    From Shop floor to top floor. An exploratory study of sustainable progression in the retial sector: the case of Morrisons

    There has been increasing political and media attention given to the issue of social mobility in recent times. The interest has been sparked by research which suggests that social mobility may have stalled or even declined in the UK during the post-war period. Various factors have been identified as inhibiting social mobility including early years experiences in the home and at school, education and health along with area based influences. Employment and labour market experiences are also key factors contributing to social mobility with the importance of ‘getting a job’ and ‘sustainable progression’ increasingly recognised as an important means of improving social mobility. There now appears consensus that occupational mobility and career development is a key factor in overcoming social mobility, and the extent to which organisations develop pathways and support careers is a critical element in pursuing social mobility. This exploratory study has been commissioned by Morrisons to investigate the factors which impact on progression in the workplace and the effect of these on the social mobility of research participants. The study adopts the framework of ‘career development’ as a construct to explore sustainable progression, drawing on the employment and labour market experiences of employees at Morrisons who, having started on the ‘shop floor’ have progressed to senior management levels in the company. The study is based on their ‘life stories’ to identify the key factors associated with a successful career at Morrisons

    Work Based Learning, Enterprise Education and small family business

    The world of education and work are adapting to the ever-changing political, economic and policy landscape as evidenced by approaches to enterprise education and work based learning. Work based learning often focuses on the development of employability skills of learners (e.g. Costley and Armsby, 2007) whereas the focus of enterprise education is often placed on developing skills, attributes and behaviours to encourage entrepreneurship and to help people cope with the uncertainties associated with the flexible market ‘gig’ economy (e.g. Gibb, 1993). This paper compares and contrasts these two educational concepts and in so doing looks at where they overlap and what one might learn from the other (Jones and Iredale, 2014). The paper draws on the literature to build theory and assesses the blending of the two concepts via case study analysis founded upon two Erasmus+ programme projects that tackle succession planning in small family businesses. This paper addresses issues around succession planning in small family businesses and the design of a higher education curriculum that brings together Work Based Learning (WBL) and Enterprise Education. The blending of the two concepts is justified in that WBL is embedded in the structures, processes, practices and cultures of small family businesses. In such environments learning is largely informal, is passed on and acquired as part of everyday activity. At the point of succession there is a need to adopt an Enterprise Education approach to help re-think the original entrepreneurial skills, attributes and behaviours of the business founder for a new generation, changing environment and set of business circumstances

    A Framework for Work-Based Learning: Basic Pillars and the interactions between them

    Purpose: European policy is placing an increasing emphasis on involving employers and labour market institutions in the design and delivery of higher education programmes that match curricula to current and future needs of the economy. The aim of this paper is to investigate the curriculum development process for work-based learning (WBL) programmes and to connect it to the basic pillars, organizational and pedagogical strategies and key stages that enable higher education institutions to foster students’ learning, employability and innovation. Design/methodology/approach: Case studies of 14 European WBL programmes in Higher Education are reported using interviews and document analysis. These case studies are used to develop a final framework and examples of practice. Findings: A framework was designed to develop WBL programmes that include three basic pillars and the interactions between them to enhance learning quality (Doctrine), provide authentic experiences (Authenticity) and respect the ways of developing and delivering WBL (Culture). Research limitations/implications: While selecting the cases on this ‘best practices’ basis, some important pitfalls were not discussed. Thus, rather than offering a definitive theory, we provide a framework of issues that should at least be taken into account in the different stages of planning, delivery and reflection. Practical implications: The framework is simultaneously a kind of “checklist” for WBL curriculum developers. Originality/value The research presents 14 case studies from programmes recognized in 6 European countries and develops an original WBL programme planning, delivery and evaluation framework that can also be used as a checklist for HEIs offering work-based learning programmes

    Methodology of a diabetes prevention translational research project utilizing a community-academic partnership for implementation in an underserved Latino community

    <p>Abstract</p> <p>Background</p> <p>Latinos comprise the largest racial/ethnic group in the United States and have 2–3 times the prevalence of type 2 diabetes mellitus as Caucasians.</p> <p>Methods and design</p> <p>The Lawrence Latino Diabetes Prevention Project (LLDPP) is a community-based translational research study which aims to reduce the risk of diabetes among Latinos who have a ≥ 30% probability of developing diabetes in the next 7.5 years per a predictive equation. The project was conducted in Lawrence, Massachusetts, a predominantly Caribbean-origin urban Latino community. Individuals were identified primarily from a community health center's patient panel, screened for study eligibility, randomized to either a usual care or a lifestyle intervention condition, and followed for one year. Like the efficacious Diabetes Prevention Program (DPP), the LLDPP intervention targeted weight loss through dietary change and increased physical activity. However, unlike the DPP, the LLDPP intervention was less intensive, tailored to literacy needs and cultural preferences, and delivered in Spanish. The group format of the intervention (13 group sessions over 1 year) was complemented by 3 individual home visits and was implemented by individuals from the community with training and supervision by a clinical research nutritionist and a behavioral psychologist. Study measures included demographics, Stern predictive equation components (age, gender, ethnicity, fasting glucose, systolic blood pressure, HDL-cholesterol, body mass index, and family history of diabetes), glycosylated hemoglobin, dietary intake, physical activity, depressive symptoms, social support, quality of life, and medication use. Body weight was measured at baseline, 6-months, and one-year; all other measures were assessed at baseline and one-year. All surveys were orally administered in Spanish.</p> <p>Results</p> <p>A community-academic partnership enabled the successful recruitment, intervention, and assessment of Latinos at risk of diabetes with a one-year study retention rate of 93%.</p> <p>Trial registration</p> <p>NCT00810290</p

    A randomized clinical trial of a peri-operative behavioral intervention to improve physical activity adherence and functional outcomes following total knee replacement

    <p>Abstract</p> <p>Background</p> <p>Total knee replacement (TKR) is a common and effective surgical procedure to relieve advanced knee arthritis that persists despite comprehensive medical treatment. Although TKR has excellent technical outcomes, significant variation in patient-reported functional improvement post-TKR exists. Evidence suggests that consistent post-TKR exercise and physical activity is associated with functional gain, and that this relationship is influenced by emotional health. The increasing use of TKR in the aging US population makes it critical to find strategies that maximize functional outcomes.</p> <p>Methods/Design</p> <p>This randomized clinical trial (RCT) will test the efficacy of a theory-based telephone-delivered Patient Self-Management Support intervention that seeks to enhance adherence to independent exercise and activity among post- TKR patients. The intervention consists of 12 sessions, which begin prior to surgery and continue for approximately 9 weeks post-TKR. The intervention condition will be compared to a usual care control condition using a randomized design and a probabilistic sample of men and women. Assessments are conducted at baseline, eight weeks, and six- and twelve- months. The project is being conducted at a large healthcare system in Massachusetts. The study was designed to provide greater than 80% power for detecting a difference of 4 points in physical function (SF36/Physical Component Score) between conditions (standard deviation of 10) at six months with secondary outcomes collected at one year, assuming a loss to follow up rate of no more than 15%.</p> <p>Discussion</p> <p>As TKR use expands, it is important to develop methods to identify patients at risk for sub-optimal functional outcome and to effectively intervene with the goal of optimizing functional outcomes. If shown efficacious, this peri-TKR intervention has the potential to change the paradigm for successful post-TKR care. We hypothesize that Patient Self-Management Support to enhance adherence to independent activity and exercise will enhance uniform, optimal improvement in post-TKR function and patient autonomy, the ultimate goals of TKR.</p> <p>Trial Registration</p> <p>ClinicalTrials.gov: <a href="http://www.clinicaltrials.gov/ct2/show/NCT00566826">NCT00566826</a></p

    Patient-reported outcome measures of the impact of cancer on patient’s everyday lives: a systematic review

    Purpose: Patients with advanced disease are living longer and commonly used patient-reported outcome measures (PROMs) may miss relevant elements of the quality of extended survival. This systematic review examines the measures used to capture aspects of the quality of survival including impact on patients’ everyday lives such as finances, work and family roles. Methods: Searches were conducted in MEDLINE, EMBASE, CINAHL and PsycINFO restricted to English language articles. Information on study characteristics, instruments and outcomes was systematically extracted and synthesised. A predefined set of criteria was used to rate the quality of studies. Results: From 2761 potentially relevant articles, 22 met all inclusion criteria, including 10 concerning financial distress, 3 on roles and responsibilities and 9 on multiple aspects of social well-being. Generally, studies were not of high quality; many lacked bias free participant selection, had confounding factors and had not accounted for all participants. High levels of financial distress were reported and were associated with multiple demographic factors such as age and income. There were few reports concerned with impacts on patients’ roles/responsibilities in everyday life although practical and emotional struggles with parenting were identified. Social difficulties were common and associated with multiple factors including being a caregiver. Many studies were single time-point surveys and used non-validated measures. Exceptions were employment of the COST and Social Difficulties Inventory (SDI), validated measures of financial and social distress respectively. Conclusions: Impact on some important parts of patients’ everyday lives is insufficiently and inconsistently captured. Further PROM development focussing on roles and responsibilities, including work and caring for dependents, is warranted. Implications for Cancer Survivors: Factors such as finances, employment and responsibility for caring for dependents (e.g. children and elderly relatives) can affect the well-being of cancer survivors. There is a need to ensure that any instruments used to assess patients’ social well-being are broad enough to include these areas so that any difficulties arising can be better understood and appropriately supported

    Protocol of a randomized controlled trial of the effectiveness of physician education and activation versus two rehabilitation programs for the treatment of Whiplash-associated Disorders: The University Health Network Whiplash Intervention Trial

    Background: Whiplash injuries are an important public health problem that is associated with significant disability and high health care utilization. Recent cohort studies suggest that physician care may be the most effective treatment for patients with whiplash-associated disorders. However, these findings have not been tested in a randomized controlled trial. The purpose of this study is to determine which of physician care or two rehabilitation programs of care is most effective in improving recovery of patients with recent whiplash associated disorders. Methods and Design: We designed a pragmatic randomized clinical trial. A total of 444 participants (148 in each of three arms) who reside in Southern Ontario, Canada will be recruited from a large insurer. We will include individuals who are 18 years of age or older and who are diagnosed with Grade I or II Whiplash-associated Disorders. Participants will be randomized to physician-based education and activation or one of two rehabilitation programs of care currently in use in Ontario. Our primary outcome, self-rated global recovery and all secondary outcomes (neck pain intensity, whiplash disability, health-related quality of life, depressive symptomatology and satisfaction with care) will be measured at baseline by a trial coordinator and at 6 weeks, 3, 6, 9 and 12 months follow-up by an interviewer who is blind to the participants' baseline characteristics and treatment allocation. We will also collect information on general health status, other injuries, comorbidities, expectation of recovery, work status, pain coping, legal representation, and co-interventions. The primary intention-to-treat analysis will compare time to recovery between the three interventions. This trial will have 90% power at an alpha of 0.05 to detect a 20% difference in the rate of perceived recovery at one year. Secondary analyses will compare the health outcomes, rate of recurrence and the rate of adverse events between intervention groups. Conclusion: The results of this study will provide the public, clinicians and policy makers much needed evidence on the effectiveness of common approaches used to manage whiplash-associated disorders. © 2008 Côté et al; licensee BioMed Central Ltd

    Body image disturbance and surgical decision making in egyptian post menopausal breast cancer patients

    <p>Abstract</p> <p>Background</p> <p>In most developing countries, as in Egypt; postmenopausal breast cancer cases are offered a radical form of surgery relying on their unawareness of the subsequent body image disturbance. This study aimed at evaluating the effect of breast cancer surgical choice; Breast Conservative Therapy (BCT) versus Modified Radical Mastectomy (MRM); on body image perception among Egyptian postmenopausal cases.</p> <p>Methods</p> <p>One hundred postmenopausal women with breast cancer were divided into 2 groups, one group underwent BCT and the other underwent MRM. Pre- and post-operative assessments of body image distress were done using four scales; Breast Impact of Treatment Scale (BITS), Impact of Event Scale (IES), Situational Discomfort Scale (SDS), and Body Satisfaction Scale (BSS).</p> <p>Results</p> <p>Preoperative assessment showed no statistical significant difference regarding cognitive, affective, behavioral and evaluative components of body image between both studied groups. While in postoperative assessment, women in MRM group showed higher levels of body image distress among cognitive, affective and behavioral aspects.</p> <p>Conclusion</p> <p>Body image is an important factor for postmenopausal women with breast cancer in developing countries where that concept is widely ignored. We should not deprive those cases from their right of less mutilating option of treatment as BCT.</p
