Wstęp Przestrzeganie zasad prawidłowego pomiaru ciśnienia tętniczego jest warunkiem uzyskania wiarygodnych wyników samodzielnych pomiarów domowych ciśnienia tętniczego (SBPM, self-blood pressure monitoring). Celem pracy była ocena znajomości i przestrzegania powyższych zasad przez chorych na nadciśnienie tętnicze stosujących SBPM. Materiał i metody Grupę losowo dobranych 201 chorych na nadciśnienie tętnicze (101 — pacjenci ZPOZ, 100 — pacjenci Poradni Nadciśnienia Tętniczego przy Klinice Kardiologii, średni wiek: 57,80 ± 12,51 lat, 53,7% kobiet), którzy regularnie stosują SBPM, poddano ankiecie utworzonej na podstawie zaleceń European Society of Hypertension (2005,2003) dotyczących pomiarów ciśnienia krwi. Wyniki Aż 20,9% respondentów używa aparatów nadgarstkowych. Spośród pacjentów użytkujących aparaty z mankietem na ramię (79,1% ogółu) tylko 45,3% sprawdziło, czy rozmiar mankietu ich ciśnieniomierza jest dla nich odpowiedni. W ciągu godziny przed pomiarem 26,3% badanych zdarza się spożywać obfity posiłek, 22,9% — wykonywać ciężką/męczącą pracę fizyczną, 18,9% — pić kawę, 3,5% — pić alkohol. Pięciominutowego odpoczynku przed pomiarem nie stosuje 20,4% ankietowanych. Co 3 chory mierzy ciśnienie wkrótce po zażyciu leków hipotensyjnych. Niewielu badanych uwzględnia czynniki podnoszące ciśnienie krwi, takie jak wypełnienie pęcherza moczowego, gorączka, zbyt wysoka lub zbyt niska temperatura otoczenia (11,4%). Pacjenci poradni przyklinicznej oraz osoby lepiej wykształcone mają nieco większą wiedzę na temat czynników zwiększających ciśnienie tętnicze. Długość terapii nadciśnienia tętniczego pozostawała bez wpływu na poziom wiedzy dotyczącej prawidłowych zasad prowadzenia pomiarów ciśnienia krwi. Wnioski Wybór sfigmomanometru w SBPM jest często niewłaściwy, a dobór odpowiedniego rozmiaru mankietu zaniedbywany. Wiedza pacjentów odnośnie do warunków prawidłowego pomiaru ciśnienia krwi jest niekompletna.Background An appropriate blood pressure (BP) measurement technique is crucial in obtaining reliable results in Self-Blood Pressure Monitoring (SBPM). The aim of the study was to assess to what extent hypertensive patients who perform SBPM know and follow the rules of the correct blood pressure measurement. Material and methods A random group of 201 hypertensive individuals (100 primary care patients and 101 high reference ambulatory patients, aged 57.80 ± 12.51 years, 53.7% female) who regularly perform SBPM have undergone an inquiry study based on the European Society of Hypertension 2005 and 2003 recommendations for blood pressure (BP) measurement. Results As many as 20.9% respondents use wrist devices. Among patients using sphygmomanometers with arm cuff system (79.1% of all) only 45.3% check if the cuff size matches their arm circumference. In the hour preceding BP measurement 26.3% of the studied individuals happen to eat an abundant meal, 22.9% perform strenuous exercise, 18.9% drink coffee, 3.5% consume alcohol. Fiveminute resting before taking BP is not practiced by 20.4% of the enquired. One in three patients takes their BP soon after antihypertensive drug intake. Few responders identify bladder distension, fever, too high or too low temperature of the surrounding (11.4%–22.9%) with the impact on the blood pressure. High reference patients and higher educated ones are slightly better informed on the factors increasing BP. Hypertension therapy duration did not influence the knowledge of the rules of correctly performed SBPM. Conclusions Sphygmomanometer selection for SBPM is often inappropriate and correct cuff size matching neglected. Patients’ knowledge about the rules of proper BP measurement is incomplete

Gender does not influence event-free survival in patients with ischaemic heart disease undergoing non-emergency coronary angiography : a single centre analysis
Background: Although gender-related differences in cardiovascular risk in patients with acute coronary syndromes have been investigated several times in Poland, there are few data on the effects of gender on management of patients with stable ischaemic heart disease (IHD). In addition, available data on the prognosis in this condition in men and women are also scarce. Aim: To assess gender-related differences in treatment and event-free survival in patients with IHD undergoing non-emergency coronary angiography. Methods: 960 consecutive patients with IHD undergoing coronary angiography were included. Study end points were ascertained over 4.5-year follow-up in 925 patients, of whom 187 were women and 738 were men. The primary end-point consisted of cardiovascular death, myocardial infarction, stroke, cardiac arrest, PCI, CABG or heart transplantation. Results: Female gender was independently related to higher prescription rate of beta-blockers [odds ratio 1.89 (95% CI 1.08-3.29)], ACE inhibitors [1.47 (1.01-2.16)] and calcium antagonists [1.65 (1.08-2.53)] at the time of coronary angiography. On the other hand, female gender was not independently related to the probability of PCI [1.01 (0.69-1.49)] or CABG [0.91 (0.57-1.48)]. We did not find any gender-related difference in the risk of the primary end point [hazard ratio 0.94 (95% CI 0.67-1.34)] or the three predefined secondary end points. Conclusions: Female gender was independently related to a higher prescription rate of beta-blockers, ACE inhibitors and calcium antagonists. No gender-related revascularisation was found in respect of myocardial revascularisation. Gender was not an independent factor affecting event-free survival in patients with IHD undergoing non-emergency coronary angiographyWstęp: Wyniki badań oceniających wpływ płci na rokowanie u pacjentów z ostrymi zespołami wieńcowymi są sprzeczne. Natomiast wyniki badań oceniających wpływ płci na rokowanie u pacjentów ze stabilną chorobą niedokrwienną serca (IHD) są nieliczne i również rozbieżne. W Polsce nie analizowano dotąd wpływu płci na rokowanie w tej grupie chorych. Także wyniki badań dotyczących relacji między płcią a częstością stosowania leków kardiologicznych oraz częstością rewaskularyzacji mięśnia sercowego nie są zgodne. Cel: Ocena wpływu płci na częstość stosowania leków kardiologicznych, częstość wykonywania zabiegów angioplastyki wieńcowej (PCI) oraz operacji pomostowania aortalno-wieńcowego (CABG), a także rokowanie u osób z IHD poddawanych koronarografii w trybie planowym. Metodyka:Do badania zakwalifikowano 960 kolejnych pacjentów z IHD poddawanych koronarografii. Chorobę niedokrwienną serca zdefiniowano jako co najmniej jedno zwężenie >50% w tętnicy wieńcowej lub zawał mięśnia sercowego lub rewaskularyzację w wywiadzie. Występowanie powikłań sercowo-naczyniowych oceniono u 925 osób (738 mężczyzn oraz 187 kobiet). Pierwszorzędowy punkt końcowy zdefiniowano jako zgon z przyczyn sercowo-naczyniowych, zawał serca, udar mózgu, zatrzymanie krążenia, PCI, CABG lub transplantację serca. In vivo and in vitro studies of efficient mephedrone adsorption over zirconium-based metal-organic frameworks corroborated by DFT+D modeling
Zirconium-based metal–organic frameworks as acriflavine cargos in the battle against coronaviruses : a theoretical and experimental approach
[Image: see text] In this study, we present a complementary approach for obtaining an effective drug, based on acriflavine (ACF) and zirconium-based metal–organic frameworks (MOFs), against SARS-CoV-2. The experimental results showed that acriflavine inhibits the interaction between viral receptor-binding domain (RBD) of spike protein and angiotensin converting enzyme-2 (ACE2) host receptor driving viral cell entry. The prepared ACF@MOF composites exhibited low (MOF-808 and UiO-66) and high (UiO-67 and NU-1000) ACF loadings. The drug release profiles from prepared composites showed different release kinetics depending on the local pore environment. The long-term ACF release with the effective antiviral ACF concentration was observed for all studied ACF@MOF composites. The density functional theory (DFT) calculations allowed us to determine that π–π stacking together with electrostatic interaction plays an important role in acriflavine adsorption and release from ACF@MOF composites. The molecular docking results have shown that acriflavine interacts with several possible binding sites within the RBD and binding site at the RBD/ACE2 interface. The cytotoxicity and ecotoxicity results have confirmed that the prepared ACF@MOF composites may be considered potentially safe for living organisms. The complementary experimental and theoretical results presented in this study have confirmed that the ACF@MOF composites may be considered a potential candidate for the COVID-19 treatment, which makes them good candidates for clinical trials
