The improvement process entails three general phases. Diagnostic analysis is an interdisciplinary field study to thoroughly understand the actual performance of an irrigation system. Areas of high and low performance are identified. Management planning is a process for organizational change. The process uses the information and understanding obtained from the diagnostic analysis to make important changes in physical structures and/or management procedures for improving irrigation system performance. Management performance is carrying out the management plan. Monitoring and evaluation is included to assist in management decision making and to measure the impact of the changes on system performance. The management planning is done by the key managers in an irrigated area, farmer representatives, and representatives from other involved organizations. Conscious, deliberate applications of the processes offer important advantages to the farming community and to water management professionals. The results are effective, appropriate solutions to many relevant problems in irrigation

    Psoriasis is a chronic, incurable, auto-immune disorder with cutaneous manifestations. New evidence on the central role of the immune system in the pathogenesis of psoriasis increasingly provides insight into pathogenic steps that can be modulated to provide disease control. Numerous biological therapies are in various stages of clinical development, with expectation of providing enhanced safety and efficacy over currently available psoriasis therapies. Efalizumab, a recombinant humanized monoclonal IgG1 antibody, is a novel targeted T-cell modulator that inhibits multiple steps in the immune cascade that result in the production and maintenance of psoriatic plaques, including initial T-cell activation and T-cell trafficking into sites of inflammation, including psoriatic skin, with subsequent reactivation in these sites. This article reviews the pharmacodynamic, pharmacokinetic and clinical effects observed during phase I, II and III efalizumab trials in patients with moderate to severe chronic plaque psoriasis.     Albumin-based cancer therapeutics for intraperitoneal drug delivery : a review

    Albumin is a remarkable carrier protein with multiple cellular receptor and ligand binding sites, which are able to bind and transport numerous endogenous and exogenous compounds. The development of albumin-bound drugs is gaining increased importance in the targeted delivery of cancer therapy. Intraperitoneal (IP) drug delivery represents an attractive strategy for the local treatment of peritoneal metastasis (PM). PM is characterized by the presence of widespread metastatic tumor nodules on the peritoneum, mostly originating from gastro-intestinal or gynaecological cancers. Albumin as a carrier for chemotherapy holds considerable promise for IP delivery in patients with PM. Data from recent (pre)clinical trials suggest that IP albumin-bound chemotherapy may result in superior efficacy in the treatment of PM compared to standard chemotherapy formulations.     The California Legacy Survey I. A Catalog of 178 Planets from Precision Radial Velocity Monitoring of 719 Nearby Stars over Three Decades

    We present a high-precision radial velocity (RV) survey of 719 FGKM stars, which host 164 known exoplanets and 14 newly discovered or revised exoplanets and substellar companions. This catalog updated the orbital parameters of known exoplanets and long-period candidates, some of which have decades-longer observational baselines than they did upon initial detection. The newly discovered exoplanets range from warm sub-Neptunes and super-Earths to cold gas giants. We present the catalog sample selection criteria, as well as over 100,000 radial velocity measurements, which come from the Keck-HIRES, APF-Levy, and Lick-Hamilton spectrographs. We introduce the new RV search pipeline RVSearch that we used to generate our planet catalog, and we make it available to the public as an open-source Python package. This paper is the first study in a planned series that will measure exoplanet occurrence rates and compare exoplanet populations, including studies of giant planet occurrence beyond the water ice line, and eccentricity distributions to explore giant planet formation pathways.     Two Planets Straddling the Habitable Zone of The Nearby K dwarf Gl 414A

    We present the discovery of two planets orbiting the nearby (D = 11.9 pc) K7 dwarf Gl 414A. Gl 414A b is a sub-Neptune mass planet with M_b sin i_b = 9.28^(+3.19)_(−2.54) M_⊕ and a semi-major axis of 0.24 ± 0.01 au. Gl 414A c is a sub-Saturn mass planet with M_c sin i_c = 59.48^(+9.98)_(−9.69) M_⊕ and a semi-major axis of 1.43 ± 0.06 au. We jointly analyzed radial velocity data from Keck/HIRES and the Automated Planet Finder at Lick Observatory, as well as photometric data from KELT, to detect the two planets as well as two additional signals related to the rotationally-modulated activity and the long term magnetic activity cycle of the star. The outer planet in this system is a potential candidate for future direct imaging missions

    Ideologies implicated in IT innovation in government: a critical discourse analysis of Mexico’s international trade administration

    We develop a perspective of IT innovation in the public sector as a process that involves three complementary areas of ideology and concomitant dispute: first, the widespread view of e-government as a transformative force that leads to major improvements of public sector functions for the benefit of society at large; second, ideologies concerning the substantive policies enacted by public sector organizations; and third, ideology regarding public sector modernization. Our research examines how the objectives of IT projects and their actual effects in government are influenced by such ideologies and contestations that surround them. We develop our theoretical contribution with a critical discourse analysis that traces the ideological underpinnings of two consecutive IT projects for the administration of international trade in Mexico. This analysis associates the objectives of the IT projects with the emergence and ensuing contestation in Mexican politics of two ideologies: the first ideology concerns free international trade as imperative for economic development; the second ideology concerns public sector modernization that sought to overcome historically formed dysfunctionalities of public administration bureaucracies by adopting management practices from the private sector. The analysis then identifies the effects of the ideologically shaped IT projects on two key values of public administration: efficiency and legality. The insights of this research on the role of ideology in IT innovation complement organizational perspectives of e-government; socio-cognitive perspectives that focus on ideas and meaning, such as technology frames and organizing visions; and perspectives that focus on politics in IT innovation

    Intraoperative radiotherapy electron boost in advanced and recurrent epithelial ovarian carcinoma: a retrospective study

    <p>Abstract</p> <p>Background</p> <p>Relapses of epithelial ovarian carcinoma (EOC) have a poor prognosis and are almost always fatal. The aim of this study was to evaluate the clinical outcome and toxicity of intraoperative electron beam radiation therapy (IOERT) in advanced and recurrent EOC.</p> <p>Methods</p> <p>Forty-five women with EOC were treated with IOERT. Twenty-five patients had primary disease (PD) without distant metastasis at IOERT, and 20 patients had an isolated local recurrence (ILR) after surgery. All 45 patients in this series underwent optimal cytoreductive (≤ 1 cm) surgery. The whole pelvic (WP) radiotherapy was intraoperatively delivered using 12 Mev electron beam; 43 patients received 18-20 Gy and two patients received 10 Gy. Thirty-three patients received postoperateive intraperitoneal (IP) chemotherapy, while seven patients received intravenous (IV) chemotherapy. Five patients refused concurrent chemotherapy. Overall survival (OS) rates were analyzed using the Kaplan-Meier method.</p> <p>Results</p> <p>Tumor recurrence and metastasis were observed in 16 patients (35.6%). Of those, 14 patients (31.1%) relapsed and two patients (4.4%) had distant metastasis alone. Eight of 25 (32%) local failures were observed in the PD group, as compared to 6/20 (30%) in the ILR group (<it>P </it>= 0.885). Actuarial local control at five year follow-up was 31/45 (68.9%). Seventeen of the total 45 (37.8%) patients died. Nine of 25 (36%) in the PD group died, as compared to 8 of 20 (40%) in the ILR group. The 5-year OS and disease-free survival (DFS) rates were 28/45 (62.2%) and 25/45 (55.6%), respectively. In the PD group, the 5-year OS and DFS rates were 16/25 (64%) and 14/25 (56%) (<it>P </it>> 0.05, <it>vs</it>. the ILR group at 12/20 and 11/20, respectively). The OS and DFS in the IOERT plus IP group were 25/33 (75.8%) and 23/33 (69.7%), respectively, which were superior to the rates achieved with IOERT plus IV chemotherapy (<it>P </it>< 0.05, 2/7 and 1/7, respectively). The major complication of IOERT was neuropathy. Five (11.1%) patients developed peripheral neurotoxicity.</p> <p>Conclusions</p> <p>IOERT may be feasible and effective as a boosting technique for advanced and recurrent ovarian cancer. IOERT plus IP chemotherapy may achieve high locoregional disease control and survival benefit with a low risk of toxicity. Peripheral nerves in the IOERT field are dose-limiting structures requiring nerve protection policies or a dose compromise to ensure against severe neurological damage.</p
