    Validation of a modified clinical risk score to predict cancer-specific survival for stage II colon cancer

    Many patients with stage II colon cancer will die of their disease despite curative surgery. Therefore, identification of patients at high risk of poor outcome after surgery for stage II colon cancer is desirable. This study aims to validate a clinical risk score to predict cancer-specific survival in patients undergoing surgery for stage II colon cancer. Patients undergoing surgery for stage II colon cancer in 16 hospitals in the West of Scotland between 2001 and 2004 were identified from a prospectively maintained regional clinical audit database. Overall and cancer-specific survival rates up to 5 years were calculated. A total of 871 patients were included. At 5 years, cancer-specific survival was 81.9% and overall survival was 65.6%. On multivariate analysis, age ≥75 years (hazard ratio (HR) 2.11, 95% confidence intervals (CI) 1.57–2.85; P<0.001) and emergency presentation (HR 1.97, 95% CI 1.43–2.70; P<0.001) were independently associated with cancer-specific survival. Age and mode of presentation HRs were added to form a clinical risk score of 0–2. The cancer-specific survival at 5 years for patients with a cumulative score 0 was 88.7%, 1 was 78.2% and 2 was 65.9%. These results validate a modified simple clinical risk score for patients undergoing surgery for stage II colon cancer. The combination of these two universally documented clinical factors provides a solid foundation for the examination of the impact of additional clinicopathological and treatment factors on overall and cancer-specific survival

    Design improvement of a pump wear ring labyrinth seal

    The investigation was successful in obtaining two improved designs for the impeller wear ring seal of the liquid hydrogen turbopump of interest. A finite difference computer code was extensively used in a parametric computational study in determining a cavity configuration with high flow resistance due to turbulence dissipation. These two designs, along with that currently used, were fabricated and tested. The improved designs were denoted Type O and Type S. The measurements showed that Type O and Type S given 67 and 30 percent reduction in leakage over the current design, respectively. It was found that the number of cavities, the step height and the presence of a small stator groove are quite important design features. Also, the tooth thickness is of some significance. Finally, the tooth height and an additional large cavity cut out from the stator (upstream of the step) are of negligible importance

    Systemic inflammation predicts all-cause mortality: a Glasgow Inflammation Outcome Study

    Introduction: Markers of the systemic inflammatory response, including C-reactive protein and albumin (combined to form the modified Glasgow Prognostic Score), as well as neutrophil, lymphocyte and platelet counts have been shown to be prognostic of survival in patients with cancer. The aim of the present study was to examine the prognostic relationship between these markers of the systemic inflammatory response and all-cause, cancer, cardiovascular and cerebrovascular mortality in a large incidentally sampled cohort.<p></p> Methods: Patients (n = 160 481) who had an incidental blood sample taken between 2000 and 2008 were studied for the prognostic value of C-reactive protein (>10mg/l, albumin (>35mg/l), neutrophil (>7.5×109/l) lymphocyte and platelet counts. Also, patients (n = 52 091) sampled following the introduction of high sensitivity C-reactive protein (>3mg/l) measurements were studied. A combination of these markers, to make cumulative inflammation-based scores, were investigated.<p></p> Results: In all patients (n = 160 481) C-reactive protein (>10mg/l) (HR 2.71, p<0.001), albumin (>35mg/l) (HR 3.68, p<0.001) and neutrophil counts (HR 2.18, p<0.001) were independently predictive of all-cause mortality. These associations were also observed in cancer, cardiovascular and cerebrovascular mortality before and after the introduction of high sensitivity C-reactive protein measurements (>3mg/l) (n = 52 091). A combination of high sensitivity C-reactive protein (>3mg/l), albumin and neutrophil count predicted all-cause (HR 7.37, p<0.001, AUC 0.723), cancer (HR 9.32, p<0.001, AUC 0.731), cardiovascular (HR 4.03, p<0.001, AUC 0.650) and cerebrovascular (HR 3.10, p<0.001, AUC 0.623) mortality. Conclusion The results of the present study showed that an inflammation-based prognostic score, combining high sensitivity C-reactive protein, albumin and neutrophil count is prognostic of all-cause mortality

    Bird Monitoring at Homestead National Monument of America, Nebraska, Status Report 2009–2017

    Executive Summary In 2009, the Heartland Inventory Monitoring Network initiated breeding bird surveys on Homestead National Monument of America, Nebraska, to address two objectives: (1) to monitor changes in bird community composition and abundance, and (2) to improve our understanding of relationships between breeding birds and habitat, and the effects of management actions on those relationships. This report evaluates trends in the park’s breeding bird populations in the context of trends observed within the North American Bird Conservation Initiative’s (NABCI) Central Mixed Grass Prairie Bird Conservation Region, the region in which the park is located. By doing so, we can assess the influence of park habitat management on bird populations with an understanding of regional population trends that are outside the influence of natural resource management activities at the Homestead national Monument of America. Eighty-six species of birds were recorded during May and June site visits in the nine years since initiating monitoring. Seventy-four of the species are considered breeding species because they are permanent or summer residents. Two of the breeding species recorded on Homestead National Monument of America are species of concern for the Central Mixed Grass Prairie Bird Conservation Region. Thirteen species were observed during the survey period in sufficient numbers to calculate annual abundances and trends with some degree of confidence. The American Goldfinch (Spinus tristis), Dickcissel (Spiza americana), House Wren (Troglodytes aedon), and Red-winged Blackbird (Agelaius phoeniceus) were the most abundant and widespread species on Homestead National Monument of America. Comparing population trends of the thirteen most recorded species on the park with regional trends for the Central Mixed Grass Prairie Bird Conservation Region were inconclusive, but suggest that trends in populations on the park were similar to trends in populations found region-wide for most species present. However, some differences in population trends were observed between the park and the region. This report provides current regional and local trends for breeding birds for future comparisons with bird data collected as part of the long-term monitoring efforts at Homestead National Monument of America. This information will help park staff plan management objectives, and assess the effectiveness of management alternatives. These monitoring data also provide park staff with additional information for interpreting natural resources

Comparing population trends on the park with regional trends suggest that populations of the most common species on the park were similar to those of the region, especially for American Goldfinch (Spinus tristis), Common Grackle (Quiscalus quiscula), Common Yellowthroat (Geothlypis trichas), Dickcissel (Spiza americana), and Mourning Dove (Zenaida macroura). The population of Eastern Meadowlark (Sturnella magna) on the park was doing slightly better than the population region-wide. One of the species that was common and widespread on the park, American Robin, had an uncertain park population trend, but a region-wide trend that was increasing. The other common and widespread species, Red-winged Blackbird, had a population trend that was declining both on the park and across the region. This report provides current regional and local trends for breeding birds for future comparisons with bird data collected as part of the long-term monitoring efforts at Herbert Hoover National Historic Site. This information will help park staff plan management objectives and assess the effectiveness of management alternatives.     Quahogs in Eastern North America: Part II, History by Province and State

    The northern quahog, Mercenaria mercenaria, ranges along the Atlantic Coast of North America from the Canadian Maritimes to Florida, while the southern quahog, M. campechiensis, ranges mostly from Florida to southern Mexico. The northern quahog was fished by native North Americans during prehistoric periods. They used the meats as food and the shells as scrapers and as utensils. The European colonists copied the Indians treading method, and they also used short rakes for harvesting quahogs. The Indians of southern New England and Long Island, N.Y., made wampum from quahog shells, used it for ornaments and sold it to the colonists, who, in turn, traded it to other Indians for furs. During the late 1600’s, 1700’s, and 1800’s, wampum was made in small factories for eventual trading with Indians farther west for furs. The quahoging industry has provided people in many coastal communities with a means of earning a livelihood and has given consumers a tasty, wholesome food whether eaten raw, steamed, cooked in chowders, or as stuffed quahogs. More than a dozen methods and types of gear have been used in the last two centuries for harvesting quahogs. They include treading and using various types of rakes and dredges, both of which have undergone continuous improvements in design. Modern dredges are equipped with hydraulic jets and one type has an escalator to bring the quahogs continuously to the boats. In the early 1900’s, most provinces and states established regulations to conserve and maximize yields of their quahog stocks. They include a minimum size, now almost universally a 38-mm shell width, and can include gear limitations and daily quotas. The United States produces far more quahogs than either Canada or Mexico. The leading producer in Canada is Prince Edward Island. In the United States, New York, New Jersey, and Rhode Island lead in quahog production in the north, while Virginia and North Carolina lead in the south. Connecticut and Florida were large producers in the 1990’s. The State of Tabasco leads in Mexican production. In the northeastern United States, the bays with large openings, and thus large exchanges of bay waters with ocean waters, have much larger stocks of quahogs and fisheries than bays with small openings and water exchanges. Quahog stocks in certified beds have been enhanced by transplanting stocks to them from stocks in uncertified waters and by planting seed grown in hatcheries, which grew in number from Massachusetts to Florida in the 1980’s and 1990’s

    Metazoans of redoxcline sediments in Mediterranean deep-sea hypersaline anoxic basins

    Background: The deep-sea hypersaline anoxic basins (DHABs) of the Mediterranean (water depth similar to 3500 m) are some of the most extreme oceanic habitats known. Brines of DHABs are nearly saturated with salt, leading many to suspect they are uninhabitable for eukaryotes. While diverse bacterial and protistan communities are reported from some DHAB haloclines and brines, loriciferans are the only metazoan reported to inhabit the anoxic DHAB brines. Our goal was to further investigate metazoan communities in DHAB haloclines and brines. Results: We report observations from sediments of three DHAB (Urania, Discovery, L'Atalante) haloclines, comparing these to observations from sediments underlying normoxic waters of typical Mediterranean salinity. Due to technical difficulties, sampling of the brines was not possible. Morphotype analysis indicates nematodes are the most abundant taxon; crustaceans, loriciferans and bryozoans were also noted. Among nematodes, Daptonema was the most abundant genus; three morphotypes were noted with a degree of endemicity. The majority of rRNA sequences were from planktonic taxa, suggesting that at least some individual metazoans were preserved and inactive. Nematode abundance data, in some cases determined from direct counts of sediments incubated in situ with CellTracker (TM) Green, was patchy but generally indicates the highest abundances in either normoxic control samples or in upper halocline samples; nematodes were absent or very rare in lower halocline samples. Ultrastructural analysis indicates the nematodes in L'Atalante normoxic control sediments were fit, while specimens from L'Atalante upper halocline were healthy or had only recently died and those from the lower halocline had no identifiable organelles. Loriciferans, which were only rarely encountered, were found in both normoxic control samples as well as in Discovery and L'Atalante haloclines. It is not clear how a metazoan taxon could remain viable under this wide range of conditions. Conclusions: We document a community of living nematodes in normoxic, normal saline deep-sea Mediterranean sediments and in the upper halocline portions of the DHABs. Occurrences of nematodes in mid-halocline and lower halocline samples did not provide compelling evidence of a living community in those zones. The possibility of a viable metazoan community in brines of DHABs is not supported by our data at this time
