    EFFICACY OF AN AYURVEDIC INTERVENTION IN THE MANAGEMENT OF URETERIC CALCULI: A CASE STUDY

    Urinary calculi is one of the most common and painful urological disorder of the urinary tract affecting 10-12 % of the population. It is characterized by frequent micturition, intermittent pain occasionally radiates to flanks, sweating and restlessness during the pain. Most of the patients taking allopathic medicine but calculi was not expel or crushed from the urinary tract. Endoscopic procedures such as ureterostomy, percutaneous nephrolithiotomy and extracorporeal shockwave lithotripsy or other surgical procedures is the treatment of choice in case of ureteric calculi in the conventional system. Non-invasive and safe study had designed for the management of this entity. In the present study, An Ayurvedic intervention Chandraprabha Vati (500 mg ) and Swetparpati (250 mg) along with decoction of natural herbs 30 ml twice in a day for 90 days used for ureteric calculi. The response to the treatment is encouraging by symptoms relief and through lower abdomen ultrasonography (USG). Ureteric calculi has expel from the tract through this management.

    Taking Multiple Infections of Cells and Recombination into Account Leads to Small Within-Host Effective-Population-Size Estimates of HIV-1

    Whether HIV-1 evolution in infected individuals is dominated by deterministic or stochastic effects remains unclear because current estimates of the effective population size of HIV-1 in vivo, Ne, are widely varying. Models assuming HIV-1 evolution to be neutral estimate Ne∼102–104, smaller than the inverse mutation rate of HIV-1 (∼105), implying the predominance of stochastic forces. In contrast, a model that includes selection estimates Ne>105, suggesting that deterministic forces would hold sway. The consequent uncertainty in the nature of HIV-1 evolution compromises our ability to describe disease progression and outcomes of therapy. We perform detailed bit-string simulations of viral evolution that consider large genome lengths and incorporate the key evolutionary processes underlying the genomic diversification of HIV-1 in infected individuals, namely, mutation, multiple infections of cells, recombination, selection, and epistatic interactions between multiple loci. Our simulations describe quantitatively the evolution of HIV-1 diversity and divergence in patients. From comparisons of our simulations with patient data, we estimate Ne∼103–104, implying predominantly stochastic evolution. Interestingly, we find that Ne and the viral generation time are correlated with the disease progression time, presenting a route to a priori prediction of disease progression in patients. Further, we show that the previous estimate of Ne>105 reduces as the frequencies of multiple infections of cells and recombination assumed increase. Our simulations with Ne∼103–104 may be employed to estimate markers of disease progression and outcomes of therapy that depend on the evolution of viral diversity and divergence

    Effect of mitratapide on body composition, body measurements and glucose tolerance in obese Beagles

    The objective of this study was to confirm that weight loss after treatment with mitratapide (Yarvitan®) is loss of adipose tissue. Obese dogs were treated with the recommended treatment schedule of mitratapide. Dual-energy X-ray absorptiometry (DEXA) was done before and after the treatment schedule. Body weight, feed consumption and pelvic circumference were recorded and a glucose tolerance test was performed. Dual-energy X-ray absorptiometry measurements showed an impressive loss of fat tissue, corresponding to a mean loss of approximately 41.6% of the body fat mass recorded before treatment. After treatment with mitratapide, the mean body fat percentage had returned within the normal range. At the end of the study, the dogs had lost on average 14.2% of their body weight and 15.2% of their pelvic circumference compared to baseline. The results also suggest that losing weight with mitratapide might help to reverse insulin resistance

    Glucose challenge increases circulating progenitor cells in Asian Indian male subjects with normal glucose tolerance which is compromised in subjects with pre-diabetes: A pilot study

    <p>Abstract</p> <p>Background</p> <p>Haematopoietic stem cells undergo mobilization from bone marrow to blood in response to physiological stimuli such as ischemia and tissue injury. The aim of study was to determine the kinetics of circulating CD34<sup>+ </sup>and CD133<sup>+</sup>CD34<sup>+ </sup>progenitor cells in response to 75 g glucose load in subjects with normal and impaired glucose metabolism.</p> <p>Methods</p> <p>Asian Indian male subjects (n = 50) with no prior history of glucose imbalance were subjected to 2 hour oral glucose tolerance test (OGTT). 24 subjects had normal glucose tolerance (NGT), 17 subjects had impaired glucose tolerance (IGT) and 9 had impaired fasting glucose (IFG). The IGT and IFG subjects were grouped together as pre-diabetes group (n = 26). Progenitor cell counts in peripheral circulation at fasting and 2 hour post glucose challenge were measured using direct two-color flow cytometry.</p> <p>Results</p> <p>The pre-diabetes group was more insulin resistant (p < 0.0001) as measured by homeostasis assessment model (HOMA-IR) compared to NGT group. A 2.5-fold increase in CD34<sup>+ </sup>cells (p = 0.003) and CD133<sup>+</sup>CD34<sup>+ </sup>(p = 0.019) cells was seen 2 hours post glucose challenge in the NGT group. This increase for both the cell types was attenuated in subjects with IGT. CD34<sup>+ </sup>cell counts in response to glucose challenge inversely correlated with neutrophil counts (ρ = -0.330, p = 0.019), while post load counts of CD133<sup>+</sup>CD34<sup>+ </sup>cells inversely correlated with serum creatinine (ρ = -0.312, p = 0.023).</p> <p>Conclusion</p> <p>There is a 2.5-fold increase in the circulating levels of haematopoietic stem cells in response to glucose challenge in healthy Asian Indian male subjects which is attenuated in subjects with pre-diabetes.</p
