    Tratamiento conservador en los hematomas intracerebrales supratentoriales espontáneos

    Se realiza un estudio prospectivo de 50 pacientes diagnosticados por medio de tomografía computarizada craneal (TC) y angiografía carotídea de hematomas intracerebrales espontáneos (HIS) tratados de forma conservadora. Se analizan características clínicas y radiológicas de la fase aguda y la evolución de los pacientes durante el tiempo de ingreso hospitalario. La mortalidad global fue del 32%. Los factores que influyeron significativamente en el pronóstico fueron: el mayor grado de déficit motor al ingreso, el empeoramiento neural post-ingreso, el mayor volumen del hematoma y la presencia de efecto de masa en los estudios de TC y de angiografía carotídea. Todos los pacientes fueron evaluados clínicamente atendiendo a la escala de Glasgow para el Coma (EGC), con índice de mal pronóstico en todos aquellos que tenían una puntuación igual o inferior a ocho puntos. Se concluye que, de todos los factores evaluados en nuestro estudio, el volumen del hematoma con valor crítico de 50 ml y una puntuación menor de ocho puntos en la EGC son las principales causas en la mala evolución de estos enfermos

    Se realizó un estudio con los pacientes hipertensos dispensarizados de la Policlínica "Omar Ranedo Pubillones" de Guantánamo, del 1 de abril  de 1997 al 30 de abril de 1998. De la  totalidad de dichos pacientes en el área se obtuvo una muestra sistemática representativa, con el propósito de evaluar el control de la enfermedad en dicha área de salud. Se encontró que el 58,2% de ellos estaban controlados. La edad y el sexo no influyeron en el grado de control de la presión arterial. La patología asociada que se presentó con mayor frecuencia fue la cardiopatía isquémica. El mejor resultado en el control de la enfermedad se obtuvo con el tratamiento combinado farmacológico y no farmacológico. Se concluye que hay un bajo grado de control de la presión arterial en la muestra estudiada

    The purpose of this study is to compare the economic activity of Mexican tourist destinations through the input-output matrix, in order to estimate the economic investment multipliers for these destinations. It was carried out based on input-output techniques with data on tourist arrivals for the year 2010, as well as the Gross Domestic Product per capita for each destination, the data was taken from the Municipal Human Development Index. The results obtained indicate that Mexico City is the best positioned and investment in tourism in that place has a greater influence on the rest of the destinations studied. This study allows to quantify the real contribution of the tourist demand in the economy in the tourist destinations.  El proposito de este estudio es comparar la actividad econĂłnomica de destinos turisticos mexicanos mediante la matriz insumo-producto, con la finalidad de estimar los multiplicadores econĂłmicos de inversiĂłn para dichos destinos. Se realizo a partir de tĂ©cnicas de insumo-producto con datos de llegadas de turistas del año 2010, asĂ­ como el Producto Interno Bruto per cápita por cada destino, los datos se tomaron del ĂŤndice de Desarrollo Humano Municipal. Los resultados obtenidos indican la Ciudad de MĂ©xico es la mejor posicionada y la inversiĂłn en turismo en dicho lugar influye en mayor proporciĂłn sobre el resto de los destinos estudiados.     Mortalidad por enfermedad cerebrovascular en el Hospital General Docente “Dr. Agostinho Neto”. Estudio del quinquenio 1993-1997

     Se  realizó  un  estudio tipo caso control  con  el  objetivo  de caracterizar  aspectos de interés relacionados con la  mortalidad por   enfermedad  cerebrovascular  (ECV)  en  el  hospital   "Dr. Agostinho  Neto"  durante  el quinquenio 1993 -  1997.  El  grupo estudio  (fallecidos; n=300) y un grupo control  (vivos;  n=600), ambos  con  diagnóstico de ECV, fueron  seleccionados aleatoriamente  y apareados con una relación 2:1. Los  resultados más  sobresalientes fueron los siguientes: la tasa de  incidencia mostró  una  tendencia  ascendente,  la  ECV  oclusiva  tuvo  una incidencia mayor que la de la ECV hemorrágica. El análisis demostró que  hay una asociación estadísticamente significativa  entre  la ECV y la hipertensión arterial (OR = 830), la edad (OR = 5.78)  y con  el antecedente de ECV previa (OR = 4.33). La  bronconeumonía fue la complicación más frecuente y la causa directa de muerte en el 45.7 % de los fallecidos

    Nuclear DNA content variation in life history phases of the Bonnemaisoniaceae (Rhodophyta)

    Nuclear DNA content in gametophytes and sporophytes or the prostrate phases of the following species of Bonnemaisoniaceae (Asparagopsis armata, Asparagopsis taxiformis, Bonnemaisonia asparagoides, Bonnemaisonia clavata and Bonnemaisonia hamifera) were estimated by image analysis and static microspectrophotometry using the DNA-localizing fluorochrome DAPI (4′, 6-diamidino-2-phenylindole, dilactate) and the chicken erythrocytes standard. These estimates expand on the Kew database of DNA nuclear content. DNA content values for 1C nuclei in the gametophytes (spermatia and vegetative cells) range from 0.5 pg to 0.8 pg, and for 2C nuclei in the sporophytes or the prostrate phases range from 1.15-1.7 pg. Although only the 2C and 4C values were observed in the sporophyte or the prostrate phase, in the vegetative cells of the gametophyte the values oscillated from 1C to 4C, showing the possible start of endopolyploidy. The results confirm the alternation of nuclear phases in these Bonnemaisoniaceae species, in those that have tetrasporogenesis, as well as those that have somatic meiosis. The availability of a consensus phylogenetic tree for Bonnemaisoniaceae has opened the way to determine evolutionary trends in DNA contents. Both the estimated genome sizes and the published chromosome numbers for Bonnemaisoniaceae suggest a narrow range of values consistent with the conservation of an ancestral genome

    A large repertoire of spatiotemporal activity patterns in the brain is the basis for adaptive behaviour. Understanding the mechanism by which the brain's hundred billion neurons and hundred trillion synapses manage to produce such a range of cortical configurations in a flexible manner remains a fundamental problem in neuroscience. One plausible solution is the involvement of universal mechanisms of emergent complex phenomena evident in dynamical systems poised near a critical point of a second-order phase transition. We review recent theoretical and empirical results supporting the notion that the brain is naturally poised near criticality, as well as its implications for better understanding of the brain

    Optimization of sentinel lymph node biopsy in breast cancer using an operative gamma camera

    <p>Abstract</p> <p>Background</p> <p>Sentinel lymph node (SLN) procedure is now a widely accepted method of LN staging in selected invasive breast cancers (unifocal, size ≤ 2 cm, clinically N0, without previous treatment). Complete axillary clearance is no longer needed if the SLN is negative. However, the oncological safety of this procedure remains to be addressed in randomized clinical trials. One main pitfall is the failure to visualize SLN, resulting in incorrect tumor staging, leading to suboptimal treatment or axillary recurrence. Operative gamma cameras have therefore been developed to optimize the SLN visualization and the quality control of surgery.</p> <p>Case presentation</p> <p>A 44-year-old female patient with a 14-mm infiltrative ductal carcinoma underwent the SLN procedure. An operative gamma camera was used during and after the surgery. The conventional lymphoscintigraphy showed only one SLN, which was also detected by the operative gamma camera, then removed and measured (9.6 kBq). It was analyzed by frozen sections, showing no cancer cells. During this analysis, the exploration of the axillary area with the operative gamma camera enabled the identification of a second SLN with low activity (0.5 kBq) that conventional lymphoscintigraphy, surgical probe and blue staining had failed to visualize. Histological examination revealed a macrometastasis. Axillary clearance was then performed, followed by a postoperative image proving that no SLN remained. Therefore, the use of the operative gamma camera prevented an under-estimation of staging which would have resulted in a suboptimal treatment for this patient.</p> <p>Conclusion</p> <p>This case report illustrates that an efficient operative gamma camera may be able to decrease the risk of false negative rate of the SLN procedure, and could be an additional tool to control the quality of the surgery.</p> <p>Trial Registration</p> <p>ClinicalTrials.gov Identifier: NCT00357487</p
