    Assessing Vegetation Cover Change Using Remote Sensing: Case Study at Binh Duong Province, Vietnam

    This study aims to present the application of remote sensing in monitoring vegetation change in Binh Duong Province, Vietnam. The study used Landsat 5 images in the year 2010 and Landsat 8 images in the years 2015 and 2020 to investigate the area of vegetation. The maximum likelihood classification method (MLC) was used to classify land cover and an accuracy matrix was computed to validate the classification results. The references data were collected to support classification and accuracy assessment processes including land use maps in 2010, 2015, and 2020. In addition, collected field points and UAV (unmanned aerial vehicle) in 2020 were used. The overall accuracies are 81.27%, 84.41%, and 83.86%, and Kappa indices were 0.76, 0.80, and 0.80, corresponding to 2010, 2015, and 2020. The results showed that as compared to 2010 and 2015, the area of vegetation in 2020 decreased 10% and 8%, respectively. The average vegetation cover per capita was 740 m2 person-1 in 2020, compared to 1000 m2 person-1 in 2015 and 1200 m2 person-1 in 2010. This reduction was obvious in urban areas in the province, due to the need for construction and development. The study provides meaningful information on vegetation change and green area per capita in Binh Duong Province from 2010 to 2020

    Evaluation of the Luminex xTAG Respiratory Viral Panel FAST v2 assay for detection of multiple respiratory viral pathogens in nasal and throat swabs in Vietnam.

    BACKGROUND: Acute respiratory infections (ARI) are among the leading causes of hospitalization in children ≤5 years old. Rapid diagnostics of viral pathogens is essential to avoid unnecessary antibiotic treatment, thereby slowing down antibiotic-resistance. We evaluated the diagnostic performance of the Luminex xTAG Respiratory Viral Panel FAST v2 against viral specific PCR as reference assays for ARI in Vietnam. METHODS: Four hundred and forty two nose and throat swabs were collected in viral transport medium, and were tested with Luminex xTAG Respiratory Viral Panel FAST v2. Multiplex RT-PCR and single RT-PCR were used as references.    Results: Overall, viral pathogens were detected in a total count of 270/294 (91.8%, 95% CI 88.1-94.7) by the Luminex among reference assays, whilst 112/6336 (1.8%, 95% CI, 1.4-2.1) of pathogens were detected by the Luminex, but not by reference assays. Frequency of pathogens detected by Luminex and reference assays was 379 and 292, respectively. The diagnostic yield was 66.7% (295/442, 95%CI 62.1-71.1%) for the Luminex assay and 54.1% (239/442, 95% CI, 49.3-58.8%) for reference assays. The Luminex kit had higher yields for all viruses except influenza B virus, respiratory syncytial virus, and human bocavirus. High agreements between both methods [mean (range): 0.91 (0.83-1.00)] were found for 10/15 viral agents. CONCLUSIONS: The Luminex assay is a high throughput multiplex platform for rapid detection of common viral pathogens causing ARI. Although the current high cost may prevent Luminex assays from being widely used, especially in limited resource settings where ARI are felt most, its introduction in clinical diagnostics may help reduce unnecessary use of antibiotic prescription

    Evaluation of Luminex xTAG Gastrointestinal Pathogen Panel Assay for Detection of Multiple Diarrheal Pathogens in Fecal Samples in Vietnam.

    Diarrheal disease is a complex syndrome that remains a leading cause of global childhood morbidity and mortality. The diagnosis of enteric pathogens in a timely and precise manner is important for making treatment decisions and informing public health policy, but accurate diagnosis is a major challenge in industrializing countries. Multiplex molecular diagnostic techniques may represent a significant improvement over classical approaches. We evaluated the Luminex xTAG gastrointestinal pathogen panel (GPP) assay for the detection of common enteric bacterial and viral pathogens in Vietnam. Microbiological culture and real-time PCR were used as gold standards. The tests were performed on 479 stool samples collected from people admitted to the hospital for diarrheal disease throughout Vietnam. Sensitivity and specificity were calculated for the xTAG GPP for the seven principal diarrheal etiologies. The sensitivity and specificity for the xTAG GPP were >88% for Shigellaspp.,Campylobacterspp., rotavirus, norovirus genotype 1/2 (GI/GII), and adenovirus compared to those of microbiological culture and/or real-time PCR. However, the specificity was low (∼60%) for Salmonella species. Additionally, a number of important pathogens that are not identified in routine hospital procedures in this setting, such as Cryptosporidiumspp. and Clostridium difficile, were detected with the GPP. The use of the Luminex xTAG GPP for the detection of enteric pathogens in settings, like Vietnam, would dramatically improve the diagnostic accuracy and capacity of hospital laboratories, allowing for timely and appropriate therapy decisions and a wider understanding of the epidemiology of pathogens associated with severe diarrheal disease in low-resource settings

    Awareness and preparedness of healthcare workers against the first wave of the COVID-19 pandemic: A cross-sectional survey across 57 countries.

    BACKGROUND: Since the COVID-19 pandemic began, there have been concerns related to the preparedness of healthcare workers (HCWs). This study aimed to describe the level of awareness and preparedness of hospital HCWs at the time of the first wave. METHODS: This multinational, multicenter, cross-sectional survey was conducted among hospital HCWs from February to May 2020. We used a hierarchical logistic regression multivariate analysis to adjust the influence of variables based on awareness and preparedness. We then used association rule mining to identify relationships between HCW confidence in handling suspected COVID-19 patients and prior COVID-19 case-management training. RESULTS: We surveyed 24,653 HCWs from 371 hospitals across 57 countries and received 17,302 responses from 70.2% HCWs overall. The median COVID-19 preparedness score was 11.0 (interquartile range [IQR] = 6.0-14.0) and the median awareness score was 29.6 (IQR = 26.6-32.6). HCWs at COVID-19 designated facilities with previous outbreak experience, or HCWs who were trained for dealing with the SARS-CoV-2 outbreak, had significantly higher levels of preparedness and awareness (p<0.001). Association rule mining suggests that nurses and doctors who had a 'great-extent-of-confidence' in handling suspected COVID-19 patients had participated in COVID-19 training courses. Male participants (mean difference = 0.34; 95% CI = 0.22, 0.46; p<0.001) and nurses (mean difference = 0.67; 95% CI = 0.53, 0.81; p<0.001) had higher preparedness scores compared to women participants and doctors. INTERPRETATION: There was an unsurprising high level of awareness and preparedness among HCWs who participated in COVID-19 training courses. However, disparity existed along the lines of gender and type of HCW. It is unknown whether the difference in COVID-19 preparedness that we detected early in the pandemic may have translated into disproportionate SARS-CoV-2 burden of disease by gender or HCW type

    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Inflammatory mediators drive neuroinflammation in autism spectrum disorder and cerebral palsy

    Abstract Inflammation conditions are associated with autism spectrum disorder (ASD) and cerebral palsy (CP), primarily observed in the peripheral immune system. However, the extent of neuro-inflammation and neuro-immune dysregulation remains poorly studied. In this study, we analyzed the composition of cerebrospinal fluid (CSF) to uncover the inflammatory mediators driving the neuro-immune system in ASD and CP patients. Our findings revealed that ASD patients had elevated levels of four inflammatory cytokines (TNF-α, IL-4, IL-21, and BAFF) compared to controls, while CP patients exhibited increased levels of eight inflammatory cytokines (IFN-γ, GM-CSF, TNF-α, IL-2, IL-4, IL-6, IL-17A and IL-12), one anti-inflammatory cytokine (IL-10), and five growth factors (GFs) (NGF-β, EGF, GDF-15, G-CSF and BMP-9) compared to both controls and ASD patients. Additionally, intrathecal infusion of autologous bone marrow mononuclear cells (BMMNCs) led to a slight decrease in TGF-β and GDF-15 levels in the CSF of ASD and CP patients, respectively. Our study provides new insights into the molecular composition of CSF in ASD and CP patients, with the potential to develop more effective diagnosis methods and improved treatment for these diseases.     Projections of Future Climate Change in the Vu Gia Thu Bon River Basin, Vietnam by Using Statistical DownScaling Model (SDSM)

    Future projections of anthropogenic climate change play a pivotal role in devising viable countermeasures to address climate-related risks. This study strove to construct future daily rainfall and maximum and minimum temperature scenarios in Vu Gia Thu Bon river basin by employing the Statistical DownScaling Model (SDSM). The model performance was evaluated by utilizing a Taylor diagram with dimensioned and dimensionless statistics. During validation, all model-performance measures show good ability in simulating extreme temperatures and reasonable ability for rainfall. Subsequently, a set of predictors derived from HadCM3 and CanESM2 was selected to generate ensembles of each climatic variables up to the end of 21st century. The generated outcomes exhibit a consistent increase in both extreme temperatures under all emission scenarios. The greatest changes in maximum and minimum temperature were predicted to increase by 2.67&ndash;3.9 &deg;C and 1.24&ndash;1.96 &deg;C between the 2080s and reference period for the worst-case scenarios. Conversely, there are several discrepancies in the projections of rainfall under different emission scenarios as well as among considered stations. The predicted outcomes indicate a significant decrease in rainfall by approximately 11.57%&ndash;17.68% at most stations by 2099. Moreover, all ensemble means were subjected to the overall and partial trend analysis by applying the Innovative-Şen trend analysis method. The results exhibit similar trend patterns, thereby indicating high stability and applicability of the SDSM. Generally, it is expected that these findings will contribute numerous valuable foundations to establish a framework for the assessment of climate change impacts at the river basin scale

    Nonlinear Buckling Behavior of Spiral Corrugated Sandwich FGM Cylindrical Shells Surrounded by an Elastic Medium

    This paper presents a semi-analytical approach for investigating the nonlinear buckling and postbuckling of spiral corrugated sandwich functionally graded (FGM) cylindrical shells under external pressure and surrounded by a two-parameter elastic foundation based on Donnell shell theory. The improved homogenization theory for the spiral corrugated FGM structure is applied and the geometrical nonlinearity in a von Karman sense is taken into account. The nonlinear equilibrium equation system can be solved by using the Galerkin method with the three-term solution form of deflection. An explicit solution form for the nonlinear buckling behavior of shells is obtained. The critical buckling pressure and the postbuckling strength of shells are numerically investigated. Additionally, the effects of spiral corrugation in enhancing the nonlinear buckling behavior of spiral corrugated sandwich FGM cylindrical shells are validated and discussed
