Mechanisms of urethral continence and their clinical application
Controversy about the basic nature of urethral function does not preclude accurate clinical assessment of disorders of function. While the precise method of treatment of urethral continence dysfunction varies from institution to institution, the basic techniques are quite similar. It is the application of a treatment method to a particular case which causes difficulty. It is important, therefore, to have some understanding of the functional elements in the urethral continence mechanism to be able to determine which element does not function. Most cases of intractable incontinence are associated with poor function of the involuntary part of the sphincter. Bedeutung von mikroRNA-1 und -328 für atriales Remodeling bei Vorhofflimmern
Das Ziel dieser Arbeit ist es, mittels RP (rapid pacing) in vitro bzw. RAP (rapid atrial pacing) in vivo die Bedeutung der miRNAs-1 und -328 für Remodeling-Vorgänge bei Vorhofflimmern zu untersuchen. Von Interesse ist dabei einerseits die Bestimmung des zeitabhängigen Expressionsniveaus der miRNAs unter RP, während andererseits die Auswirkungen ihres funktionellen „knock-down“ auf das Protein-Expressionsmuster von Kardiomyozyten untersucht werden. Für die geplanten Analysen (RT-qPCR, 2-D-Gelelektrophorese) solletn primäre Schweine- und Mauskardiomyozyten sowie die murine HL-1-Kardiomyozytenlinie als Modelle dienen. Im Zentrum der Untersuchungen stehen dabei akut eintretende Veränderungen, da diese für den pathophysiologischen Übergang in die persistierende Rhythmusstörung relevant sind, aber aufgrund der möglichen reversiblen Natur interessante therapeutische Optionen eröffnen könnten. Aufbauend auf die bereits gewonnenen Erkenntnisse in der Literatur soll diese Arbeit einen Beitrag für das Verständnis von atrialen Remodeling-Prozessen leisten, die das Vorhofflimmern zu einer chronischen Krankheit mit erheblichen Komplikationen machen.With the help of different rapid pacing models we wanted to assess in vivo and in vitro changes of miRNA-1 and miRNA-328 expression levels and their meaning for atrial remodeling during atrial fibrillation. Next to thesechanges we investigated with proteome analysis potential targets to evaluate their capability of influencing structural and electrical remodeling. We used primary murine and porcine cardiomyocytes as well as an atrial tumor-cell-line (HL1-cells) and one in vivo approach in pigs. Clinical results after reconstruction of severe acetabular bone defects using the pedestal cup
GesamtdissertationDie Hypothese dieser Arbeit war, dass die Implantation einer Sockelpfanne bei
Pfannendefekten PAPROSKY Typ 2B - 3B ähnlich gute klinische Ergebnisse wie der
Literaturvergleich mit alternativen Implantaten bietet. In der überwiegenden
Zahl der Fälle konnte die vorbestehende Beinlängendifferenz auf die Hälfte
reduziert oder ausgeglichen werden. Sowohl der HARRIS-Hip-Score als auch der
MERLE D´AUBIGNE Score verbesserten sich durch die Operation signifikant in der
Größenordnung des Literaturvergleiches. Durch die Implantation der
Sockelpfanne konnte die Anzahl Trendelenburg positiver Patienten deutlich von
28 auf 23 gesenkt werden. Diese für Revisionsoperationen zufriedenstellenden
Ergebnisse wurden in der subjektiven Bewertung durch die Patienten bestätigt.
Die durchschnittliche Operationsdauer mit durchschnittlich 129 Minuten war
deutlich geringer als im Literaturvergleich. Auch der Blutverlust mit
durchschnittlich 1450 ml lag unterhalb der in der Literatur zum Vergleich
angegebenen Werte. Im Rahmen der operationstechnischen Lernkurve traten 2
implantatspezifische Komplikationen auf. Bei 9 Implantaten kam es früh -
postoperativ zu Luxationsereignissen, davon konnten 8 Fälle geschlossen
reponiert werden. In einem Fall musste offen reponiert werden. In 3 Fällen kam
es wiederholt zu Luxationsereignissen. Alle luxierten Implantate hatten einen
Kopf der Größe 28 mm. In 8 Fällen wurde primär auf ein überhöhtes Inlay
verzichtet. Es ist daher zu postulieren, dass durch die Verwendung großer
Köpfe und überhöhter Inlays gerade bei muskelschwachen Patienten die
Luxationsrate verringert werden kann. Zum Zeitpunkt der Nachuntersuchung waren
4 Implantate revidiert worden, davon 2 aufgrund einer septischen und 2
aufgrund einer aseptischen Implantatlockerung. Als prädiktiver Faktor für das
funktionelle Ergebnis zeigte sich weder der Body-mass-Index noch das
Geschlecht oder das Alter. Lediglich die Körpergröße korrelierte positiv mit
dem HARRIS-Hip-Score zum Zeitpunkt der Nachuntersuchung. Zusammenfassend
stellt die Sockelpfanne aufgrund der zu den etablierten Revisionsimplantaten
vergleichbaren klinischen Ergebnissen der jedoch rascheren und blutärmeren
Operationstechnik gerade für den alten und morbiden Patienten eine Alternative
dar.The hypothesis of this study was that the implantation of a pedestal cup in
patients with acetabular defects of PAPROSKY type 2B to 3B leads to similar
satisfactory clinical results as alternative implants in the conducted
literature review. The pre-existing leg length discrepancy has been reduced by
at least 50 % or fully adjusted in the predominant number of patients. The
HARRIS-HIP-SCORE as well as the MERLE-D AUBIGNE-SCORE have improved
significantly through the operation similar to the results of the literature
review of other implants. The number of patients with a positive TRENDELENBURG
test was reduced considerably from 28 to 23 through the implantation of the
pedestal cup. The subjective patient rating confirmed these - for revision
surgery satisfactory - results. The average duration of surgery was with 129
minutes considerably shorter than stated in the literature. The average Blood
loss of 1450 ml was lower than described in the literature as well. Two
implant associated complications occurred during the period of the technical
surgical learning curve. Nine patients suffered from early postoperative
luxation. Out of these, eight were repositioned through closed reduction, and
one had to be openly reduced. In three cases repeated luxations were seen. All
of these had a 28 mm diameter head. In eight patients constrained liners have
not been implanted primarily. Therefore, it can be postulated that the number
of luxations in patients with insufficient musculature may be reduced through
the use of large heads and constrained liners. At the time of follow-up four
implants had been revised, two out of these due to septic loosening, and two
due to aseptic loosening of the implant. Neither body mass index nor sex or
age had a predictive value on the functional out-come. Solely, the height of
the patient correlated positively with the HARRIS-HIP-SCORE at follow-up. It
can be concluded, that due to the comparable clinical results and the shorter
duration of the surgery with less blood loss the pedestal cup may be an
alternative implant to the established revision implants especially for old
and morbid patients
