Nephrotic Syndrome in Children: From Bench to Treatment
Idiopathic nephrotic syndrome (INS) is the most frequent form of NS in children. INS is defined by the association of the clinical features of NS with renal biopsy findings of minimal changes, focal segmental glomerulosclerosis (FSGS), or mesangial proliferation (MP) on light microscopy and effacement of foot processes on electron microscopy. Actually the podocyte has become the favourite candidate for constituting the main part of the glomerular filtration barrier. Most cases are steroid sensitive (SSINS). Fifty percents of the latter recur frequently and necessitate a prevention of relapses by nonsteroid drugs. On the contrary to SSINS, steroid resistant nephrotic syndrome (SRINS) leads often to end-stage renal failure. Thirty to forty percents of the latter are associated with mutations of genes coding for podocyte proteins. The rest is due to one or several different circulating factors. New strategies are in development to antagonize the effect of the latter
Effectiveness of an extracellular matrix graft (OASIS Wound Matrix) in the treatment of chronic leg ulcers: A randomized clinical trial
BackgroundVenous leg ulcers are a major cause of morbidity, economic loss, and decreased quality of life in affected patients. Recently, biomaterials derived from natural tissue sources have been used to stimulate wound closure. One such biomaterial obtained from porcine small-intestine submucosa (SIS) has shown promise as an effective treatment to manage full-thickness wounds. Our objective was to compare the effectiveness of SIS wound matrix with compression vs compression alone in healing chronic leg ulcers within 12 weeks.MethodsThis was a prospective, randomized, controlled multicenter trial. Patients were 120 patients with at least 1 chronic leg ulcer. Patients were randomly assigned to receive either weekly topical treatment of SIS plus compression therapy (n = 62) or compression therapy alone (n = 58). Ulcer size was determined at enrollment and weekly throughout the treatment. Healing was assessed weekly for up to 12 weeks. Recurrence after 6 months was recorded. The primary outcome measure was the proportion of ulcers healed in each group at 12 weeks.ResultsAfter 12 weeks of treatment, 55% of the wounds in the SIS group were healed, as compared with 34% in the standard-care group (P = .0196). None of the healed patients treated with SIS wound matrix and seen for the 6-month follow-up experienced ulcer recurrence.ConclusionsThe SIS wound matrix, as an adjunct therapy, significantly improves healing of chronic leg ulcers over compression therapy alone

The purpose of this paper is to make the application as a whole that integrated the frontend and backend operations that help the process from ordering food, serving customer to managing restaurant data such as restaurant menus, employees, and transactions. The method used is the method of analysis (study of literature, analysis of similar applications, questionnaires, and interviews) and extreme programming design method. The results of this research is frontend application for food ordering and restaurant service and a backend system that has additional features such as restaurant profiles, restaurant menu data includes descriptions, recommended menuand images, promos, restaurant rating or menu rating, the waiter call feature, view bill and request features bill. The conclusion of this research is the developed application can help ordering process and restaurant servicesprocess in a restaurant with good customer satisfaction and good application performance
Excitation and decay of projectile-like fragments formed in dissipative peripheral collisions at intermediate energies
Projectile-like fragments (PLF:15<=Z<=46) formed in peripheral and
mid-peripheral collisions of 114Cd projectiles with 92Mo nuclei at E/A=50 MeV
have been detected at very forward angles, 2.1 deg.<=theta_lab<=4.2 deg.
Calorimetric analysis of the charged particles observed in coincidence with the
PLF reveals that the excitation of the primary PLF is strongly related to its
velocity damping. Furthermore, for a given V_PLF*, its excitation is not
related to its size, Z_PLF*. For the largest velocity damping, the excitation
energy attained is large, approximately commensurate with a system at the

Weather on Other Worlds. I. Detection of Periodic Variability in the L3 Dwarf DENIS-P J1058.7-1548 with Precise Multi-wavelength Photometry
Photometric monitoring from warm Spitzer reveals that the L3 dwarf DENIS-P J1058.7-1548 varies sinusoidally in brightness with a period of 4.25^(+0.26)_(-0.16) hr and an amplitude of 0.388% ± 0.043% (peak-to-valley) in the 3.6 μm band, confirming the reality of a 4.31 ± 0.31 hr periodicity detected in J-band photometry from the SOAR telescope. The J-band variations are a factor of 2.17 ± 0.35 larger in amplitude than those at 3.6 μm, while 4.5 μm Spitzer observations yield a 4.5 μm/3.6 μm amplitude ratio of only 0.23 ± 0.15, consistent with zero 4.5 μm variability. This wide range in amplitudes indicates rotationally modulated variability due to magnetic phenomena and/or inhomogeneous cloud cover. Weak Hα emission indicates some magnetic activity, but it is difficult to explain the observed amplitudes by magnetic phenomena unless they are combined with cloud inhomogeneities (which might have a magnetic cause). However, inhomogeneous cloud cover alone can explain all our observations, and our data align with theory in requiring that the regions with the thickest clouds also have the lowest effective temperature. Combined with published vsin (i) results, our rotation period yields a 95% confidence lower limit of R_* ≥ 0.111 R_☉, suggesting upper limits of 320 Myr and 0.055 M_☉ on the age and mass. These limits should be regarded cautiously because of ~3σ inconsistencies with other data; however, a lower limit of 45° on the inclination is more secure. DENIS-P J1058.7-1548 is only the first of nearly two dozen low-amplitude variables discovered and analyzed by the Weather on Other Worlds project
Evaluating and improving the Community Land Model's sensitivity to land cover
Modeling studies have shown the importance of biogeophysical effects of deforestation on local climate conditions but have also highlighted the lack of agreement across different models. Recently, remote-sensing observations have been used to assess the contrast in albedo, evapotranspiration (ET), and land surface temperature (LST) between forest and nearby open land on a global scale. These observations provide an unprecedented opportunity to evaluate the ability of land surface models to simulate the biogeophysical effects of forests. Here, we evaluate the representation of the difference of forest minus open land (i.e., grassland and cropland) in albedo, ET, and LST in the Community Land Model version 4.5 (CLM4.5) using various remote-sensing and in situ data sources. To extract the local sensitivity to land cover, we analyze plant functional type level output from global CLM4.5 simulations, using a model configuration that attributes a separate soil column to each plant functional type. Using the separated soil column configuration, CLM4.5 is able to realistically reproduce the biogeophysical contrast between forest and open land in terms of albedo, daily mean LST, and daily maximum LST, while the effect on daily minimum LST is not well captured by the model. Furthermore, we identify that the ET contrast between forests and open land is underestimated in CLM4.5 compared to observation-based products and even reversed in sign for some regions, even when considering uncertainties in these products. We then show that these biases can be partly alleviated by modifying several model parameters, such as the root distribution, the formulation of plant water uptake, the light limitation of photosynthesis, and the maximum rate of carboxylation. Furthermore, the ET contrast between forest and open land needs to be better constrained by observations to foster convergence amongst different land surface models on the biogeophysical effects of forests. Overall, this study demonstrates the potential of comparing subgrid model output to local observations to improve current land surface models' ability to simulate land cover change effects, which is a promising approach to reduce uncertainties in future assessments of land use impacts on climate
Neutron to proton ratios of quasiprojectile and midrapidity emission in the Zn + Zn reaction at 45 MeV/nucleon
Simultaneous measurement of both neutrons and charged particles emitted in
the reaction Zn + Zn at 45 MeV/nucleon allows comparison of the
neutron to proton ratio at midrapidity with that at projectile rapidity. The
evolution of N/Z in both rapidity regimes with increasing centrality is
examined. For the completely re-constructed midrapidity material one finds that
the neutron-to-proton ratio is above that of the overall Zn + Zn
system. In contrast, the re-constructed ratio for the quasiprojectile is below
that of the overall system. This difference provides the most complete evidence
to date of neutron enrichment of midrapidity nuclear matter at the expense of
the quasiprojectile
Construction and commissioning of a technological prototype of a high-granularity semi-digital hadronic calorimeter
A large prototype of 1.3m3 was designed and built as a demonstrator of the
semi-digital hadronic calorimeter (SDHCAL) concept proposed for the future ILC
experiments. The prototype is a sampling hadronic calorimeter of 48 units. Each
unit is built of an active layer made of 1m2 Glass Resistive Plate
Chamber(GRPC) detector placed inside a cassette whose walls are made of
stainless steel. The cassette contains also the electronics used to read out
the GRPC detector. The lateral granularity of the active layer is provided by
the electronics pick-up pads of 1cm2 each. The cassettes are inserted into a
self-supporting mechanical structure built also of stainless steel plates
which, with the cassettes walls, play the role of the absorber. The prototype
was designed to be very compact and important efforts were made to minimize the
number of services cables to optimize the efficiency of the Particle Flow
Algorithm techniques to be used in the future ILC experiments. The different
components of the SDHCAL prototype were studied individually and strict
criteria were applied for the final selection of these components. Basic
calibration procedures were performed after the prototype assembling. The
prototype is the first of a series of new-generation detectors equipped with a
power-pulsing mode intended to reduce the power consumption of this highly
granular detector. A dedicated acquisition system was developed to deal with
the output of more than 440000 electronics channels in both trigger and
triggerless modes. After its completion in 2011, the prototype was commissioned
using cosmic rays and particles beams at CERN.Comment: 49 pages, 41 figure
