On Conditional Chisini Means and Risk Measures
Given a real valued functional T on the space of bounded random variables, we
investigate the problem of the existence of a conditional version of nonlinear
means. We follow a seminal idea by Chisini (1929), defining a mean as the
solution of a functional equation induced by T. We provide sufficient
conditions which guarantee the existence of a (unique) solution of a system of
infinitely many functional equations, which will provide the so called
Conditional Chisini mean. We apply our findings in characterizing the
scalarization of conditional Risk Measures, an essential tool originally
adopted by Detlefsen and Scandolo (2005) to deduce the robust dual
representation
Risk Sharing with Deep Neural Networks
We consider the problem of optimally sharing a financial position among
agents with potentially different reference risk measures. The problem is
equivalent to computing the infimal convolution of the risk metrics and finding
the so-called optimal allocations. We propose a neural network-based framework
to solve the problem and we prove the convergence of the approximated
inf-convolution, as well as the approximated optimal allocations, to the
corresponding theoretical values. We support our findings with several
numerical experiments

Stromal Gene Signatures as Source of Targets for Drug-Repositioning and Prognostic Biomarkers in Prostate Cancer
The interplay between cancer cells and adjacent stroma is fundamental for the development and progression of tumours. In fact, this crosstalk sustains carcinogenesis and promotes the acquisition of additional malignant features. In prostate cancer (PCa), cancer-associated fibroblasts (CAFs) have been shown to fuel tumour development and metastasis by mutually interacting with tumour cells. In this regard, defining stromal gene signatures relevant for cancer progression could be a successful strategy to identify novel therapeutic targets and prognostic biomarkers for PCa. Recently, we have performed a gene expression profiling analysis comparing CAFs and fibroblasts stimulated with IL-6 or TGF-β, to examine the molecular signatures involved in fibroblast activation. The gene set enriched analysis revealed that CAFs are characterized by positive enrichment of genes coding for calcium, sodium, and potassium cation channels and genes related to extracellular matrix (ECM) remodelling. This evidence suggests a crucial role of characteristic stromal-related pathways in fibroblast conversion to a phenotype able to sustain PCa progression. In addition, the definition of reactive stroma players that are crucial for cancer progression could be a strategy to identify novel therapeutic targets and biomarkers for the early detection of aggressive PCa.
To better understand the involvement of cation channels in CAFs activation and potentially revert such activated phenotype, in this work we treated CAFs with specific cation channel inhibitors, such as antiarrhythmics. Interestingly, we found that antiarrhythmics are able to interfere with crucial features of prostate CAFs, as indicated by the modulation of specific activation markers and reduction of contraction and migration capabilities. Moreover, co-culture experiments indicate that antiarrhythmics are able to impair CAF-induced proliferative spur and EMT in PCa cells, and ultimately induce tumour growth delay in vivo.
Given the ability of a reactive stroma to support cancer progression, the prognostic relevance of selected genes found up-regulated in activated fibroblasts was tested in relation to clinical-pathological characteristics and patient outcome.
Among crucial factors found up-regulated in prostate CAFs compared to normal fibroblasts we identified a set o ECM-remodelling protein, including periostin. Interestingly, the immunostaining analysis we performed in a series of prostatectomies from PCa patients confirmed that periostin is mainly expressed in the stroma components surrounding tumour foci, and that its staining correlates with tumour grade and cribriform gland morphology, which is indicative of adverse outcome. Most importantly, high levels of stromal periostin expression were associated with a significantly increased probability to experience biochemical recurrence after prostatectomy. In light of the positive correlation observed between periostin stromal staining and aggressive disease, we also investigated the prognostic potential of circulating periostin in patients with potentially indolent tumours followed in the context of an active surveillance protocol. However, results from the study indicated that baseline plasma levels of periostin displayed a moderate capability to identify patients with aggressive disease.
Overall these studies may open new opportunities for the translation of drugs already used in different fields into the targeting of tumour-associated fibroblasts with the aim to interrupt their tumour-supportive spur. Finally, the characterisation of relevant stromal gene signatures may improve diagnostic tools for an early identification of high-risk PCa
Entwicklung der transapikalen Transkatheter Aortenklappenimplantation am Universitätsklinikum Marburg unter besonderer Berücksichtigung von Zeit und Klappentyp
Die kalzifizierende Aortenklappenstenose ist eine chronisch progrediente Krankheit, welche die dritthäufigste kardiovaskuläre Krankheit nach der arteriellen Hypertonie und der koronaren Herzkrankheit darstellt.
Der herkömmliche operative Aortenklappenersatz war bis vor Kurzem noch Goldstandard zur Therapie von kalzifizierenden Aortenklappenstenosen, jedoch konnte dieser bei Patienten über 80 Jahren oder mit einer Ejektionsfraktion von unter 40 % aufgrund eines zu hohen Risikos oft nicht mehr durchgeführt werden.
Die hier beschriebene transapikale sowie auch die transfemorale kathetergesteuerte Variante des Aortenklappenersatzes sind die Therapie der Wahl bei Patienten mit einer hochgradigen Aortenklappenstenose mit mittlerem bis hohem peri- und postoperativen Risiko.
Zu den zentralen Patientencharakteristika zur Indikationsstellung eines kathetergestützten Aortenklappenersatzes zählen ein hohes Alter sowie multiple Komorbiditäten.
Zu den hier beschriebenen Klappensystemen gehören die Ballon-expandierenden Bioprothesen Edwards Sapien XT und Edwards Sapien 3.
Diese Arbeit befasst sich mit dem Vergleich postoperativer Komplikationen nach einer transapikalen TAVI-Prozedur bei insgesamt 183 Patienten im Verlauf von 3 verschiedenen Zeitperioden: 2009–2011, 2012–2014 und 2015–2016. Des Weiteren wird das Augenmerk auf 2 verschiedene Prothesentypen (Edwards Sapien XT und Edwards Sapien 3) und einen möglichen Zusammenhang zwischen dem Prothesentyp und postoperativen Komplikationen gelegt.
Die Patienten waren im Altersdurchschnitt 80,5 Jahre alt und zum größeren Teil weiblich.
Eine postoperative Dislokation der Prothese war hauptsächlich in den ersten Jahren nach Etablierung der transapikalen TAVI-Prozedur am Universitätsklinikum Marburg zu beobachten. Je länger sich die Prozedur in der Klinik etablierte, desto weniger Fälle einer Dislokation konnten festgestellt werden (Gruppe 1: 7,4 %; Gruppe 2: 2,4 %; Gruppe 3: 0 %).
Bei der Inzidenz einer postoperativen Blutung konnte über den Beobachtungszeitraum ein statistisch signifikanter Anstieg der Fälle festgestellt werden, wofür jedoch bisher keine Erklärung gefunden werden konnte (Gruppe 1: 0 %; Gruppe 2: 8,3 %; Gruppe 3: 13,6 %). Bezüglich des postoperativen Deliriums konnte ein statistisch insignifikanter Aufwärtstrend zwischen Gruppe 2 und Gruppe 3 festgestellt werden (Gruppe 2: 7,1 %; Gruppe 3: 18,2 %). Eine residuale Aortenklappeninsuffizienz bei Entlassung konnte im gesamten Patientenkollektiv bei 34,1 % festgestellt werden. Davon waren jedoch 31,8 % leichtgradig (Grad I oder niedriger) und nur 2,1 % mittel- bis hochgradig (Grad II oder höher). Vergleicht man die Zahlen im Verlauf, kann eine stark sinkende Inzidenz festgestellt werden (Gruppe 1: 35,5 %; Gruppe 2: 40,5 %; Gruppe 3: 4,5 %). Dieses kann durch den Übergang von der Edwards Sapien XT- auf die Edwards Sapien 3-Bioprothese nach der Zeitperiode von 2009 bis 2011 erklärt werden, welche eine signifikant niedrigere Inzidenz einer residualen Insuffizienz aufwies (Sapien XT: 38,3 %, Sapien 3: 0 %).
Die durchschnittliche Liegezeit von Patienten auf der Intensivstation lag bei 4,04 Tagen, mit 68,9 % konnte eine hohe Zahl von Patienten noch am Operationstag extubiert werden.
Die 30-Tages-Mortalität lag hier bei insgesamt 6,8 %. In den verschiedenen Zeitperioden lag sie in Gruppe 1 bei 10,5 %, in Gruppe 2 bei 2,4 % und in Gruppe 3 bei 4,5 %.
Anhand der hier gewonnenen Daten konnte eine vielversprechende Entwicklung der transapikalen TAVI-Prozedur am Universitätsklinikum Marburg festgestellt werden, somit gilt sie als geeignete alternative Therapieoption für Patienten mit einer hochgradigen Aortenklappenstenose
FORMULASI GRANUL MUKOADHESIF GLIBENKLAMID MENGGUNAKAN POLIMER KITOSAN DAN HIDROKSIPROPIL SELULOSA
Glibenklamid merupakan obat golongan sulfonil urea yang digunakan sebagai terapi diabetes melitus tipe II. Salah satu permasalahan dalam formulasi glibenklamid yaitu waktu paruh yang pendek, yakni 4-6 jam. Penelitian ini bertujuan untuk memformulasi granul mukoadhesif glibenklamid menggunakan polimer kitosan dan hidroksipropil selulosa (HPC), yang ditujukan untuk meminimalkan frekuensi pemberian dan memperlama waktu tinggal obat di usus, serta membandingkan daya bioadhesi antara polimer kitosan dengan HPC. Granul mukoadhesi glibenklamid dibuat dengan metoda granulasi basah dalam lima formulasi, yaitu: formula 1 (F1) dan formula 2 (F2) menggunakan polimer HPC berbagai konsentrasi, formula 3 (F3) dan formula 4 (F4) menggunakan polimer kitosan, dan formula 5 (F5) tanpa polimer sebagai pembanding. Granul yang dihasilkan dievaluasi meliputi penetapan kadar glibenklamid dalam granul, sudut angkat, bobot jenis nyata, bobot jenis mampat, uji kelembaban, distribusi ukuran partikel, profil disolusi, serta uji daya mukoadhesif dengan uji mukoadhesif in vitro dan uji wash off. Uji mukoadhesif in vitro menunjukkan bahwa polimer kitosan dan hidroksipropil selulosa memiliki daya mukoadhesif yang baik, peningkatan konsentrasi penggunaan polimer dapat meningkatkan daya mukoadhesif. Hasil uji mukoadhesif in vitro menunjukan (F2) menggunakan polimer HPC 30% memiliki daya mukoadhesif yang lebih baik yakni dengan pelekatan 100% dibandingkan dengan (F4) yang menggunakan polimer kitosan 30% hanya menunjukan pelekatan 96% di jaringan usus. Pada profil disolusi terlihat adanya penurunan laju disolusi oleh formula yang menggunakan polimer kitosan dan formula yang menggunakan HPC dibandingkan dengan formula tanpa polimer. Peningkatan kosentrasi juga berpengaruh terhadap laju disolusi dimana semakin tinggi konsentrasi polimer yang digunakan menyebabkan semakin rendahnya laju disolusi granul mukoadhesif glibenklamid
