    Validation of p53 Immunohistochemistry (PAb240 Clone) in Canine Tumors with Next-Generation Sequencing (NGS) Analysis

    In human medicine, p53 immunohistochemistry (IHC) is a common method that is used for the identification of tumors with TP53 mutations. In veterinary medicine, several studies have performed IHC for p53 in canine tumors, but it is not known how well it actually predicts the mutation. The aim of this study was to estimate the accuracy of the IHC method for p53 (clone PAb240) using a lab-developed NGS panel to analyze TP53 mutations in a subset of malignant tumors in dogs. A total of 176 tumors were analyzed with IHC and then 41 were subjected to NGS analysis; among them, 15 were IHC positive and 26 were negative, and 16 out of 41 (39%) were found to be inadequate for NGS analysis. Excluding the non-evaluable cases at NGS, of the remaining eight IHC-positive cases, six were mutants and two were wild-type. Among the 17 IHC-negative cases, 13 were wild type, and 4 were mutants. The sensitivity was 60%, specificity was 86.7%, and the accuracy was 76%. These results suggest that when using IHC for p53 with this specific antibody to predict mutation, up to 25% wrong predictions can be expected

    Measuring habitual physical activity in adults with cystic fibrosis

    SummaryBackgroundThe aim of the present study was to determine whether different methods of recording physical activity (PA), i.e., accelerometers vs questionnaires, provided similar information in adults with cystic fibrosis (CF).Methods20 CF (age 33 ± 8SD yrs, FEV1 68 ± 16% predicted) and 11 age-matched healthy controls completed the Habitual Activity Estimation Scale (HAES) questionnaire and wore a biaxial accelerometer (SenseWear Pro3 Armband). Exercise tolerance was measured in CF.ResultsPatients had similar values in PA compared with controls. None of PA categories estimated by HAES questionnaire correlated with PA categories measured by the accelerometer; in CF the HAES questionnaire overestimated the effective levels of PA measured by the accelerometer. There were no differences between weekdays and weekend days PA levels provided by the accelerometer. In CF the questionnaire detected different time reported during the “somewhat inactive” and “somewhat active” categories (z = 2.651; p = 0.008; z = −2.651; p = 0.008), weekdays vs weekend; patients reported more time spent in activity (somewhat active & very active) during the weekend (z = −2.203; p = 0.02). Peak oxygen uptake correlated with accelerometer activities of “moderate” (>4.8 metabolic equivalents (METS)) and “vigorous” (>7.2 METS) intensity (r = 0.503, p = 0.02; r = 0.545, p = 0.01).ConclusionsIn adults with cystic fibrosis PA levels are better evaluated by the accelerometer and are similar to the controls. PA measured by the accelerometer is similar during the week and correlates with exercise tolerance

    Impact of clonally-related Burkholderia contaminans strains in two patients attending an Italian cystic fibrosis centre. A case report

    Background: Burkholderia contaminans is one of the 20 closely related bacterial of the Burkholderia cepacia complex, a group of bacteria that are ubiquitous in the environment and capable of infecting people with cystic fibrosis (CF). This species is an emerging pathogen and it has been widely isolated from CF patients in Argentina, Spain, Portugal, Australia, Canada, USA with a low prevalence in Ireland, France, Russia, Switzerland, Czech Republic, and Italy. This is the first report of B. contaminans affecting two Italian CF patients attending the same CF Centre. We correlate B. contaminans colonisation with lung function decline and co-infection with other clinically relevant CF pathogens. Case presentation: B. contaminans was identified by Multi Locus Sequence Typing in routine sputum analysis of two Caucasian CF women homozygous for Phe508del CFTR mutation. Sequence Type 102 was detected in both strains. It is known that B. contaminans ST102 was isolated both from CF and non-CF patients, with an intercontinental spread across the world. Random Amplified Polymorphic DNA analysis revealed the genetic relatedness between the two strains. We examined their susceptibility to antimicrobial agents, comparing the latter with that recorded for other B. contaminans isolated from different countries. We also described key virulence factors possibly linked with a clinical outcome. Specifically, we attempted to correlate colonization with the incidence of acute exacerbation of symptoms and lung function decline. Conclusions: This case presentation suggests that acquisition of B. contaminans ST102 is not directly associated with a lung function decline. We retain that the presence of other CF pathogens (i.e. MRSA and Trichosporon) along with B. contaminans ST102 might have contributed to the worsening of clinical conditions in our CF patients. The circumstances leading to the establishment of B. contaminans ST102 infections are still unknown.     Large Cell Neuroendocrine Carcinoma of the Lung: Current Understanding and Challenges

    Large cell neuroendocrine carcinoma of the lung (LCNEC) is a rare and highly aggres-sive type of lung cancer, with a complex biology that shares similarities with both small-cell lung cancer (SCLC) and non-small-cell lung cancer (NSCLC). The prognosis of LCNEC is poor, with a median overall survival of 8–12 months. The diagnosis of LCNEC requires the identification of neuroendocrine morphology and the expression of at least one of the neuroendocrine markers (chromogranin A, synaptophysin or CD56). In the last few years, the introduction of next-generation sequencing allowed the identification of molecular subtypes of LCNEC, with prognostic and potential therapeutic implications: one subtype is similar to SCLC (SCLC-like), while the other is similar to NSCLC (NSCLC-like). Because of LCNEC rarity, most evidence comes from small retrospective studies and treatment strategies that are extrapolated from those adopted in patients with SCLC and NSCLC. Nevertheless, limited but promising data about targeted therapies and immune checkpoint inhibitors in patients with LCNEC are emerging. LCNEC clinical management is still controversial and standardized treatment strategies are currently lacking. The aim of this manuscript is to review clinical and molecular data about LCNEC to better understand the optimal management and the potential prognostic and therapeutic implications of molecular subtypes

Bisulfite sequencing studies have identified 5-methylcytosine (5 mC) DNA methylation as one of the epigenetic changes that may be involved in this process. However, analysis of samples by bisulfite sequencing is a time-consuming procedure.     Correlation of molecular alterations with pathological features in hepatocellular carcinoma: Literature review and experience of an Italian center

    Hepatocellular carcinoma (HCC) represents the primary carcinoma of the liver and the fourth leading cause of cancer-related deaths. The World Health Organization estimates an increase in cases in the coming years. The risk factors of HCC are multiple, and the incidence in different countries is closely related to the different risk factors to which the population is exposed. The molecular mechanisms that drive HCC tumorigenesis are extremely complex, but understanding this multistep process is essential for the identification of diagnostic, prognostic, and therapeutic markers. The development of multigenic nextgeneration sequencing panels through the parallel analysis of multiple markers can provide a landscape of the genomic status of the tumor. Considering the literature and our preliminary data based on 36 HCCs, the most frequently altered genes in HCCs are TERT, CTNNB1, and TP53. Over the years, many groups have attempted to classify HCCs on a molecular basis, but a univocal classification has never been achieved. Nevertheless, statistically significant correlations have been found in HCCs between the molecular signature and morphologic features, and this leads us to think that it would be desirable to integrate the approach between anatomic pathology and molecular laboratories

    MutLα heterodimers modify the molecular phenotype of Friedreich ataxia

    This article has been made available through the Brunel Open Access Publishing Fund.Background: Friedreich ataxia (FRDA), the most common autosomal recessive ataxia disorder, is caused by a dynamic GAA repeat expansion mutation within intron 1 of FXN gene, resulting in down-regulation of frataxin expression. Studies of cell and mouse models have revealed a role for the mismatch repair (MMR) MutS-heterodimer complexes and the PMS2 component of the MutLα complex in the dynamics of intergenerational and somatic GAA repeat expansions: MSH2, MSH3 and MSH6 promote GAA repeat expansions, while PMS2 inhibits GAA repeat expansions. Methodology/Principal Findings: To determine the potential role of the other component of the MutLα complex, MLH1, in GAA repeat instability in FRDA, we have analyzed intergenerational and somatic GAA repeat expansions from FXN transgenic mice that have been crossed with Mlh1 deficient mice. We find that loss of Mlh1 activity reduces both intergenerational and somatic GAA repeat expansions. However, we also find that loss of either Mlh1 or Pms2 reduces FXN transcription, suggesting different mechanisms of action for Mlh1 and Pms2 on GAA repeat expansion dynamics and regulation of FXN transcription. Conclusions/Significance: Both MutLα components, PMS2 and MLH1, have now been shown to modify the molecular phenotype of FRDA.     Swin UNETR for Tumor and Lymph Node Segmentation Using 3D PET/CT Imaging:A Transfer Learning Approach

    Delineation of Gross Tumor Volume (GTV) is essential for the treatment of cancer with radiotherapy. GTV contouring is a time-consuming specialized manual task performed by radiation oncologists. Deep Learning (DL) algorithms have shown potential in creating automatic segmentations, reducing delineation time and inter-observer variation. The aim of this work was to create automatic segmentations of primary tumors (GTVp) and pathological lymph nodes (GTVn) in oropharyngeal cancer patients using DL. The organizers of the HECKTOR 2022 challenge provided 3D Computed Tomography (CT) and Positron Emission Tomography (PET) scans with ground-truth GTV segmentations acquired from nine different centers. Bounding box cropping was applied to obtain an anatomic based region of interest. We used the Swin UNETR model in combination with transfer learning. The Swin UNETR encoder weights were initialized by pre-trained weights of a self-supervised Swin UNETR model. An average Dice score of 0.656 was achieved on a test set of 359 patients from the HECKTOR 2022 challenge. Code is available at: https://github.com/HC94/swin_unetr_hecktor_2022.</p
