    Management of Vatarakta w.s.r. to Gout through Panchakarma

    Vatarakta is more distressing and common metabolic disorder prevalent in present era. It is Vatapradhana Tridoshaja Vatavyadhi where Rakta is main Dushya. Vata is a predominant Dosha in Vatarakta, when it is excessively aggravated with vitiated Rakta is called Vatarakta. Vatarakta is correlated with gout in modern science. Gout is an abnormality of purine metabolism causes hyperuricemia and deposition of monosodium urate crystals in joints. Pain is predominant symptom of gout, which disturbs day-today life of the patients. The Panchakarma is not only a important component of Ayurvedic treatment but it is also forms the fundamental basis of Ayurveda therapy. The different procedures like Swedana, Vamana, Virechana, Basti, Sirovirechana, Raktamokshana focus on the purification which detoxifies the human body, i.e. the correction of the metabolism at molecular level.     THE DETERMINANTS OF REVERSE MORTGAGE CHOICE OF INDIAN ELDERLY HOMEOWNERS FOR SUSTAINABLE LIVELIHOOD: A LOGIT ANALYSIS

    Purpose: In the regime of stretched old-age social security, federals and policymakers are presuming housing wealth as a means of sustainable livelihood for elderly homeowners.  The current study attempts to discover which demographic and financial factors are significant determinants of home equity liquidation through reverse mortgage of Indians in later life. Methodology: Binary logistic regression is applied to survey-based primary data of 410 elderly homeowners through SPSS software. Main Findings: Results of binary logistic regression model depicts that elderly considering an RM likely to be female, older, having poor health, childless or having girl child only, long life expectancy, resident of metro, employed, cash-constrained, not having any kind of insurance cover and those children are financially well are significantly more willing to opt for RM scheme. Implication: Study renders implications for Government and NHB, to provide refinancing facility to commercial banks so that home equity liquidation product like Reverse Mortgage can be able to fulfill income needs of greying India. Novelty/Originality: Length of research in European and western countries have been carried out to explore the attitude of older homeowner for housing wealth liquidation but Indian context, is largely untapped that how Indian older homeowner perceive their housing wealth and which factor influences them to delete it.     Lifestyle disorder is an umbrella term given to all the diseases which arise because of unhealthy lifestyle. Lifestyle diseases, a subset of non-communicable diseases (NCDs) are a result of the way we live, work and go about our everyday lives. Developing countries like India is predicted to have most number of Lifestyle related patients in the next 10 years in the entire world. Health promotion and preventive health approaches are proven effective strategies in reducing disease burden with lower costs compared to the cost of NCD treatment. There is a definite need of an approach to understand the psycho-somatic impact on the human body and emerging lifestyle diseases. And the hope to treat Lifestyle diseases definitely brighten with the broader approach of Ayurveda. Ayurveda provides better solution in the forms of proper dietary management, lifestyle advises, Panchakarma like detoxification and bio-purification procedure and rejuvenation therapies. If the factors involved in pathogenesis of these lifestyle disorders are reviewed on the basis of Ayurveda, the symptoms shows Agnidushti, Kaphadushti, Mansavaha and Medovahadushti.     Nowadays Pilonidal Sinus (PNS) is becoming common disease in between 20-30 years of age, in men and mostly dense hairy persons. Commonly it occurs midline over the coccyx. It results in purulent discharge, pain and discomfort. In Ayurvedic practice, there are many surgeons who are practicing classical Ksharasutra management to treat PNS, which is very effective but there are some problems using classical Ksharasutra in the management of PNS, such as discomfort, burning sensation, itching and irritation due to Snuhi-ksheera. So to overcome these problems Guggulu Apamarga Ksharasutra has been used, which is very effective in curing PNS and has no complications.     Headache is one among the most common reasons patients seek medical attention, on a global basis being responsible for more disability than any other neurological problem. Migraine is the 2nd most common primary headache, also known as ‘Hemicrania’, means ‘half of the head’. It constitutes 16% of the primary headache and affects ~10-20% of general population, i.e. about 15% of women and 6% of men. It is characterized by recurrent episodes of headache that is moderate to severe and pulsatile in nature and lasting for 2-72 hours. Associated symptoms are nausea, vomiting, sensitivity to light, sound, or smell. It is not a lethal disorder but it can disturb patient’s life very widely, so its treatment requires more attention and care. Unfortunately, there is no preventive medication for migraine has been stabilized so far, only symptomatic medication are available, which medications have a lot of adverse effects and make dependence. In Ayurveda it can be correlated to Ardhavabhedaka. Very elaborative treatment has been given in Ayurvedic Samhitas; especially Panchakarma offers significant relief in this disease

    Management of complex fistula-in-ano by IFTAK technique - A Case Report

    Bhagandara is included in one among Ashtamahagada in Sushruta Samhita. Ksharasutra concept has been explained for the treatment of Nadivrana and Bhagandara. Ksharasutra is a medicated thread which helps in cutting as well as drainage of fistulous tract. Although, it is a gold standard technique but in spite of having many benefits it has some drawbacks like long duration of treatment, mild to moderate pain, discomfort after. However, IFTAK or BHU technique is designed to minimize the duration of treatment with minimal post operative scar mark. A 38 years male patient came to Shalya OPD with complaints of pus discharge from perianal region. After local examination, he was diagnosed as a case of Bhagandara (fistula-in-ano). The patient was treated with IFTAK technique and he was completely cured after 7 weeks with no complications.
