    Needle stick injuries among dental students: risk factors and recommendations for prevention

    Aim: To evaluate the risk factors of needle stick injuries (NSIs) sustained by undergraduate dental students and nurse students at the King’s College London (KCL) Dental Institute.Materials and methods: A retrospective study evaluated the incident reports relating to NSIs reported over a period of 2 years. Factors including the dental department, study year, and when the injury took place during administration of local anaesthesia (LA) and recapping conventional syringe or clearing work surface or during disposal.Results: This report showed that students are at the highest risk of NSIs at the fourth year of their 5-year BDS course. About one-third of injuries were reported among this group of students followed by year 5 students (25%). Oral surgery clinics were the major source of incident reporting when compared with other specialised dental clinics within the institute. The left hands of the students were the most frequently affected by such injuries and then the right hands of student dental nurses. The attempt of needle recapping of conventional syringes was the least reported mechanism of injuries and constituted only 15% of the total injuries and mainly occurred in third year students. The most frequent injuries among student nurses were during disposal of the needle.Conclusion: Less NSIs occur when using safety syringes. A non-recapping policy with immediate disposal of either the conventional or safety syringe systems after injection would prevent all clearance-related NSIs sustained by nurses. To avoid NSIs, education plays a vital role particularly with effective implementation of the change to safety syringes with appropriate training.Keywords: needle stick injuries; local anaesthetic syringes; safety syringes; dental students; occupational hazards; dental anaesthetic syringe

    Examination of the effects of a new compression garment on skin tissue oxygenation in healthy volunteers

    Objective: Compression devices have been shown to reduce venous stasis, increase blood flow and skin tissue oxygenation (StO2), promoting healthy tissue. This study aimed to explore the efficacy of a new compression garment in three different positions in healthy adults. Methods: In this quantitative study, potential participants were screened and recruited using the Physical Activity Readiness Questionnaire (PAR-Q, Canada). Participants attended three separate, one-hour sessions to evaluate StO2 in supine-lying, chair-sitting and long-sitting positions. StO2 was recorded for 20 minutes pre-, during and post- a 20-minute intervention using a compression garment, TributeWrap (Lohmann-Rauscher, Germany). A repeated-measures analysis of variance (ANOVA) was followed by post-hoc pairwise comparisons. Results: A total of 28 healthy volunteers took part (aged 24.6 ±8.4years; 13 males, 15 females). A significant difference was seen between the three positions (p<0.001). Chair-sitting had the lowest StO2 pre-intervention, increasing StO2 significantly (32.25%, p<0.001) during wear of the compression garment (24.8% higher than baseline post-intervention). No significant difference was seen between long sitting and supine-lying (p=1.000). In contrast, long-sitting and supine-lying StO2 was higher pre-intervention compared with chair-sitting and only increased post-intervention (11% and 16.8% respectively, p<0.001) compared with baseline. Conclusion: The compression garment significantly increased StO2 levels in both seating positions. Further studies are required to determine if increasing StO2 through short intervention sessions with this device has the potential to improve self-management of tissue health in individuals with reduced mobility, oedema or venous insufficiency

    Design thinking and design doing: Describing a process of people-centred innovation

    The research forms part of the author’s long-term enquiry into the challenges of implementing Design Thinking (DT) and its relationship to Inclusive Design (ID), something that has been understudied and under-researched. Both fields advocate research with users and have a history spanning decades, but they have remained largely separate in terms of academic research and practical application. The author was approached by the series editor for his expertise at the intersection of ID and DT. “State of the art” DT ideas and approaches were evaluated through study of circa 50 recent academic publications, papers and journal articles. Field research was based on personal leadership of over 70 ID projects with government, business, and the voluntary sector leading to frontline discoveries and insights. Peer-reviewed academic publication, conference presentation, and keynote delivery helped test ideas before making the link between ID and DT and delivering the publication. This 13,000 word, sole-authored chapter outlines gaps in the long-term effectiveness of DT, outlining five principles that aim to engender a more sustainable approach by aligning it to ID. These five ideas give an overview of newly-articulated frameworks, tools and methods for academic and industry application. The chapter sets a context for 16 other chapters within the publication and establishes the need for more empirical research to link between DT and ID. The ideas in the chapter have been used to direct practice-based research projects and education programmes at the RCA as well as organisations in its network. 700 Hong Kong civil servants have been trained using these ideas, alongside numerous industry organisations including Tata Consulting Services and Panasonic
