    A small satellite mission devoted to mid-low latitude earth observation

    This paper aims at assessing the feasibility of a small mission devoted to observe the mid-low latitude regions. The satellite will be equipped with three optical sensors: a medium-high spatial resolution VIS-NIR multi-spectral sensor, allowing the surface monitoring and land-use and land-cover studies; a medium spatial-resolution 3-bands thermal (MIR-TIR) sensor allowing the surface temperature (LST, SST) estimate and hot-spots (fires, volcanic eruption, etc.) detection; a panchromatic VIS-NIR camera for night-time observation able to reveal artificial and natural lights. The selected orbit, called multi-sun-synchronous (MSS), represents an innovation with respect to the classical sun-synchronous orbit much suitable for observing tropical regions, allowing an enhanced revisit frequency. Further, such an orbit allows the observation of the same region of the Earth at different local-time. In this way, the diurnal cycle of surface temperatures can be reconstructed with a 2-hours local-time step. An analysis of the capability of the selected ground stations to acquire the data gathered by the remote sensing sensors has been carried out. Orbital perturbations have been taken into account and an estimate of the propellant required for ground track control has been performed in order to verify its compatibility with a small mission requirements

    Resident training in urology: Bipolar transurethral resection of the prostate - a safe method in learning endoscopic surgical procedure

    Introduction: Modern medicine uses increasingly innovative techniques that require more and more capabilities for acquisition. In the urological department is increasing the presence of patients with lower urinary tract symptoms (LUTS) and transurethral resection of the prostate (TURP) is the standard of care in their surgical treatment. We report our surgical experience and learning curve of using bipolar plasmakinetic devices in the training of urological residents to benign prostatic hyperplasia (BPH) treatment. Materials and Methods: 80 patients with benign prostatic enlargement due to BPH were enrolled in the study. TURP has been performed by three urological residents and by an expe- rienced urologist. Patients were evaluated before and 6 months after the endoscopic bipolar plasmakinetic resection using the International Prostate Symptom Score (IPSS), maximum uri- nary flow rate (Qmax), postvoid residual urine (PVR) and prostate specific antigen (PSA). Results: Overall 60 procedures were performed, 18 PlasmaKinetic (PK)-TURP procedures were completed by the three residents. In the other 42 cases the procedures were completed by the experienced urologist. In eight cases there was a capsular perforation and the experienced urol- ogist replaced the resident to complete the resection. No complications have been reported in the procedures completed by the senior urologist. All complications caused by the residents were man- aged intraoperatively without changing the course of the procedure. Statistical differences were observed regarding IPSS, quality of life (QoL), and PVR at 6-month follow-up when procedures completed by urological residents were compared to those completed by the senior urologist. Conclusion: Bipolar device represents appropriate tools to acquire endoscopic skills. It is safe and it can be used at the first experience of BPH treatment by a resident who has not previ- ously approached this endoscopic surgical procedure

    The determination of some minor components of extra virgin olive oil, and in particular of the polyphenolic fraction of pigments and fragrances, can contribute to the characterization of the monovarietal productions: the quali-quantitative assessment of these substances can in fact contribute to the valorisation of typicalities and, at the same time, consent to the optimisation of the operational techniques during the processing and the eventual blending. The analytical techniques, applied here for the characterisation of extra virgin olive oil of Abruzzo and Istria, have highlighted that chemical composition can enhance the valorisation of these typical products. Nevertheless, further sampling program is needed to evidence typical composition profile that might be used as ā€œorigin markersā€.Određivarije nekih manjih komponenata ekstra djevičanskog maslinovog ulja, naročito polifenolne frakcije pigmemta i mirisa, može doprinijeti svojstvima monovarijetetnih proizvoda: kvalitativna i kvalitativna procjena tih supstancija može zapravo doprinijeti valorizaciji tipičnosti te, istodobno omogućiti optimizaciju operativnih tehnika za vrijeme prerade i mijeÅ”anja.     40Arā€“39Ar dating of volcanogenic products from the AND-2A core (ANDRILL Southern McMurdo Sound Project, Antarctica): correlations with the Erebus Volcanic Province and implications for the age model of the core

    The AND-2A drillcore (Antarctic Drilling Programā€”ANDRILL) was successfully completed in late 2007 on the Antarctic continental margin (Southern McMurdo Sound, Ross Sea) with the aim of tracking ice proximal to shallow marine environmental fluctuations and to document the 20-Ma evolution of the Erebus Volcanic Province. Lava clasts and tephra layers from the AND-2A drillcore were investigated from a petrographic and stratigraphic point of view and analyzed by the 40Arā€“39Ar laser technique in order to constrain the age model of the core and to gain information on the style and nature of sediment deposition in the Victoria Land Basin since Early Miocene. Ten out of 17 samples yielded statistically robust 40Arā€“39Ar ages, indicating that the AND-2A drillcore recovered ā‰¤230 m of Middle Miocene (āˆ¼128ā€“358 m below sea floor, āˆ¼11.5ā€“16.0 Ma) and >780 m of Early Miocene (āˆ¼358ā€“1093 m below sea floor, āˆ¼16.0ā€“20.1 Ma). Results also highlight a nearly continuous stratigraphic record from at least 358 m below sea floor down hole, characterized by a mean sedimentation rate of āˆ¼19 cm/ka, possible oscillations of no more than a few hundreds of ka and a break within āˆ¼17.5ā€“18.1 Ma. Comparison with available data from volcanic deposits on land, suggests that volcanic rocks within the AND-2A core were supplied from the south, possibly with source areas closer to the drill site for the upper core levels, and from 358 m below sea floor down hole, with the ā€œproto-Mount Morningā€ as the main source

    Background. One-third of Crohnā€™s disease (CD) patients present perianal fistula. The gold standard in the diagnosis and treatment of symptomatic perianal disease (PAD) in CD is the exploration of the anal canal and distal rectum under anesthesia (EUA). This procedure is mainly conducted as a day case surgery. Unfortunately, it is not always possible to proceed within the ideal timing and any delay may well represent a relevant clinical issue. The aim of this study was to evaluate the feasibility of outpatient treatment of symptomatic perianal fistulas in CD patients. Methods. All CD patients under regular follow-up at our inflammatory bowel disease referral center, presenting with symptomatic perianal fistulas, were offered surgical consultation. The data of patients were prospectively collected for three years (February 2014 to February 2017) for the purpose of the study. All clinical information, including previous EUA and/or records from MRI and endoscopic ultrasound, was included. Outpatient anal canal and distal rectum exploration and treatment (OE) were undertaken during the specialist surgical consultation. Fistulas were classified according to Parksā€™s classification; the type of outpatient treatment and compliance of patients were recorded. Pain was assessed by VAS at the time of the procedure and during the first control. Patients were followed up in the surgical clinic in relation to the study. Results. Ninety-two CD patients with symptomatic perianal fistulas had surgical consultation during the study period. OE was offered to all but 18 patients who fulfilled the exclusion criteria or had an extremely severe disease; six patients refused the OE (8.11%). Of the 68 patients undergoing OE, eleven (16.18%) had previous surgery for perianal disease. The OE was accomplished in sixty-one patients (89.71%), while in 7 patients, it was abandoned for scarce compliance. Nine patients (14.75%) underwent drainage of perianal abscess; in 3 of them, it was possible to probe the fistula tract, find the internal orifice, and pass a loose seton. Overall, setonage was performed in 50 patients (81.97%). Rectovaginal setons were placed in 3 patients and more than one seton (up to 3) in 6 cases. Fistulotomy was performed in 4 simple subcutaneous fistulous tracts. Concordance with the preoperative findings was found in 54 out of 61 patients. EUA was scheduled at the time of OE for the 7 patients who did not complete the procedure. All sixty-one patients who had the OE were followed up for a minimum of 12 months. Conclusions. This preliminary study indicates that OE in CD patients with symptomatic perianal fistulas is safe and feasible in a high-volume referral center. It might provide several benefits, including patientsā€™ logistics, reduce or remove patientsā€™ symptoms and discomfort, allow for a timely start of medical therapy, and avoid further complications

    Late Pleistocene-Holocene volcanic activity in northern Victoria Land recorded in Ross Sea (Antarctica) marine sediments

    Eight pyroclastic fall deposits have been identified in cores of Late Pleistocene-Holocene marine sediments from the Ross Sea (Antarctica), and their components, granulometry and clast morphologies were analysed. Sedimentological, petrographic and geochemical analysis of clasts, with 40Ar-39Ar dating of alkali feldspar grains, indicate that during this period at least five explosive eruptions of mid to high intensity (plinian to subplinian) occurred, and that three of these eruptions took place from Mount Melbourne volcanic complex, between 137.1 \ub1 3.4 and 12 ka. Geochemical comparison of the studied tephra with micro and crypto-tephra recovered from deep Antarctic ice cores and from nearby englacial tephra at Frontier Mountain indicates that eruptive activity in the Melbourne Volcanic Province of northern Victoria Land was intense during the Late Pleistocene-Holocene, but only a general area of provenance for the majority of the identified tephra can be identified

    Super selective arterial embolization to treat radiation-induced hemorrhagic gastritis: a case report and review of the literature

    Radiation-induced hemorrhagic gastritis (RIHG) is a rare but potentially fatal event following radiotherapy for locally advanced gastric cancer; the treatment of this condition is not standardized. Only few cases of RIHG have been reported, treated with different therapeutic approaches. Here we report the case of a 79-year-old patient who underwent subtotal gastrectomy for gastric cancer, followed by adjuvant chemo-radiotherapy. Approximately 3 months after the end of the treatment, she developed recurrent diffuse bleeding originating from the entire mucosa of the gastric pouch and from a marginal ulcer. As the bleeding was refractory to several endoscopic treatments and surgery was not indicated, the patient underwent two sessions of transcatheter selective arterial embolization, with resolution of bleeding. Arterial embolization has already been reported for the treatment of hemorrhagic cystitis, developing after irradiation of the pelvis for prostate, bladder, rectum, and cervix cancer. However, to our knowledge, it has never been reported as a treatment for hemorrhagic gastritis. Based on this case, we suggest arterial embolization as an option in the management of RIHG, when standard endoscopic treatment fails
