    L’endocardite Ă  Bartonella en Tunisie: ParticularitĂ©s lĂ©sionnelles et Ă©volutives

    L'endocardite Ă  Bartonalla est une infection ubiquitaire, son diagnostic est difficile vu qu'il s'agit souvent d'endocardite Ă  hĂ©moculture nĂ©gative. Lebut de cette Ă©tude est d'analyser les particularitĂ©s lĂ©sionnelles et  Ă©volutives de cette entitĂ© dans un pays du nord d'Afrique, la Tunisie et dedĂ©montrer la gravitĂ© de cette infection. Nous avons Ă©tudié  rĂ©trospectivement les dossiers mĂ©dicaux de 20 patients atteints  d'endocardite Ă  Bartonella, confirmĂ©e selon les critĂšres de Dukes modifiĂ©s. L'Ăąge moyen de nos patients Ă©tait 37 ans avec une prĂ©dominance  masculine (SR=3). Tous nos malades avaient un niveau socio-Ă©conomique bas. Le motif essentiel de consultation Ă©tait la dyspnĂ©e, 6 patients Ă©taient admis dans un tableau d'insuffisance cardiaque congestive. Une  prĂ©dilection des lĂ©sions au niveau de la valve aortique a Ă©tĂ© notĂ©e (14 cas). Quatorze patients avaient des vĂ©gĂ©tations endocarditiques avec une taille qui dĂ©passe 10 mm chez 8 malades. La majoritĂ© des patients (18 patients) prĂ©sentaient une rĂ©gurgitation valvulaire massive en rapport principalement avec des mutilations importantes (6 cas de ruptures de cordages mitraux, 2 cas de dĂ©chirures des sigmoĂŻdes aortiques, un cas de perforation valvulaire aortique, un cas de dĂ©sinsertion de prothĂšse  mitrale). Quinze malades (3/4) avaient nĂ©cessitĂ© une chirurgie Ă  la phase active de la maladie, l'indication majeure Ă©tait l'insuffisance cardiaque. Une complication neurologique Ă©tait notĂ©e chez 2 malades et une  complication rĂ©nale chez 3 malades. Treize patients Ă©taient guĂ©ris, 5 malades Ă©taient dĂ©cĂ©dĂ©s et 2 malades opĂ©rĂ©s ont prĂ©sentĂ© une  rĂ©infection Ă  staphylococcus aureus et Ă  candida albicans en  postopĂ©ratoire. L'endocardite Ă  Bartonella est une infection grave. Cette BactĂ©rie possĂšde un potentiel destructif important. Le recours Ă  la chirurgie est quasi constant. La morbi-mortalitĂ© est Ă©levĂ©e. La recherche de cette bactĂ©rie devrait ĂȘtre alors systĂ©matique chez nos malades suspects d'endocardite d'autant plus que la bartonellose est endĂ©mique sur nos terres

    AbstractMR imaging findings of West Nile virus meningoencephalitis during pregnancy are unknown. We report the first case of serologically proved West Nile virus meningoencephalitis complicating pregnancy with MRI findings. MR imaging of the brain revealed abnormal hyperintensity in the periventricular white matter near the left frontal horn and insular left lobe on fluid-attenuated inversion recovery and T2-weighted images. Evolution was favorable, and no obvious fetal consequences of infection were noted after birth. Recognition of the MR imaging appearance of this entity is important because of the expanding epidemic

    Chemical composition, antibacterial and antioxidant activities of essential oils from flowers, leaves and aerial parts of Tunisian Dittrichia Viscosa

    The objective of this work was to determine the chemical composition, the antioxidant and antibacterial activities of the essential oils (EOs) extracted by hydrodistillation from different organs of the D. viscosa: flowers, leaves and aerial parts. The main compounds identified by GC/MS are oxygenated sesquiterpenes. Among these compounds, (E)-nerolidol (40.7%) is the most abundant constituent of flowers’ essential oil while caryophyllene oxide (9.9%), isolongifolan-7-α-ol (10.3%) and α -eudesmol (9.1%) are the major constituents of the leaves’ essential oil. The presence of these compounds in the aerial parts’ essential oil is solely due to those of the flowers and leaves that constitute these aerial parts. The volatile extracts showed antioxidant effects with IC50 values ranging between 9.25 and 9.75 mg.mL−1. On the other hand, EOs showed antibacterial effects on both Gram-positive and Gram-negative bacteria.     Isokinetic muscle strengthening after acquired cerebral damage: A literature review

    AbstractObjectiveIsokinetic strengthening is a rehabilitation technique rarely used in stroke patients. However, the potential benefits of force and endurance training in this population are strongly suspected.MethodThis literature review synthesizes the results of clinical trials on this topic. The research was conducted on PubMed, using “Stroke”, “rehabilitation”, “isokinetic”, “upper limb” and “training” as keywords.ResultsSeventeen studies focusing on the use of isokinetics in assessment or rehabilitation (six studies) following stroke were reviewed. For the lower limb, muscle strength and walking ability improved after isokinetic rehabilitation programs. For the upper limb, the only two studies found in the literature suggest improvement in the strength of the trained muscles, of grip force, of the Fugl-Meyer motor score and of global functional capacities. This review does not reveal any consensus on the protocols to be implemented: type of muscle contraction, velocities
.ConclusionWhile isokinetic strengthening has not proven its efficiency in rehabilitation of the upper limb following stroke, its interest with regard to rehabilitation of the lower limbs has been recognized. Randomized controlled trials in this field are necessary to confirm its efficiency, especially concerning upper arm rehabilitation

    The low-temperature rotational (de-) excitation of C3 (X 1∑g 1) by collisions with He (1S) is studied using an ab initio potential energy surface (PES). This PES has been calculated using the single- and double-excitation coupled-cluster approach with noniterative perturbational treatment of triple excitations [CCSD(T)] and the augmented correlation-consistent triple-ζ basis set (aug-cc-pVTZ) with bond functions. This PES is then incorporated in full close-coupling quantum scattering calculations for collision energies between 0.1 and 50 cm -1 in order to deduce the rate constants for rotational levels of C3 up to j = 10, covering the temperature range 5-15 K. © 2008.     Oil quality and aroma composition of 'Chemlali' olive trees (Olea europaea L.) under three irrigation regimes

    The present work focused on the chemical composition of monovarietal virgin olive oil from the cultivar Chemlali cultivated in the South of Tunisia: sub-arid zone under three different irrigation regimes: stressed, moderate and well irrigation treatment with the restitution of 50, 75 and 100% of crop evapotranspiration (ETc), respectively. Quality characteristics (acidity and peroxide value) and chemical data (antioxidant compound, fatty acids volatile compounds and oxidative stability) were studied in addition to the pomological characteristic of olive fruit. Results show that there were significant differences observed in oil composition according to the irrigation regime applied. Total phenols, bitterness intensity and LOX products content showed the highest values for low irrigation regime, whereas polyunsaturated fatty acid and oxidative susceptibility values had highest values for olive oil from well irrigated trees.     Localisation automatique de champs de saisie sur des images de formulaires couleur par isomorphisme de sous-graphe

    International audienceThis paper presents an approach for spotting textual fields in colored forms. We proceed by locating these fields thanks to their neighboring context which is modeled with a structural representation. First, informative zones are extracted. Second, forms are represented by graphs in which nodes represent colored rectangles while edges represent neighboring links. Finally, the context of the queried region of interest is modeled as a graph. Subgraph isomorphism is applied in order to locate this ROI in the structural representation of a whole document. Evaluated on a 130-document image dataset, experimental results show up that our approach is efficient and that the requested information is found even if its position is changed.Cet article prĂ©sente une approche permettant la localisation de champs de saisie sur des images couleur de formulaires. Ces champs sont localisĂ©s grĂące Ă  une modĂ©lisation structurelle reprĂ©sentant leur contexte. Dans un premier temps, les zones informatives sont ex-traites. Les formulaires sont ensuite reprĂ©sentĂ©s par des graphes au sein desquels les noeuds reprĂ©sentent des rectangles de couleur uniforme tandis que les arcs modĂ©lisent les relations de voisinage.     Developmental variations in plasma leptin, leptin soluble receptor and their molar ratio in healthy infants

    <p>Abstract</p> <p>Background</p> <p>Leptin and its soluble receptor (sOB-R) are important to regulation of body composition but there are no data on the developmental variations in these plasma variables and their relationship with body composition measurements,</p> <p>Methods</p> <p>Weight, length, and body composition (bone, fat and lean mass) by dual energy absorptiometry, and plasma variables were measured in healthy infants at 2, 4, 8 and 12 months.</p> <p>Results</p> <p>15 whites and 29 African Americans (21 males and 23 females) with mean birth weight 3357 +/- 45 (SEM) g and gestation of 39.3 +/- 0.17 weeks were studied. The overall Z score for weight, length and weight for length during the study were 0.00 +/- 0.15, -0.08 +/- 0.11 and 0.12 +/- 0.14 respectively. With increasing age, plasma leptin (1.0 to 18.2, median 5.5 ng/mL) and sOB-R:leptin molar ratio (10.1 to 247.4, median 59.9) were lowered (r = -0.47, p < 0.01; and r = -0.37, p < 0.05 respectively), best predicted by weight Z score and percentage of fat mass, and higher in African American and female. Presence of body composition measurements eliminated the race and gender effect on the plasma variables. Plasma sOB-R (49.5 to 173.9, median 81.3 ng/mL) did not change significantly with age and was correlated and predicted only by body composition measurements.</p> <p>Conclusion</p> <p>In healthy growing infants, plasma leptin but not sOB-R decreases with age. Gender, race and anthropometric measurements are additional physiological determinants predictive of plasma leptin and the receptor:ligand ratio. However, body composition is the only variable that can predict plasma leptin and its soluble receptor and the receptor: ligand ratio; and body composition measurements eliminated the race and gender effect on these plasma variables.</p
