline
in some countries has not been studied.
Objective: The aim of this study was to compare the cost-effectiveness
of varenicline to that of bupropion, nicotine-replacement therapy (NRT),
and unaided cessation in the Greek health care setting. The analysis
takes into account a societal security (third-party payer) perspective.
Methods: To perform the analyses of the benefits of smoking cessation in
terms of smoking-related morbidity, mortality, and associated medical
costs, a Markov model was used that simulated the progress of a
hypothetical cohort of current smokers making a single attempt to quit
smoking at the beginning of the timeframe of the analysis. The
robustness of the results was assessed using a series of 1-way
sensitivity analyses.
Results: Varenicline was associated with the potential prevention of
14.1, 14.2, and 35.1 additional cases of the 4 smoking-related diseases
incorporated into the model, per 1000 smokers willing to quit, versus
bupropion, NRT, and unaided cessation, respectively. Potentially avoided
smoking-related deaths with varenicline were estimated at 3.24, 3.26,
and 7.5 per 1000 quitters versus the 3 comparators. Varenicline led to a
potential gain of 33.78, 33.91, and 83.97 QALYs per 1000 persons willing
to make a quit attempt versus the 3 comparators. Varenicline was
associated with cost-savings against both active comparators for the
lifetime horizon. Overall, the cost per additional quitter with
varenicline, considering only the costs of the smoking-cessation
strategy, was (sic)2659 ((sic)1015) for a lifetime horizon compared with
bupropion (NRT); however, when all direct costs were incorporated into
the analysis, varenicline was cost-saving.
Conclusion: The findings from the present study suggest that, compared
with the widely used treatment options bupropion and NRT, as well as
unaided cessation, varenicline may enhance smoking-cessation treatment
outcomes while substantially reducing the overall costs of smoking to

Cost Effectiveness of Apixaban versus Warfarin or Aspirin for Stroke Prevention in Patients with Atrial Fibrillation: A Greek Perspective
Background Strokes attributed to atrial fibrillation (AF) represent a
major cause of adult disability and a great burden to society and
healthcare systems.
Objectives Our objective was to assess the cost effectiveness of
apixaban, a direct acting oral anticoagulant (DOAC), versus warfarin or
aspirin for patients with AF in the Greek healthcare setting.
Methods We used a previously published Markov model to simulate clinical
events for patients with AF treated with apixaban, the vitamin K
antagonist (VKA) warfarin, or aspirin. Clinical events (ischemic and
hemorrhagic stroke, intracranial hemorrhage, other major bleed,
clinically relevant non-major bleed, myocardial infarction, and
cardiovascular [CV] hospitalizations) were modeled using efficacy data
from the ARISTOTLE and AVERROES clinical trials. The cohort’s baseline
characteristics also sourced from these trials. Among VKA-suitable
patients, 64.7% were men with a mean age of 70 years and average
CHADS(2) (cardiac failure, hypertension, age, diabetes, stroke(2)) score
of 2.1, whereas 58.5% of VKA-unsuitable patients were men with a mean
age of 70 years and a CHADS(2) score of 2.0. A panel of experts
(cardiologists and internists) provided information on the resource use
associated with the management of AF. Cost calculations reflect the
local clinical setting and a third-party payer perspective (epsilon,
discounted at 3%).
Results Based on a simulation of 1000 VKA-suitable patients over a
lifetime horizon, the use of apixaban versus warfarin resulted in 26
fewer strokes and systemic embolisms in total, 65 fewer bleeds, 41 fewer
myocardial infarctions, and 29 fewer CV-related deaths, with an
incremental cost-effectiveness ratio (ICER) of epsilon
14,478/quality-adjusted life-year (QALY). For VKA-unsuitable patients,
apixaban versus aspirin resulted in 72 fewer strokes and systemic
embolisms and 57 fewer CV-related deaths, with an ICER of
(sic)7104/QALY. Sensitivity analyses indicated that results were robust.
Conclusions Based on the present analysis, apixaban represents a
cost-effective treatment option versus warfarin and aspirin for the
prevention of stroke in patients with AF from a Greek healthcare payer
perspective over a lifetime horizon
