Victims of child abuse dropping out of trauma‑focused treatment: A meta‑analysis of risk factors
A substantial number of children who experienced child maltreatment drop out of evidence-based trauma-focused treatments (TF-CBT). Identifying child, family, and treatment-related factors associated with treatment dropout is important to be able to prevent this from happening and to effectively treat children’s trauma-related symptoms. Methods: A quantitative review was performed based on a systematic synthesis of the literature on potential risk factors for dropout of trauma-focused treatment in maltreated children. Results: Eight studies were included, that examined TF-CBT, reporting on 139 effects of potential risk factors for dropout. Each factor was classified into one of ten domains. Small but significant effects were found for the “Demographic and Family” risk domain (r=.121), with factors including being male, child protective services involvement or placement, and minority status, and for the “Youth Alliance” risk domain (r=.207), with factors including low therapist-child support and low youth perception of parental approval. Moderator analyses suggested that family income and parental education may better predict the risk for TF-CBT dropout than other variables in the “Demographic and Family” domain. Proximity effect and strong coupling superconductivity in nanostructures built with an STM
We present high resolution tunneling spectroscopy data at very low
temperatures on superconducting nanostructures of lead built with an STM. By
applying magnetic fields, superconductivity is restricted to length scales of
the order of the coherence length. We measure the tunneling conductance and
analyze the phonon structure and the low energy DOS. We demonstrate the
influence of the geometry of the system on the magnetic field dependence of the
tunneling density of states, which is gapless in a large range of fields. The
behavior of the features in the tunneling conductance associated to phonon

Meta-analysis comparing upfront surgery with neoadjuvant treatment in patients with resectable or borderline resectable pancreatic cancer
Background: Studies comparing upfront surgery with neoadjuvant treatment in pancreatic cancer may report only patients who underwent resection and so survival will be skewed. The aim of this study was to report survival by intention to treat in a comparison of upfront surgery versus neoadjuvant treatment in resectable or borderline resectable pancreatic cancer.
Methods: MEDLINE, Embase and the Cochrane Library were searched for studies reporting median overall survival by intention to treat in patients with resectable or borderline resectable pancreatic cancer treated with or without neoadjuvant treatment. Secondary outcomes included overall and R0 resection rate, pathological lymph node rate, reasons for unresectability and toxicity of neoadjuvant treatment.
Results: In total, 38 studies were included with 3484 patients, of whom 1738 (49·9 per cent) had neoadjuvant treatment. The weighted median overall survival by intention to treat was 18·8months for neoadjuvant treatment and 14·8months for upfront surgery; the difference was larger among patients whose tumours were resected (26·1 versus 15·0months respectively). The overall resection rate was lower with neoadjuvant treatment than with upfront surgery (66·0 versus 81·3 per cent; P<0·001), but the R0 rate was higher (86·8 (95 per cent c.i. 84·6 to 88·7) versus 66·9 (64·2 to 69·6) per cent; P<0·001). Reported by intention to treat, the R0 rates were 58·0 and 54·9 per cent respectively (P=0·088). The pathological lymph node rate was 43·8 per cent after neoadjuvant therapy and 64·8 per cent in the upfront surgery group (P<0·001). Toxicity of at least grade III was reported in up to 64 per cent of the patients.
Conclusion: Neoadjuvant treatment appears to improve overall survival by intention to treat, despite lower overall resection rates for resectable or borderline resectable pancreatic cancer. PROSPERO registration number: CRD42016049374
Phonon-mediated anisotropic superconductivity in the Y and Lu nickel borocarbides
We present scanning tunneling spectroscopy and microscopy measurements at low
temperatures in the borocarbide materials RNi2B2C (R=Y, Lu). The characteristic
strong coupling structure due to the pairing interaction is unambiguously
resolved in the superconducting density of states. It is located at the
superconducting gap plus the energy corresponding to a phonon mode identified
in previous neutron scattering experiments. These measurements also show that
this mode is coupled to the electrons through a highly anisotropic
electron-phonon interaction originated by a nesting feature of the Fermi
surface. Our experiments, from which we can extract a large electron-phonon
coupling parameter lambda (between 0.5 and 0.8), demonstrate that this
anisotropic electron-phonon coupling has an essential contribution to the
pairing interaction. The tunneling spectra show an anisotropic s-wave

Subdural hematomas: glutaric aciduria type 1 or abusive head trauma? A systematic review

154022.pdf (publisher's version ) (Open Access)PURPOSE: Glutaric aciduria type 1 (GA1) is a rare metabolic disorder of glutaryl-CoA-dehydrogenase enzyme deficiency. Children with GA1 are reported to be predisposed to subdural hematoma (SDH) development due to stretching of cortical veins secondary to cerebral atrophy and expansion of CSF spaces. Therefore, GA1 testing is part of the routine work-up in abusive head trauma (AHT). This systematic review addresses the coexistence of GA1 and SDH and the validity of GA1 in the differential diagnosis of AHT. METHODS: A systematic literature review, with language restriction, of papers published before 1 Jan 2015, was performed using Pubmed, PsychINFO, and Embase. Inclusion criteria were reported SDHs, hygromas or effusions in GA1 patients up to 18 years of age. Of 1599 publications, 20 publications were included for analysis. RESULTS: In total 20 cases, 14 boys and 6 girls, were included. In eight cases (40 %) a child abuse work-up was performed, which was negative in all cases. Clinical history revealed the presence of trauma in eight cases (40 %). In only one case neuroradiology revealed no abnormalities related to GA1 according to the authors, although on evaluation we could not exclude AHT. CONCLUSION: From this systematic review we conclude that SDHs in 19/20 children with GA1 are accompanied by other brain abnormalities specific for GA1. One case with doubtful circumstances was the exception to this rule
A systematic review of antithrombotic treatment of venous thromboembolism in patients with myeloproliferative neoplasms

232734.pdf (Publisher’s version ) (Open Access)Patients with myeloproliferative neoplasms (MPNs), polycythemia vera, essential thrombocythemia, and primary myelofibrosis, have an increased risk of thrombosis. Risk of recurrent thrombosis can be reduced with antithrombotic therapy and/or cytoreduction, but the optimal long-term management in patients with MPN with a history of venous thromboembolism (VTE) is unknown, and clinical practice is heterogeneous. We performed a systematic review and meta-analysis of randomized trials and observational studies evaluating anticoagulant and/or antiplatelet therapy, with or without cytoreduction, in MPN patients with a history of VTE. A total of 5675 unique citations were screened for eligibility. No randomized trials were identified. Ten observational studies involving 1295 patients with MPN were included in the analysis. Overall, 23% had an arterial or recurrent venous thrombotic event on follow-up. The recurrence risk was lowest for patients on oral anticoagulation plus cytoreduction (16%); 55 of 313 (18%) with vitamin K antagonists (VKA) and 5 of 63 (8%) with direct oral anticoagulants (DOACs). In 746 analyzed patients, the risk of recurrent VTE ranged up to 33% (median 13%) and was low in 63 DOAC plus cytoreduction-treated patients (3.2%). All types of antithrombotic treatments were associated with a lower risk of recurrent VTE when combined with cytoreduction. Most studies had a high risk of bias, whereas clinical and statistical heterogeneity led to inconsistent and imprecise findings. In summary, evidence on the optimal antithrombotic treatment of VTE in patients with MPN is based on observational studies only with low certainty for all strategies. Our data suggest that a combination of anticoagulation and cytoreduction may provide the lowest recurrence risk
