Viral dynamics of acute HIV-1 infection.
Viral dynamics were intensively investigated in eight patients with acute HIV infection to define the earliest rates of change in plasma HIV RNA before and after the start of antiretroviral therapy. We report the first estimates of the basic reproductive number (R(0)), the number of cells infected by the progeny of an infected cell during its lifetime when target cells are not depleted. The mean initial viral doubling time was 10 h, and the peak of viremia occurred 21 d after reported HIV exposure. The spontaneous rate of decline (alpha) was highly variable among individuals. The phase 1 viral decay rate (delta(I) = 0.3/day) in subjects initiating potent antiretroviral therapy during acute HIV infection was similar to estimates from treated subjects with chronic HIV infection. The doubling time in two subjects who discontinued antiretroviral therapy was almost five times slower than during acute infection. The mean basic reproductive number (R(0)) of 19.3 during the logarithmic growth phase of primary HIV infection suggested that a vaccine or postexposure prophylaxis of at least 95% efficacy would be needed to extinguish productive viral infection in the absence of drug resistance or viral latency. These measurements provide a basis for comparison of vaccine and other strategies and support the validity of the simian immunodeficiency virus macaque model of acute HIV infection
Complexity without chaos: Plasticity within random recurrent networks generates robust timing and motor control
It is widely accepted that the complex dynamics characteristic of recurrent
neural circuits contributes in a fundamental manner to brain function. Progress
has been slow in understanding and exploiting the computational power of
recurrent dynamics for two main reasons: nonlinear recurrent networks often
exhibit chaotic behavior and most known learning rules do not work in robust
fashion in recurrent networks. Here we address both these problems by
demonstrating how random recurrent networks (RRN) that initially exhibit
chaotic dynamics can be tuned through a supervised learning rule to generate
locally stable neural patterns of activity that are both complex and robust to
noise. The outcome is a novel neural network regime that exhibits both
transiently stable and chaotic trajectories. We further show that the recurrent
learning rule dramatically increases the ability of RRNs to generate complex
spatiotemporal motor patterns, and accounts for recent experimental data
showing a decrease in neural variability in response to stimulus onset
Magnetic control of graphitic microparticles in aqueous solutions
Graphite is an inexpensive material with useful electrical, magnetic, thermal, and optical properties. It is also biocompatible and used universally as a substrate. Micrometer-sized graphitic particles in solution are therefore ideal candidates for novel lab-on-a-chip and remote manipulation applications in biomedicine, biophysics, chemistry, and condensed-matter physics. However, submerged graphite is not known to be amenable to magnetic manipulation, the optimal manipulation method for such applications. Here, we exploit the diamagnetism of graphite and demonstrate contactless magnetic positioning control of graphitic microflakes in diamagnetic aqueous solutions. We develop a theoretical model for magnetic manipulation of graphite microflakes and demonstrate experimentally magnetic transport of such particles over distances [Formula: see text] with peak velocities [Formula: see text] in inhomogeneous magnetic fields. We achieve fully biocompatible transport for lipid-coated graphite in NaCl aqueous solution, paving the way for previously undiscovered biomedical applications. Our results prove that micrometer-sized graphite can be magnetically manipulated in liquid media
Decay of correlations for maps with uniformly contracting fibers and logarithm law for singular hyperbolic attractors
We consider two dimensional maps preserving a foliation which is uniformly
contracting and a one dimensional associated quotient map having exponential
convergence to equilibrium (iterates of Lebesgue measure converge exponentially
fast to physical measure). We prove that these maps have exponential decay of
correlations over a large class of observables. We use this result to deduce
exponential decay of correlations for the Poincare maps of a large class of

Characteristics and outcome of patients with newly diagnosed advanced or metastatic lung cancer admitted to intensive care units (ICUs)
BACKGROUND: Although patients with advanced or metastatic lung cancer have poor prognosis, admission to the ICU for management of life-threatening complications has increased over the years. Patients with newly diagnosed lung cancer appear as good candidates for ICU admission, but more robust information to assist decisions is lacking. The aim of our study was to evaluate the prognosis of newly diagnosed unresectable lung cancer patients. METHODS: A retrospective multicentric study analyzed the outcome of patients admitted to the ICU with a newly diagnosed lung cancer (diagnosis within the month) between 2010 and 2013. RESULTS: Out of the 100 patients, 30 had small cell lung cancer (SCLC) and 70 had non-small cell lung cancer. (Thirty patients had already been treated with oncologic treatments.) Mechanical ventilation (MV) was performed for 81 patients. Seventeen patients received emergency chemotherapy during their ICU stay. ICU, hospital, 3- and 6-month mortality were, respectively, 47, 60, 67 and 71%. Hospital mortality was 60% when invasive MV was used alone, 71% when MV and vasopressors were needed and 83% when MV, vasopressors and hemodialysis were required. In multivariate analysis, hospital mortality was associated with metastatic disease (OR 4.22 [1.4-12.4]; p = 0.008), need for invasive MV (OR 4.20 [1.11-16.2]; p = 0.030), while chemotherapy in ICU was associated with survival (OR 0.23, [0.07-0.81]; p = 0.020). CONCLUSION: This study shows that ICU management can be appropriate for selected newly diagnosed patients with advanced lung cancer, and chemotherapy might improve outcome for patients with SCLC admitted for cancer-related complications. Nevertheless, tumors' characteristics, numbers and types of organ dysfunction should be taken into account in the decisional process before admitting these patients in ICU.Peer reviewe

Stella modulates transcriptional and endogenous retrovirus programs during maternal-to-zygotic transition
The maternal-to-zygotic transition (MZT) marks the period when the embryonic genome is activated and acquires control of development. Maternally inherited factors play a key role in this critical developmental process, which occurs at the 2-cell stage in mice. We investigated the function of the maternally inherited factor Stella (encoded by Dppa3) using single-cell/embryo approaches. We show that loss of maternal Stella results in widespread transcriptional mis-regulation and a partial failure of MZT. Strikingly, activation of endogenous retroviruses (ERVs) is significantly impaired in Stella maternal/zygotic knockout embryos, which in turn leads to a failure to upregulate chimeric transcripts. Amongst ERVs, MuERV-L activation is particularly affected by the absence of Stella, and direct in vivo knockdown of MuERV-L impacts the developmental potential of the embryo. Does respiratory health contribute to the effects of long-term air pollution exposure on cardiovascular mortality?
BACKGROUND: There is growing epidemiological evidence that short-term and long-term exposure to high levels of air pollution may increase cardiovascular morbidity and mortality. In addition, epidemiological studies have shown an association between air pollution exposure and respiratory health. To what extent the association between cardiovascular mortality and air pollution is driven by the impact of air pollution on respiratory health is unknown. The aim of this study was to investigate whether respiratory health at baseline contributes to the effects of long-term exposure to high levels of air pollution on cardiovascular mortality in a cohort of elderly women. METHOD: We analyzed data from 4750 women, aged 55 at the baseline investigation in the years 1985–1994. 2593 of these women had their lung function tested by spirometry. Respiratory diseases and symptoms were asked by questionnaire. Ambient air pollution exposure was assessed by the concentrations of NO(2 )and total suspended particles at fixed monitoring sites and by the distance of residency to a major road. A mortality follow-up of these women was conducted between 2001 and 2003. For the statistical analysis, Cox' regression was used. RESULTS: Women with impaired lung function or pre-existing respiratory diseases had a higher risk of dying from cardiovascular causes. The impact of impaired lung function declined over time. The risk ratio (RR) of women with forced expiratory volume in one second (FEV(1)) of less than 80% predicted to die from cardiovascular causes was RR = 3.79 (95%CI: 1.64–8.74) at 5 years survival time and RR = 1.35 (95%CI: 0.66–2.77) at 12 years. The association between air pollution levels and cardiovascular death rate was strong and statistically significant. However, this association did only change marginally when including indicators of respiratory health into the regression analysis. Furthermore, no interaction between air pollution and respiratory health on cardiovascular mortality indicating a higher risk of those with impaired respiratory health could be detected. CONCLUSION: Respiratory health is a predictor for cardiovascular mortality. In women followed about 15 years after the baseline investigation at age 55 years long-term air pollution exposure and impaired respiratory health were independently associated with increased cardiovascular mortality
