Cesarean scar defects and placental abnormalities – a 3 year survey study
The placenta is an essential organ for embryonic and fetal development, ensuring nutrient uptake, gas exchange (via the mother\u27s blood supply), waste elimination, thermo-regulation, immunological and hormonal factors, etc. The most common placental abnormalities are represented by placenta previa, and a morbidly adherent placenta (in the form of accreta, increta, and percreta placenta). This study was performed on a sample of 99 patients diagnosed with abnormalities of placentation who underwent cesarian delivery during a period of 3 years in Bucur Maternity Hospital. Seven patients were diagnosed with morbidly adherent placenta (5 accreta and 2 percreta subtypes), the others having placenta previa (65 with lateral disposition, 18 marginal, and 9 central insertion). All patients had been diagnosed by ultrasound (which was also used for general monitoring), being confirmed during operation and histopathologically. Complications required 4 emergency peripartum hysterectomies, with no maternal mortality but with fetal death in one case. The research literature shows that about half of women with placenta previa have several episodes of bleeding, being the leading cause of antepartum hemorrhage. For some women with placenta previa/accrete, hemorrhaging is severe and requires hysterectomy as a necessary step to control the life-threatening situation. Thus, such patients should be carefully monitored to avoid as much as possible the medical, social, and psychological implications of this critical therapeutic procedure
The correlation between histopathological and ultrasound findings regarding Cesarean section scars – A three-year survey study
The cesarean operation represents a major surgery, with a higher risk of postoperative complications and longer postoperative recovery than vaginal delivery.
Due to the increasing frequency of cesarean sections, the ultrasound imaging of the uterine scar has become a particularly useful tool in identifying its potential long-term complications. This should be done pre-conceptively and quarterly or whenever necessary during pregnancy.
Currently, there are only few histopathological studies on the uterine scar, trying to assess the myometrial repair and certain factors that influence the quality of the scar.
The study was performed on a batch of 123 patients with previous C-sections, with multiple ultrasound exams during pregnancy and post-operative pathologic evaluation of the uterine scar in order to assess the possibility of a new prognostic score by correlating these two factors.
Our study found solid evidence related to possible correlations between histopathological and ultrasound data on the cesarean section scar, which could lead to a possible predictive algorithm with implications for both prognostic and therapeutic fields

ChatGPT Performances in Explanation of Physiopathology by Example
Physiopathology constitutes one of the basic disciplines that helps understand the mechanisms of diseases and, consequently, access effective and personalized therapies. ChatGPT is a working tool that promptly provides detailed answers to targeted questions, revolutionizing the way medical professionals interact with artificial intelligence (AI). The purpose of this study was to compare the answers provided by human intelligence with experience and professional qualification with the answers provided by ChatGPT, regarding the targeted questions addressed to one clinical case, to decipher the key pathophysiological mechanisms. The input was represented by cases published in Bulboacă et al.1 ChatGPT-3.5 (https://chat.openai.com/) free version was used in our study. We provide one case (in Romanian language) and ask ChatGPT to answer five individual questions. We collected the answers on August 7, 2023. The following request was included in the ChatGPT window: We have the following case: “[case description]”. Answer the following questions: [questions]. The evaluation of the responses given by the ChatGPT was done by one researcher and classified as correct, partially correct, or incorrect. The AI generated answers were partially correct in 4 out of 5 questions and incorrect in one question. ChatGPT can be a valuable instrument for medical teaching, but answer analyses must be added to ensure the validity of the responses and explanations

Discovery and physical characterization as the first response to a potential asteroid collision: The case of 2023 DZ2
Near-Earth asteroids (NEAs) that may evolve into impactors deserve detailed
threat assessment studies. Early physical characterization of a would-be
impactor may help in optimizing impact mitigation plans. We first detected NEA
2023~DZ on 27--February--2023. After that, it was found to have a Minimum
Orbit Intersection Distance (MOID) with Earth of 0.00005~au as well as an
unusually high initial probability of becoming a near-term (in 2026) impactor.
We aim to perform a rapid but consistent dynamical and physical
characterization of 2023~DZ as an example of a key response to mitigate
the consequences of a potential impact. We use a multi-pronged approach,
drawing from various methods (observational/computational) and techniques
(spectroscopy/photometry from multiple instruments), and bringing the data
together to perform a rapid and robust threat assessment.} The visible
reflectance spectrum of 2023~DZ is consistent with that of an X-type
asteroid. Light curves of this object obtained on two different nights give a
rotation period =6.27430.0005 min with an amplitude
=0.570.14~mag. We confirm that although its MOID is among the smallest
known, 2023~DZ will not impact Earth in the foreseeable future as a
result of secular near-resonant behaviour. Our investigation shows that
coordinated observation and interpretation of disparate data provides a robust
approach from discovery to threat assessment when a virtual impactor is
identified. We prove that critical information can be obtained within a few

Employing theoretical resources from Transactional Analysis (TA) and drawing from interviews with managers dealing with social or environmental issues in their role, we explain how CSR activity provides a context for dramas in which actors may ignore, or discount aspects of self, others, and the contexts of their work as they maintain and reproduce the roles of Rescuers, Persecutors and Victims. In doing so, we add to knowledge about CSR by providing an explanation for how the contradictions of CSR are avoided in practice even when actors may be aware of them. Specifically, we theorise how CSR work can produce dramatic stories where adversity is apparently overcome, whilst little is actually achieved at the social level. Systematic review of the evidence relating FEV1 decline to giving up smoking
<p>Abstract</p> <p>Background</p> <p>The rate of forced expiratory volume in 1 second (FEV<sub>1</sub>) decline ("beta") is a marker of chronic obstructive pulmonary disease risk. The reduction in beta after quitting smoking is an upper limit for the reduction achievable from switching to novel nicotine delivery products. We review available evidence to estimate this reduction and quantify the relationship of smoking to beta.</p> <p>Methods</p> <p>Studies were identified, in healthy individuals or patients with respiratory disease, that provided data on beta over at least 2 years of follow-up, separately for those who gave up smoking and other smoking groups. Publications to June 2010 were considered. Independent beta estimates were derived for four main smoking groups: never smokers, ex-smokers (before baseline), quitters (during follow-up) and continuing smokers. Unweighted and inverse variance-weighted regression analyses compared betas in the smoking groups, and in continuing smokers by amount smoked, and estimated whether beta or beta differences between smoking groups varied by age, sex and other factors.</p> <p>Results</p> <p>Forty-seven studies had relevant data, 28 for both sexes and 19 for males. Sixteen studies started before 1970. Mean follow-up was 11 years. On the basis of weighted analysis of 303 betas for the four smoking groups, never smokers had a beta 10.8 mL/yr (95% confidence interval (CI), 8.9 to 12.8) less than continuing smokers. Betas for ex-smokers were 12.4 mL/yr (95% CI, 10.1 to 14.7) less than for continuing smokers, and for quitters, 8.5 mL/yr (95% CI, 5.6 to 11.4) less. These betas were similar to that for never smokers. In continuing smokers, beta increased 0.33 mL/yr per cigarette/day. Beta differences between continuing smokers and those who gave up were greater in patients with respiratory disease or with reduced baseline lung function, but were not clearly related to age or sex.</p> <p>Conclusion</p> <p>The available data have numerous limitations, but clearly show that continuing smokers have a beta that is dose-related and over 10 mL/yr greater than in never smokers, ex-smokers or quitters. The greater decline in those with respiratory disease or reduced lung function is consistent with some smokers having a more rapid rate of FEV<sub>1 </sub>decline. These results help in designing studies comparing continuing smokers of conventional cigarettes and switchers to novel products.</p

Fourteen days of smoking cessation improves muscle fatigue resistance and reverses markers of systemic inflammation
Cigarette smoking has a negative effect on respiratory and skeletal muscle function and is a risk factor for various chronic diseases. To assess the effects of 14 days of smoking cessation on respiratory and skeletal muscle function, markers of inflammation and oxidative stress in humans. Spirometry, skeletal muscle function, circulating carboxyhaemoglobin levels, advanced glycation end products (AGEs), markers of oxidative stress and serum cytokines were measured in 38 non-smokers, and in 48 cigarette smokers at baseline and after 14 days of smoking cessation. Peak expiratory flow (p = 0.004) and forced expiratory volume in 1 s/forced vital capacity (p = 0.037) were lower in smokers compared to non-smokers but did not change significantly after smoking cessation. Smoking cessation increased skeletal muscle fatigue resistance (p < 0.001). Haemoglobin content, haematocrit, carboxyhaemoglobin, total AGEs, malondialdehyde, TNF-α, IL-2, IL-4, IL-6 and IL-10 (p < 0.05) levels were higher, and total antioxidant status (TAS), IL-12p70 and eosinophil numbers were lower (p < 0.05) in smokers. IL-4, IL-6, IL-10 and IL-12p70 had returned towards levels seen in non-smokers after 14 days smoking cessation (p < 0.05), and IL-2 and TNF-α showed a similar pattern but had not yet fully returned to levels seen in non-smokers. Haemoglobin, haematocrit, eosinophil count, AGEs, MDA and TAS did not significantly change with smoking cessation. Two weeks of smoking cessation was accompanied with an improved muscle fatigue resistance and a reduction in low-grade systemic inflammation in smokers
