    CT-based tumour response criteria compared after combined treatment for liver metastases of colorectal cancer

    open6noPurpose: The aim of this analysis is to compare different tumour response criteria (TRC) after chemotherapy combined with bevacizumab in liver metastases from colorectal cancer (mCRC) to ascertain the best early prognostic indicator of response. Methods and Materials: 103 target liver metastases from 65 mCRC patients treated with chemoterapy plus bevacizumab were examined at the Istituto Oncologico Veneto IOV-IRCSS (March 2008-January 2013). All patients had baseline CT and at least one follow-up scan. Tumour response was retrospectively analyzed by two radiologists using RECIST1.1, modified Choi, and Chun morphologic criteria. Tumour response, classified as good (complete or partial response) or poor (stable or progressive disease), was compared with progression-free survival (PFS) at first follow-up (t1) and time of best response. Interobserver agreement and concordance between TRC were measured. Results: At t1, 32.31% showed a good response according to RECIST1.1 (median PFS 11.1), 84.62% according to Choi (median PFS 10.8). These percentages rose to 49.23% (median PFS 12.1) and 87.69% (median PFS 10.8), respectively, at the time of best response. According to Chun, 67.69% showed a good response at the time of best response (median PFS 10.8). The Choi criteria detected a higher proportion of good responders at t1, showing a better correlation with PFS; all methods correlated with PFS at the time of best response.     Diversity of cervical microbiota in asymptomatic chlamydia trachomatis genital infection: a pilot study

    Chlamydia trachomatis genital infection continues to be an important public health problem worldwide due to its increasing incidence. C. trachomatis infection can lead to severe sequelae, such as pelvic inflammatory disease, obstructive infertility, and preterm birth. Recently, it has been suggested that the cervico-vaginal microbiota may be an important defense factor toward C. trachomatis infection as well as the development of chronic sequelae. Therefore, the investigation of microbial profiles associated to chlamydial infection is of the utmost importance. Here we present a pilot study aiming to characterize, through the metagenomic analysis of sequenced 16s rRNA gene amplicons, the cervical microbiota from reproductive age women positive to C. trachomatis infection. The main finding of our study showed a marked increase in bacterial diversity in asymptomatic C. trachomatis positive women as compared to healthy controls in terms of Shannon's diversity and Shannon's evenness (P = 0.031 and P = 0.026, respectively). More importantly, the cervical microbiota from C. trachomatis positive women and from healthy controls significantly separated into two clusters in the weighted UniFrac analysis (P = 0.0027), suggesting that differences between the two groups depended entirely on the relative abundance of bacterial taxa rather than on the types of bacterial taxa present. Furthermore, C. trachomatis positive women showed an overall decrease in Lactobacillus spp. and an increase in anaerobes. These findings are part of an ongoing larger epidemiological study that will evaluate the potential role of distinct bacterial communities of the cervical microbiota in C. trachomatis infection

    Cognitive Health of Nonagenarians in Southern Italy: A Descriptive Analysis from a Cross-Sectional, Home-Based Pilot Study of Exceptional Longevity (Cilento Initiative on Aging Outcomes Or CIAO).

    Background: Nonagenarians and centenarians (NCs) are an extremely fragile population, particularly in regard to their physical and cognitive function. The aim of this study was to define the neurocognitive profiles among 29 NCs and their 49 younger cohabitants aged 50-75 years from The Cilento Initiative on Aging Outcomes (CIAO) Pilot study in the South of Italy that had provided initial hypotheses regarding positive psychological traits related to exceptional longevity. Methods: During the home visits, lifestyle information with specific questionnaires, functional autonomy and the neuropsychological Mini Mental Scale Examination (MMSE), and the Alzheimer's Disease Assessment Scale-Cognitive (ADAS-Cog) scale were obtained by qualified study personnel. The total blood oxidative capacity was also determined by testing the reactive derivative of oxygen metabolites (d-ROM) and by the Biological Antioxidant Potential (BAP). In all individuals, the APOE genotype determination was also performed. Results: All the subjects in both groups showed high adherence to the Mediterranean Diet. None of the NCs had severe cognitive impairment, and a very low incidence of dementia was found. The data obtained on the Activities ed Instrumental Activities of Daily Living (ADL-IADL) scale showed that the majority of NCs (16/29) were autonomous in daily life activities. The comparative assessment of NCs and cohabitants showed no significant differences in the laboratory assessment of oxidative stress and APOE genotype. Conclusion: In the Cilento Region of Southern Italy, NCs seemed to have good cognitive status when compared to younger cohabitants aging 50-65 years without significant differences in oxidative stress markers or APOE genotype. These results might be related to optimal adherence to the Mediterranean diet, although other lifestyle factors and positive personality traits may also contribute to their healthy aging. Further studies on a larger population should be performed to confirm the results of this pilot study

    Ileocecal duplication cysts: Is the loss of the valve always necessary?

    Ileocecal (IC) duplication cysts are enteric duplications located at the IC junction, not clearly identified in all the published series. The reported treatment is IC resection and ileocolic anastomosis. It is well known that the loss of the IC valve has several adverse effects. This study is aimed at demonstrating that cyst removal together with the common ileal wall and following enterorrhapy is possible, safe, and effective in preserving the IC region. Methods Medical records of 3 patients who underwent surgery for IC duplication between 2003 and 2013 were retrospectively reviewed evaluating follow-up results. Results All patients had an antenatal diagnosis of intrabdominal cystic mass. In two cases associated malformations were reported. The lesions presented at newborn age with intermittent small bowel obstruction and required removal. No patients underwent IC resection. The diagnosis of duplication cyst was confirmed by histo-pathologic examination. The postoperative course was uneventful, even in the long-term follow-up

    The shallow magma pathway geometry at Mt. Etna volcano

    A fundamental goal of volcano seismology is to understand the dynamics of active magmatic systems in order to assess eruptive behavior and the associated hazard. Imaging of magma conduits, quantification of magma transport and investigation of long-period seismic sources, together with their temporal variations, are crucial for the comprehension of eruption-triggering mechanisms. At Mt. Etna volcano, several intense episodes of tremor activity were recorded during 2007, in association with strombolian activity and/or intense fire fountaining episodes occurring from the South East Crater (SEC). The locations of the tremor sources and of the long-period seismic events are used here to constrain both the area and the depth range of magma degassing, highlighting the geometry of the shallow conduits feeding SEC. The imaged conduits consist of two connected resonating dike-like bodies, NNW-SSE and NW-SE oriented, extending from sea level to the surface. In addition, we show how tremor, long-period (LP) and very-long-period (VLP) event locations and signatures reflect pressure fluctuations in the plumbing system associated with the ascent/discharge of gas-rich magma linked to the lava fountains. The evidence here reported, also corroborated by ground deformation variations, can help develop a better prediction and early-warning system for those eruptions (effusive or explosive) that apparently manifest no clear precursors

    Production of laccase by Botrytis cinerea and fermentation studies with strain F226

    After induction, seven strains ofBotrytis cinerea released into the culture broth considerable amounts of laccase in a brief production time. The set-up of a suitable production process was studied with a selected strain in a 10-L fermenter. The optimum fermentation conditions were a 3% inoculum with a high degree of sporulation, a simple medium containing 20 g L−1 of glucose and 2 g L−1 of yeast extract at pH 3.5, 2 g L−1 gallic acid as inducer, added after 2 days of growth, an agitation speed of 300 rpm, an aeration rate of 1.2 vvm and a temperature of 24°C. By optimizing the culture conditions, the enzyme activity reached 28 U ml−1 in 5 days with a specific activity of 560 U mg−1 protein. The best procedure to obtain a suitable crude enzyme preparation was concentration of the supernatant medium to 10% of the initial volume by ultrafiltration, followed by a fractional precipitation with ethanol. The optimum pH and temperature for laccase activity were 5.5 and 40°C, respectively, with syringaldazine as the substrate

    Bonaccorso et al. [2011a] investigated the source and magma dynamics of the 10 May 2008 lava fountain at the South-East Crater (SEC) of Mount Etna through a multidisciplinary approach that integrated a wide data set ranging from bulk rock compositions of the erupted products to seismic tremor and long-period events, tilt and gravity signals. Using a large dataset, the study provided a robust framework in which the mechanism of the 10 May 2008 lava fountain is explained as a violent release of bubble-rich magma layer previously trapped at the top of a shallow reservoir located between −0.5 and 1.5 km above sea level (asl). This result is in agreement with recent relevant literature [Allard et al., 2005; Vergniolle and Ripepe, 2008; Aiuppa et al., 2010; Andronico and Corsaro, 2011; Bonaccorso et al., 2011b; Calvari et al., 2011; Vergniolle and Gaudemer, 2012]. In the introduction of their comment Carbone and Patanè [submitted] affirm that in their opinion the interpretation that “the lava fountain was generated by the fragmentation of a foam layer trapped at the top of shallow reservoir” is not soundly based. This comment’s conclusion is puzzling because one of the comment’s authors (D. Patanè) is also a co-author on the paper by Aiuppa et al. [2010] where the same conclusion, now criticized, was well supported (see figure 5 and conclusions of that paper). In particular, in the conclusions Aiuppa et al. [2010] reported that “The paroxysmal SEC episodes mark the violent release of a bubble-rich magma layer, with bubbles having relatively shallow reservoir ...", that is, the same conclusion now criticized in the comment. After this, the comment raises issues concerning the analysis and interpretation of gravity and tilt data in the multidisciplinary approach presented by Bonaccorso et al. [2011a]. The comment by Carbone and Patanè is divided into 4 paragraphs, labelled “1. Introduction”, “2. Gravity changes”, “3. Tilt changes” and “4. Concluding remarks” with only paragraphs 2 and 3 containing specific comments. In this reply, we address these two paragraphs, and we shall show how the assumptions underlying the comment are merely speculative and why the results presented by Bonaccorso et al.[2011a] remain valid

    Advances in the study of geophysical signals from Mt. Etna volcano.

    Long-period seismic signals, including LP events and tremor, observed on many volcanoes worldwide, may play a crucial role in the forecasting of volcanic eruptions as these signals are direct indicators of sub-surface magma dynamics. Their location is widely agreed to be useful for mapping the extension and geometry of the plumbing system and also for quantifying pressure transients caused by resonance or movement of fluids along the conduits. At Mt. Etna detailed investigations on these signals started systematically only after the installation of a broad-band seismic network (since 2003). Thereafter, we present the main results recently obtained on this volcano by analysing tremor, LP and VLP events during two eruptive episodes in the second half of 2007. We also discuss results of cross analysis performed between tremor and gravity sequences acquired simultaneously at Mt. Etna during the 2002-03 eruption and during the December 2005-January 2006 period of quiescence. We detected common anomalies which are indicative of a quasi-closed system, becoming progressively enriched in volatiles. These studies, carried out in the framework of the VOLUME Project, provide new insight into the shallow plumbing system of Etna and grant valuable tools for volcanic hazard forecasting and risk mitigation
