    Are antipsychotic prescribing patterns different in older and younger adults? : a survey of 1357 psychiatric inpatients in Toronto

    Objective: To compare antipsychotic prescribing patterns in younger (aged 59 years or younger) and older (aged 60 years or older) patients with psychotic or mood disorders. Method: Pharmacy records of all patients discharged from the Centre for Addiction and Mental Health over a 21-month period were reviewed. A total of 1357 patients who were prescribed an antipsychotic at the time of their discharge were included in the analysis (956 with a primary psychotic disorder and 401 with a primary mood disorder). World Health Organization-defined daily doses were used as the standardized dosing unit. Results: Both in patients with a primary psychotic disorder and in patients with a primary mood disorder, the prescribing patterns were similar in older and younger patients, with no statistical difference in the proportions receiving first-generation antipsychotics, second-generation antipsychotics (SGAs), multiple antipsychotics, or long-acting (depot) antipsychotics. Overall, the mean daily antipsychotic doses were lower only in the older group of patients with a primary mood disorder. However, the mean dose of SGAs was about 30% lower in older patients in both diagnostic groups. Regardless of age, patients with a mood disorder were prescribed lower doses of antipsychotics than those with a psychotic disorder. Conclusions: Our data suggest that older patients are prescribed lower antipsychotic dosages primarily when using SGAs. This finding emphasizes the need for dose-finding studies assessing both the efficacy and the safety of antipsychotics in older patients with a psychotic or mood disorder.peer-reviewe

    Physical (and weak) regularity conditions are used to determine and classify all the possible types of spherically symmetric dust spacetimes in general relativity. This work unifies and completes various earlier results. The junction conditions are described for general non-comoving (and non-null) surfaces, and the limits of kinematical quantities are given on all comoving surfaces where there is Darmois matching. We show that an inhomogeneous generalisation of the Kantowski-Sachs metric may be joined to the Lemaitre-Tolman-Bondi metric. All the possible spacetimes are explicitly divided into four groups according to topology, including a group in which the spatial sections have the topology of a 3-torus.     Magnetic Coherence as a Universal Feature of Cuprate Superconductors

    Recent inelastic neutron scattering (INS) experiments on La2−x_{2-x}Srx_xCuO4_4 have established the existence of a {\it magnetic coherence effect}, i.e., strong frequency and momentum dependent changes of the spin susceptibility, χ′′\chi'', in the superconducting phase. We show, using the spin-fermion model for incommensurate antiferromagnetic spin fluctuations, that the magnetic coherence effect establishes the ability of INS experiments to probe the electronic spectrum of the cuprates, in that the effect arises from the interplay of an incommensurate magnetic response, the form of the underlying Fermi surface, and the opening of the d-wave gap in the fermionic spectrum. In particular, we find that the magnetic coherence effect observed in INS experiments on La2−x_{2-x}Srx_xCuO4_4 requires that the Fermi surface be closed around (π,π)(\pi,\pi) up to optimal doping. We present several predictions for the form of the magnetic coherence effect in YBa2_2Cu3_3O6+x_{6+x} in which an incommensurate magnetic response has been observed in the superconducting state.Comment: 9 pages, 12 figures; extended version of Phys.     A Fully-Flexible Solution-Processed Autonomous Glucose Indicator

    We present the first demonstration of a fully-flexible, self-powered glucose indicator system that synergizes two flexible electronic technologies: a flexible self-powering unit in the form of a biofuel cell, with a flexible electronic device - a circuit-board decal fabricated with biocompatible microbial nanocellulose. Our proof-of-concept device, comprising an enzymatic glucose fuel cell, glucose sensor and a LED indicator, does not require additional electronic equipment for detection or verification; and the entire structure collapses into a microns-thin, self-adhering, single-centimeter-square decal, weighing less than 40 mg. The flexible glucose indicator system continuously operates a light emitting diode (LED) through a capacitive charge/discharge cycle, which is directly correlated to the glucose concentration. Our indicator was shown to operate at high sensitivity within a linear glucose concentration range of 1 mM-45 mM glucose continuously, achieving a 1.8 VDC output from a flexible indicator system that deliver sufficient power to drive an LED circuit. Importantly, the results presented provide a basis upon which further development of indicator systems with biocompatible diffusing polymers to act as buffering diffusion barriers, thereby allowing them to be potentially useful for low-cost, direct-line-of-sight applications in medicine, husbandry, agriculture, and the food and beverage industries

    The Geography of the International System: The CShapes Dataset

    We describe CShapes, a new dataset that provides historical maps of state boundaries and capitals in the post-World War II period. The dataset is coded according to both the Correlates of War and the Gleditsch and Ward (1999) state lists, and is therefore compatible with a great number of existing databases in the discipline. Provided in a geographic data format, CShapes can be used directly with standard GIS software, allowing a wide range of spatial computations. In addition, we supply a CShapes package for the R statistical toolkit. This package enables researchers without GIS skills to perform various useful operations on the GIS maps. The paper introduces the CShapes dataset and structure and gives three examples of how to use CShapes in political science research. First, we show how results from quantitative analysis can be depicted intuitively as a map. The second application gives an example of computing indicators on the CShapes maps, which can then be used in statistical tests. Third, we illustrate the use of CShapes for generating different weights matrices in spatial statistical applications. All the examples can be replicated using the freely available R package and do not require specialized GIS skills.     Attitudes of healthcare workers in U.S. hospitals regarding smallpox vaccination

    BACKGROUND: The United States is implementing plans to immunize 500,000 hospital-based healthcare workers against smallpox. Vaccination is voluntary, and it is unknown what factors drive vaccine acceptance. This study's aims were to estimate the proportion of workers willing to accept vaccination and to identify factors likely to influence their decisions. METHODS: The survey was conducted among physicians, nurses, and others working primarily in emergency departments or intensive care units at 21 acute-care hospitals in 10 states during the two weeks before the U.S. national immunization program for healthcare workers was announced in December 2002. Of the questionnaires distributed, 1,165 were returned, for a response rate of 81%. The data were analyzed by logistic regression and were adjusted for clustering within hospital and for different number of responses per hospital, using generalized linear mixed models and SAS's NLMIXED procedure. RESULTS: Sixty-one percent of respondents said they would definitely or probably be vaccinated, while 39% were undecided or inclined against it. Fifty-three percent rated the risk of a bioterrorist attack using smallpox in the United States in the next two years as either intermediate or high. Forty-seven percent did not feel well-informed about the risks and benefits of vaccination. Principal concerns were adverse reactions and the risk of transmitting vaccinia. In multivariate analysis, four variables were associated with willingness to be vaccinated: perceived risk of an attack, self-assessed knowledge about smallpox vaccination, self-assessed previous smallpox vaccination status, and gender. CONCLUSIONS: The success of smallpox vaccination efforts will ultimately depend on the relative weight in people's minds of the risk of vaccine adverse events compared with the risk of being exposed to the disease. Although more than half of the respondents thought the likelihood of a bioterrorist smallpox attack was intermediate or high, less than 10% of the group slated for vaccination has actually accepted it at this time. Unless new information about the threat of a smallpox attack becomes available, healthcare workers' perceptions of the vaccine's risks will likely continue to drive their ongoing decisions about smallpox vaccination

    Impact of air pollution on educational attainment for respiratory health treated students: A cross sectional data linkage study

    Introduction: There is some evidence that exam results are worse when students are acutely exposed to air pollution. Studies investigating the association between air pollution and academic attainment have been constrained by small sample sizes. Methods: Cross sectional educational attainment data (2009–2015) from students aged 15–16 years in Cardiff, Wales were linked to primary health care data, modelled air pollution and measured pollen data, and analysed using multilevel linear regression models. Annual cohort, school and individual level confounders were adjusted for in single and multi-pollutant/pollen models. We stratified by treatment of asthma and/or Seasonal Allergic Rhinitis (SAR). Results: A unit (10μg/m3) increase of short-term exposure to NO2 was associated with 0.044 (95% CI: −0.079, −0.008) reduction of standardised Capped Point Score (CPS) after adjusting for individual and household risk factors for 18,241 students. This association remained statistically significant after controlling for other pollutants and pollen. There was no association of PM2.5, O3, or Pollen with standardised CPS remaining after adjustment. We found no evidence that treatment for asthma or SAR modified the observed NO2 effect on educational attainment. Conclusion: Our study showed that short-term exposure to traffic-related air pollution, specifically NO2, was associated with detrimental educational attainment for students aged 15–16. Longitudinal investigations in different settings are required to confirm this possible impact and further work may uncover the long-term economic implications, and degree to which impacts are cumulative and permanent
