Barriers to gender-equitable HIV testing: going beyond routine screening for pregnant women in Nova Scotia, Canada
<p>Abstract</p> <p>Background</p> <p>Women and men face different gender-based health inequities in relation to HIV, including HIV testing as well as different challenges in accessing HIV care, treatment and support programs and services when testing HIV-positive. In this article, we discuss the findings of a mixed methods study exploring the various individual and structural barriers and facilitators to HIV counselling and testing experienced among a sample of adult women and men living in Nova Scotia, Canada.</p> <p>Methods</p> <p>Drawing from testing demographics, qualitative interview data and a review of existing testing policies and research, this paper focuses on understanding the gendered health inequities and their implications for HIV testing rates and behaviours in Nova Scotia.</p> <p>Results</p> <p>The findings of this research serve as the basis to further our understanding of gender as a key determinant of health in relation to HIV testing. Recognizing gender as a key determinant of health in terms of both vulnerability to HIV and access to testing, this paper explores how gender intersects with health equity issues such as access to HIV testing, stigma and discrimination, and sexual behaviours and relationships.</p> <p>Conclusions</p> <p>Drawing on the current gender and HIV literatures, in conjunction with our data, we argue that an enhanced, gender-based, context-dependent approach to HIV counselling and testing service provision is required in order to address the health equity needs of diverse groups of women and men living in various settings. Further, we argue that enhanced HIV testing efforts must be inclusive of both men and women, addressing uniquely gendered barriers to accessing HIV counselling and testing services and in the process moving beyond routine HIV testing for pregnant women.</p
Removing Orbital Debris with Lasers
Orbital debris in low Earth orbit (LEO) are now sufficiently dense that the
use of LEO space is threatened by runaway collisional cascading. A problem
predicted more than thirty years ago, the threat from debris larger than about
1 cm demands serious attention. A promising proposed solution uses a high power
pulsed laser system on the Earth to make plasma jets on the objects, slowing
them slightly, and causing them to re-enter and burn up in the atmosphere. In
this paper, we reassess this approach in light of recent advances in low-cost,
light-weight modular design for large mirrors, calculations of laser-induced
orbit changes and in design of repetitive, multi-kilojoule lasers, that build
on inertial fusion research. These advances now suggest that laser orbital
debris removal (LODR) is the most cost-effective way to mitigate the debris
problem. No other solutions have been proposed that address the whole problem
of large and small debris. A LODR system will have multiple uses beyond debris
removal. International cooperation will be essential for building and operating

High-temperature performance of ferritic steels in fireside corrosion regimes: temperature and deposits
The paper reports high temperature resistance of ferritic steels in fireside corrosion regime in terms of temperature and deposits aggressiveness. Four candidate power plant steels: 15Mo3, T22, T23 and T91 were exposed under simulated air-fired combustion environment for 1000 h. The tests were conducted at 600, 650 and 700 °C according to deposit-recoat test method. Post-exposed samples were examined via dimensional metrology (the main route to quantify metal loss), and mass change data were recorded to perform the study of kinetic behavior at elevated temperatures. Microstructural investigations using ESEM-EDX were performed in order to investigate corrosion degradation and thickness of the scales. The ranking of the steels from most to the least damage was 15Mo3 > T22 > T23 > T91 in all three temperatures. The highest rate of corrosion in all temperatures occurred under the screening deposit
Adaptive response and enlargement of dynamic range
Many membrane channels and receptors exhibit adaptive, or desensitized,
response to a strong sustained input stimulus, often supported by protein
activity-dependent inactivation. Adaptive response is thought to be related to
various cellular functions such as homeostasis and enlargement of dynamic range
by background compensation. Here we study the quantitative relation between
adaptive response and background compensation within a modeling framework. We
show that any particular type of adaptive response is neither sufficient nor
necessary for adaptive enlargement of dynamic range. In particular a precise
adaptive response, where system activity is maintained at a constant level at
steady state, does not ensure a large dynamic range neither in input signal nor
in system output. A general mechanism for input dynamic range enlargement can
come about from the activity-dependent modulation of protein responsiveness by
multiple biochemical modification, regardless of the type of adaptive response
it induces. Therefore hierarchical biochemical processes such as methylation
and phosphorylation are natural candidates to induce this property in signaling

The Ghrelin Signalling System Is Involved in the Consumption of Sweets
The gastric-derived orexigenic peptide ghrelin affects brain circuits involved in energy balance as well as in reward. Indeed, ghrelin activates an important reward circuit involved in natural- as well as drug-induced reward, the cholinergic-dopaminergic reward link. It has been hypothesized that there is a common reward mechanism for alcohol and sweet substances in both animals and humans. Alcohol dependent individuals have higher craving for sweets than do healthy controls and the hedonic response to sweet taste may, at least in part, depend on genetic factors. Rat selectively bred for high sucrose intake have higher alcohol consumption than non-sucrose preferring rats and vice versa. In the present study a group of alcohol-consuming individuals selected from a population cohort was investigated for genetic variants of the ghrelin signalling system in relation to both their alcohol and sucrose consumption. Moreover, the effects of GHS-R1A antagonism on voluntary sucrose- intake and operant self-administration, as well as saccharin intake were investigated in preclinical studies using rodents. The effects of peripheral grelin administration on sucrose intake were also examined. Here we found associations with the ghrelin gene haplotypes and increased sucrose consumption, and a trend for the same association was seen in the high alcohol consumers. The preclinical data show that a GHS-R1A antagonist reduces the intake and self-administration of sucrose in rats as well as saccharin intake in mice. Further, ghrelin increases the intake of sucrose in rats. Collectively, our data provide a clear indication that the GHS-R1A antagonists reduces and ghrelin increases the intake of rewarding substances and hence, the central ghrelin signalling system provides a novel target for the development of drug strategies to treat addictive behaviours
Association Between Vitamin D Insufficiency and Impaired Bone Density Among Adolescents With Perinatally Acquired HIV Infection
Background: Stunting and pubertal delay are common among children growing up with human immunodeficiency virus (HIV) and are associated with bone and muscle impairments. We investigated factors associated with bone density and muscle function in adolescents living with HIV (ALWH). Methods: The VITALITY trial (PACTR202009897660297) investigated whether vitamin D and calcium supplementation improves musculoskeletal health among ALWH. A total of 842 ALWH aged 11–19 years, established on antiretroviral therapy (ART) for ≥6 months, were enrolled from HIV clinics in Zambia and Zimbabwe. Clinical history and examination were undertaken, and serum 25-hydroxyvitamin D3 (25[OH]D3) was measured. Dual-energy X-ray absorptiometry measured total-body-less-head bone mineral density adjusted for height (TBLH-BMDHT), and lumbar spine bone mineral apparent density (LS-BMAD) z scores. The association between a priori–defined covariates and musculoskeletal outcomes were investigated using baseline enrollment data and multivariable logistic regression. Results: TBLH-BMDHT z scores were impaired (mean, −1.42 for male and −0.63 female participants), as were LS-BMAD z scores (mean −1.15 for male and −0.47 for female participants). In bivariate analysis, early pubertal stage, less physical activity, and older age at ART initiation were associated with lower TBLH-BMDHT z scores. Younger age, early pubertal stage, and low socioeconomic status were associated with lower LS-BMAD z scores. Grip-strength-for-height and jump-power-for-height z scores were associated with lower TBLH-BMDHT and LS-BMAD z scores. Low dietary vitamin D and calcium were associated with lower adjusted TBLH-BMDHT z scores. Lower 25(OH)D3 was associated with lower adjusted TBLH-BMDHT and LS-BMAD z scores. Conclusions: Deficits in bone density are common in ALWH. Vitamin D and calcium supplementation and promotion of exercise may improve musculoskeletal health among perinatally infected ALWH
Assessment of a rapid liquid-based cytology method for measuring sputum cell counts
Differential sputum cell counting is not widely available despite proven clinical utility in the management of asthma. We compared eosinophil counts obtained using liquid-based cytology (LBC), a routine histopathological processing method, and the current standard method. Eosinophil counts obtained using LBC were a strong predictor of sputum eosinophilia (≥3%) determined by the standard method suggesting LBC could be used in the management of asthma
Multivariate Modelling of Pedestrian Fatality Risk Through on the Spot Accident Investigation
Pedestrians are the most vulnerable users of public roads and represent one of the largest groups of road casualties; their death rate around the world due to vehicle-pedestrian collisions is high and tending to rise. In Spain, as in other countries of the European Union, steps have been taken to reduce the number and consequences of such accidents, with encouraging results in recent years. A key to countering this concern is the accident research activity that has obtained remarkable achievements in different fields, especially when multidisciplinary approaches are taken. This paper describes the development of a multivariate model that is able to detect the most influential parameters on the consequences of vehicle-pedestrian collision and to quantify their impact on pedestrian fatality risk. First, an accident database containing detailed information and parameters of vehicle-pedestrian collisions in Madrid has been developed. The accidents were investigated on the spot by INSIA accident investigation teams and analyzed using advanced reconstruction techniques. The model was then developed with two components: (1) a classification tree that characterizes and selects the explanatory variables, identifying their interactions, and (2) a binary logistic regression to quantify the influence of each variable and interaction resulting from the classification tree. The whole model represents an important tool for identifying, quantifying and predicting the potential impact of measures aimed at reducing injuries in vehicle-pedestrian collisions
