    This case report describes the findings of a 18 year-old black male from Bahia, a Northeastern state in Brazil, with the sickle cell trait, who developed bilateral hyphema and vitreous hemorrhage with blindness in the course of leptospirosis. The patient started to complain of blurred vision four days after the start of fever and muscular pain and approximately twelve hours after the introduction of penicillin. The severity of the leptospirosis in conjunction with sickle cell trait was considered to be the most likely explanation for this ocular complication.Este relato de caso descreve os achados em homem negro com 18 anos de idade, da Bahia, estado do nordeste do Brasil, com estigma falciforme que desenvolveu hifema bilateral e hemorragia vítrea com cegueira no curso de leptospirose. O paciente começou a se queixar de visão embaçada quatro dias após o início da febre e da dor muscular e aproximadamente 12 horas após o início da penicilina.     Sacrospinous Ligament Suspension With Transobturator Mesh Versus Sacral Colpopexy For Genital Prolapse

    To compare the safety and efficacy of abdominal sacral colpopexy and sacrospinous ligament suspension with the use of vaginal mesh for apical prolapse. METHOD: This retrospective study was conducted from 2005 to 2012 and included 89 women with apical prolapse who underwent surgery. Assessments included pre- and postoperative Pelvic Organ Prolapse Quantification (POP-Q) stage. Rates of objective cure and immediate/late complications were compared. RESULTS: In total, 41 of the 89 women underwent sacrospinous ligament suspension, and 48 of the women underwent abdominal sacral colpopexy. A total of 40.4% of the women had vault prolapse (p=0.9361). Most of them had no complications (93.2%) (p=0.9418). Approximately 30% of the women had late complications; local pain was the main symptom and was found only in women who underwent the abdominal procedure (25.6%) (p=0.001). Only the women who were submitted to the vaginal procedure had mesh exposure (18.4%). The objective success rate and the rate of anterior vaginal prolapse (p=0.2970) were similar for both techniques. CONCLUSION: Sacrospinous ligament suspension was as effective and had a similar objective success rate as abdominal sacral colpopexy for the treatment of apical prolapse. Sacrospinous ligament suspension performed with the use of vaginal mesh in the anterior compartment was effective in preventing anterior vaginal prolapse after surgery.71948749

    Generation of multiple ultrashort solitons in a third-order nonlinear composite medium with self-focusing and self-defocusing nonlinearities

    Theoretical consideration of the propagation of femtosecond-Gaussian pulses in a 1D composite medium, consisting of alternating self-focusing (SF) and self-defocusing (SDF) waveguide segments with normal group-velocity dispersion predicts the generation of trains of bright solitons when an optical pulse first propagates in the SF segment, followed by the SDF one. The multiple temporal compression (MTC) process, based on this setting, offers a method for controllable generation of multiple ultrashort temporal solitons. Numerical solutions of the generalized nonlinear Schr\"{o}dinger equation modeling this system demonstrate that the intrapulse Raman scattering plays a major role in the temporal and spectral dynamics. Collisions between ultrashort solitons with different central wavelengths are addressed too. The paper provides, for the first time, a procedure for producing controllable trains of ultrashort temporal solitons by incident optical pulses propagating in a composite medium.Comment: To be published in Phys.     Generation of robust temporal soliton trains by the multiple-temporal-compression (MTC) method

    We report results of systematic numerical analysis for multiple soliton generation by means of the recently reported multiple temporal compression (MTC) method, and compare its efficiency with conventional methods based on the use of photonic crystal fibers (PCFs) and fused silica waveguides (FSWs). The results show that the MTC method is more efficient to control the soliton fission, giving rise to a larger number of fundamental solitons with high powers, that remain nearly constant over long propagation distances.     Plastic adjustments in xylem vessel traits to drought events in three Cedrela species from Peruvian Tropical Andean forests

    Cedrela species occur within the Tropical montane cloud forest (TMCF) and rainforest in North America (Mexico), Central and South America. We assessed the hypothesis that functional xylem hydraulic architecture might be influenced by specific climatic variations. We investigated the effect of climate on tree-ring width and vessel traits (diameter, vessel density, vulnerability index and hydraulic diameter) of three relict-endemic and threatened Cedrela species (Cedrela fissilis, C. nebulosa and C. angustifolia) inhabiting Peruvian Tropical Andean cloud forests. All Cedrela species showed a significant reduction in radial growth and adjusted vessel trait linked with temperature, precipitation, and evapotranspiration. Ring-width and vessel traits showed adaptation within Cedrela species, crucial to understanding a rough indication of the plant’s ability to withstand drought-induced embolism or cavitation. Our results provide evidence for hydraulic mechanisms that determine specific wood anatomical functionality to climatic variation and drought responses. Therefore, changing the frequency or intensity of future drought events might exceed the adaptive limits of TMCF tree species, resulting in a substantial reduction of hydraulic functionality in Peruvian Cedrela species.Fil: Rodríguez Ramírez, Ernesto C..
