    Quality Improvement of Semi-Wet Terasi by Optimizing the Starter Culture Ratio of Controlled Fermentation

    Terasi is a traditional fermented shrimp paste used in Indonesian dish as condiments. Due to its affordability, the paste is widely consumed among the general population, and thus has a great impact in Indonesia. Currently, small-scale or home industry is common for terasi production, and natural fermentation process is the traditional method. Fermentation process is considered complete when desired aromatic odors are obtained. However, this makes the fermentation process subjective, because the decision is solely dependent on the producer. Additionally, natural fermentation poses a higher risk for contamination of microbial pathogens. As a result, the quality of the final product varies greatly from region to region. Therefore, it is necessary to improve the quality of terasi by means of controlled fermentation. Hence, the objective of the research is to optimize the controlled fermentation condition of terasi by determining the most optimal ratio of mixed starter culture. Optimal fermentation conditions were determined by analyzing the effect of the various starter inoculum on the inner microbial community, and results indicated that mixed culture of Staphylococcus saprophyticus, Bacillus subtilis, and Lactobacillus murinus with ratio of 2:1:2 was the most effective for suppressing the growth of unwanted microorganisms.     Association between the size of healthcare facilities and the intensity of hypertension therapy: a cross-sectional comparison of prescription data from insurance claims data

    Hypertension is a heterogeneous disease for which role sharing in treatment between specialized facilities and small clinics is needed for efficient healthcare provision. However, the Japanese healthcare system has a "free access" attribute; therefore, nobody can control treatment resource allocation. We aimed to describe the current situation of role sharing by comparing antihypertensive therapies among different types of medical facilities. We analyzed 1% sampled Japanese medical insurance claims data related to outpatient care as of October 2014. We divided the target patients into four groups according to the size of the facilities that issued the insurance claim for them. Among these groups, we compared the number of antihypertensive drugs and proportion of difficult-to-treat hypertensive cases and performed a stratified analysis. The proportion of patients with hypertension and diabetes mellitus receiving renin-angiotensin-aldosterone system inhibitors (RAASis) as the first-choice drug was also compared. We identified 3465, 1797, 2323, and 34, 734 claims issued from large, medium-sized, small hospitals, and clinics, respectively. The mean number of hypertensive drugs was 1.96, 1.87, 1.81, and 1.69, respectively, and the proportion of difficult-to-treat hypertensive cases was 18.9, 17.0, 14.3, and 12.0%, respectively, with both showing significant differences. Stratified analysis showed similar results. The proportion of patients with hypertension and diabetes mellitus receiving RAASis as the first-choice drug was higher in large hospitals than in clinics. In conclusion, facility size is positively associated with the number of antihypertensive drugs and proportions of difficult-to-treat hypertensive cases.     Substellar Companions to Seven Evolved Intermediate-Mass Stars

    We report the detections of substellar companions orbiting around seven evolved intermediate-mass stars from precise Doppler measurements at Okayama Astrophysical Observatory. o UMa (G4 II-III) is a giant with a mass of 3.1 M_sun and hosts a planet with minimum mass of m_2sini=4.1 M_J in an orbit with a period P=1630 d and an eccentricity e=0.13. This is the first planet candidate (< 13 M_J) ever discovered around stars more massive than 3 M_sun. o CrB (K0 III) is a 2.1 M_sun giant and has a planet of m_2sini=1.5 M_J in a 187.8 d orbit with e=0.19. This is one of the least massive planets ever discovered around ~2 M_sun stars. HD 5608 (K0 IV) is an 1.6 M_sun subgiant hosting a planet of m_2sini=1.4 M_J in a 793 d orbit with e=0.19. The star also exhibits a linear velocity trend suggesting the existence of an outer, more massive companion. 75 Cet (G3 III:) is a 2.5 M_sun giant hosting a planet of m_2sini=3.0 M_J in a 692 d orbit with e=0.12. The star also shows possible additional periodicity of about 200 d and 1880 d with velocity amplitude of ~7--10 m/s, although these are not significant at this stage. nu Oph (K0 III) is a 3.0 M_sun giant and has two brown-dwarf companions of m_2sini= 24 M_J and 27 M_J, in orbits with P=530.3 d and 3190 d, and e=0.126 and 0.17, respectively, which were independently announced by Quirrenbach et al. (2011). The ratio of the periods is close to 1:6, suggesting that the companions are in mean motion resonance. We also independently confirmed planets around k CrB (K0 III-IV) and HD 210702 (K1 IV), which had been announced by Johnson et al. (2008) and Johnson et al. (2007a), respectively.     Prediction and visualization of acute kidney injury in intensive care unit using one-dimensional convolutional neural networks based on routinely collected data

    Background: Acute kidney injury (AKI) occurs frequently in in-hospital patients, especially in the intensive care unit (ICU), due to various etiologies including septic shock. It is clinically important to identify high-risk patients at an early stage and perform the appropriate intervention. Methods: We proposed a system to predict AKI using one-dimensional convolutional neural networks (1D-CNN) with the real-time calculation of the probability of developing AKI, along with the visualization of the rationale behind prediction using score-weighted class activation mapping and guided backpropagation. The system was applied to predicting developing AKI based on the KDIGO guideline in time windows of 24 to 48 h using data of 0 to 24 h after admission to ICU. Results: The comparison result of multiple algorithms modeling time series data indicated that the proposed 1D-CNN model achieved higher performance compared to the other models, with the mean area under the receiver operating characteristic curve of 0.742 ± 0.010 for predicting stage 1, and 0.844 ± 0.029 for stage 2 AKI using the input of the vital signs, the demographic information, and serum creatinine values. The visualization results suggested the reasonable interpretation that time points with higher respiratory rate, lower blood pressure, as well as lower SpO2, had higher attention in terms of predicting AKI, and thus important for prediction. Conclusions: We presumed the proposed system's potential usefulness as it could be applied and transferred to almost any ICU setting that stored the time series data corresponding to vital signs

    A Double Planetary System around the Evolved Intermediate-Mass Star HD 4732

    We report the detection of a double planetary system orbiting around the evolved intermediate-mass star HD 4732 from precise Doppler measurements at Okayama Astrophysical Observatory (OAO) and Anglo-Australian Observatory (AAO). The star is a K0 subgiant with a mass of 1.7 M_sun and solar metallicity. The planetary system is composed of two giant planets with minimum mass of msini=2.4 M_J, orbital period of 360.2 d and 2732 d, and eccentricity of 0.13 and 0.23, respectively. Based on dynamical stability analysis for the system, we set the upper limit on the mass of the planets to be about 28 M_J (i>5 deg) in the case of coplanar prograde configuration.Comment: 12 pages, 7 figures, accepted for publication in Ap

    A Substellar Companion to the Intermediate-Mass Giant 11 Com

    We report the detection of a substellar companion orbiting the intermediate-mass giant star 11 Com (G8 III). Precise Doppler measurements of the star from Xinglong station and Okayama Astrophysical Observatory (OAO) revealed Keplerian velocity variations with an orbital period of 326.03 +/- 0.32 days, a semiamplitude of 302.8 +/- 2.6 m/s, and an eccentricity of 0.231 +/- 0.005. Adopting a stellar mass of 2.7 +/- 0.3 M_solar, the minimum mass of the companion is 19.4 +/- 1.5 M_Jup, well above the deuterium burning limit, and the semimajor axis is 1.29 +/- 0.05 AU. This is the first result from the joint planet search program between China and Japan aiming at revealing statistics of substellar companions around intermediate-mass giants. 11 Com b emerged from 300 targets of the planet search program at OAO.
