    Geometry of contours and Peierls estimates in d=1 Ising models

    Following Fr\"ohlich and Spencer, we study one dimensional Ising spin systems with ferromagnetic, long range interactions which decay as ∣x−y∣−2+α|x-y|^{-2+\alpha}, 0≤α≤1/20\leq \alpha\leq 1/2. We introduce a geometric description of the spin configurations in terms of triangles which play the role of contours and for which we establish Peierls bounds. This in particular yields a direct proof of the well known result by Dyson about phase transitions at low temperatures.Comment: 28 pages, 3 figure

    Short-term efficacy of ultramicronized palmitoylethanolamide in peripheral neuropathic pain.

    Introduction. This study evaluates the efficacy of palmitoylethanolamide ultramicronized (PEA-um) as an add-on treatment in patients with diabetic or traumatic neuropathic pain (NP). Methods. 30 patients with chronic NP were assessed with Visual Analogue Scale (VAS), NP Symptom Inventory (NPSI), and Health Questionnaire Five Dimensions (EQ-5D), both at baseline and after 10 and 40 days of treatment with 1200 mg/die of PEA-um. All other therapies were maintained stable during the follow-up period. Results. VAS mean score significantly improved within the first 10 days, ranging from 8.20 ± 1.53 to 6.40 ± 1.83 (P<0.002), with a further decrease to 5.80 ± 2.04 (P<0.001) after 40 days of PEA-um administration. Moreover, NPSI total score improved from 5.2 ± 1.5 to 3.8 ± 2.1 (P: 0.025) and EQ-5D ranged from −0.30 ± 0.65 to 0.5 ± 0.34 (P<0.001) between T0 and T2. Conclusions. This study reports the prospective short-term efficacy data of oral PEA-um in patients with diabetic or traumatic NP. A significant improvement was observed both in VAS and NPSI scores and in quality of life scales after 40 days of treatment, although some limitations should be considered, including the short followup and the open-label study design

    The Human Cytomegalovirus UL116 Glycoprotein Is a Chaperone to Control gH-Based Complexes Levels on Virions

    Human cytomegalovirus (HCMV) relies in large part upon the viral membrane fusion glycoprotein B and two alternative gH/gL complexes, gH/gL/gO (Trimer) and gH/gL/UL128/UL130/UL131A (Pentamer) to enter into cells. The relative amounts of Trimer and Pentamer vary among HCMV strains and contribute to differences in cell tropism. Although the viral ER resident protein UL148 has been shown to interact with gH to facilitate gO incorporation, the mechanisms that favor the assembly and maturation of one complex over another remain poorly understood. HCMV virions also contain an alternative non-disulfide bound heterodimer comprised of gH and UL116 whose function remains unknown. Here, we show that disruption of HCMV gene UL116 causes infectivity defects of ∼10-fold relative to wild-type virus and leads to reduced expression of both gH/gL complexes in virions. Furthermore, gH that is not covalently bound to other viral glycoproteins, which are readily detected in wild-type HCMV virions, become undetectable in the absence of UL116 suggesting that the gH/UL116 complex is abundant in virions. We find evidence that UL116 and UL148 interact during infection indicating that the two proteins might cooperate to regulate the abundance of HCMV gH complexes. Altogether, these results are consistent with a role of UL116 as a chaperone for gH during the assembly and maturation of gH complexes in infected cells

    Smoke Evacuation During Laparoscopic Surgery: A Problem Beyond the COVID-19 Period. A Quantitative Analysis of CO2 Environmental Dispersion Using Different Devices

    Background. The COVID-19 pandemic leads to several debates regarding the possible risk for healthcare professionals during surgery. SAGES and EAES raised the issue of the transmission of infection through the surgical smoke during laparoscopy. They recommended the use of smoke evacuation devices (SEDs) with CO2 filtering systems. The aim of the present study is to compare the efficacy of different SEDs evaluating the CO2 environmental dispersion in the operating theater. Methods. We prospectively evaluated the data of 4 group of patients on which we used different SEDs or standard trocars: AIRSEAL system (S1 group), a homemade device (S2 group), an AIRSEAL system + homemade device (S3 group), and with standard trocars and without SED (S4 group). Quantitative analysis of CO2 environmental dispersion was carried out associated to the following data in order to evaluate the pneumoperitoneum variations: a preset insufflation pressure, real intraoperative pneumoperitoneum pressure, operative time, total volume of insufflated CO2, and flow rate index. Results. 16 patients were prospectively enrolled. The [CO2] mean value was 711 ppm, 641 ppm, 593 ppm, and 761 ppm in S1, S2, S3, and S4 groups, respectively. The comparison between data of all groups showed statistically significant differences in the measured ambient CO2 concentration. Conclusion. All tested SEDs seem to be useful to reduce the CO2 environmental dispersion respect to the use of standard trocars. The association of AIRSEAL system and a homemade device seems to be the best solution combining an adequate smoke evacuation and a stable pneumoperitoneum during laparoscopic surgery

    Quality control for more reliable integration of deep learning-based image segmentation into medical workflows

    Machine learning algorithms underpin modern diagnostic-aiding software, whichhas proved valuable in clinical practice, particularly in radiology. However,inaccuracies, mainly due to the limited availability of clinical samples fortraining these algorithms, hamper their wider applicability, acceptance, andrecognition amongst clinicians. We present an analysis of state-of-the-artautomatic quality control (QC) approaches that can be implemented within thesealgorithms to estimate the certainty of their outputs. We validated the mostpromising approaches on a brain image segmentation task identifying whitematter hyperintensities (WMH) in magnetic resonance imaging data. WMH are acorrelate of small vessel disease common in mid-to-late adulthood and areparticularly challenging to segment due to their varied size, anddistributional patterns. Our results show that the aggregation of uncertaintyand Dice prediction were most effective in failure detection for this task.Both methods independently improved mean Dice from 0.82 to 0.84. Our workreveals how QC methods can help to detect failed segmentation cases andtherefore make automatic segmentation more reliable and suitable for clinicalpractice.<br

    Laparoscopic treatment of abdominal unicentric castleman's disease: a case report and literature review.

    BACKGROUND: Castleman's disease is a rare lymphoproliferative disorder of unknown etiology that most commonly presents as a mediastinal nodal mass. It is exceptionally uncommon for Castleman's disease to present in the mesentery and, only 53 cases have ever been described in the literature. Standard treatment for this lymphoproliferative disorder involving a single node is a complete "en bloc" surgical resection which has proven to be a curative approach in almost all cases without recurrence after 20 years of follow up. All 53 reported cases of mesenteric Castleman's disease, except one, were treated with laparotomy. CASE PRESENTATION: We report on a case of mesenteric Castleman's disease localized in the mesentery which is the second reported case if its kind and was treated by a laparoscopic-assisted procedure. Our female patient had an uneventful postoperative course and was discharged in the 5th post-operative day. No signs of recurrence were present as evidenced by physical examination and total body CT scan 24 months after the operation. We compare our case with the other reported cases in which Castleman's disease presented as an isolated mass in the abdomen. CONCLUSION: Although a rare disease, Unicentric Castleman's disease should always be considered when a solid asymptomatic abdominal mass is occasionally presented. The laparoscopic approach (LA) allows for the achievement of better results than open surgery, including a reduction in postoperative pain and length of hospital stay. In cases of masses of an uncertain nature, LA must be considered the last diagnostic tool and the first treatment one

    Archeologia preventiva a Pontelatone (CE): nuovi dati sulle dinamiche insediative in prossimità del fiume Volturno

    The research project, carried out as part of the preliminary planning for the environmental requalification of the Barignano area (Pontelatone, CE), consists in a multidisciplinary application model for the best practice of preventive archaeology. The project started thanks to a recent agreement between the Municipality of Pontelatone (CE) and the Institute for Technologies Applied to Cultural Heritage (ITABC) of the Italian CNR, encharged of drawing up an archaeological map of the territory of Barignano and the surrounding territory. Recent surveys increased the knowledge about the exploitation of the Pontelatone district since prehistoric times and defined a more articulated settlement model of the perifluvial part of the territory, providing it with a different economic position in the ancient world. Research data, confronted with geographically and historically comparable areas, propose a new territorial and economic development model for the areas located on the Volturno river, both in Roman and in earlier periods. Starting from archive research and published scientific literature, the survey was supported by remote sensing data and new software to map land markers - both historical and archaeological - and for their 3D representation. The dataset have been organized in different topics and informative layers on a GIS platform. The survey and the interpretation of remote sensing data provided new elements for the topography of the area. Traces that suggest a different environmental model for some regions of the Volturno plain have been identified. Furthermore, today’s research supported by targeted geophysical surveys represents an actual prospect for future research
