    Can topological defects mimic the BICEP2B-mode signal?

    We show that the B-mode polarization signal detected at low multipoles by BICEP2 cannot be entirely due to topological defects. This would be incompatible with the high-multipole B-mode polarization data and also with existing temperature anisotropy data. Adding cosmic strings to a model with tensors, we find that B modes on their own provide a comparable limit on the defects to that already coming from Planck satellite temperature data. We note that strings at this limit give a modest improvement to the best fit of the B-mode data, at a somewhat lower tensor-to-scalar ratio of r ≃ 0.15

    Assessment and management of iatrogenic withdrawal syndrome and delirium in pediatric intensive care units across Europe: an ESPNIC survey.

    Analgesia and sedation are essential for the care of children in the pediatric intensive care unit (PICU); however, when prolonged, they may be associated with iatrogenic withdrawal syndrome (IWS) and delirium. We sought to evaluate current practices on IWS and delirium assessment and management (including non-pharmacologic strategies as early mobilization), and to investigate associations between presence of an analgosedation protocol and IWS and delirium monitoring, analgosedation weaning, and early mobilization. A multicenter cross-sectional survey-based study collecting data from one experienced physician or nurse per PICU in Europe was conducted from January to April 2021. We then investigated differences among PICUs that did or did not follow an analgosedation protocol. Among 357 PICUs, 215 (60%) responded across 27 countries. IWS was systematically monitored with a validated scale in 62% of PICUs, mostly using the Withdrawal Assessment Tool-1 (53%). Main first-line treatment for IWS was a rescue bolus with interruption of weaning (41%). Delirium was systematically monitored in 58% of PICUs, mostly with the Cornell Assessment of Pediatric Delirium scale (48%) and the Sophia Observation Scale for Pediatric Delirium (34%). Main reported first-line treatment for delirium was dexmedetomidine (45%) or antipsychotic drugs (40%). Seventy-one percent of PICUs reported to follow an analgosedation protocol. Multivariate analyses adjusted for PICU characteristics showed that PICUs using a protocol were significantly more likely to systematically monitor IWS (Odds Ratio [OR ]1.92, 95% Confidence Interval [CI] 1.01-3.67) and delirium (OR 2.00, 95% CI 1.07-3.72), use a protocol for analgosedation weaning (OR 6.38, 95% CI 3.20-12.71), and promote mobilization (OR 3.38, 95% CI 1.63-7.03). Monitoring and management of IWS and delirium are highly variable among European PICUs. The use of an analgosedation protocol was associated with increased likelihood of monitoring IWS and delirium, performing a structured analgosedation weaning, and promoting mobilization. Education on this topic and interprofessional collaborations are highly needed to help reduce the burden of analgosedation-associated adverse outcomes

Il risultato pi\uf9 innovativo dello studio \ue8 stato il miglioramento delle capacit\ue0 cognitive riscontrato dopo il periodo di allenamento, in entrambi i Gruppi, ma con un\u2019evidenza significativa maggiore per il gruppo sperimentale, che evidenzia l\u2019importanza di comprendere, in un programma di APA, esercizi di educazione respiratoria e routine di esercizi motori e cognitivi combinati, con particolare riferimento alla fluidit\ue0 verbale, alla capacit\ue0 attentiva nonch\ue9 le abilit\ue0 prassiche spesso legate a deficit cognitivi nelle ADL.     Identification of llama KRTAP7-1 and KRTAP8-1 fiber genes and polymorphism screening

    Keratin-associated proteins (KAP)are one of the main structural components of hair fiber. Within this protein group, high glycine tyrosine (HGT)-KAP play a crucial role in the definition of their physical-mechanical properties. Polymorphisms in HGT genes have been associated with the variation of different wool traits in sheeps and goats. The Argentine llama is a fiber-producing animal which is valued by the textile industry. However, the genes encoding for fiber proteins have not yet been identified in this species. Here, we focus on studying the HGT-KRTAP7-1 and KRTAP8-1 genes and their variation using the High Resolution Melting (HRM)technique in a sample of 117 llamas. Four single nucleotide polymorphisms (SNPs)were detected in KRTAP7-1, two of which were non-synonymous substitutions leading to amino acid changes in the protein. Of the 5 polymorphisms identified in KRTAP8-1, c.1-5A > G was located in the Kozak sequence, known to regulate protein synthesis level. The other four, two SNPs and one double nucleotide polymorphism (DNP), were found in the coding region and produced three amino acid replacement: c.43 T > C and c.45C > A (p.Y15Q), c.46 G > T (p.G16W)and c.173 A > G (p.Y58C). In summary, most of the polymorphisms found in both KRTAP7-1 and KRTAP8-1 genes produce non-conservative amino acid changes involving tyrosine and glycine residues, which are essential to maintain HGT protein properties. Therefore, these mutations as well as the regulatory SNP here identified could modify the fiber characteristics. We discuss the possible impact of these polymorphisms on KAP7-1 and KAP8-1 structure and/or interaction with other fiber proteins.Fil: Daverio, Maria Silvana.     The upstream Variable Number Tandem Repeat polymorphism of the monoamine oxidase type A gene influences trigeminal pain-related evoked responses

    Monoamines have an important role in neural plasticity, a key factor in cortical pain processing that promotes changes in neuronal network connectivity. Monoamine oxidase type A (MAOA) is an enzyme that, due to its modulating role in monoaminergic activity, could play a role in cortical pain processing. The X-linked MAOA gene is characterized by an allelic variant of length, the MAOA upstream Variable Number Tandem Repeat (MAOA-uVNTR) region polymorphism. Two allelic variants of this gene are known, the high-activity MAOA (HAM) and low-activity MAOA (LAM). We investigated the role of MAOA-uVNTR in cortical pain processing in a group of healthy individuals measured by the trigeminal electric pain-related evoked potential (tPREP) elicited by repeated painful stimulation. A group of healthy volunteers was genotyped to detect MAOA-uVNTR polymorphism. Electrical tPREPs were recorded by stimulating the right supraorbital nerve with a concentric electrode. The N2 and P2 component amplitude and latency as well as the N2-P2 inter-peak amplitude were measured. The recording was divided into three blocks, each containing 10 consecutive stimuli and the N2-P2 amplitude was compared between blocks. Of the 67 volunteers, 37 were HAM and 30 were LAM. HAM subjects differed from LAM subjects in terms of amplitude of the grand-averaged and first-block N2-P2 responses (HAM>LAM). The N2-P2 amplitude decreased between the first and third block in HAM subjects but not LAM subjects. The MAOA-uVNTR polymorphism seemed to influence the brain response in a repeated tPREP paradigm and suggested a role of the MAOA as a modulator of neural plasticity related to cortical pain processing

    Ulysses contract in psychiatry

    Over the last twenty years we have witnessed a growing focus on the rights of the ill people. The debate on informed consent and a new redefinition of the therapeutic relationship is constantly evolving. With this article, we propose a critical literature review of the so-called "Ulysses contract" or "psychiatric advance directives". It refers to the will that a subject expresses in writing, or orally, about the treatments he or she wishes or does not wish to be subject to if the time comes when it may be impossible to express his/her consent. This can especially occur in those with psychiatric disorders with serious clinical involvement and remitting-relapse (typically bipolar disorder, but also chronic delusional disorders and schizophrenia). In this context, the question is whether during intercritical periods the patient may or may not leave instructions to their care-givers. This aspect opens up to a series of interdisciplinary problems. In this article, we want to show the complexity of this debate from a clinical, ethical, legal and psychodynamic point of view, emphasizing the strengths and the major criticisms of the psychiatric advance directives for each area
