    Examination of offsite radiological emergency protective measures for nuclear reactor accidents involving core melt

Evaluations were conducted using a modified version of the Reactor Safety Study consequence model. Models representing each measure were developed and are discussed. Potential PWR core-melt radioactive material releases are separated into two categories, "Melt-through" and "Atmospheric," based upon the mode of containment failure. Protective measures are examined and compared for each category in terms of projected doses to the whole body and thyroid. Measures for "Atmospheric" accidents are also examined in terms of their influence on the occurrence of public health effects. For "Melt-through" accidents, few, if any, early public health effects are likely, and doses in excess of Protective Action Guides (PAGs) are "confined" to areas within 10 miles of the reactor.Evacuation appears to provide the largest reduction in whole body dose for this category. However, sheltering, particularly when basements are readily available, may be an acceptable alternative. Both evacuation and iodine prophylaxis can substantially reduce the dose to the thyroid. For "Atmospheric" accidents, PAGs are likely to be exceeded at very large distances, and significant numbers of early public health effects are possible. However, most early fatalities occur within 10 miles of the reactor. Within 5 miles, evacuation appears to be more effective than sheltering in reducing the number of early health effects. Beyond 5 miles, this distinction is less, or not, apparent. Within 10 miles, early health effects are strongly influenced by the speed and efficiency with which protective measures are implemented. Outside of 10 miles, they are not.The projected total number of thyroid nodules is not substantially reduced unless iodine prophylaxis is administered over very large areas (distances).     HIV-1 Evolutionary Patterns Associated with Metastatic Kaposi's Sarcoma during AIDS.

    Kaposi's sarcoma (KS) in HIV-infected individuals can have a wide range of clinical outcomes, from indolent skin tumors to a life-threatening visceral cancer. KS tumors contain endothelial-related cells and inflammatory cells that may be HIV-infected. In this study we tested if HIV evolutionary patterns distinguish KS tumor relatedness and progression. Multisite autopsies from participants who died from HIV-AIDS with KS prior to the availability of antiretroviral therapy were identified at the AIDS and Cancer Specimen Resource (ACSR). Two patients (KS1 and KS2) died predominantly from non-KS-associated disease and KS3 died due to aggressive and metastatic KS within one month of diagnosis. Skin and visceral tumor and nontumor autopsy tissues were obtained (n = 12). Single genome sequencing was used to amplify HIV RNA and DNA, which was present in all tumors. Independent HIV tumor clades in phylogenies differentiated KS1 and KS2 from KS3, whose sequences were interrelated by both phylogeny and selection. HIV compartmentalization was confirmed in KS1 and KS2 tumors; however, in KS3, no compartmentalization was observed among sampled tissues. While the sample size is small, the HIV evolutionary patterns observed in all patients suggest an interplay between tumor cells and HIV-infected cells which provides a selective advantage and could promote KS progression

    Demographic and Psychiatric Correlates of Compulsive Sexual Behaviors in Gambling Disorder

    Background and aims: Gambling disorder (GD) and compulsive sexual behavior (CSB) may commonly co-occur. Yet, the psychiatric correlates of these co-occurring disorders are an untapped area of empirical scrutiny, limiting our understanding of appropriate treatment modalities for this dual-diagnosed population. This study examined the demographic and clinical correlates of CSB in a sample of treatment-seeking individuals with GD (N = 368) in São Paulo, Brazil. Methods: Psychiatrists and psychologists conducted semi-structured clinical interviews to identify rates of CSB and other comorbid psychiatric disorders. The Shorter PROMIS Questionnaire was administered to assess additional addictive behaviors. The TCI and BIS-11 were used to assess facets of personality. Demographic and gambling variables were also assessed. Results: Of the total sample, 24 (6.5%) met diagnostic criteria for comorbid CSB (GD + CSB). Compared to those without compulsive sexual behaviors (GD − CSB), individuals with GD + CSB were more likely to be younger and male. No differences in gambling involvement emerged. Individuals with GD + CSB tended to have higher rates of psychiatric disorders (depression, post-traumatic stress disorder, and bulimia nervosa) and engage in more addictive behaviors (problematic alcohol use, drug use, and exercise) compared to GD − CSB. Those with GD + CSB evidenced less self-directedness, cooperativeness, self-transcendence, and greater motor impulsivity. Logistic regression showed that the predictors of GD + CSB, which remained in the final model, were being male, a diagnosis of bulimia, greater gambling severity, and less self-transcendence. Discussion and conclusion: Given those with GD + CSB evidence greater psychopathology, greater attention should be allocated to this often under studied comorbid condition to ensure adequate treatment opportunities

    AbstractOver the past three decades, polymeric gas separation membranes have become widely used for a variety of industrial gas separations applications. This review presents the fundamental scientific principles underpinning the operation of polymers for gas separations, including the solution-diffusion model and various structure/property relations, describes membrane fabrication technology, describes polymers believed to be used commercially for gas separations, and discusses some challenges associated with membrane materials development. A description of new classes of polymers being considered for gas separations, largely to overcome existing challenges or access applications that are not yet practiced commercially, is also provided. Some classes of polymers discussed in this review that have been the focus of much recent work include thermally rearranged (TR) polymers, polymers of intrinsic microporosity (PIMs), room-temperature ionic liquids (RTILs), perfluoropolymers, and high-performance polyimides

    Botulinum toxin type A injections for the management of muscle tightness following total hip arthroplasty: a case series

    <p>Abstract</p> <p>Background</p> <p>Development of hip adductor, tensor fascia lata, and rectus femoris muscle contractures following total hip arthroplasties are quite common, with some patients failing to improve despite treatment with a variety of non-operative modalities. The purpose of the present study was to describe the use of and patient outcomes of botulinum toxin injections as an adjunctive treatment for muscle tightness following total hip arthroplasty.</p> <p>Methods</p> <p>Ten patients (14 hips) who had hip adductor, abductor, and/or flexor muscle contractures following total arthroplasty and had been refractory to physical therapeutic efforts were treated with injection of botulinum toxin A. Eight limbs received injections into the adductor muscle, 8 limbs received injections into the tensor fascia lata muscle, and 2 limbs received injection into the rectus femoris muscle, followed by intensive physical therapy for 6 weeks.</p> <p>Results</p> <p>At a mean final follow-up of 20 months, all 14 hips had increased range in the affected arc of motion, with a mean improvement of 23 degrees (range, 10 to 45 degrees). Additionally all hips had an improvement in hip scores, with a significant increase in mean score from 74 points (range, 57 to 91 points) prior to injection to a mean of 96 points (range, 93 to 98) at final follow-up. There were no serious treatment-related adverse events.</p> <p>Conclusion</p> <p>Botulinum toxin A injections combined with intensive physical therapy may be considered as a potential treatment modality, especially in difficult cases of muscle tightness that are refractory to standard therapy.</p
