Minimal-invasive endovaskulΓ€re Therapie zerebraler Aneurysmen mittels Stents und selektiv ablΓΆsbarer Spiralen : eine tierexperimentelle Studie am Kaninchenmodell
The aim of this study was to use an animal model to investigate the different available therapies of intracranial aneurysms. We compared the established therapy with Guglielmi Detachable Coils and the combination with stent placement across the neck of the aneurysm and coiling though the stent meshes and the therapy with uncovered stents. We created aneurysms in the right common carotid artery in 30 rabbits by using elastase and an endovascular technique. A few days after the intervention an aneurysm was formed at the bifurcation of the right common carotid artery and the brachiocephalic trunk. Two weeks later 10 animals were treated with the current gold standard by using Guglielmi Detachable Coils, ten animals were treated with a combination of uncovered stents and coiling through the meshes and 10 animals were treated only with uncovered stents. Five animals of each group were observed for one month, the other animals were observed for three month. After this time we investigated the occlusion aneurysms by using a DSA for the final angiography. Therefore the animals were euthanasized and the brachiocephalic trunc, the right subclavian artery and the aneurysm were resected and send for histological analysis.The most important results:1. The therapy with coils remains the gold standard. Problems will be appearing in aneurysms with a high dome-neck-ratio.2. Comparable results are presented in the combination of stenting with following coiling through the meshes of the stent. Fibrinolytical reorganisation in the neck and coil compaction will be a problem in giant aneurysms.3. A dense packing of the coils without dislocation in the parent vessel is possible in the combined therapy with stents and following coiling. But the risk of compaction is also present in this group. The risk of reperfusion especially in the region of the neck is furthermore present, especially in giant aneurysms.4. The complete occlusion of the aneurysm with uncovered stents is satisfying and not predictable.5. The delivery of the stent in the parent vessel could be difficult, concerning vessel curvature and the navigation through intracranial arteries. The flexibility of the stent is very important. Self-expanding stents are showing promising results.6. The risk of a stenosis after stent implantation is not completly influenced. A progressive vessel wall reaction or proliferation is in the intracranial vessel not tolerable.7. The formation of the thrombus is detectable after one month. The occlusion of the aneurysm is about 95% in most cases.8. After three months the occlusion is regressive because of fibrinolytical processes in the area of the neck and compaction. The occlusion rate is between 70 and 90%9. The risk of compaction of the coil increased in the course of time and especially in giant aneurysms.10. Is the dome-neck-ratio lower than 1.5, the risk of dislocation into the parent vessel and the risk of reperfusion is increasing.This study presents the risks of the endovascular therapy in intracranial aneurysms. Especially the successful therapy of aneurysms with wide necks or giant aneurysms are very difficult. At the barrier between the aneurysm and the blood flow, fibrinolytical processes causes a reperfusion of the aneurysms. The consequence is a dilatation and regrowth of the aneurysm. The clinical results, especially long-term-studies, for the therapy with stents and the combination stent plus coiling are inexistent. The therapy with Guglielmi Detachable Coils will be furthermore the gold standard for intracranial aneurysms
