    Comparative study of modified Stoppa approach and ilioinguinal approach for pelviacetabular fractures

    Background: The present study was done to study advantages and disadvantages of modified Stoppa approach (MSA) and ilioinguinal approach (IIA) for surgery of pelviacetabular fractures involving anterior column, anterior wall, quadrilateral plate with protrusion and complex fractures, pelvic ring fractures with pubic diastasis or sacroiliac joint disruptions which needs to be stabilized anteriorly.Methods: Study was conducted in Department of Orthopaedics, Government Medical College and Hospital, Aurangabad on patients with pelviacetabular fractures during June 2018 to March 2020. In our study of 25 patients, they were divided into group A containing 13 patients operated using MSA and group B containing 12 patients operated using IIA. Follow up period was 12-18 months (mean=15) and 12-16 months (mean=14) for group A and B, respectively. Patients assessed using modified Merle d’Aubigné score and Matta’s score.Results: Mean modified Merle d’Aubigné score was 16 and 14 for group A and B, respectively (p value=0.89). Mean blood loss and operative time were less in MSA. Superficial infection was found in one patient each of group A and B whereas one patient developed deep infection in group B. One patient each of both group had hip pain suggestive of early arthrosis. In group B, two patients developed meralgia paresthetica. One patient developed external iliac artery thrombosis and inguinal hernia in group B. one patient from group A developed incisional hernia.Conclusions: MSA was better and simpler than IIA with due adequate training and practice to achieve direct access for pelviacetabular fracture reduction and also it requires less operative time, less blood loss and better postoperative outcome.

    DEVELOPMENT AND VALIDATION OF HPLC METHOD FOR SIMULTANEOUS QUANTIFICATION OF VASICINE, GLYCYRRHIZIN AND PIPERINE IN POLY HERBAL COUGH SYRUP

    Objective: The present study was undertaken to develop a rapid, simple, specific and economic high performance liquid chromatographic (HPLC) method has been developed, validated and used for simultaneous quantification of Vasicine, Glycyrrhizin and Piperine in poly herbal cough syrup Methods: An Agilent technologies 1200 Series quaternary pump combined with an Agilent 1200 series photo diode array detector (USA), an Agilent 1200 series vacuum degasser (USA) and an Agilent autosampler injector. Chromatographic separation was performed on a Hiber, prepacked column, C18, Size 250x 4.60 mm, 5µ maintained at 25 °C. PJ (solvent A) and HPLC grade Acetonitrile (solvent B). Results: The HPLC developed for quantization was simple, accurate and specific. The drug follows the beer’s lambert’s law in the concentration range of of Vasicine in concentration range 25–250 μg/ml, glycyrrhizin in concentration range 100–1000 μg/ml and Piperine in concentration range of 20–100μg/ml and exhibited good correlation coefficient and excellent mean recovery. Percentage RSD for precision and accuracy of the method was found to be less than 2%. Conclusion: The present standardization provides a specific and rapid tool in the herbal research, permitting to set quality specifications for identity, transparency and reproducibility of these Phytoconsituents in the herbal Cough syrup

    Clinical profile and obstetric outcome in pregnancies complicated by heart disease: a five year Indian rural experience

    Background: Cardiac diseases are a major cause of morbidity and mortality in pregnant women in the developing world. This study illustrates the problem in rural India focusing on patterns of diseases, clinical features, and pregnancy outcome in these women.Methods: A retrospective analysis of five year data, from 2006 to 2010 for all patients admitted with cardiac disease in pregnancy.Results: Past history of heart disease was present in 70%. Rheumatic heart disease was predominant type in 83% and the chief complaint at admission was breathlessness in 44%.   Mitral stenosis was the commonest lesion in 55% and mitral regurgitation with or without stenosis in 48%. Preeclampsia was seen in 20% and preterm labor in 10%. Spontaneous vaginal delivery was seen in 41% and cesarean in 20.6%.Conclusions: Heart disease in pregnancy is a high risk condition has a major impact on pregnancy. Associated obstetric complications along with lack of knowledge and ignorance regarding the pathology lead to unpleasant obstetric outcomes

    A study on Feasibility of Bioremediation of Crude Oil contaminated soil from Kalol with Indigenous Mixed Culture

    Bioremediation is an efficient technique for treatment of various kinds of contaminants with application of microorganisms and the provision of their kinetics renders implementation of various biochemical characterizations based on rates of decomposition. The present study is based on feasibility of bioremediation for crude oil contaminated soil from agricultural land of Kalol area of Ahmedabad district, Gujarat, India. The experiment was arranged in five batches with descending levels of contamination measured in terms of Total Petroleum Hydrocarbon (TPH); batch A and B with initial contamination of 11.7% TPH and batch C, D, E with 7.3%, 7.24%, 2.3% TPH respectively. The indigenous consortium from collected soil sample was cultured in lab and applied to batch A, C, E whereas unknown culture provided by OTBL (ONGC TERI Biotech Limited) was applied to batch B, C and treated as reference to the other three batches. The growth pattern of indigenous consortium was observed from total colony counts in CFU/ml/day that revealed diauxic growth pattern during stages of development after lag phase. The kinetics of microbial growth using Verhulst model based on diauxic isotherm was plotted. Also the degradation of crude oil in all batches of soil was estimated using solvent extraction technique at regular intervals of time. The degradation rate of crude oil contamination within soil was studied using integral method which showed first order kinetics. Other characteristics of indigenous consortium including morphology and substrate utilization were observed using standard determination methods. The variation in physical and chemical properties of soil such as pH, ORP, conductivity, colour, TPH are observed on prior and latter basis of bioremediation

    The PROMISES study: A mixed methods approach to explore the acceptability of salivary progesterone testing for preterm birth risk among pregnant women and trained frontline healthcare workers in rural India

    Introduction India has an overall neonatal mortality rate of 28/1000 live births, with higher rates in rural India. Approximately 3.5 million pregnancies in India are affected by preterm birth (PTB) annually and contribute to approximately a quarter of PTBs globally. Embedded within the PROMISES study (which aims to validate a low-cost salivary progesterone test for early detection of PTB risk), we present a mixed methods explanatory sequential feasibility substudy of the salivary progesterone test. Methods A pretraining and post-training questionnaire to assess Accredited Social Health Activists (ASHAs) (n=201) knowledge and experience of PTB and salivary progesterone sampling was analysed using the McNemar test. Descriptive statistics for a cross-sectional survey of pregnant women (n=400) are presented in which the acceptability of this test for pregnant women is assessed. Structured interviews were undertaken with ASHAs (n=10) and pregnant women (n=9), and were analysed using thematic framework analysis to explore the barriers and facilitators influencing the use of this test in rural India. Results Before training, ASHAs' knowledge of PTB (including risk factors, causes, postnatal support and testing) was very limited. After the training programme, there was a significant improvement in the ASHAs' knowledge of PTB. All 400 women reported the salivary test was acceptable with the majority finding it easy but not quick or better than drawing blood. For the qualitative aspects of the study, analysis of interview data with ASHAs and women, our thematic framework comprised of three main areas: implementation of intervention; networks of influence and access to healthcare. Qualitative data were stratified and presented as barriers and facilitators. Conclusion This study suggests support for ongoing investigations validating PTB testing using salivary progesterone in rural settings

    Effects of Rapid Heating on Solutionizing Characteristics of Al-Si-Mg Alloys Using a Fluidized Bed

    Effects of rapid heat transfer using a fluidized bed on the heat-treating response of Al-Si-Mg alloys (both unmodified and Sr modified) were investigated. The heating rate in the fluidized bed is greater than in conventional air convective furnaces. Particle size analyses of eutectic Si showed that the high heating rate during fluidized bed solution heat treatment causes faster fragmentation and spherodization of Si particles compared to conventional air convective furnaces. The mechanism of Si fragmentation through fluidized bed processing is through both brittle fracture and neck formation and its propagation. In contrast to this, the mechanism of Si fragmentation using a conventional air convective furnace is through neck formation and propagation. The Sr-modified D357 alloy showed a faster spherodizing rate than the unmodified alloy. Thermal analyses showed an exothermic reaction during solution heat treatment using a fluidized bed due to recrystallization, and coarsening of eutectic Al grains. Whereas the alloy solutionized using a conventional air convective furnace showed two exothermic reactions, one due to annihilation of point defects and the other due to recrystallization, and coarsening of the eutectic grains in the aluminum matrix. The recrystallization temperature of the alloy solutionized in the fluidized bed is lower than those in the conventional air convective furnace. Both tensile strength and elongation of fluidized bed solutionized alloys are greater than those solutionized using the air convective furnace. The optimum heat-treatment time for T4 temper using a fluidized bed for unmodified and Sr-modified alloy was reduced to 60 and 30 minutes, respectively

    SN2019wxt: An ultrastripped supernova candidate discovered in the electromagnetic follow-up of a gravitational wave trigger

    We present optical, radio, and X-ray observations of a rapidly evolving transient SN2019wxt (PS19hgw), discovered during the search for an electromagnetic counterpart to the gravitational-wave (GW) trigger S191213g. Although S191213g was not confirmed as a significant GW event in the off-line analysis of LIGO-Virgo data, SN2019wxt remained an interesting transient due to its peculiar nature. The optical/near-infrared (NIR) light curve of SN2019wxt displayed a double-peaked structure evolving rapidly in a manner analogous to currently known ultrastripped supernovae (USSNe) candidates. This double-peaked structure suggests the presence of an extended envelope around the progenitor, best modeled with two components: (i) early-time shock-cooling emission and (ii) late-time radioactive 56Ni decay. We constrain the ejecta mass of SN2019wxt at Mej ≈ 0.20M⊙, which indicates a significantly stripped progenitor that was possibly in a binary system. We also followed up SN2019wxt with long-term Chandra and Jansky Very Large Array observations spanning ∼260 days. We detected no definitive counterparts at the location of SN2019wxt in these long-term X-ray and radio observational campaigns. We establish the X-ray upper limit at 9.93 × 10−17 erg cm−2 s−1 and detect an excess radio emission from the region of SN2019wxt. However, there is little evidence for SN1993J- or GW170817-like variability of the radio flux over the course of our observations. A substantial host-galaxy contribution to the measured radio flux is likely. The discovery and early-time peak capture of SN2019wxt in optical/NIR observations during EMGW follow-up observations highlight the need for dedicated early, multiband photometric observations to identify USSNe
