Sozial- und Gesundheitsdaten sind als personenbezogene Daten aber auch datenschutzrechtlich einem engmaschigen Rechtsrahmen unterworfen. Ihre Nutzung in anderen Kontexten bedarf eines tiefgründigen Verständnisses der Regelungen auf europäischer und nationaler Ebene. Dieser Band bietet einen Überblick über den aktuellen Rechtsrahmen und eine wichtige Orientierung zur rechtssicheren Nutzung von Sozial- und Gesundheitsdaten. Das Werk ermöglicht es, Lösungen und Produkte zu entwickeln, die die Qualität der Versorgung verbessern. Im ersten Teil wird der sozialrechtliche Rahmen zur Nutzung von Sozialdaten für die Forschung dargelegt. Im zweiten Teil wird ein Überblick zum Umgang mit Forschungsdaten nach Anwendung der DSGVO und entsprechender nationaler Anpassungen des Rechtsrahmens gegeben

    "Much of the data collected in the statutory health insurance system is of great value for the further development and improvement of health care. Social and health data as personal data are also subject to a tight legal framework in terms of data protection law. Their use in other contexts requires a deep understanding of the regulations at European and national level. This volume provides an overview of the current legal framework and an important orientation for the legally secure use of social and health data. The plant makes it possible to develop solutions and products that improve the quality of care. The first part sets out the social law framework for the use of social data for research. The second part provides an overview of how research data is handled after the application of the GDPR and corresponding national adjustments to the legal framework.

Conventional weightometers are, however, too expensive to allow a widespread installation in these processes. This study investigates more economic means to measure the massflow on horizontal conveyor belts. A deterioration of accuracy resulting from an instrument using this technique is expected, but the advantages of being able to instrument entire plants is very attractive. The study uses a model a whereby the kinetic energy of the belt system is determined during a perturbation to the system. From this the massflow can be determined

    Color-coded summation images for the evaluation of blood flow in endovascular aortic dissection fenestration

    BACKGROUND: To analyze the benefit of color-coded summation images in the assessment of target lumen perfusion in patients with aortic dissection and malperfusion syndrome before and after fluoroscopy-guided aortic fenestration. METHODS: Between December 2011 and April 2020 25 patients with Stanford type A (n = 13) or type B dissection (n = 12) and malperfusion syndromes were treated with fluoroscopy-guided fenestration of the dissection flap using a re-entry catheter. The procedure was technically successful in 100% of the cases and included additional iliofemoral stent implantation in four patients. Intraprocedural systolic blood pressure measurements for gradient evaluation were performed in 19 cases. Post-processed color-coded DSA images were obtained from all DSA series before and following fenestration. Differences in time to peak (dTTP) values in the compromised aortic lumen and transluminal systolic blood pressure gradients were analyzed retrospectively. Correlation analysis between dTTP and changes in blood pressure gradients was performed. RESULTS: Mean TTP prior to dissection flap fenestration was 6.85 ± 1.35 s. After fenestration, mean TTP decreased significantly to 4.96 ± 0.94 s (p < 0.001). Available systolic blood pressure gradients between the true and the false lumen were reduced by a median of 4.0 mmHg following fenestration (p = 0.031), with significant reductions in Stanford type B dissections (p = 0.013) and minor reductions in type A dissections (p = 0.530). A moderate correlation with no statistical significance was found between dTTP and the difference in systolic blood pressure (r = 0.226; p = 0.351). CONCLUSIONS: Hemodynamic parameters obtained from color-coded DSA confirmed a significant reduction of TTP values in the aortic target lumen in terms of an improved perfusion in the compromised aortic region. Color-coded DSA might thus be a suitable complementary tool in the assessment of complex vascular patterns prevailing in aortic dissections, especially when blood pressure measurements are not conclusive or feasible

    Granulomatous dermatitis: a rare pitfall in lymphoma staging with [18F]FDG-PET/CT

    A 36-year-old male with anaplastic large T cell lymphoma (A; nodal manifestations in the left iliac and left inguinal region, arrow) who had received six cycles of chemotherapy (brentuximab vedotin, cyclophosphamide, doxorubicin, and prednisone) presented for restaging prior to autologous stem cell transplantation. A few days earlier, the patient had noticed multiple new tender subcutaneous nodules, and erythemato-squamous, polymorphous, partially atrophic plaques all over his body (B1). [18F]FDG-positron emission tomography/computed tomography (PET/CT) showed complete metabolic response of primary lymphoma manifestations but revealed intense tracer accumulation in the disseminated subcutaneous nodules (B, axial image B2). Besides cutaneous involvement by T cell lymphoma, differential diagnoses included cutaneous sarcoidosis and pityriasis rosea. Biopsy of a subcutaneous nodule revealed no evidence of malignancy but granulomatous inflammation (B3) most consistent with reactive granulomatous dermatitis. After initiation of prednisone therapy, all (sub-)cutaneous lesions quickly resolved, and the patient was eligible for stem cell transplantation. Follow-up [18F]FDG-PET/CT demonstrated only residual tracer uptake of some lesions and a sustained complete lymphoma response (C). Reactive granulomatous dermatitis is a very rare skin disease with only several hundred cases reported worldwide so far [1], most commonly associated with autoimmune disorders such as rheumatoid arthritis. Furthermore, it has been associated with hematologic malignancies, including — in approximately 3% of cases — (B cell) lymphoma [1,2,3]. To our knowledge, this is one of the very first cases of granulomatous dermatitis in anaplastic large T cell lymphoma [4], and the first visualization of granulomatous dermatitis by [18F]FDG-PET/CT mimicking cutaneous lymphoma manifestations

    Lung perfusion assessed by SPECT/CT after a minimum of three months anticoagulation therapy in patients with SARS-CoV-2-associated acute pulmonary embolism: a retrospective observational study

    Background Anticoagulant treatment is recommended for at least three months after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-related acute pulmonary embolism (PE), but the persistent pulmonary clot burden after that time is unknown. Methods Lung perfusion was assessed by ventilation-perfusion (V/Q) SPECT/CT in 20 consecutive patients with SARS-CoV-2-associated acute PE after a minimum of three months anticoagulation therapy in a retrospective observational study. Results Remaining perfusion defects after a median treatment period of six months were observed in only two patients. All patients (13 men, seven women, mean age 55.6 ± 14.5 years) were on non-vitamin K direct oral anticoagulants (DOACs). No recurrent venous thromboembolism or anticoagulant-related bleeding complications were observed. Among patients with partial clinical recovery, high-risk PE and persistent pulmonary infiltrates were significantly more frequent (p < 0.001, respectively). Interpretation Temporary DOAC treatment seems to be safe and efficacious for resolving pulmonary clot burden in SARS-CoV-2-associated acute PE. Partial clinical recovery is more likely caused by prolonged SARS-CoV-2-related parenchymal lung damage rather than by persistent pulmonary perfusion defects

    In-vivo somatostatin-receptor expression in small cell lung cancer as a prognostic image biomarker and therapeutic target

    Background: Given the dismal prognosis of small cell lung cancer (SCLC), novel therapeutic targets are urgently needed. We aimed to evaluate whether SSTR expression, as assessed by positron emission tomography (PET), can be applied as a prognostic image biomarker and determined subjects eligible for peptide receptor radionuclide therapy (PRRT). Methods: A total of 67 patients (26 females; age, 41–80 years) with advanced SCLC underwent SSTR-directed PET/computed tomography (somatostatin receptor imaging, SRI). SRI-avid tumor burden was quantified by maximum standardized uptake values (SUVmax) and tumor-to-liver ratios (T/L) of the most intense SCLC lesion. Scan findings were correlated with progression-free (PFS) and overall survival (OS). In addition, subjects eligible for SSTR-directed radioligand therapy were identified, and treatment outcome and toxicity profile were recorded. Results: On a patient basis, 36/67 (53.7%) subjects presented with mainly SSTR-positive SCLC lesions (>50% lesions positive); in 10/67 patients (14.9%), all lesions were positive. The median SUVmax was found to be 8.5, while the median T/L was 1.12. SRI-uptake was not associated with PFS or OS, respectively (SUVmax vs. PFS, ρ = 0.13 with p = 0.30 and vs. OS, ρ = 0.00 with p = 0.97; T/L vs. PFS, ρ = 0.07 with p = 0.58 and vs. OS, ρ = −0.05 with p = 0.70). PRRT was performed in 14 patients. One patient succumbed to treatment-independent infectious complications immediately after PRRT. In the remaining 13 subjects, disease control was achieved in 5/13 (38.5%) with a single patient achieving a partial response (stable disease in the remainder). In the sub-group of responding patients, PFS and OS were 357 days and 480 days, respectively. Conclusions: SSTR expression as detected by SRI is not predictive of outcome in patients with advanced SCLC. However, it might serve as a therapeutic target in selected patient
