    Cost-effectiveness of treatments for superficial venous refluxin patients with chronic venous ulceration.

    Background Venous leg ulcers impair quality of life significantly, with substantial costs to health services. The aim of this study was to estimate the cost‐effectiveness of interventional procedures alongside compression therapy versus compression therapy alone for the treatment of chronic venous leg ulceration. Methods A Markov decision analytical model was developed. The main outcome measures were quality‐adjusted life‐years (QALYs) and lifetime costs per patient, from the perspective of the UK National Health Service at 2015 prices. Resource use included the initial procedures, compression therapy, primary care and outpatient consultations. The interventional procedures included superficial venous surgery, endothermal ablation and ultrasound‐guided foam sclerotherapy (UGFS). The study population was patients with a chronic venous ulcer who were eligible for either compression therapy or an interventional procedure. Data were obtained from systematic review and meta‐analysis of RCTs. Results Surgery gained 0·112 (95 per cent c.i. −0·011 to 0·213) QALYs compared with compression therapy alone, with a difference in lifetime costs of €−1330 (−3570 to 1262). Given the expected savings in community care, the procedure would pay for itself within 4 years. There was insufficient evidence regarding endothermal ablation and UGFS to draw conclusions. Discussion This modelling study found surgery to be more effective and less costly than compression therapy alone. Further RCT evidence is required for both endothermal ablation and UGFS

    Primary succession on a seasonal tropical rocky shore: The relative roles of spatial heterogeneity and herbivory

    Hong Kong is within the tropics and has a seasonal climate. In winter, shores support patches of ephemeral macroalgae and areas of seemingly bare rock close to crevices where molluscan herbivores are abundant. Using a factorial design of herbivore exclusions in areas far and close to crevices, the development of algal assemblages was monitored in mid-shore, cleared areas, in winter. To estimate the role of herbivore mucus deposition, half the treatments received a mucus application. Algal development was estimated from macroalgal and biofilm development and chlorophyll a levels. In all areas, biofilms (diatoms, unicellular cyanobacteria) developed rapidly in herbivore exclusions followed by ephemeral macroalgae (Enteromorpha spp. and Porphyra suborbiculata). In herbivore access treatments, however, the algal assemblage was influenced by treatment location; few macroalgae developed in areas close to crevices, and the rock was dominated by cyanobacteria. A negative relationship between macroalgae and biofilms suggested that ephemeral algae were competitively dominant. In areas distant from herbivore refuges, ephemeral macroalgae did develop, illustrating that the effectiveness of molluscan herbivores was limited to 50 to 100 cm from these refuges. The absence of large herbivorous fish, and the sparse numbers of herbivorous crabs at this site, means that algae can achieve a spatial escape from consumption, and where this occurs competition between producers is important in assemblage development. Mucus appeared to play a limited role, only sometimes stimulating initial stages of unicellular cyanobacteria and macroalgae. With the onset of summer, macroalgae died back, and rock space became available for colonization. Unicellular cyanobacteria developed rapidly but were replaced in all treatments by the encrusting macroalga, Hapalospongidion gelatinosum, which dominated treatments until the end of the experiment. On seasonal, tropical shores processes influencing community structure can, therefore, be temporally variable and their relative importance, even at the same shore level, can change with season.published_or_final_versio

    Palliative Care in Dementia

    An optimal approach to palliative care for people with dementia has been defined by the European Association for Palliative Care with 11 domains including applicability of palliative care; person-centered care, communication, and shared decision-making; and setting care goals and advance care planning. Not all people with dementia will require specialist palliative care, and all involved in dementia care should be able to provide palliative care focusing on care and treatment which aims to increase the comfort and quality of life of the individual and supporting their family. There are many complications and symptoms which may arise for someone with dementia including increased infections, shortness of breath, swallowing difficulties, and pain which the individual may not be able to clearly express. These complications can lead to difficult decisions which need to be made by not only practitioners but also family caregivers as proxy. There should be a shared decision-making approach to these complications and symptoms, with advance care planning performed where possible. Caring for someone with dementia is one of the most difficult caring roles; support for family caregivers as part of a palliative approach is essential. Each person with dementia is different, and needs should be assessed on an individual basis, adopting a person-centered approach to care

This review examines how the subunit composition of heterologously expressed receptors determines their insecticide-specific pharmacology and how the pharmacology of expressed receptors differs from those found in the insect nervous system. We find that the insecticide-specific pharmacology of some receptors, like that containing subunits of the Rdl encoded GABA receptor, can be reconstituted with very few of the naturally occurring subunits expressed. In contrast, workers have struggled even to express functional insect nicotinic acetylcholine receptors (nAChRs), and work has therefore often relied upon the expression of vertebrate receptor subunits in their place. We also examine the extent to which insecticide-resistance-associated mutations, such as those in the para encoded voltage-gated sodium channel, can reveal details of insecticide-binding sites and mode of action. In particular, we examine whether mutations are present in the insecticide-binding site and/or at sites that allosterically affect the drug preferred conformation of the receptor. We also discuss the ryanodine receptor as a target for the recently developed diamides. Finally, we examine the lethality of the genes encoding these receptor subunits and discuss how this might determine the degree of conservation of the resistance-associated mutations found
