Expression and function of calcium-activated potassium channels following in-stent restenosis in a porcine coronary artery model
AbstractIn-stent restenosis (ISR) occurs due to proliferation and migration of smooth muscle cells from media to intima resulting in re-narrowing of the vessel lumen. This study aims to investigate changes in the three main KCa channels in response to stent implantation in porcine coronary arteries as their expression and function in ISR is yet to be defined. Twenty-eight days after stent implantation, immunofluorescent labelling with anti-desmin and anti-vWF confirm the presence of both endothelial and smooth muscle cells within the neointimal layer. Using real-time PCR, significant increase in the SK3 and IKCa and BKCa channel mRNA was observed within this layer alone. Western blot analysis confirms the expression of KCa channels in neointima. Although expression of BKCa was increased in the neointima in comparison with medial region of the artery, microelectrode recordings showed that the function of this channel was unchanged. However, the presence of functional BKCa in both medial and intimal cells suggests that smooth muscle cells migration may contribute to neointimal hyperplasia.Functional analysis using 1-EBIO and Bradykinin produced hyperpolarization of neointimal but not medial myocytes, which indicated the expression of functional endothelial SK3 and IKCa in the former and not in the latter. The expression of IKCa and SK3 within the neointimal layer suggested that some degree of recovery of both endothelial as well as smooth muscle regeneration had occurred. Future development of selective modulators of IKCa and SK3 channels may decrease the progression of ISR and improve coronary vascular function after stent placement, and is an area for future investigation

Talk CPR - a technology project to improve communication in do not attempt cardiopulmonary resuscitation decisions in palliative illness
Background
A national Do Not Attempt Cardiopulmonary Resuscitation policy was rolled out for the National Health Service in Wales in 2015. A national steering group led on producing information videos and a website for patients, carers and healthcare professionals, forming part of a quality improvement program. Videos were planned, scripted and produced with healthcare professionals and patient/carer representatives, and were completed with both English and Welsh language versions. The TalkCPR videos encourage and promote open discussion about Cardiopulmonary Resuscitation (CPR) and DNACPR in palliative care situations.
Methods
We worked with patient/carer groups to evaluate whether video resources to convey the salient facts involved in CPR and DNACPR decisions for people with palliative and life-limiting illness were acceptable or not. We conducted a mixed-method design service review in five phases to evaluate whether this technological resource could help. After creating video and website materials, they were evaluated by doctors, nurses and a patient/carer group. We also sent out one lightweight TalkCPR video media pad to each practice in Wales. These rechargeable electronic video media pads had communication videos pre-loaded for easy viewing, especially in areas with poor roaming data coverage.
Results
Videos were demonstrably acceptable to both patient and carer groups, and improved healthcare professional confidence and understanding. Videos went live on the TalkCPR website, in all Welsh Health Boards and on Youtube, and are now used in routine practice throughout Wales.
Conclusion
This is the first time that DNACPR information videos are aimed directly at palliative care patients and carers, to explore this sensitive subject with them, and to encourage them to approach their doctor or nurse about it. The website, app and video media pads were developed by patients, the Digital Legacy Association, Welsh NHS IT services, Welsh Government, the Bevan Commission and the Dying Matters Charity in Wales âByw Nawrâ. The GMC, the Royal College of General Practitioners and NICE have listed TalkCPR as a learning resource. There has also been a collaboration with Falmouth University Art College, who helped produce graphic designs to facilitate and encourage discussions about CPR and end of life care
Iris pigmentation as a quantitative trait: variation in populations of European, East Asian and South Asian ancestry and association with candidate gene polymorphisms
In this study, we present a new quantitative method to measure iris colour based on highâresolution photographs. We applied this method to analyse iris colour variation in a sample of individuals of East Asian, European and South Asian ancestry. We show that measuring iris colour using the coordinates of the CIELAB colour space uncovers a significant amount of variation that is not captured using conventional categorical classifications, such as âbrownâ, âblueâ or âgreenâ. We tested the association of a selected panel of polymorphisms with iris colour in each population group. Six markers showed significant associations with iris colour in the European sample, three in the South Asian sample and two in the East Asian sample. We also observed that the marker HERC 2 rs12913832, which is the main determinant of âblueâ versus âbrownâ iris colour in European populations, is also significantly associated with central heterochromia in the European sample
Access to justice for people with disabilities as victims of crime in Ireland
International literature recognises that people with disabilities are at greater risk of crime than their able-bodied counterparts, but that crime against people with disabilities is significantly under-reported and often fails to proceed to prosecution. However, little is known in the Irish context about how the criminal justice system responds to the needs of people with disabilities as victims of crime. This study aims to: Explore the barriers that people with disabilities who report a crime face in accessing the criminal justice system in Ireland and internationally; Compare the legislative tools and frameworks across different jurisdictions which seek to protect the rights of people with disabilities who report crime and abuse; Analyse the specific policies and practices that agencies of the criminal justice system have in place to facilitate people with disabilitiesâ access to justice; Explore national and international innovations which may contribute to strengthening the way in which the Irish criminal justice system responds to the needs of people with disabilities. Sprouty2 lossâinduced IL6 drives castrationâresistant prostate cancer through scavenger receptor B1
Metastatic castrationâresistant prostate cancer (mCRPC) is a lethal form of treatmentâresistant prostate cancer and poses significant therapeutic challenges. Deregulated receptor tyrosine kinase (RTK) signalling mediated by loss of tumour suppressor Sprouty2 (SPRY2) is associated with treatment resistance. Using preâclinical human and murine mCRPC models, we show that SPRY2 deficiency leads to an androgen selfâsufficient form of CRPC. Mechanistically, HER2âIL6 signalling axis enhances the expression of androgen biosynthetic enzyme HSD3B1 and increases SRB1âmediated cholesterol uptake in SPRY2âdeficient tumours. Systemically, IL6 elevated the levels of circulating cholesterol by inducing host adipose lipolysis and hepatic cholesterol biosynthesis. SPRY2âdeficient CRPC is dependent on cholesterol bioavailability and SRB1âmediated tumoral cholesterol uptake for androgen biosynthesis. Importantly, treatment with ITX5061, a clinically safe SRB1 antagonist, decreased treatment resistance. Our results indicate that cholesterol transport blockade may be effective against SPRY2âdeficient CRPC
Risk factors for negative experiences during psychotherapy
Background: It is estimated that between 3% and 15% of patients have a negative experience of psychotherapy, but little is understood about this. Aims: The aim of this study was to investigate the factors associated with patientsâ negative therapy experiences. Method: The data comprised 185 patient and 304 therapist questionnaires, 20 patient and 20 therapist interviews. Patients reported on an unhelpful or harmful experience of therapy, and therapists on a therapy where they thought the patient they were working with had a poor or harmful experience. These were transcribed and analysed using thematic analysis. Results: There was a Lack of fit between Patient needs, Therapist skills, and Service structures. This could result in Fault Lines, a tension between Safety and containment and Power and control. This tension led to Strain and Poor Engagement, which led to Consequences following the negative therapy experience. Conclusions: Patients require clear information, choice, involvement in decision-making, explicit contracting and clarity about sessions and progress. Opportunities for patient feedback should be the norm, where the therapist and service are vigilant for signs of deterioration and solutions considered.Clinical and methodological significance of this article: Estimates of âunwanted effects,â including long-lasting effects, of psychotherapy have ranged from 3% to 15%. Few empirical studies have been conducted in this area. This study aimed to address this gap and provide clinicians with a model of risk factors for negative therapy effects. The findings of this study indicate the importance of providing patients with a supportive service structure that offers clear information, choice and involvement in decision-making. Explicit contracting at the beginning of therapy and clarity about sessions and progress are also important in managing patient expectations throughout. Opportunities for patient feedback should be provided
Obesity-related perivascular adipose tissue damage is reversed by sustained weight loss in the rat
Objective â Perivascular adipose tissue (PVAT) exerts an anticontractile effect in response to various vasoconstrictor agonists and this is lost in obesity. A recent study reported that bariatric surgery reverses the damaging effects of obesity on PVAT function. However, PVAT function has not been characterised following weight loss induced by caloric restriction, which is often the first line treatment for obesity.
Approach and Results â Contractility studies were performed using wire myography on small mesenteric arteries with and without PVAT from control, diet-induced obese, calorie restricted and sustained weight loss rats. Changes in the PVAT environment were assessed using immunohistochemistry. PVAT from healthy animals elicited an anticontractile effect in response to norepinephrine. This was abolished in diet-induced obesity through a mechanism involving increased local TNFα and reduced nitric oxide bioavailability within PVAT. Sustained weight loss led to improvement in PVAT function associated with restoration of adipocyte size, reduced TNFα and increased nitric oxide synthase function. This was associated with reversal of obesity-induced hypertension and normalisation of plasma adipokine levels, including leptin and insulin.
Conclusions â We have shown that diet-induced weight loss reverses obesity-induced PVAT damage through a mechanism involving reduced inflammation and increased nitric oxide synthase activity within PVAT. These data reveal inflammation and nitric oxide synthase, particularly eNOS, as potential targets for the treatment of PVAT dysfunction associated with obesity and the metabolic syndrome
Development of a Tool for Navigating the Evidence concerning Land Managers and Woodland Creation in the United Kingdom
Woodland creation has become an important objective for a variety of stakeholders to help tackle the climate and biodiversity crises. One of the key evidence needs is a better understanding of the multiple factors influencing the willingness and ability of landowners and managers to establish new woodlands. To address this gap, a systematic map of evidence was prepared comprising publications from academic journals and grey literature accessed through bibliographic databases (Web of Science, Scopus, and CAB Abstracts), libraries, direct requests to relevant organisations and individuals, and citation tracking from past reviews. A screening process refined the evidence base to 226 studies within the UK. The systematic evidence map codes the content of each study against a comprehensive list comprising actors, drivers of or barriers to woodland creation, and outcomes. These are presented as a freely accessible, interactive online dashboard detailing sources of evidence. The systematic evidence map helps users navigate the evidence, demonstrating where the bulk of the evidence lies and, conversely, several evidence gaps where there is comparatively little evidence. The findings serve as a basis for dialogue with stakeholders to determine priorities for future primary research
