Poszttranszplantációs lymphoproliferativ betegség gyermekkorban
Bevezetés: A poszttranszplantációs lymphoproliferativ betegség a leggyakoribb szolid szerv- vagy allogén vérképző őssejt-transzplantációt követően kialakuló malignitás gyermekkorban. E széles spektrumú betegség immunszuppresszió talaján jön létre, kialakulásában kiemelkedő jelentőségű az Epstein–Barr-vírus immunsejteket módosító hatása. Célkitűzés: A szerzők célul tűzték ki a Magyarországon előforduló gyermekkori esetek összefoglaló felmérését. Módszer: A Semmelweis Egyetem I. Gyermekgyógyászati Klinikáján gondozott vese-, máj- és tüdőtranszplantált, valamint a Szent László Kórházban kezelt őssejttranszplantált gyermekek képezték a vizsgált betegpopulációt. Hetvennyolc vese-, 109 máj-, 17 tüdő-, illetve 243 allogén őssejttranszplantált gyermek adatait dolgozták fel. Eredmények: 1998 és 2012 között 13 transzplantált gyermeknél alakult ki szövettanilag igazolt poszttranszplantációs lymphoproliferativ betegség. A megbetegedettek közül 8 gyermek esett át szolid szervtranszplantáción, 5 beteg pedig őssejtátültetésen. A 8 szervtranszplantált közül hármat, míg az 5 őssejttranszplantált közül 4 beteget veszítettünk el. A poszttranszplantációs lymphoproliferativ betegség legmagasabb arányban a tüdőtranszplantáltak között alakult ki (17,6%). Következtetések: Az eredmények alátámasztják, hogy a poszttranszplantációs lymphoproliferativ betegség ritka, ám nagyon magas letalitású kórkép. A legfontosabb terápiás lehetőség az immunszuppresszió csökkentésén túl a kemoterápia és a rituximab. A kimenetelt döntően befolyásolja a korán felállított diagnózis, amelynek fontos eszköze a magas kockázatú betegek Epstein–Barr-vírus-szűrése polimeráz láncreakcióval. Orv. Infliximabrezisztens Crohn-beteg gyermek adalimumabkezelése = Adalimumab treatment in infliximab resistant pediatric patient with Crohn’s disease
Hagyományos kezelésre refrakter, fistulás Crohn-betegek kezelésében a chimer antitumornekrózis-faktor-alfa- (infliximab-) terápia igen hatékony, bár a betegek egy részében infliximabrezisztencia alakulhat ki. A szerzők egy infliximabrezisztens, 12 éves Crohn-beteg fiú esetét ismertetik, akinek luminális gyulladása mellett 3 aktív perianalis fistulája is volt. Teljesen humanizált antitumornekrózis-faktor-alfa- (adalimumab-) kezelést követően a 10. héten a fistulák bezáródtak, a korábbi magas aktivitási index normalizálódott. Validált életminőségi mutatók szintén szignifikánsan javultak. Az eset alapján infliximabrezisztens Crohn-beteg gyermeknél az adalimumabterápia sikerrel alkalmazható.

Treatment with the chimeric monoclonal antibody (infliximab) is highly effective in refractory and fistulising Crohn’s disease, nevertheless, infliximab resistance may occur. Authors report a 12-year-old boy with infliximab refractory luminal Crohn’s disease including 3 active perianal fistulas. The patient was treated successfully with adalimumab, a fully human anti-tumor necrosis factor alpha monoclonal antibody. After 10 weeks of therapy, the previously high activity index returned to normal and the fistulas were closed. Quality of life using validated questionare improved significantly also. Adalimumab might be a suitable therapy even in pediatric Crohn’s disease patients with infliximab resistance
A Helicobacter pylori szerepe a gyermekkori idiopathiás immunthrombocytopeniában = The role of Helicobacter pylori infection in children with chronic immune thrombocytopenic purpura
A Helicobacter pylori kóroki szerepe a felnőttkori immunthrombocytopeniás purpurában bizonyított, ám a gyermekkori esetekről eddig csak kevés és egymásnak ellentmondó adatok ismeretesek. Jelen felmérésükben a szerzők a Helicobacter pylori-infekció előfordulását vizsgálták immunthrombocytopeniás purpurában szenvedő gyermekekben. Ezenkívül a Helicobacter pylori
-pozitív gyermekek thrombocytaszámának változását követték nyomon az eradikációt követően.
Betegek és módszer:
A vizsgálatukba 27 immunthrombocytopeniás purpurás gyermeket vontak be (13 fiú, 14 lány, átlagéletkor: 8,2 év, tartomány: 18 hó–18 év). A kontrollcsoportba 38 egészséges gyermek tartozott (20 fiú, 18 lány, átlagéletkor: 9,6 év, tartomány: 4–18 év). A

Eredmény:
Az immunthrombocytopeniás purpurás betegekben a 27 főből 2 gyermek (7,7%) bizonyult
Helicobacter pylori
-pozitívnak. A kontrollokban hasonló előfordulási arányt találtak (3/38, 7,9%). A nyomon követés során (átlagosan 10 hónap, tartomány: 6–14 hó) a
Helicobacter pylori
eradikációját követően a betegek thrombocytaszáma nem emelkedett szignifikánsan. A
Helicobacter pylori
-pozitív immunthrombocytopeniás purpurás betegek eradikációja a kontroll

C urea-kilégzési teszt alapján sikeresnek bizonyult.
Következtetés:
Vizsgálatok alapján a
Helicobacter pylori
kóroki szerepe nem igazolható a gyermekkori immunthrombocytopeniás purpurában. Ennek megfelelően a kórképben szenvedő gyermekekben a
Helicobacter pylori
szűrővizsgálata nem javasolt.

Helicobacter pylori
infection has been associated with immune thrombocytopenic purpura in adults, however, the few studies concerning this issue showed conflicting results in the paediatric field. Therefore, authors prospectively investigated the incidence of
Helicobacter pylori
infection and the effect of
Helicobacter pylori
eradication on platelet count recovery in children with immune thrombocytopenic purpura.
Patients and methods:
The study included 27 children with immune thrombocytopenic purpura (13 boys and 14 girls) with a median age of 8.2 years (range, 18 months–18 years). 38 healthy children (controls) were also enrolled (20 boys, 18 girls, median age, 9.6 years, range, 4–18 years).

infection was diagnosed using

C urea breath test. Eradication was assessed at 6 weeks following the end of the treatment by
13
C urea breath.
Results:
2 children out of 27 patients with immune thrombocytopenic purpura proved to be positive for
Helicobacter pylori
infection (7.7%) and were treated with combination therapy. Controls showed similar rate of
Helicobacter pylori
infection (3/38, 7.9%) in comparison to those with immune thrombocytopenic purpura. In the follow-up period (mean, 10 months, range, 6–14 months) platelets did not show any improvement in infected children following the eradication treatment and were identical to those of the uninfected patients. Eradication was successful in the 2

infected patients with immune thrombocytopenic purpura.
Conclusion:
The results do not suggest a pathogenetic role of
Helicobacter pylori
infection in the etiology of immune thrombocytopenic purpura. Screening of
Helicobacter pylori
infection among paediatric patients with immune thrombocytopenic purpura is not recommended
Immune phenotype in children with therapy-naïve remitted and relapsed Crohn’s disease
AIM To characterize the prevalence of subpopulations of CD4+
cells along with that of major inhibitor or stimulator cell
types in therapy naive childhood Crohn s disease (CD) and to
test whether abnormalities of immune phenotype are normalized
with the improvement of clinical signs and symptoms of disease
METHODS We enrolled 26 pediatric patients with CD 14 therapy
naive CD children, of those, 10 children remitted on
conventional therapy and formed the re mission group We also
tested another group of 12 children who relapsed with
conventional therapy and were given infliximab, and 15 healthy
children who served as controls The prevalence of Th1 and Th2,
naive and memory, activated and regulatory T cells, along with
the members of innate immunity such as natural killer (NK), NK
T, myeloid and plasmocytoid dendritic cells (DCs), monocytes and
Toll like receptor (TLR) 2 and TLR 4 expression were determined
in peripheral blood samples RESULTS Children with therapy naive
CD and those in relapse showed a decrease in Th1 cell prevalence
Simultaneously, an increased prevalence of memory and activated
lymphocytes along with that of DCs and monocytes was observed In
addition, the ratio of myeloid/plasmocytoid DCs and the
prevalence of TLR2 or TLR4 positive DCs and monocytes were also
higher in therapy naive CD than in controls The majority of
alterations diminished in remitted CD irrespective of whether
remission was obtained by conventional or biological therapy
CONCLUSION The finding that immune phenotype is normalized in
remission suggests a link between immune phenotype and disease
activity in childhood CD Our observations support the

Szervtranszplantáció gyermekkorban
Absztrakt:
A gyermekkorban végzett szervátültetés mára hazánkban is minden, az átültetésre
alkalmas gyermek számára elérhetővé vált. Fontos ismernünk és tudnunk, hogy a
végállapotú szervelégtelenség kialakulásához vezető okok szinte minden szerv
esetében jelentősen különböznek a felnőttekéitől. Gyermekkorban mindezek mellett
mind sebészi, mind gyermekgyógyászi oldalról más kihívásokkal kell megküzdenünk,
mint a felnőtteknél (a szervek és a recipiens mérete, más és más formában zajló
infekciók, az immunszuppresszív szerek eltérő farmakokinetikája és
farmakodinamikája, noncompliance). A gyermekkori szervtranszplantáció ugyanakkor
az elmúlt évtizedek egyik sikertörténete, amely csak sok szakterület gondos és

Abstract:
Paediatric organ transplantation today is considered and accepted and widely
available therapy in children with end-stage organ failure. It is important to
know that in childhood, diseases leading to end-stage organ failure differ from
those in adults. Beside this, in children there are different surgical and
paediatric challenges before and after transplantation (size differences of the
patient and donor organ, special and paediatric infections, different
pharmacokinetics and pharmacodynamics of immunosuppressive drugs,
noncompliance). However, paediatric organ transplantation in the last decades
became a success story of the Hungarian health care owing to several working
