    Professionalization of Public Health – an exploratory case study

    Introduction: Public health is continuously challenged by a shortage of workforce. There are various reasons for this: 1) public health is less visible than traditional health professions and people may be unfamiliar with the nature and opportunities involved in entering this career field; 2) lack of official recognition of public health as a professional category; and 3) no umbrella organization that supports its members and governs professional standards as is the case of other more established professions. To adequately address the challenges of public health for the 21st century, a key policy element will need to focus on adequately cultivating, training and growing the future workforce of professionals in the field. The aim of this study was to examine why professionalization of public health in Europe is not as robust as it deserves to be and what steps can be taken to assure an adequate supply of professionals with the proper education and training background, and career guidance to tackle the public health needs of the future. Method: A case study approach was used collecting data via a scoping literature review, a focus group with public health students and interviews with public health experts for convergence. Data was analysed using directed content analysis and pattern matching logic. Results: Public health fulfilled five out of seven attributes of a profession, such as skills, training and education, certification and an altruistic service. Recognition of Public Health as multidisciplinary and multi-professional field, derived from the interviews as an additional characteristic. A code of ethics and professional conduct and a formal organization were missing. Conclusion: Public health professionals and organisations that govern best practices in this field should consider introducing a shared code of ethics and professional conduct as well as establishing a coordinated body to help advance the public status as a the profession to increase interest in studying and specializing in this area.     Conflicts of interest: None declared. &nbsp

    The Transtheoretical model for fruit, vegetable and fish consumption: associations between intakes, stages of change and stage transition determinants

    BACKGROUND: Cardiovascular diseases are caused by multiple behavioral factors, including different dietary factors. We examined to what extent fruit, vegetable and fish consumption are related, and whether behavioral determinants vary across these dietary behaviors from a Transtheoretical model perspective. METHODS: Data were collected among 1142 participants (T0; response rate 46%) selected from an Internet panel, who were followed-up one-week later (T1; N = 1055, response rate 92%). Mean age was 35.4 (SD = 11.9) years, 35% was male, and most respondents were of Dutch origin (90%). Of the respondents, 13%, 44% and 43% had a low, medium or high level of education, respectively. Electronic questionnaires assessed fruit, vegetable and fish intake (food frequency questionnaires), stages of change, decisional balance and self-efficacy, for each of these three behaviors. RESULTS: Stages of change and (changes in) fruit, vegetable and fish intake were only weakly associated; decisional balance and self-efficacy were more strongly associated. Some presumed predictors of stage transitions were similar for fruit, vegetable, and fish intake, i.e., strong pros predicted progress out of precontemplators and low self-efficacy predicted relapse from action/maintenance for all behaviors. However, progress out of contemplation and out of preparation showed different patterns for fruit, vegetable and fish intake. CONCLUSION: The weak associations between intakes and potential determinants for fruit, vegetable, and fish consumption do not warrant an integrated dietary change approach targeting the same determinants for each behavior

    Simulation as a Key Training Method for Inculcating Public Health Leadership Skills: A Mixed Methods Study

    BACKGROUND: Successful management of public health challenges requires developing and nurturing leadership competencies. We aimed to evaluate the effectiveness of training simulations to assess public health leadership and decision-making competencies during emergencies as an effective learning and training method. METHODS: We examined the effects of two simulation scenarios on public health school students in terms of their experience (compared to face-to-face learning) and new skills acquired for dealing with similar emergent situations in the future. A mixed-methods design included developing a validated and pre-tested questionnaire with open-and closed-ended questions that examined the simulation impact and the degree of student satisfaction with the conditions in which it was conducted. Semi-structured in-depth interviews were conducted with the students after going through the simulations. The questionnaire results were evaluated using descriptive analytics. The interviews were analyzed using thematic analyses. All data were collected during June 2022. RESULTS: The questionnaire results indicate that students strengthened their interpersonal communication skills and learned about the importance of listening to the opinions of others before formulating their positions. Four themes emerged from 16 in-depth interviews, according to Kolb\u27s experimental learning cycle. Students emphasized the effectiveness of experiential learning versus traditional classroom learning. The simulation scenarios were felt to realistically convey critical issues regarding leadership, decision-making, and teamwork challenges. They effectively conveyed the importance of building a culture of conducting substantive and respectful discussions. CONCLUSION: Simulation is a powerful pedagogical training tool for public health leadership competencies. Simulations were seen to be advantageous over face-to-face learning in imparting a range of leadership skills and hands-on practice. We recommend integrating simulations in all public health leadership training programs

    Parental versus child reporting of fruit and vegetable consumption

    <p>Abstract</p> <p>Background</p> <p>The purpose of this study was to (1) compare parental and child recording of children's fruit and vegetable (F&V) consumption, including family-related factors, and (2) investigate the potential differences in the relation of children's and parents' perceptions of family-related factors.</p> <p>Methods</p> <p>Children were recruited from Dutch seventh and eighth grade classrooms. Each child and one of their parents completed parallel questionnaires. A total of 371 matched child-parent surveys were included in the analyses. To compare parental and child reports of consumption and family-related factors regarding F&V intake several techniques were used such as paired sample t-test, chi-square tests, Pearson's correlations and Cohens's kappa as measurement of agreement. To investigate potential differences between the parent's and children's perceptions of family-related factors, linear regression analyses were conducted.</p> <p>Results</p> <p>The results indicated weak agreement for F&V consumption (Cohen's kappa coefficients of .31 and .20, respectively) but no differences in mean consumption of fruit at the group level. Regarding the family-environmental factors related to fruit consumption, significant differences were found between the perceptions of subjective norm, and the availability and accessibility of fruit. Perceptions of subjective norm, parental modelling and exposure regarding vegetable consumption were also viewed differently by the two groups. The family-environmental factors reported by the children were similarly associated with F&V consumption compared to those reported by their respective parents. However, parents rated these factors more favourably than their children did.</p> <p>Conclusion</p> <p>The results indicated a low level of agreement between parental and child reporting of F&V intake and their assessment of family-environmental factors on individual level. This has important implications for the development and evaluation of interventions and we recommend that researchers clearly indicate which source of information they use in their studies as it remains unclear which source is more valid.</p> <p>However, when the effects of interventions are studied at the group level, our results suggest that it makes no difference whether children or parents report the child's fruit consumption. The same holds for determinant studies; both parental and child reports can be used. However, perceptions of these factors differ significantly.</p
