Chimeric antigen receptor T in the treatment of multiple myeloma – state of the art and future directions
In spite of the introduction of several new drugs in the last 10 years, multiple myeloma (MM) remains incurable. Thus, an adoptive cellular therapy using chimeric antigen receptor T (CART), a strategy to increase the frequency of tumor-directed and functionally active T cells targeting antigens present on the cancer cell, might change the treatment in MM as it did in lymphoma and ALL. There are several targets for CART therapy in MM on different levels of development, which are discussed in the manuscript. B-cell maturation antigen (BCMA) being tested in the studies of phase 1–2 is the most promising, but so far CART has not been approved in the cure of MM and remains an experimental approach. The hematological society is facing a new technology which with its potential ability to cure MM, in spite of its complexity, cost, and toxicity, will definitely and soon change the landscape of myeloma in Europe and world-wide
“Translational research” in multiple myeloma – Polish opportunity to accelerate?
Although multiple myeloma still remains an incurable disease, the introduction of new drugs like thalidomide, bortezomib and lenalidomide, and recently carfilzomib and pomalidomide (not in the EU) has significantly prolonged life of patients with multiple myeloma. One of the key factors affecting the dramatic improvement in the effectiveness of the therapy is the development of so-called translational research which is defined by intensive laboratory work, mainly based on molecular studies, focused on the extremely fast introduction of this knowledge in clinical practice. The use of modern methods of molecular biology by identifying new therapeutic targets in preclinical studies has allowed the introduction of new particles such as perifosine or elotuzumab for clinical trials, which will probably result in their rapid introduction into everyday clinical practice. “Translational research” is also used in the work on the optimization of treatment with substances already registered as exemplified by studies using GEP and proteomics. Consolidated and consistent actions of hematological centers in Poland, in cooperation with the Polish non-clinical research centers, as well as with foreign institutions, given the large clinical population of patients with multiple myeloma, it is a huge Polish opportunity to develop knowledge about the disease. This will give a chance to people for easier access to modern forms of therapy to ultimately make myeloma from “incurable” to “chronic” disease
Low-energy fractures — a new complication of type 2 diabetes?
Niska masa kostna była dotychczas tradycyjnie łączona z cukrzycą typu 1. Ostatnie badania potwierdziły, że chorzy na cukrzycę typu 2 doznają złamań niskoenergetycznych częściej niż osoby zdrowe, tymczasem gęstość masy kostnej jest u nich podwyższona lub taka sama jak u osób zdrowych. Wydaje się, że ocena gęstości masy kostnej za pomocą badania densytometrycznego nie oddaje złożoności patologii tkanki kostnej u chorych na cukrzycę, co współcześnie jest przedmiotem wielu badań. Wzrost ryzyka złamania uwarunkowany jest prawdopodobnie m.in. długością trwania choroby, jej wyrównaniem metabolicznym, obecnością powikłań, rodzajem stosowanego leczenia, zwiększoną podatnością na upadki. Upośledzona jest prawdopodobnie tzw. jakość kości, co jest efektem nagromadzenia w kości zaawansowanych produktów glikacji, nieprawidłowego usieciowania kolagenu oraz niskiego tempa obrotu kostnego. Wykazano także, że u chorych na cukrzycę parametry strukturalne i geometryczne kości są gorsze. W świetle wzrastającego zagrożenia złamaniami wśród kobiet po menopauzie wskazana jest kalkulacja ryzyka złamania także wśród starszych chorych z cukrzycą typu 2.Low bone mineral density has traditionally been linked with type 1 diabetes. Latest research has confirmed that type 2 diabetics have increased fracture risk, whereas their bone mineral density is normal or increased when compared to healthy peers. It seems the assessment of bone mineral density using DXA (dual X-ray absorptiometry) does not reflect the complexity of bone pathology in diabetes, which has recently been the subject of debate. Increase in fracture risk is probably related to i.a. disease duration, metabolic disease, presence of complications, type of therapy, the increased susceptibility to falls. The underlying mechanism whereby diabetes type 2 exerts its detrimental effect on bone health is still not clear and often described as poor bone quality. This might be due to the accumulation of advanced glycation end products in bone, abnormal collagen cross-linking, accompanied by low rate of bone turnover. It was also shown that patients with diabetes have impaired structural and geometrical bone parameters. In light of increasing fracture incidence in postmenopausal women, fracture risk calculation is indicated also among older patients with type 2 diabetes

Ostatnia dekada przyniosła wzrastające obciążenie społeczno-ekonomiczne dwoma palącymi problemami: ogromnym wzrostem występowania
otyłości na całym świecie oraz - w związku ze starzeniem się społeczeństwa - dużym odsetkiem ludzi dotkniętych osteoporozą.
Otyłość trzewna, wyrażona zwiększonym obwodem talii, jest w świetle aktualnych wytycznych zasadniczą komponentą zespołu
metabolicznego (MS, metabolic syndrome). Dotychczas nie łączono występowania obu jednostek chorobowych, gdyż w badaniach udowadniano
ochronny wpływ dużej masy ciała na gęstość mineralną kości (BMD, bone mineral density). Wiadomo jednak, że tkanka tłuszczowa
jest aktywną endokrynnie tkanką wydzielającą wiele czynnych biologicznie związków mających wpływ także na kość, a adipocyt
i osteoblast wywodzą się ze wspólnej komórki prekursorowej. Coraz więcej dowodów przemawia za wspólną determinantą genetyczną
obu chorób. Pojawiają się także dane pochodzące z badań eksperymentalnych oraz epidemiologicznych, mówiące, że sama otyłość,
a także poszczególne elementy zespołu metabolicznego mogą mieć negatywny wpływ na kościotworzenie. Udowodniono na przykład, że
u chorych z tym rozpoznaniem zarówno gęstość mineralna kości, jak i ryzyko złamania były zwiększone. Zjawisko to obserwowano także
w przypadku wysokich wartości ciśnienia tętniczego. Do tej pory nie wyjaśniono przekonująco sprzeczności między zwiększoną gęstością
mineralną kości a dużym ryzykiem złamania niskoenergetycznego u chorych z cukrzycą typu 2. W pracy omówiono doniesienia i kontrowersje
związane ze współistnieniem i wzajemnymi interakcjami między elementami zespołu metabolicznego a osteoporozą i jej powikłaniami.In recent years an increasing socioeconomic burden of two pressing problems has been observed: enormous prevalence of obesity and
- along with aging of populations - great number of people affected with osteoporosis. Visceral obesity, expressed by increased waist
circumference, is according to the latest guidelines a crucial component of metabolic syndrome (MS). So far the two entities have not been
linked, as studies have proven beneficial effect of increased body weight on bone mineral density (BMD). It has been shown however, that
adipose tissue is actually an endocrine tissue, secreting numerous biologically active substances that influence also bone. Moreover, an
adipocyte and osteoblast derive from a common precursor cell. There is also increasing number of evidence showing common genetic
determination of the diseases. Data from experimental and epidemiological studies proves that obesity itself as well as remaining components
of MS might have negative influence on bone. BMD as well as fracture risk have been shown to be raised in patients diagnosed with
MS. The same has been observed for high blood pressure. What’s more, contradiction between high BMD and - simultaneously - high
fracture risk in subjects with type 2 diabetes has not been convincingly explained. The paper discusses reports and controversies on

Outpatient CAR-T therapy
Chimeric antigen receptor T (CAR-T) therapy has recently revolutionized the treatment of aggressive lymphomas and acute lymphoblastic leukemia, and will soon do the same for myeloma and other hematological malignancies. Due to the risk of potentially life-threatening complications such as cytokine release syndrome (CRS) and immune effector cell associated neurological syndrome (ICANS), patients have been hospitalized for the time when those symptoms may have occurred. However, due to improved prognostic factors, diagnostics and treatment of CRS and ICANS, it is possible that in the near future certain groups of patients will be treated with CAR as outpatients. That would allow broader access to CAR therapy, lowering overall costs and improving patient quality of life. Patient selection for outpatient CAR treatment is a topic that has been extensively discussed but, even based on the experience we already have, can already be effectively performed. CAR as an outpatient could be particularly useful for younger patients with a low tumor burden who have an educated caregiver and whose CAR center is logistically capable of providing outpatient care
Stimulating preventive procedures in primary care : effect of PIUPOZ program on the delivery of preventive procedures
Introduction: Educational meetings are one of the most frequently used strategies to change doctors' professional behavior; however, their effectiveness as a single intervention is limited. This study evaluated the effect of a multifactorial intervention, based on interactive workshops, on the GPs' knowledge and the delivery rates of preventive procedures in primary care.
Material and methods: The study population comprised 106 GPs working in the Wielkopolska region recruited to the PIUPOZ program (Improving Quality in Primary Care). The intervention in the program consisted of lectures, interactive workshops and an audit, before and three months after the training. Trained medical students directly observed GPs to register which of 12 studied preventive procedures were performed during the consultation in patients aged 40+.
Results: A total of 1060 consultations were recorded, during which 4899 preventive procedures were delivered: 2115 before and 2784 after workshops. The mean number of preventive procedures per patient before and after workshops was 3.84 and 5.25 respectively (p < 0.0001). The most commonly performed preventive procedures were blood pressure, blood glucose and lipid profile measurement. Mean number of correct answers for 16 questions in the initial knowledge test was 8.7 and 12.7 in the final test (p < 0.0001).
Conclusions: The observed number of delivered preventive procedures was below the recommended range. Preventive procedures based on laboratory tests were performed more often than lifestyle counseling
A discussion of the intervention thresholds in osteoporosis treatment in Poland
Introduction: Epidemiological prognoses regarding the global spread of post-menopausal osteoporosis can prove somewhat nebulous.
But it is clear that low-energy fractures and their consequences will become an increasingly serious health problem. Therefore it is crucial
to implement prognostic procedures which could more effectively predict the incidence of osteoporosis and its complications.
Material and methods: The study involved 378 female patients aged 40–86 years for whom clinical risk factors of osteoporotic fracture
were analysed. Densitometry (DPX) was performed at femoral neck. The 10–year risk of fracture was assessed according to the British
model of FRAX calculator.
Results: The study group was divided into two, depending on the history of low-energy fractures. Previous osteoporotic fractures were
confirmed in 128 patients. In this group, the mean bone mineral density (BMD) values (0.717 g/cm2) were lower than in the group without
fracture history (0.735 g/cm2). In 33.3% of patients aged 50-59 years and 17% of women aged 60-79 who required medical treatment for
their clinical status (previous fracture), the FRAX value did not meet the criterion of pharmacotherapy administration. Considering BMD
in the calculation of FRAX produced an even higher underestimation of the fracture risk. Of women aged 40-49, 25% were qualified for
pharmacotherapy of osteoporosis. In that particular age category, BMD did not affect the FRAX value. BMD measurement had a higher
discriminatory value among patients aged 50-79, increasing the number of patients requiring therapy by more than 50%.
Conclusions:
1. The FRAX calculator does not always consider the history of low-energy fractures as a criterion sufficient for therapy implementation.
2. Designing a FRAX calculator specifically for the Polish population would be advisable.

z tym konsekwencje złamań niskoenergetycznych będą stanowiły coraz większy problem zdrowotny społeczeństw. Konieczne staje się
zatem wdrożenie postępowań, które będą skuteczniej przewidywały występowanie osteoporozy pomenopauzalnej i powikłań choroby.
Materiał i metody: Występowanie klinicznych czynników złamania oceniano w grupie 378 pacjentek w wieku 40-86 lat. Dodatkowo
u wszystkich kobiet przeprowadzono badanie densytometryczne bliższego końca kości udowej. W celu obliczenia 10-letniego ryzyka
złamania posłużono się modelem brytyjskim kalkulatora FRAX.
Wyniki: W niniejszej pracy grupę badaną podzielono na dwie części w zależności od wywiadu dotyczącego wystąpienia niskoenergetycznego
złamania. U 128 pacjentek stwierdzono w przeszłości złamanie osteoporotyczne. W tej grupie średnie wartości gęstości mineralnej
kości (BMD, bone mineral density) (0,717 g/cm2> ) były niższe niż w grupie bez złamań (0,735 g/cm2>).
W grupie między 50.-59. rokiem życia, która ze względu na stan kliniczny (wcześniejsze złamanie) wymagała leczenia, wartość FRAX
w 33,3%, a między 60.-79. rokiem życia w 17%, nie spełniała kryterium włączenia farmakoterapii. Uwzględniając FRAX BMD w grupie
między 60.-79. rokiem życia, stwierdzono jeszcze większe niedoszacowanie ryzyka złamań, sięgające 25%.
Wśród kobiet między 40.-49. rokiem życia, bez złamań, 25% kwalifikowała się do farmakoterapii osteoporozy pomenopauzalnej. W tej kategorii
wiekowej dodanie informacji BMD nie wpływało na wysokość FRAX-10. Badanie BMD miało większą wartość dyskryminacyjną wśród chorych
między 50.-79. rokiem życia, zwiększając liczbę włączonych według wskazań do terapii farmakologicznej nawet o ponad 50%.
Wnioski:
1. Kalkulator FRAX nie zawsze uwzględnia wcześniejsze złamanie niskoenergetyczne jako wystarczające kryterium do wdrożenia terapii.
2. Wskazane jest opracowanie kalkulatora FRAX dla polskiej populacji.

Role of transplantation in treatment of multiple myeloma in era of novel agents
Multiple myeloma (MM) is a B-cell malignancy characterized by clonal proliferation of plasma cells. Despite the introduction of novel agents such as immunomodulatory drugs, proteasome inhibitors, and monoclonal antibodies, high-dose chemotherapy with autologous transplantation remains the primary treatment for patients with newly diagnosed multiple myeloma. This review presents the results of clinical trials assessing the effectiveness and safety of various kinds of transplantation such as single, allogeneic, tandem and salvage. Nowadays, in the era of access to new therapies, the following questions should be asked: when is the best time to perform autologous transplantation? What is the significance of allogeneic or tandem transplantation? Is the use of a second or third salvage transplant justified? Will chimeric antigen receptor T-cell (CAR-T) therapy become a valuable therapeutic method in MM? In this article, we will try to answer these questions
Pregnancy, lactation and bone mineral density
Abstract Evidence shows that pregnancy and lactation lead to the decrease of bone mineral density (BMD), which is related to mobilization of skeletal calcium. Numeorous mechanisms are involved in maternal adaptation to the increased demand for calcium. BMD has been proved to return to the baseline level after weaning. Pregnancies and long or repeated periods of feeding do not seem to determine the diagnosis of osteoporosis in later life. In fact, women with multiple pregnancies have been shown to have the same or higher BMD and lower fracture risk compared with nulliparous women. Pregnancy and lactation-associated osteoporosis (PLO) is a rare disease entity. It manifests itself as low back pain, and often leads to a number of vertebral compression fractures and, therefore, to potentially serious health consequences
Evaluation of plasminogen activator inhibitor 1 concentration in patients with simple obesity
WSTĘP. Związek otyłości z rozwojem miażdżycy i jej klinicznymi
konsekwencjami jest dobrze udokumentowany. Wśród analizowanych
czynników, uczestniczących w progresji zmian miażdżycowych,
znajdują się zarówno „stare” — klasyczne czynniki ryzyka,
jak i nowe, których miejsce i znaczenie kliniczne jest wciąż przedmiotem
licznych badań. W patogenezie zwiększonej aktywności
prozakrzepowej obserwowanej u otyłych osób analizuje się potencjalny
udział nadprodukcji inhibitora aktywatora plazminogenu
typu 1 (PAI-1).
Celem badania była ocena stężenia PAI-1 u pacjentów z otyłością
prostą oraz poszukiwanie jego związku z wybranymi parametrami
antropometrycznymi i biochemicznymi.
MATERIAŁ I METODY. Do badania włączono 88 pacjentów z otyłością
prostą oraz 25 zdrowych ochotników jako grupę kontrolną.
U wszystkich wykonano pomiary antropometryczne, oceniono stężenia:
glukozy na czczo, parametrów gospodarki lipidowej, insuliny
(metodą radioimmunometryczną), czynnika martwicy nowotworów
alfa (TNFa) oraz PAI-1 (metodą immunoenzymatyczną).
Stopień insulinooporności wyliczono na podstawie wskaźnika
HOMA-IR.
WYNIKI. Stężenia TNFa, PAI-1 oraz wartości wskaźnika HOMA-IR
były znamiennie większe w grupie pacjentów z otyłością niż w grupie
kontrolnej. Stwierdzono dodatnią korelację między stężeniami
insuliny oraz wartościami HOMA-IR a PAI-1.
WNIOSKI.
1. Otyłych pacjentów charakteryzują zwiększona aktywacja procesu
zapalnego, predyspozycja prozakrzepowa oraz insulinooporność.
2. W patogenezie zwiększonej aktywności prozakrzepowej osocza
u otyłych pacjentów, wyrażonej w badaniu autorów zwiększonymi
stężeniami PAI-1, należy rozważyć zjawisko insulinooporności.INTRODUCTION. Relationship between obesity and atherosclerosis
development is well-documented. Among risk factors of atherosclerosis
progression are both „old” and „new” — which clinical
value is still intensively studied. In the pathogenesis of increased
procoagulat state observed in obese patients potential role of plasminogen
activator inhibitor 1 (PAI-1) is analysed.
The aim of the study was to evaluate PAI-1 concentration and its
relationship with selected anthropometric and biochemical parameters.
MATERIAL AND METHODS. 88 obese patients and 25 healthy subjects
as a control were studied. Anthropometric parameters, concentrations
of: fasting glucose, lipids, insulin with use of radioimmunomethric method, tumor necrosis factor alpha (TNFa) and PAI-1
with use of immunoenzymatic method were assessed. Insulin resistance
in the participants was evaluated according to the homeostasis
model assessment–insulin resistance (HOMA-IR) protocol.
RESULTS. Serum concentrations TNFa, PAI-1 and HOMA-IR index
were significantly higher in the studied group as compared to
the control. Positive correlation between insulin concentration,
HOMA-IR and PAI-1 were found.
CONCLUSIONS.
1. Obese patients characterise increased activation of inflammatory
process, procoagulant predisposition and insulin resistance.
2. In the pathogenesis of increased procoagulant activity in obese
patients, observed in our study as elevated PAI-1 concentration,
insulin resistance role should be considered
