    Granuloma debridement and the use of an injectable calcium phosphate bone cement in the treatment of osteolysis in an uncemented total knee replacement

    Polyethylene particulate debris-induced periprosthetic osteolysis is a known complication of knee arthroplasty surgery, and may result in the need for revision surgery. The management of these bony defects can be surgically challenging, and full revisions of well-fixed total knee components can lead to substantial bone loss. We present the case of a 71 year old man who developed knee pain and osteolysis around an uncemented total knee replacement. Due to significant medical comorbidies he was treated by percutaneous cyst granuloma debridement and grafting using an injectable calcium phosphate bone substitute. There were no wound complications, and the patient was allowed to fully weight-bear post-operatively. Histopathology and microbiology of the cyst material confirmed polyethylene granulomata without any evidence of infection. At 6 weeks post-operatively the patient's previous knee pain had resolved, he was able to comfortably fully weight-bear. Preoperative scores (Knee Society Score (KSS) 41, WOMAC score 46.2, and Oxford Knee Score 39) had all improved at the 12-month post-operative review KSS 76, WOMAC 81.7 and Oxford Knee score 21). This is a safe and effective technique with minimal morbidity and may be an appropriate treatment modality when more extensive revision surgery is not possible.     Polymeric Piezoelectric Transducers for Hydrophone Applications

    Conventional ceramic piezoelectric materials have been used in hydrophones for sonarapplications since 1940's. In the last few years since the discovery of polymeric piezoelectrichydrophones, the technology has matured, applications have emerged in extraordinary number ofcases such as underwater navigation, biomedical applications, biomimetics, etc. Hydrophones areused underwater at high hydrostatic pressures. In the presence of hydrostatic pressures, theanisotropic piezoelectric response of ceramic materials is such that it has poor hydrophone performancecharacteristics whereas polymeric piezoelectric materials show enough hydrostatic piezoelectriccoefficients. Moreover, piezoelectric polymers have low acoustic impedance, which is only 2-6 timethat of water, whereas in piezoelectric ceramics, it is typically 11-time greater than that of water. Aclose impedance match permits efficient transduction of acoustic signals in water and tissues. Newlydeveloped hydrostatic-mode polyvinylidene flouride (PVDF) hydrophones use a pressure-releasesystem to achieve improved sensitivity. Recently, voided PVDF materials have been used for makinghydrophones having higher sensitivity and figure of merit than unvoided PVDF materials

    Cystic hygromas, also known as lymphangiomas, are unusual congenital malformations of the lymphatic system and commonly involve the head and neck region or axilla. Involvement of other sites such as breasts is very rare. The preferred mode of treatment for lymphangioma of the breast in adults or children is surgery.     Microencapsulation and Application of Probiotic Bacteria Lactiplantibacillus plantarum 299v Strain

    Microencapsulation is an up-and-coming technology for maintaining the viability of probiotics. However, the effect of core-to-wall ratios and ratios of polysaccharides on the protection of the Lactiplantibacillus plantarum 299v strain has not been deeply discussed. Lyophilization of the Lp. plantarum 299v strain was conducted, and different core-to-wall ratios and ratios of maltodextrin (MD) and resistant starch (RS) were applied. Results demonstrated that the content of MD and RS had an influence on the yield and bulk density in both core-to-wall ratios (1:1 and 1:1.5). In addition, samples coated with a core-to-wall ratio of 1:1.5 had significantly higher viability than those coated with a core-to-wall ratio of 1:1. Moreover, samples coated with core-to-wall ratios of 1:1 and MD:RS 1:1, as well as core-to-wall ratios of 1:1.5 and MD:RS 3:1, had the highest cell number after simulated gastric fluid and simulated intestinal fluid testing, respectively. Furthermore, the optimal formulation for the application of microencapsulated Lp. plantarum 299v in apple juice (serving as a functional beverage) is listed as follows: core-to-wall ratios of 1:1 and MD:RS 1:1, with the fortification method, and stored at 4 °C. After 11 weeks of storage, the cell count was 8.28 log (CFU/mL). This study provided a strategy for Lp. plantarum 299v to achieve high viability in long-term storage and provides an application in functional apple beverages
