    Comparing in-person and webinar delivery of an immunization quality improvement program: a process evaluation of the adolescent AFIX trial

    Abstract Background Immunization quality improvement programs are often limited by the cost and inconvenience associated with delivering face-to-face consultations to primary care providers. To investigate a more efficient mode of intervention delivery, we conducted a process evaluation that compared in-person consultations to those delivered via interactive webinar. Methods The Centers for Disease Control and Prevention’s Assessment, Feedback, Incentives, and eXchange (AFIX) Program is an immunization quality improvement program implemented in all 50 states. In 2011, we randomly assigned 61 high-volume primary care clinics in North Carolina to receive an in-person or webinar AFIX consultation focused on adolescent immunization. We used surveys of participating vaccine providers and expense tracking logs to evaluate delivery modes on participation, satisfaction, and cost. Clinics served 71,874 patients, ages 11 to 18. Results Clinics that received in-person and webinar consultations reported similar levels of participation on key programmatic activities with one exception: more webinar clinics reported improving documentation of previously administered, ‘historical’ vaccine doses. Both in-person and webinar clinics showed sustained improvement in confidence to use reminder/recall systems (both p < 0.05). Participants rated delivery modes equally highly on satisfaction measures such as convenience (mean = 4.6 of 5.0). Delivery cost per clinic was 152forinpersonconsultationsversus152 for in-person consultations versus 100 for webinar consultations. Conclusions In-person and webinar delivery modes were both well received, but webinar AFIX consultations cost substantially less. Interactive webinar delivery shows promise for considerably extending the reach of immunization quality improvement programs. Trial registration Clinicaltrials.gov, NCT0154476

    The extracellular microenvironment is an integral component of normal and diseased tissues that is poorly understood owing to its complexity. To investigate the contribution of the microenvironment to lung fibrosis and adenocarcinoma progression, two pathologies characterized by excessive stromal expansion, we used mouse models to characterize the extracellular matrix (ECM) composition of normal lung, fibrotic lung, lung tumors, and metastases. Using quantitative proteomics, we identified and assayed the abundance of 113 ECM proteins, which revealed robust ECM protein signatures unique to fibrosis, primary tumors, or metastases. These analyses indicated significantly increased abundance of several S100 proteins, including Fibronectin and Tenascin-C (Tnc), in primary lung tumors and associated lymph node metastases compared with normal tissue. We further showed that Tnc expression is repressed by the transcription factor Nkx2-1, a well-established suppressor of metastatic progression. We found that increasing the levels of Tnc, via CRISPR-mediated transcriptional activation of the endogenous gene, enhanced the metastatic dissemination of lung adenocarcinoma cells. Interrogation of human cancer gene expression data revealed that high TNC expression correlates with worse prognosis for lung adenocarcinoma, and that a three-gene expression signature comprising TNC, S100A10, and S100A11 is a robust predictor of patient survival independent of age, sex, smoking history, and mutational load. Our findings suggest that the poorly understood ECM composition of the fibrotic and tumor microenvironment is an underexplored source of diagnostic markers and potential therapeutic targets for cancer patients

    Variation in Community Structure across Vertical Intertidal Stress Gradients: How Does It Compare with Horizontal Variation at Different Scales?

    In rocky intertidal habitats, the pronounced increase in environmental stress from low to high elevations greatly affects community structure, that is, the combined measure of species identity and their relative abundance. Recent studies have shown that ecological variation also occurs along the coastline at a variety of spatial scales. Little is known, however, on how vertical variation compares with horizontal variation measured at increasing spatial scales (in terms of sampling interval). Because broad-scale processes can generate geographical patterns in community structure, we tested the hypothesis that vertical ecological variation is higher than fine-scale horizontal variation but lower than broad-scale horizontal variation. To test this prediction, we compared the variation in community structure across intertidal elevations on rocky shores of Helgoland Island with independent estimates of horizontal variation measured at the scale of patches (quadrats separated by 10s of cm), sites (quadrats separated by a few m), and shores (quadrats separated by 100s to 1000s of m). The multivariate analyses done on community structure supported our prediction. Specifically, vertical variation was significantly higher than patch- and site-scale horizontal variation but lower than shore-scale horizontal variation. Similar patterns were found for the variation in abundance of foundation taxa such as Fucus spp. and Mastocarpus stellatus, suggesting that the effects of these canopy-forming algae, known to function as ecosystem engineers, may explain part of the observed variability in community structure. Our findings suggest that broad-scale processes affecting species performance increase ecological variability relative to the pervasive fine-scale patchiness already described for marine coasts and the well known variation caused by vertical stress gradients. Our results also indicate that experimental research aiming to understand community structure on marine shores should benefit from applying a multi-scale approach

    Rock Refuges Are Strongly Associated with Increased Urban Occupancy in the Western Fence Lizard, <i>Sceloporus occidentalis</i>

    Urbanization has dramatically altered habitats for local species worldwide. While some species are unable to meet the challenges that these alterations bring, others are able to persist as long as a threshold for suitable habitat is met. For reptiles, a key feature for persistence in urban areas can be access to suitable refuges from predation, high temperatures, and/or other environmental challenges. We tested for effects of local and landscape variables affecting urban occupancy in the Western Fence Lizard, Sceloporus occidentalis, in transects across an urban–rural gradient, with a specific focus on the presence of rock, tree, and shrub refuges. We found that fence lizards were much more likely to be present in areas with more rock cover, and in parks or low-density residential areas. Occupancy was also positively related to canopy cover in the general vicinity, though negatively related to number of trees along the transects. Our results highlight the importance of assessing local habitat features to successfully predict the occupancy of reptile species in urban habitats, and present directions for future research with concrete conservation and management applications

Factors affecting the use of different information sources to obtained precision farming information such as crop consultants, farm dealers, trade shows, Extension, and media are analyzed using a multivariate probit approach that accounts for simultaneous and/or correlation among the different information-source use decisions. Empirical findings suggest that the decision to use private, Extension, and/or Media information sources are correlated.     Precision Farming Information Sources Used by Cotton Farmers and Implications for Extension

    Cotton farmers in Alabama, Arkansas, Florida, Georgia, Louisiana, Mississippi, Missouri, North and South Carolina, Tennessee, and Virginia were asked where they obtained information about precision farming. Farmers use Extension as a source of precision farming information, but in combination with other sources, including media, crop consultants, trade shows, and other farmers. Farmers using Extension as a source of information tend to be younger, with more education and higher incomes, compared to producers not seeking information from Extension. Understanding the profiles of producers using Extension resources for precision farming information may help Extension design programs to improve information delivery to clients
