Gold versus Palladium: A Regioselective Cycloisomerization of Aromatic Enynes
International audienceAromatic enynes can be transformed into arylnaphthalenes or benzofulvenes depending on the reaction conditions. Under gold(I) catalysis, exclusive or major 6-endo-dig cyclization took place leading to arylnaphthalenes. However, a catalytic system based on palladium iodide/1,3-Bis(diphenylphosphino)propane, in the presence of cesium carbonate as a base was necessary to furnish exclusively 5-exo-dig cyclization pattern, regardless to the electronic effects of the substituents. In the latter transformation, a mechanistic study (Kinetic Isotopic Effect, Density Functional Theory) involving a C-H activation is suggested for the exclusive benzofulvenes formation
InCoder: A Generative Model for Code Infilling and Synthesis
Code is seldom written in a single left-to-right pass and is instead
repeatedly edited and refined. We introduce InCoder, a unified generative model
that can perform program synthesis (via left-to-right generation) as well as
editing (via infilling). InCoder is trained to generate code files from a large
corpus of permissively licensed code, where regions of code have been randomly
masked and moved to the end of each file, allowing code infilling with
bidirectional context. Our model is the first generative model that is able to
directly perform zero-shot code infilling, which we evaluate on challenging
tasks such as type inference, comment generation, and variable re-naming. We
find that the ability to condition on bidirectional context substantially
improves performance on these tasks, while still performing comparably on
standard program synthesis benchmarks in comparison to left-to-right only
models pretrained at similar scale. The InCoder models and code are publicly

Supporting positive patient experiences for rare disease care during disruptive times: findings from a multicentre study
Introduction: We describe the perceptions and experiences of health care services during the COVID-19 pandemic of those newly diagnosed with a rare, chronic inflammatory eye disorder.
Methods: We undertook a cross-sectional study nested within a longitudinal multi-centre inception cohort study. Participants were families and young people (aged under 18 years) newly diagnosed with childhood uveitis. Using a validated tool, the Health Foundation COVID-19 Survey, we captured qualitative and quantitative data. Quantitative data were analysed using descriptive summary statistics. Qualitative, free text responses were analysed using content and thematic analysis.
Results: Responses received from 60 families between 1st September 2020 and 30th March 2022 were analysed. Despite two in five reporting challenges in accessing medication, the majority felt comfortable accessing healthcare services (8%, 95% CI 3 - 18%, of participants expressed discomfort, versus 28%, 95% CI 26 – 28% of general population). Thematic analysis identified five themes: the value of protected spaces to safely access care; the positive role of digital health tools the experience of immature telemedicine; disintegration of care; and dealing with uncertainty.
Discussion: Our findings suggest that families of children with a rare chronic condition had greater confidence in accessing healthcare during the pandemic than the general population. Nevertheless, to ensure more robust health services for such populations in future times of disruption, developments in telemedicine should be directly informed by the experiences of those with rare disease. The development of new healthcare processes which ensure the whole healthcare team has adequate information to support families should be prioritised

Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19
IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19.
Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19.
DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022).
INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days.
MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes.
RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively).
CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.

Time integrated violation in decays (LHCb)
International audienceCKM (Cabibbo-Kobayashi-Maskawa) mechanism, describing the coupling between up-type and down-type quarks through the weak interaction, is the main CP violation contribution to the quark sector of the Standard Model (SM). In particular, the precise measurement of the CKM angle sets a benchmark for CP violation. A discrepancy between direct and indirect measurements would exhibit a strong evidence of physics beyond the SM. The CKMfitter group has notably proven that a precision on direct measurement would lead to test SM up to dozens of TeV. In this document, we present the last LHCb combination of its various measurements, which is currently the most precise determination of from a single experiment. Two new analyses are also presented, the first one measuring CP violation in the decays and the second one measuring branching fractions of and decays, which can later lead to a measurement with those channels
Diseño urbano arquitectónico en los mercados Amazonas, La Playita y zonas aledañas
El trabajo de fin de carrera consiste en la regeneración urbana – arquitectónico en el predio actual del Mercado Amazonas y su área de influencia en la ciudad de Ibarra. El proyecto se desarrolla en el lote del Mercado Amazonas que se localiza en el sector noreste de un extenso terreno cercano a la estación del tren. El crecimiento acelerado y desordenado, así como falta de visión y planificación por parte de los niveles de decisión política evidencian actualmente la situación de un sistema de abastecimiento y comercialización deficiente. Los puntos mencionados anteriormente no representan a las necesidades de la ciudad de reducir los impactos negativos que la zona comercial genera en la misma. Zona que además se apropia del espacio público y la contamina permanentemente con ruido, desechos sólidos no biodegradables. Se plantea un escenario futuro con el diseño de un espacio público y un edificio adecuado para la comercialización de productos de alimentación. Lo cual convertiría al sector en un punto de referencia, interacción y de encuentro social de la ciudad. Además, potenciaría la zona desde otro punto de vista y brindaría un dinamismo dotado de espacios públicos y culturales que ayuden al mejoramiento del entorno urbano y a mantener la identidad comercial. El trabajo se desarrolla en seis capítulos que serán explicados en el presente trabajo de titulación: fundamentación del tema, marco teórico, diagnóstico y la propuesta.The final project consists of the urban-architectural intervention in the current site of the Amazonas Market and its area of influence in the city of Ibarra. The project is developed on the Amazonas Market lot located in the northeast sector of a large plot of land near the train station. The accelerated and disorderly growth, as well as the lack of vision and planning on the part of the political decision-making levels, are evidence of the current situation of a deficient supply and commercialization system. The mentioned issues do not represent the needs of the city to reduce the negative impacts that the commercial zone generates on it. This area also appropriates public space and pollutes it permanently with noise, solid and non-biodegradable waste. A future scenario is proposed with the design of a public space and a suitable building for the commercialization of food. This would turn the sector into a point of reference, interaction, and social meeting space in the city. Moreover, it would enhance the area from another point of view and provide a dynamism endowed with public and cultural spaces that help improve the urban environment and maintain the commercial identity. The project is developed in six chapters that will be explained in the present study: the basis of the topic, theoretical framework, diagnosis, and the proposal.ARQUITECTUR
