The Study can also called as the library research because this study only based on to scunder material like material of libraries as the primary material, although in this study the writer reviewed the function of legislation of DPR RI before and after Amendments Undang-Undang Dasar Republic of Indonesian 1945 (UUD 1945) then again comphared it to get the summarizing.The formulation of this sudy is what the background of the function changes of legislation that regulated in changes of UUD 1945, and How about the different function of legislation before and after amendments UUD 1945, And what the weakness and strengths function of legislation before and after amendments UUD 1945.The result of this study showed that the background of changes the function of legislation that regulated in changes UUD 1945 are with stated out Provisionals Majelis Permusyawaratan Rakyat(MPR) is Provisionals MPR No VIII/MPR/1998 its removed Provisionals MPR Decree No IV/MPR/1983.     Accuracy is the closeness or the accuracy of the measurement results with the actual value. Accuracy can also mean a measure of the closeness results of the analysis of the actual invitation analyte levels. Accuracy is vital that must be fulfilled and owned by any measurement tool, use the measuring instrument that has high accuracy and good will produce accurate measurement data and other wise use measuring devices are not accurate, do not meet the standards of the results obtained will not be accurate. This study aims to determine the value of the mandrel roundness deviation, roundness accuracy Measuring Equipment (Roundness Tester Machine) and large deviations in the roundness measuring instrument (Roundness TesterMachine) (instrument error) and the operator or gauges (human error). From the results, the large stroundness deviation value for minimum circumscribed circle is 0,474 mm, maximum inscribed circle 0,417 mm, minimum zone circle 0,495 mm, least squares circle 0,207 mm, for the value of sphericity (roundness) for the smallest outer for minimum circumscribed circleis 0,003 mm, maximum inscribed circle 0,003 mm, minimum zone circle is 0,003 mm, least squares is 0,001 mm.     The dose makes the poison: A case report of acquired methemoglobinemia

    Background: Methemoglobinemia (MET) should be suspected in cases where cyanosis is not associated with signs and symptoms of lung and/or heart disease, or in a cyanotic child exhibiting discrepancies in the partial pressure of oxygen in the arterial blood, the blood oxygen saturation, and the clinical assessment. Case presentation: A 10-month-old girl was taken to the Pediatric Emergency Department for the acute, sudden development of significant peroral cyanosis associated with gray pigmentation of the skin. The problem was evidenced approximately one hour after she ingested a homemade puree of mixed vegetables, mainly composed of potatoes and chards that had been prepared three days before and had been kept in the refrigerator since then. Physical examination revealed that the child was very pale, conscious, and without respiratory distress. Oxygen saturation of hemoglobin in the arterial blood (SpO2) was 94%. Respiratory, cardiovascular, and abdominal evaluations did not reveal any signs of disease. A venous blood sample showed chocolate-colored blood with a pH of 7.404, a partial pressure of CO2 (pCO2) of 40.6 mmHg, a partial pressure of oxygen (pO2) of 21.3 mmHg, a bicarbonate level of 24 mmol/L, and an oxygen saturation (SO2%) of 47.7%. CO-oximetry carried out simultaneously identified a methemoglobin level of 22%. MET was suspected, and oxygen via nasal cannula at a rate of 4 L/min was given with only a slight increase in oxygen saturation (96%). Slow intravenous injection of methylene blue 1 mg/kg over a period of 5 minutes was initiated. The peripheral oxygen saturation (SpO2) gradually improved to 100% over the next 20 minutes. Forty minutes later, venous blood gas analysis showed a methemoglobin level of 0.9% with a complete resolution of cyanosis; supplemental oxygen via nasal cannula was therefore discontinued. During the next 36 hours, the patient remained hemodynamically stable with good oxygenation on room air. Conclusions: This case report shows that recognition of acquired MET in a child with sudden cyanosis onset requires a high index of suspicion. In daily activities, there is a need to pay particular attention when homemade vegetable soups for child alimentation are prepared. The consumption of vegetable soups must occur immediately after preparation. Storage in a refrigerator must last no more than 24 hours and if longer storage is needed, vegetable soups should be frozen

    First case of typhoid fever due to extensively drug-resistant Salmonella enterica serovar typhi in Italy

    Typhoid fever is a potentially severe and occasionally life-threatening bacteraemic illness caused by Salmonella enterica serovar Typhi (S. Typhi). In Pakistan, an outbreak of extensively drug-resistant (XDR) S. Typhi cases began in November 2016. We report on a five-year-old boy who contracted enteric fever while travelling in Pakistan and was diagnosed after returning to Italy in September 2019. Blood culture isolated Salmonella enterica serovar Typhi that was XDR to all first-line antibiotics, including ceftriaxone and fluoroquinolones. Empiric therapy was switched to meropenem, and the patient recovered completely. Whole-genome sequencing showed that this isolate was of haplotype H58. The XDR S. Typhi clone encoded a chromosomally located resistance region and harbored a plasmid encoding additional resistance elements, including the blaCTX-M-15 extended-spectrum β-lactamase and the qnrS fluoroquinolone resistance gene. This is the first case of typhoid fever due to XDR S. Typhi detected in Italy and one of the first paediatric cases reported outside Pakistan, highlighting the need to be vigilant for future cases. While new vaccines against typhoid are in development, clinicians should consider adapting their empiric approach for patients returning from regions at risk of XDR S. Typhi outbreak with typhoid symptoms

    Nocardiosis in a Kidney-Pancreas Transplant

    34-year-old man with chronic renal and pancreas failure in complicated diabetic disease received a kidney-pancreas transplantation. On the 32nd postoperative day, an acute kidney rejection occurred and resolved with OKT3 therapy. The patient also presented refractory urinary infection by E. Fecalis and M. Morganii, and a focal bronchopneumonia in the right-basal lobe resolved with elective chemotherapy. During the 50th post-operative day, an intense soft tissue inflammation localized in the first left metatarsal-phalangeal articulation occurred (Figure 1) followed by an abscess with a cutaneous fistula and extension to the almost totality of foot area. The radiological exam revealed a small osteo-lacunar image localized in the proximal phalanx head of the first finger foot. From the cultural examination of the purulent material, N. Asteroides was identified. An amoxicillin-based treatment was started and continued for three months, with the complete resolution of infection This case is reported for its rarity in our casuistry, and for its difficult differential diagnosis with other potentially serious infections

    Carboxymethyl guar gum nanoparticles for drug delivery applications: preparation and preliminary in-vitro investigations

    Abstract Carboxymethyl guar gum (CMGG) synthesised from commercially available polysaccharide was formulated into nanoparticles via ionic gelation using trisodium trimetaphosphate (STMP) as cross-linking agent. Characterisation using a range of analytical techniques (FTIR, NMR, GPC, TGA and DLS) confirmed the CMGG structure and revealed the effect of the CMGG and STMP concentration on the main characteristics of the obtained nanoformulations. The average nanoparticle diameter was found to be around 208 nm, as determined by dynamic light scattering (DLS) and confirmed by scanning electron microscopy (SEM) and nanoparticle tracking analysis (NTA). Experiments using simulated gastric and intestinal fluids evidenced significant pH-dependent drug release behaviour of the nanoformulations loaded with Rhodamine B (RhB) as a model drug (loading capacity in excess of 83%), as monitored by UV-Vis. While dose-dependent cytotoxicity was observed, the nanoformulations appeared completely non-toxic at concentrations below 0.3 mg/mL. Results obtained so far suggest that carboxymethylated guar gum nanoparticles formulated with STMP warrant further investigations as polysaccharide based biocompatible drug nanocarriers

    Psychological Treatment for Headache : A Pilot Study on the Efficacy of Joint Psychoeducational Group and Relaxation Training

    Headache is a severe and disabling chronic condition. Literature shows that psycho-educational interventions may be useful for several chronic pain disorders. We tested an integrated psycho-educational and relaxation intervention addressed to patients suffering from headache. The outcome indicators are a direct measure of headache (headache days per month) and two indirect measures (disability due to headache and medications used). At the end of the intervention patients also filled in a satisfaction questionnaire. Forty-seven subjects participated to the study. The treatment significantly reduced frequency of headache attacks (Wilcoxon p<0.01) and disability caused by headache (Wilcoxon p<0.001). There was also a sensible reduction in medication use. Almost all patients judged the intervention positively. An integrated approach may be very useful to ameliorate the burden of headache, to discontinue medication and to improve quality of lif
