    HPV prevalence and concordance in the cervix and oral cavity of pregnant women.

    OBJECTIVES: This investigation examined human papillomavirus (HPV) in pregnant women in order to characterize viral prevalence, types and concordance between infection in the cervix and in the oral cavity. METHODS: A total of 577 pregnant women seeking routine obstetric care were evaluated for HPV infection in their cervix during gestation and immediately before delivery, and in the oral cavity during gestation. Male partners present during the gestational clinic visit also provided a specimen from their oral cavity. HPV assessment was performed by PCR, dot blot hybridization and DNA sequencing. A sexual and health questionnaire was completed by the pregnant women. RESULTS: HPV prevalence in women was 29% in the cervix and 2.4% in the oral cavity. Among those with both gestational and delivery specimens, 35% were infected at least once and 20% had infection at both intervals. At delivery, 68% of infected women had an oncogenic HPV type in the cervix. There was no type-specific HPV concordance between the two cervical specimens, nor cervical and oral results in women, nor with cervical and oral findings between partners. CONCLUSION: The lack of association in HPV positivity and types between the cervix and oral cavity in these women suggests that self-inoculation is uncommon. This source of infection does not appear to be from oral contact with a current male partner, since there also was no concordance between partners. These results suggest either other modes of HPV transmission or differences in susceptibility to HPV infection or its clearance in the oral cavity and genital mucosa

    Cosmic ray confinement in fossil cluster bubbles

    Most cool core clusters of galaxies possess active galactic nuclei (AGN) in their centers. These AGN inflate buoyant bubbles containing non-thermal radio emitting particles. If such bubbles efficiently confine cosmic rays (CR) then this could explain ``radio ghosts'' seen far from cluster centers. We simulate the diffusion of cosmic rays from buoyant bubbles inflated by AGN. Our simulations include the effects of the anisotropic particle diffusion introduced by magnetic fields. Our models are consistent with the X-ray morphology of AGN bubbles, with disruption being suppressed by the magnetic draping effect. We conclude that for such magnetic field topologies, a substantial fraction of cosmic rays can be confined inside the bubbles on buoyant rise timescales even when the parallel diffusivity coefficient is very large. For isotropic diffusion at a comparable level, cosmic rays would leak out of the bubbles too rapidly to be consistent with radio observations. Thus, the long confinement times associated with the magnetic suppression of CR diffusion can explain the presence of radio ghosts. We show that the partial escape of cosmic rays is mostly confined to the wake of the rising bubbles, and speculate that this effect could: (1) account for the excitation of the HÎą\alpha filaments trailing behind the bubbles in the Perseus cluster, (2) inject entropy into the metal enriched material being lifted by the bubbles and, thus, help to displace it permanently from the cluster center and (3) produce observable Îł\gamma-rays via the interaction of the diffusing cosmic rays with the thermal intracluster medium (ICM).Comment: submitte

    Funding models in palliative care: lessons from international experience

    Background:Funding models influence provision and development of palliative care services. As palliative care integrates into mainstream health care provision, opportunities to develop funding mechanisms arise. However, little has been reported on what funding models exist or how we can learn from them.Aim:To assess national models and methods for financing and reimbursing palliative care.Design:Initial literature scoping yielded limited evidence on the subject as national policy documents are difficult to identify, access and interpret. We undertook expert consultations to appraise national models of palliative care financing in England, Germany, Hungary, Republic of Ireland, New Zealand, The Netherlands, Norway, Poland, Spain, Sweden, Switzerland, the United States and Wales. These represent different levels of service development and a variety of funding mechanisms.Results:Funding mechanisms reflect country-specific context and local variations in care provision. Patterns emerging include the following:Provider payment is rarely linked to population need and often perpetuates existing inequitable patterns in service provision.Funding is frequently characterised as a mixed system of charitable, public and private payers.The basis on which providers are paid for services rarely reflects individual care input or patient needs.Conclusion:Funding mechanisms need to be well understood and used with caution to ensure best practice and minimise perverse incentives. Before we can conduct cross-national comparisons of costs and impact of palliative care, we need to understand the funding and policy context for palliative care in each country of interest

    Grain boundary corrosion in TiO2 bone scaffolds doped with group II cations

    A pH drop during the inflammatory phase during bone regeneration can cause corrosion in TiO2 bone scaffolds and the loss of compressive strength. Corrosion as ion leaching and dissolution is confined to grain boundaries. Cationic doping of TiO2 showed to increase the compressive strength but increased the amount of impurities in grain boundaries as well. Therefore, this study showed the different grain boundary formation for Ca, Sr and Mg doped scaffolds and their corrosion behavior. After corrosion, the amorphous phase in grain boundaries was dissolved in all doped scaffolds. Differences occurred due to the formation of an additional crystalline phase in Sr doped scaffolds. The presence of an amorphous and crystalline phase led to an inhomogeneous dissolution in grain boundaries and a significant decrease in compressive strength already after 4 h in contact with an acidic environment. Released ions did not show any cytotoxic effect on hASCs. Mg doped TiO2 scaffolds led to sig- nificant increased osteogenic differentiation.(undefined)info:eu-repo/semantics/publishedVersio

    Increasing pattern recognition accuracy for chemical sensing by evolutionary based drift compensation

    Artificial olfaction systems, which mimic human olfaction by using arrays of gas chemical sensors combined with pattern recognition methods, represent a potentially low-cost tool in many areas of industry such as perfumery, food and drink production, clinical diagnosis, health and safety, environmental monitoring and process control. However, successful applications of these systems are still largely limited to specialized laboratories. Sensor drift, i.e., the lack of a sensor's stability over time, still limits real in dustrial setups. This paper presents and discusses an evolutionary based adaptive drift-correction method designed to work with state-of-the-art classification systems. The proposed approach exploits a cutting-edge evolutionary strategy to iteratively tweak the coefficients of a linear transformation which can transparently correct raw sensors' measures thus mitigating the negative effects of the drift. The method learns the optimal correction strategy without the use of models or other hypotheses on the behavior of the physical chemical sensors
