Sleep apnea in patients with primary chronic headache
Introduction. Sleep apnea syndrome (SAS) can accompany patients with chronic primary headache (CPH), being a frequent cause of persistence of symptoms in these patients, however, the data studying this disorder in patients with CPH are still limited.
Objective. To determine the frequency of SAS in patients with CPH.
Methods. A retrospective cohort was analyzed in patients with a diagnosis of CPH in a third level hospital. CPH was diagnosed according to the criteria of the International Headache Society and SAS with a polysomnographic record with an apnea-hypopnea index (AHI) >5/h, the frequency of SAS was determined among the total number of patients with AHI >5/h over the total number of patients undergoing polysomnography.
Results. A total of 114 subjects were included, where the SAS in CPH was 83.3%, of which 56.8% were men and 60% older than 50 years; 89% of the population had a body mass index (BMI) greater than 27 kg/m2. Fatigue and sleepiness during the day were the most frequent symptoms, reported in 89.5% of the population with SAS compared to 47.4% of the population without a diagnosis of SAS.
Conclusion. Sleep apnea is a frequent condition in patients with CPH undergoing polysomnography, mainly overweight men over 50 years of age. The main associated symptoms are fatigue and sleepiness
GARP promotes the proliferation and therapeutic resistance of bone sarcoma cancer cells through the activation of TGF-β
Sarcomas are mesenchymal cancers with poor prognosis, representing about 20% of all solid malignancies in children,
adolescents, and young adults. Radio- and chemoresistance are common features of sarcomas warranting the search
for novel prognostic and predictive markers. GARP/LRRC32 is a TGF-β-activating protein that promotes immune
escape and dissemination in various cancers. However, if GARP affects the tumorigenicity and treatment resistance of
sarcomas is not known. We show that GARP is expressed by human osteo-, chondro-, and undifferentiated
pleomorphic sarcomas and is associated with a significantly worse clinical prognosis. Silencing of GARP in bone
sarcoma cell lines blocked their proliferation and induced apoptosis. In contrast, overexpression of GARP promoted
their growth in vitro and in vivo and increased their resistance to DNA damage and cell death induced by etoposide,
doxorubicin, and irradiation. Our data suggest that GARP could serve as a marker with therapeutic, prognostic, and
predictive value in sarcoma. We propose that targeting GARP in bone sarcomas could reduce tumour burden while
simultaneously improving the efficacy of chemo- and radiotherapy.Instituto de Salud Carlos IIIEuropean Union (EU)

Demographic and clinical characteristics of chikungunya patients from six Colombian cities, 2014–2015
AbstractIn 2014, the chikungunya virus reached Colombia for the first time, resulting in a nationwide epidemic. The objective of this study was to describe the demographics and clinical characteristics of suspected chikungunya cases. Chikungunya infection was confirmed by enzyme-linked immunosorbent assay and 548 patients where included in the study. Of these patients, 295 were positive for antibodies against chikungunya (53.8%), and 27.6% (151/295) were symptomatic for chikungunya infection, with a symptomatic:asymptomatic ratio of 1.04:1. Factors associated with infection included low income and low socio-economic strata (odds ratio [OR]: 1.8; 95% confidence interval [CI]: 1.0–3.2, p = 0.003 and OR: 2.1; CI: 1.3–3.4, p = 0.002, respectively). Confirmed symptomatic cases were associated with symmetric arthritis (OR: 11.7; CI: 6.0–23.0, p < 0.001) of ankles (OR: 8.5; CI: 3.5–20.9, p < 0.001), hands (OR: 8.5; CI: 3.5–20.9, p < 0.001), feet (OR: 6.5; CI: 2.8–15.3, p < 0.001), and wrists (OR: 17.3; CI: 2.3–130.5, p < 0.001). Our study showed that poverty is associated with chikungunya infection. Public health strategies to prevent and control chikungunya should focus on poorer communities that are more vulnerable to infection. The rate of asymptomatic infections among confirmed cases was 48.8%. However, those with symptoms displayed a characteristic rheumatic clinical picture, which could help differentiate chikungunya infection from other endemic viral diseases
