    Risky sexual behavior and predisposing factors among students of Jimma University, Ethiopia

    BACKGROUND: Students of higher institutions are assumed to be exposed to many risky sexual behaviors. However, little has been explored about the magnitude of risky behavior and predisposing factors in the context of higher education institutions in Ethiopia. Thus, the objective of this study was to assess the pattern of risky sexual behaviors and predisposing factors among Jimma University students. METHODS: This cross-sectional study was conducted in November 2009 involving quantitative and qualitative methods. The quantitative study was conducted on 1010 students selected by multistage cluster sampling technique. The data were collected using self-administered questionnaire and analyzed using SPSS V.16.0. Multi-variate logistic regression was used to see association between variables. The qualitative part involved 10 focus group discussions and 17 key-informant interviews selected purposively. The qualitative data were analyzed by thematic areas. RESULTS: Among the respondents, 267(26.9%) ever had sexual intercourse. The mean age at first sexual intercourse was 17.7±2.7 years. Most, 75.6%, started sexual intercourse during secondary school. Among whoever had sex, 51.0% had sex in the last 12 months and 28.3% had multiple sexual partners. Consistent condom use with non-regular partner in the last 12 months was 69.1%. Lack of parental control, substance use, peer pressure, campus and outside environment were identified as predisposing factors. CONCLUSION: Risky sexual behaviour such as having multiple sexual partner and sexual practice without condom with non- regular partner exists.     Determination of Local Barley (Hordeum Vulgare) Crop Coefficient and Comparative Assessment of Water Productivity for Crops Grown Under the Present Pond Water in Tigray, Northern Ethiopia

    An experiment was carried out in 2010 at Mekelle, in northern Ethiopia, to measure the evapotranspiration, to estimate barley crop coefficient (kc), and to evaluate the water productivity taking into account the major crops grown under the present pond irrigation system. Four locally made lysimters were installed in the middle of barley field to measure barley evapotranspiration. The single crop coefficient approach was used to estimate barley crop coefficient. The average seasonal evapotranspiration of barley was 375 mm which is similar to many other cereal crops in the region. The single crop coefficient values for early, vegetative, mid and late crop stages were 0.6 – 0.8, 0.6 – 1.0; 1.0 – 1.05 and 0.3 - 0.4 respectively. The result showed that these crop coefficient values obtained in this experiment were similar to the crop coefficient values obtained in the past except for kc initial. Therefore, the assumption that local barley crop coefficient values differ from that of the documented values was incorrect. Furthermore, the major reason for mismanagement of irrigation water in barley fields was not due to use of wrong crop coefficient values but could be due to inadequate irrigation technical skill and knowledge of the farmer. The average economic water productivity (EWP) of barley for the very wet, wet, normal, dry and very dry seasons scenario were 0.99, 0.7, 0.65, 0.57, and 0.44 USD m-3, respectively, whereas the corresponding crop water productivity (CWP) values for grain were 1.53, 1.08, 1.0, 0.88 and 0.68 kg m-3, respectively. The EWP and CWP of barley were compared with onion and tomato under pond water irrigation at the five climatic scenarios. The crop water productivity for tomato and onion were 85 – 87% and 76 – 78% higher than that of barley, respectively. The corresponding economic water productivity for tomato and onion were 87 – 89% and 81 – 82% higher than that of barley, respectively. We concluded that growing tomato and onion would bring more income or yield per m3 of pond water supplied than growing barley. The implication is that as supply and demand determines the price of products, farmers and extension workers need to balance the crop area coverage per irrigation scheme so that undesirable price falls and rises could be avoided. Evaluation of crops based on their water productivity would improve the productivity of irrigation schemes and ultimately improve food security in the arid and semi-arid areas where water scarcity is critical problem and irrigation is a necessity for crop production.     Sero-prevalence of Hepatitis B virus surface antigen and associated factors among women of reproductive age in Bench Maji Zone, Southwest Ethiopia: community based cross-sectional study

    Background: Hepatitis B virus infection is one of the leading causes of liver diseases which occurs worldwide particularly in developing countries. It is often caused by prenatal transmission from mother to child or household transmission from a close contact during early childhood. It causes different complications like; jaundice, induces premature labor, and prematurity. Objective: The aim of this study was to estimate the sero-prevalence of hepatitis B virus surface antigen and associated factors among women of reproductive age in Bench Maji Zone, South West Ethiopia. Methods: A community-based cross-sectional study was conducted from December 15th, 2016 to February 15th, 2017. Multistage sampling technique was applied to select study participants. Logistic regression analysis was applied and p-values < 0.05 was used to see the significant association between dependent and independent variables. Results: A total of 330 participants were included in this study yielding 98.8% response rate. The sero-prevalence of HBsAg among women of reproductive age was 28(8.5%). Having multiple sexual partners (AOR = 18.73, 95% CI =3.65, 96.21) history of unprotected sex (AOR = 9.39, 95% CI =1.64, 53.77) were found to be significantly associated with sero-prevalence of HBV.Conclusions: The sero-prevalence of HBV infection among women of reproductive age was highly endemic. Hence, behavioral education and communication programs focusing on reduction of risky sexual behaviors should be designed to reduce HBV infection.     Minimum acceptable diet and its associated factors among children aged 6–23 months in Lalibela, northeast Ethiopia: a community-based cross-sectional study

    The first 2 years of life are a critical window of opportunity for ensuring optimal child growth and development. In Ethiopia, the magnitude of the minimum acceptable diet ranges from 7 to 74⋅6 %. The evidence revealed the variation and unrelated data on the prevalence of minimum acceptable diet. Therefore, the present study aimed to assess the minimum acceptable diet and its associated factors among children aged 6–23 months in Lalibela town administration, northeast Ethiopia. A community-based cross-sectional study was conducted in Lalibela town administration, northeast Ethiopia among 387 mothers/caregivers with children aged 6–23 months from May 1 to 30, 2022. The data were entered by Epidata version 3.1 and analysed by SPSS version 25.0. A multivariable binary logistic regression model was fitted to identify factors associated with minimum acceptable diet. The degrees of association were assessed using an adjusted odds ratio with a 95 % confidence interval and P-value of 0⋅05. The magnitude of minimum acceptable diet in the study area was 16⋅7 % (95 % confidence interval: 12⋅8–20⋅6 %). Sex of child, getting infant and young child feeding counselling at antenatal care, infant feeding practice-related knowledge and childhood illness are the variables that were found to be an independent predictor of minimum acceptable diet. Health facilities should strengthen infant feeding counselling starting from antenatal care visits during pregnancy for the recommended minimum acceptable diet is crucial

    Intestinal parasitic infections in Western Abaya with special reference to Schistosomiasis mansoni

    Abstract: A cross sectional investigation on the distribution and prevalence of intestinal parasitic infections was undertaken in Western Abaya, North Omo, in January 1995. As the area is potentially irrigable for modern agriculture, emphasis was placed to explore on the endemicity of schistosomiasis, including malacological assessments. Five percent of the whole population on 1473 people, in 16 villages of the area, were parasitologically examined for intestinal parasites. Among the intestinal parasitic infections, hookworm was demonstrated in all of the villages with prevalences ranging from 4.1% (Algie) to 75% (Wajifo). The majority of the villages (10/16=62.5%) had hookworm prevalences of over 50%. Schistosoma mansoni infections were found in 11 villages with prevalences of up to 53% with more males than females being affected (P<0.001). Infected Biomphalaria sudanica snails with infection rates of up to 2.1% at two sites of Lake Abaya were also found. The importance of the dominant, S. mansoni and hookworm infections in the potentially irrigable area of Western Abaya and feasible measures of their control are discussed. [Ethiop. J.     Decomposition Analysis of Antenatal Care Utilization Inequities in Kembata Tembaro Zone, Southern Ethiopia

    Background: Health equity has emerged as a global issue in the post-2015 Sustainable Development Goals, and Ethiopia is no exception. Despite positive improvements, inequities in maternal health service utilization among demographic groups continue to be one of Ethiopia’s significant challenges in decreasing maternal mortality. This study focuses on antenatal care service discrimination among a local poor group known as the “golden hands” community in Ethiopia’s Kembata Tembaro zone. The subgroup community consists of outcast artesian groups known as “golden hands,” formerly known as “Fuga,” who face discrimination in all aspects of life owing to their living conditions and ethnic background. Methods: A community-based comparative cross-sectional study was conducted in Ethiopia’s Kembata Tembaro, zone in the Southern Nations, Nationalities, and Peoples’ Region (SNNPR), from January to February 2022. The study focused on two groups, “golden hands” and “non-golden hands,” consisting of women aged 15–49 years. Using stratified and multistage cluster sampling, 1,210 participants were selected, with 440 from golden hand communities and 770 from non-golden hand communities. Data was collected through translated questionnaires, and data quality was rigorously monitored. The concentration curve and index, as well as logistic-based decomposition analysis, were used to examine inequality. The statistical significance threshold was set at p < 0.05 with a 95% confidence interval. Result: This study comprised 1,210 eligible participants, 440 of whom were golden hand community members. Discrimination accounted for 60.23% of the decreased antenatal care (ANC) service use by the golden hand community. Age, urban residence, and wealth index were the most important independent factors with statistically significant contributions to changes owing to differences in effects (discriminated difference). Conclusion: Since ANC service discrimination is prevalent, the government and nongovernmental organizations should take steps to ensure that marginalized groups in society, such as golden hand women, the poor, the uneducated, and rural people, have equal access to service utilization opportunities

    Prevalence of and Risk Factors for Trachoma in Southern Nations, Nationalities, and Peoples' Region, Ethiopia: Results of 40 Population-Based Prevalence Surveys Carried Out with the Global Trachoma Mapping Project.

    PURPOSE: We sought to estimate the prevalence of trachoma at sufficiently fine resolution to allow elimination interventions to begin, where required, in the Southern Nations, Nationalities, and Peoples' Region (SNNPR) of Ethiopia. METHODS: We carried out cross-sectional population-based surveys in 14 rural zones. A 2-stage cluster randomized sampling technique was used. A total of 40 evaluation units (EUs) covering 110 districts ("woredas") were surveyed from February 2013 to May 2014 as part of the Global Trachoma Mapping Project (GTMP), using the standardized GTMP training package and methodology. RESULTS: A total of 30,187 households were visited in 1047 kebeles (clusters). A total of 131,926 people were enumerated, with 121,397 (92.0%) consenting to examination. Of these, 65,903 (54.3%) were female. In 38 EUs (108 woredas), TF prevalence was above the 10% threshold at which the World Health Organization recommends mass drug administration with azithromycin annually for at least 3 years. The region-level age- and sex-adjusted trichiasis prevalence was 1.5%, with the highest prevalence of 6.1% found in Cheha woreda in Gurage zone. The region-level age-adjusted TF prevalence was 25.9%. The highest TF prevalence found was 48.5% in Amaro and Burji woredas. In children aged 1-9 years, TF was associated with being a younger child, living at an altitude 15°C, and the use of open defecation by household members. CONCLUSION: Active trachoma and trichiasis are significant public health problems in SNNPR, requiring full implementation of the SAFE strategy (surgery, antibiotics, facial cleanliness, and environmental improvement)

    Prevalence of Trachoma from 66 Impact Surveys in 52 Woredas of Southern Nations, Nationalities and Peoples' and Sidama Regions of Ethiopia, 2017-2019.

    PURPOSE: Trachoma is endemic in Southern Nations, Nationalities and Peoples' (SNNP) and Sidama regions of Ethiopia. We aimed to measure the prevalence of trachomatous inflammation - follicular (TF) among children aged 1 - 9 years and the prevalence of trachomatous trichiasis (TT) unknown to the health system among people aged ≥15 years following interventions for trachoma in 52 woredas of SNNP and Sidama regions. METHODS: From 2017 - 2019, 66 two-stage cluster sampling cross-sectional population-based surveys were carried out in 52 woredas (third-level administrative divisions) using a standardized World Health Organization-recommended survey methodology. This included one impact survey in 40 woredas, two consecutive impact surveys in 10 woredas and three consecutive impact surveys in two woredas. Water, sanitation and Hygiene (WASH) access was assessed using a modified version of the United Nations Children's Fund/WHO Joint Monitoring Programme questionnaire. RESULTS: By the end of this survey series, 15 (23%) of the woredas had met the active trachoma elimination threshold (TF prevalence <5%) and 12 (18%) had met the TT threshold (TT ≤ 0.2%). Regarding WASH coverage, 20% of households had access to an improved drinking water source within a 30-min journey and 3% had an improved latrine. There was strong evidence that TF was less common in 4 - 6-year-olds and 7 - 9-year-olds than 1 - 3-year-olds. CONCLUSION: Based on the findings, further antibiotic mass drug administration is required in 37 woredas and active TT case finding is needed in 40 woredas. In these surveys, access to WASH facilities was very low; WASH improvements are required

    The Global Trachoma Mapping Project: Methodology of a 34-Country Population-Based Study.

    PURPOSE: To complete the baseline trachoma map worldwide by conducting population-based surveys in an estimated 1238 suspected endemic districts of 34 countries. METHODS: A series of national and sub-national projects owned, managed and staffed by ministries of health, conduct house-to-house cluster random sample surveys in evaluation units, which generally correspond to "health district" size: populations of 100,000-250,000 people. In each evaluation unit, we invite all residents aged 1 year and older from h households in each of c clusters to be examined for clinical signs of trachoma, where h is the number of households that can be seen by 1 team in 1 day, and the product h × c is calculated to facilitate recruitment of 1019 children aged 1-9 years. In addition to individual-level demographic and clinical data, household-level water, sanitation and hygiene data are entered into the purpose-built LINKS application on Android smartphones, transmitted to the Cloud, and cleaned, analyzed and ministry-of-health-approved via a secure web-based portal. The main outcome measures are the evaluation unit-level prevalence of follicular trachoma in children aged 1-9 years, prevalence of trachomatous trichiasis in adults aged 15 + years, percentage of households using safe methods for disposal of human feces, and percentage of households with proximate access to water for personal hygiene purposes. RESULTS: In the first year of fieldwork, 347 field teams commenced work in 21 projects in 7 countries. CONCLUSION: With an approach that is innovative in design and scale, we aim to complete baseline mapping of trachoma throughout the world in 2015
