Le nuove minacce per la salute respiratoria: il fumo elettronico
Il fenomeno dello svapo si sta diffondendo moltissimo soprattutto tra i giovanissimi. Il Pediatra Pneumologo riveste un ruolo cruciale per tentare di invertire questa tendenza, avendo un duplice compito: prevenire il tabagismo, educando i propri pazienti sui rischi e le conseguenze del fumo, e individuare i pazienti che giĂ fumano o che sono esposti a fumo, per fornire aiuto e supporto nel trattamento della dipendenza
Spirometria e FeNO concetti base per l’esecuzione e l’interpretazione in età pediatrica
Le prove di funzionalitĂ respiratoria, prima tra tutte la spirometria, rappresentano
uno strumento essenziale sia nella diagnosi delle patologie respiratorie, che nel loro
follow-up. Con l’assistenza di personale qualificato la maggior parte dei bambini a
partire dai 5-6 anni riesce ad eseguire una spirometria che rispetti criteri standardizzati
di accettabilitĂ e riproducibilitĂ . La risultante curva flusso-volume consente
di distinguere disfunzioni ventilatorie di tipo ostruttivo, restrittivo o misto e di valutarne
la reversibilitĂ dopo somministrazione di salbutamolo. La spirometria fa parte
integrante del percorso diagnostico-terapeutico dell’asma, ma non ha un’elevata correlazione
con severità e controllo dei sintomi e non è sempre sufficientemente sensibile
nell’individuare la limitazione variabile del flusso aereo. Per superare questi
limiti, negli ambulatori di pneumologia pediatrica accanto alla spirometria si stanno
quindi diffondendo nuovi strumenti e nuove metodiche, tra cui la valutazione non
invasiva di markers di infiammazione, come la misurazione della frazione esalata di
ossido nitrico (FeNO). Esso rappresenta un marker di infiammazione di Tipo-2 delle
vie aeree e, in associazione alla spirometria, sembra avere un ruolo nel supportare
la diagnosi di asma, nella valutazione del rischio di riacutizzazione e nella gestione
terapeutica, ma anche nella valutazione del rischio di sviluppare asma nei bambini
con wheezing prescolare. Poiché i livelli di FeNO possono essere influenzati da molti
fattori esogeni ed endogeni, tale misurazione deve essere interpretata alla luce dei
dati clinici e strumentali e non basandosi solo sulle soglie dei cut-off indicati dalle
linee guida internazionali. Questo articolo valuta il contributo dell’esame spirometrico
e della misurazione del FeNO nella gestione ambulatoriale dei bambini con asma
bronchiale, fornendo aspetti pratici per l’esecuzione e l’interpretazione di questi test
diagnostici, al fine di ottimizzare la gestione terapeutica di questi pazienti
Climate Change and Childhood Respiratory Health: A Call to Action for Paediatricians
Climate change (CC) is one of the main contributors to health emergencies worldwide. CC appears to be closely interrelated with air pollution, as some pollutants like carbon dioxide (CO2), nitrogen oxides (NOx) and black carbon are naturally occurring greenhouse gases. Air pollution may enhance the allergenicity of some plants and, also, has an adverse effect on respiratory health. Children are a uniquely vulnerable group that suffers disproportionately from CC burden. The increasing global warming related to CC has a big impact on plants' lifecycles, with earlier and longer pollen seasons, as well as higher pollen production, putting children affected by asthma and allergic rhinitis at risk for exacerbations. Extreme weather events may play a role too, not only in the exacerbations of allergic respiratory diseases but, also, in favouring respiratory infections. Even though paediatricians are already seeing the impacts of CC on their patients, their knowledge about CC-related health outcomes with specific regards to children's respiratory health is incomplete. This advocates for paediatricians' increased awareness and a better understanding of the CC impact on children's respiratory health. Having a special responsibility for children, paediatricians should actively be involved in policies aimed to protect the next generation from CC-related adverse health effects. Hence, there is an urgent need for them to take action and successfully educate families about CC issues. This paper aims at reviewing the evidence of CC-related environmental factors such as temperature, humidity, rainfall and extreme events on respiratory allergic diseases and respiratory infections in children and proposing specific actionable items for paediatricians to deal with CC-related health issues in their clinical practice
Comorbidità nell’asma grave pediatrico
Asthma comorbidities frequently cause adverse outcomes, such as poor asthma control, frequent asthma attacks, reduced quality of life, and higher healthcare costs. Comorbidities are well-known treatable traits whose proper management can help achieve optimal asthma control. Although multimorbidity is frequent among asthmatics, comorbidities are still a potential cause of misdiagnosis and under or overtreatments, and little is known about their impact on severe pediatric asthma.
Over the years, growing scientific evidence has pointed to the existence of a clear epidemiological correlation between asthma and its comorbidities and of possible common pathogenetic mechanisms responsible for a mutual bi-directional influence between the diseases, with sometimes the possibility of describing distinct asthma phenotypes. In this light, the appropriate management of asthma comorbidities could be crucial to propose personalized or adjunctive therapies of apparent efficacy in patients with severe asthma
L’Oscillometria a impulsi nuovo test di funzionalità respiratoria per i bambini con asma
L’asma è una malattia infiammatoria cronica delle vie aeree che può interessare
l’intero albero bronchiale. Recenti evidenze dimostrano che la disfunzione delle
piccole vie aeree (o small airway dysfunction, SAD) è un fattore importante nella
patogenesi e nell’espressione clinica della malattia. A causa delle difficoltà nella
valutazione delle vie aeree periferiche con tecniche non invasive, risulta ancora
poco chiaro il ruolo della SAD nell’asma pediatrico, che è invece assodato in etĂ
adulta. Secondo recenti lavori, le piccole vie aeree sono interessate giĂ nelle prime
fasi dell’asma, ma la spirometria, il test convenzionale per la valutazione della
funzione polmonare, non esamina in modo sensibile la loro funzione, risultando
alterata solo quando la disfunzione periferica diventa molto rilevante. L’infiammazione
cronica e la SAD rappresentano fattori di rischio per la persistenza e la
gravità dell’asma, lo scarso controllo della malattia e la progressiva riduzione
della funzione polmonare con l’età . Identificare e quantificare il coinvolgimento
sia delle vie aeree centrali che periferiche risulta pertanto clinicamente molto
rilevante per una diagnosi precoce e per ottenere un buon controllo dell’asma,
ridurre l’iperreattività bronchiale e monitorare la risposta al trattamento di fondo.
Questo articolo descrive le evidenze recenti sul ruolo della SAD nello sviluppo
e nel controllo dell’asma pediatrico e valuta il contributo di una nuova tecnica
diagnostica disponibile in ambito ambulatoriale, l’oscillometria a impulsi, nella
diagnosi precoce di SAD in età prescolare e scolare, nel monitoraggio dell’asma (in
associazione alla spirometria) e nella gestione terapeutica
Il link asma-obesitĂ : aspetti patogenetici, clinico-funzionali e diagnostico-terapeutici
L’asma e l’obesità sono considerate un problema primario di salute pubblica dell’età infantile, che sta assumendo proporzioni globalmente “epidemiche”. Diversi studi epidemiologici hanno chiaramente evidenziato la presenza di un’associazione tra le due patologie. Tale complessa interazione patogenetica vede coinvolti fattori genetici, di sviluppo, di funzione
polmonare, immunologici e comportamentali; alcuni di essi sono ad oggi ancora poco studiati e conosciuti. Per tale motivo, non è possibile identificare un meccanismo prevalente sugli altri che sia alla base della relazione causale tra le due patologie. Il crescente interesse scientifico nei confronti dell’associazione tra asma e obesitĂ ha contribuito a delineare diversi fenotipi di patologia presenti nelle varie epoche della vita. La caratterizzazione clinica dei soggetti asmatici obesi è presupposto fondamentale per identificare terapie mirate a raggiungere il controllo dell’asma e contemporaneamente a ridurre il peso del soggetto prevenendo le complicanze legate all’obesitĂ
Severe asthma features in children: A case–control online survey
Background: Very few studies have explored the distinguishing features of severe asthma in childhood in Europe, and only one study was conducted in Southern Europe. The aim of this study was to provide a detailed characterization of children with severe asthma treated in specialized pediatric asthma centers across Italy. Methods: We conducted a web-based data collection of family, environmental, clinical and laboratory characteristics of 41 patients aged 6–17 years with severe asthma, defined according to the recent guidelines of the European Respiratory Society and the American Thoracic Society, and 78 age-matched peers with non-severe persistent asthma. The patients have been enrolled from 16 hospital-based pediatric pulmonology and allergy centers in Northern, Central, and Southern Italy. Logistic regression analysis assessed the relationship between patients’ characteristics and severe asthma or non-severe persistent asthma. Results: Features independently and significantly associated with severe asthma included lifetime sensitization to food allergens [Odds ratio (OR), 4.73; 95% Confidence Interval (CI), 1.21–18.53; p = 0.03], lifetime hospitalization for asthma (OR, 3.71; 95% CI, 1.11–12.33; p = 0.03), emergency-department visits for asthma during the past year (OR = 11.98; 95% CI, 2.70–53.11; p = 0.001), and symptoms triggered by physical activity (OR = 12.78; 95% CI, 2.66–61.40; p = 0.001). Quality-of-life score was worse in patients with severe asthma than in subjects with non-severe persistent asthma (5.9 versus 6.6, p = 0.005). Self-perception of wellbeing was compromised in more than 40% of patients in both groups. Children with severe asthma had lower spirometric z scores than non-severe asthmatic peers (all p < 0.001), although 56% of them had a normal forced expiratory volume in 1 s. No differences were found between the two groups for parental education, home environment, patients’ comorbidities, adherence to therapy, exhaled nitric oxide values, and serum eosinophils and IgE. Conclusions: As expected, children with severe asthma had more severe clinical course and worse lung function than peers with non-severe persistent asthma. Unlike previous reports, we found greater sensitization to food allergens and similar environmental and personal characteristics in patients with severe asthma compared to those with non-severe persistent asthma. Psychological aspects are compromised in a large number of cases and deserve further investigation
Inter-society consensus for the use of inhaled corticosteroids in infants, children and adolescents with airway diseases
Background: In 2019, a multidisciplinary panel of experts from eight Italian scientific paediatric societies developed a consensus document for the use of inhaled corticosteroids in the management and prevention of the most common paediatric airways disorders. The aim is to provide healthcare providers with a multidisciplinary document including indications useful in the clinical practice. The consensus document was intended to be addressed to paediatricians who work in the Paediatric Divisions, the Primary Care Services and the Emergency Departments, as well as to Residents or PhD students, paediatric nurses and specialists or consultants in paediatric pulmonology, allergy, infectious diseases, and ear, nose, and throat medicine.
Methods: Clinical questions identifying Population, Intervention(s), Comparison and Outcome(s) were addressed by methodologists and a general agreement on the topics and the strength of the recommendations (according to the GRADE system) was obtained following the Delphi method. The literature selection included secondary sources such as evidence-based guidelines and systematic reviews and was integrated with primary studies subsequently published.
Results: The expert panel provided a number of recommendations on the use of inhaled corticosteroids in preschool wheezing, bronchial asthma, allergic and non-allergic rhinitis, acute and chronic rhinosinusitis, adenoid hypertrophy, laryngitis and laryngospasm.
Conclusions: We provided a multidisciplinary update on the current recommendations for the management and prevention of the most common paediatric airways disorders requiring inhaled corticosteroids, in order to share useful indications, identify gaps in knowledge and drive future research
