    Validation of Geant4 nuclear reaction models for hadrontherapy and preliminary results with SMF and BLOB

    Reliable nuclear fragmentation models are of utmost importance in hadrontherapy, where Monte Carlo (MC) simulations are used to compute the input parameters of the treatment planning software, to validate the deposited dose calculation, to evaluate the biological effectiveness of the radiation, to correlate the bþ emitters production in the patient body with the delivered dose, and to allow a non- invasive treatment verification. Despite of its large use, the models implemented in Geant4 have shown severe limitations in reproducing the measured secondaries yields in ions interaction below 100 MeV/A, in term of production rates, angular and energy distributions [1–3]. We will present a benchmark of the Geant4 models with double-differential cross sec- tion and angular distributions of the secondary fragments produced in the 12C fragmentation at 62 MeV/A on thin carbon target, such a benchmark includes the recently implemented model INCL++ [4,5]. Moreover, we will present the preliminary results, obtained in simulating the same interaction, with SMF [6] and BLOB [7]. Both, SMF and BLOB are semiclassical one-body approaches to solve the Boltzmann-Langevin equation. They include an identical treatment of the mean-field propagation, on the basis of the same effective interaction, but they differ in the way fluctuations are included. In particular, while SMF employs a Uehling-Uhlenbeck collision term and introduces fluctuations as projected on the density space, BLOB introduces fluctuations in full phase space through a modified collision term where nucleon-nucleon correlations are explicitly involved. Both of them, SMF and BLOB, have been developed to sim- ulate the heavy ion interactions in the Fermi-energy regime. With the aim of enlarging the knowledge on the track stability and covering much of the possible relevant scenarios, an experimental research program has been developed in the framework of a cooperation between RFI, Italcertifer and DII. In order to perform the investigation under quite general conditions and to reduce the experimentation costs, n. 28 significant scenarios have been identified and reproduced on as many independent track segments. By applying on each track segment a transversal load, the strength of the ballast-sleeper interface has been determined. The results relative to the first four scenarios are presented in terms of applied load vs. lateral track displacement diagrams and in more synthetic numerical tables.Peer reviewe

    Apoptotic epitope-specific CD8+ T cells and interferon signaling intersect in chronic hepatitis C virus infection

    CD8(+) T cells specific to caspase-cleaved antigens derived from apoptotic T cells represent a principal player in chronic immune activation (CIA). Here, we found that both apoptotic epitope (AE)-specific and hepatitis C virus (HCV)-specific CD8(+) T cells were mostly confined within the effector memory (EM) or terminally differentiated EM CD45RA(+) cell subsets expressing a dysfunctional T-helper-1-like signature program in chronic (c)HCV infection. However, AE-specific CD8(+) T cells produced tumor necrosis factor (TNF)-α and interleukin-2 at the intrahepatic level significantly more than HCV-specific CD8(+) T cells, despite both populations acquiring high levels of programmed death-1 receptor expression. Contextually, only AE-specific CD8(+) T cells correlated with both interferon-stimulated gene levels in T cells and hepatic fibrosis score. Taken together, these data suggest that AE-specific CD8(+) T cells can sustain CIA by their capacity to produce TNF-α and be resistant to inhibitory signals more than HCV-specific CD8(+) T cells in cHCV infection

    Il monitoraggio dei movimenti e delle deformazioni con tecniche geomatiche di basso costo

    L’osservazione dell’evoluzione di fenomeni di dissesto ha una importanza strategica sul nostro territorio. Un importante precursore di fenomeni franosi è il movimento o la deformazione degli stessi: tali parametri possono essere osservati con tecniche geomatiche. Le misure topografiche o GNSS con ricevitori geodetici (multi-frequenza e multi-costellazione) sono ormai ben consolidate nel trattamento e nella precisione. La realizzazione di sistemi basati su questi sensori non è tuttavia a basso costo, specie se consideriamo tecniche GNSS applicate su molti punti di controllo per una descrizione dettagliata del fenomeno, o installazioni soggette a pericoli oggettivi e dunque da considerarsi praticamente “a perdere”. Ricevitori GNSS singola frequenza sono oggi reperibili a costi minori di cento euro e possono, sotto opportune ipotesi e ingegnerizzazioni, costituire alternative ai ricevitori geodetici. Nell’ambito di un progetto di ricerca finanziato da CSI Piemonte per lo studio di tecnologie di monitoraggio a basso costo, sono stati sviluppati sistemi di acquisizione, trasmissione ed elaborazione dati basati su sensori GNSS singola frequenza di minuscole dimensioni. Il sistema costituisce una sorta di piccola stazione permanente GNSS, integrata con accelerometri per la stima di inclinazione e vibrazioni e con sensori meteorologici. È corredata di micro PC con processore ARM e interfacce di comunicazione wireless a lungo raggio per la trasmissione in tempo reale delle misure. Il sistema risulta caratterizzato da consumi elettrici molto contenuti ed è di rapida installazione in siti anche complessi dal punto di vista dell’accesso, dell’alimentazione e comunicazione. Vengono esposti nella nota i risultati delle prove di accuratezza dei sensori GNSS eseguite in laboratorio, le procedure di calibrazione dei sensori e la loro ingegnerizzazione, oltre ai primi risultati sulle installazioni situate nel nord del Piemonte su alcune zone di dissesto

    Off-label long acting injectable antipsychotics in real-world clinical practice: a cross-sectional analysis of prescriptive patterns from the STAR Network DEPOT study

    Introduction Information on the off-label use of Long-Acting Injectable (LAI) antipsychotics in the real world is lacking. In this study, we aimed to identify the sociodemographic and clinical features of patients treated with on- vs off-label LAIs and predictors of off-label First- or Second-Generation Antipsychotic (FGA vs. SGA) LAI choice in everyday clinical practice. Method In a naturalistic national cohort of 449 patients who initiated LAI treatment in the STAR Network Depot Study, two groups were identified based on off- or on-label prescriptions. A multivariate logistic regression analysis was used to test several clinically relevant variables and identify those associated with the choice of FGA vs SGA prescription in the off-label group. Results SGA LAIs were more commonly prescribed in everyday practice, without significant differences in their on- and off-label use. Approximately 1 in 4 patients received an off-label prescription. In the off-label group, the most frequent diagnoses were bipolar disorder (67.5%) or any personality disorder (23.7%). FGA vs SGA LAI choice was significantly associated with BPRS thought disorder (OR = 1.22, CI95% 1.04 to 1.43, p = 0.015) and hostility/suspiciousness (OR = 0.83, CI95% 0.71 to 0.97, p = 0.017) dimensions. The likelihood of receiving an SGA LAI grew steadily with the increase of the BPRS thought disturbance score. Conversely, a preference towards prescribing an FGA was observed with higher scores at the BPRS hostility/suspiciousness subscale. Conclusion Our study is the first to identify predictors of FGA vs SGA choice in patients treated with off-label LAI antipsychotics. Demographic characteristics, i.e. age, sex, and substance/alcohol use co-morbidities did not appear to influence the choice towards FGAs or SGAs. Despite a lack of evidence, clinicians tend to favour FGA over SGA LAIs in bipolar or personality disorder patients with relevant hostility. Further research is needed to evaluate treatment adherence and clinical effectiveness of these prescriptive patterns
