    Plate fixation of clavicle fractures:comparison between early and delayed surgery

    Background: The optimal treatment strategy for clavicle fractures remains a topic of debate. We evaluated our step-wise treatment protocol for patients with clavicle fractures to determine our success rate of conservative treatment. In addition, we evaluated the incidence of complications after clavicle plate fixation in patients undergoing acute surgery vs. delayed surgery. Methods: This was a retrospective analysis in which we registered all patients aged 14 years or older with a clavicle fracture between January 2010 and May 2018 and at least 6 weeks' follow-up. Patients who underwent surgery were included from a prospectively maintained database. Functional outcomes were measured by Disabilities of the Arm, Shoulder and Hand and Constant-Murley scores 6 weeks after surgery. Results: Conservative treatment was successful in 1627 of 1748 patients (93%). Primary fixation was performed in 73 patients (61%) and delayed fixation in 48 (39%). In 8 patients (6.6%), radiologic widening of the acromioclavicular (AC) joint was present after surgery, suggestive of AC injury. The incidence of complications was significantly higher among patients who underwent delayed fixation vs. those who underwent primary fixation: 15 of 48 patients (31.3%) vs. 9 of 73 patients (12.3%). Conclusion: Most patients with clavicle fractures have an excellent outcome using conservative management. Acute surgery can be performed in high-demand patients, resulting in high performance scores. Delayed surgery is associated with a higher risk of complications, although the outcome is generally good. Associated AC joint dislocation found on postoperative radiographs does not influence outcomes. Shared decision making is key, and patients should be well aware of the potential risks and benefits of surgery

    Formation of the simplest amide in molecular clouds: formamide (NH2_{2}CHO) and its derivatives in H2_2O-rich and CO-rich interstellar ice analogs upon VUV irradiation

    The astronomical detection of formamide (NH2_2CHO) toward various star-forming regions and in cometary material implies that the simplest amide might have an early origin in dark molecular clouds at low temperatures. Laboratory studies have proven the efficient NH2_2CHO formation in interstellar CO:NH3_3 ice analogs upon energetic processing. However, it is still under debate, whether the proposed radical-radical recombination reactions forming complex organic molecules remain valid in an abundant H2_2O environment. The aim of this work was to investigate the formation of NH2_2CHO in H2_2O- and CO-rich ices under conditions prevailing in molecular clouds. Therefore, different ice mixtures composed of H2_2O:CO:NH3_3 (10:5:1), CO:NH3_3 (4:1), and CO:NH3_3 (0.6:1) were exposed to vacuum ultraviolet photons in an ultra-high vacuum chamber at 10 K. Fourier-transform infrared spectroscopy was utilized to monitor in situ the initial and newly formed species as a function of photon fluence. The infrared spectral identifications are complementarily secured by a temperature-programmed desorption experiment combined with a quadrupole mass spectrometer. The energetic processing of CO:NH3_3 ice mixtures mainly leads to the NH2_2CHO formation, along with its chemical derivatives such as isocyanic acid (HNCO) and cyanate ion (OCN−^-). The formation kinetics of NH2_2CHO shows an explicit dependency on ice ratios and compositions; the highest yield is found in H2_2O-rich ice. The astronomical relevance of the resulting reaction network is discussed.Comment: 16 pages, 6 figures, 2 table

    Non-western contexts: the invisible half

    Like many other disciplines within the broad area of social sciences (e.g., anthropology, gender studies, psychology, sociology, etc.), consumer research is also highly navigated by scholars from Western countries. This, however, does not mean, by any means, that consumer research is devoted to studying Western contexts only. As evident from the ever-increasing number of regional conferences (e.g., Asia-Pacific and Latin American conferences of the Association for Consumer Research) and non-Western students' enrolment in doctoral programs at Western universities, there are many more researchers (from non-Western countries) who are entering the field and enriching it by their colourful contributions. Yet, given the low number of publications on consumer research in non-Western contexts, it seems that our current knowledge in these societies has a long way to go to flourish. More specifically, and in the domain of consumption culture research, this gap is even further widened by the fact that the culture of consumption in such contexts is largely interpreted with reference to the 'grand narratives' of Western scholars (e.g., Foucault, Mafessoli, Bourdieu, Deleuze, Baudrillard, Nietzsche, Durkheim, Derrida, etc.). Therefore, from an ontological perspective, it seems that our existing knowledge about non-Western societies lies heavily on the 'theoretical structures' that are 'constructed' by Western philosophy as a set of ideas, beliefs, and practices (Said, 1978). As Belk (1995) reminds us, consumption culture always existed in all human societies. What makes contemporary societies different from that of our predecessors' is not the fact that consumption culture did not exist in those societies, but that consumption culture has become a prevailing feature in modern society (Slater, 1997; Lury, 1996; Fırat and Venkatesh, 1995; McCracken, 1988). Therefore, the nature and dynamics of consumption culture in each society should be studied not only against the sociocultural, historical, and economic background of a given context (Western or non-Western) but also with reference to the philosophical and epistemological viewpoints that analyse and interpret cultural practices of that society from within that culture. Addressing such issues, this paper discusses some of the key reasons for lack of theory development in the field from non-western contexts. The paper invites scholars in non-Western contexts to introduce the less articulated, and sometime hidden, body of knowledge from their own contexts into the field of marketing in general and consumer research in particular

    Cytokine-related genes identified from the RIKEN full-length mouse cDNA data set

    To identify novel cytokine-related genes, we searched the set of 60,770 annotated RIKEN mouse cDNA clones (FANTOM2 clones), using keywords such as cytokine itself or cytokine names (such as interferon, interleukin, epidermal growth factor, fibroblast growth factor, and transforming growth factor). This search produced 108 known cytokines and cytokine-related products such as cytokine receptors, cytokine-associated genes, or their products (enhancers, accessory proteins, cytokine-induced genes). We found 15 clusters of FANTOM2 clones that are candidates for novel cytokine-related genes. These encoded products with strong sequence similarity to guanylate-binding protein (GBP-5), interleukin-1 receptor-associated kinase 2 (IRAK-2), interleukin 20 receptor alpha isoform 3, a member of the interferon-inducible proteins of the Ifi 200 cluster, four members of the membrane-associated family 1-8 of interferon-inducible proteins, one p27-like protein, and a hypothetical protein containing a Toll/Interleukin receptor domain. All four clones representing novel candidates of gene products from the family contain a novel highly conserved cross-species domain. Clones similar to growth factor-related products included transforming growth factor beta-inducible early growth response protein 2 (TIEG-2), TGFbeta-induced factor 2, integrin beta-like 1, latent TGF-binding protein 4S, and FGF receptor 4B.     Sizes at birth of parents and their children are known to be correlated, reflecting in part the influence of fetal and maternal genes. Sociodemographic factors, regarded as aspects of the shared environment across generations, would also be expected to contribute, but evidence is limited. In the present study, the authors aimed to quantify the role of the shared environment in explaining intergenerational correlations in birth weight and length by using data across 3 consecutive generations from the Uppsala Birth Cohort Multigenerational Study in Uppsala, Sweden. That study included birth and sociodemographic data on 7,657 singletons born in Uppsala in 1915-1929 (generation 1) and their grandchildren (generation 3). Standard regression and biometric models were used to study the correlations in size at birth of generation 1-generation 3 pairs. The data showed stronger correlations in maternal pairs than in paternal pairs for birth weight (0.125 vs. 0.096, P = 0.02) but not for birth length (0.097 vs. 0.093, P = 0.77). These correlations were not reduced by adjustment for sociodemographic factors in regression models. In contrast, significant shared-environment contributions to the intergenerational correlations were identified in biometric models, averaging 14% for both birth measures. These models assumed a common latent factor for the sociodemographic variables. The present results show that the shared environment moderately but significantly contributes to intergenerational correlations

    Sorting of chromosomes by magnetic separation

    Chromosomes were isolated from Chinese hamster x human hybrid cell lines containing four and nine human chromosomes. Human genomic DNA was biotinylated by nick translation and used to label the human chromosomes by in situ hybridization in suspension. Streptavidin was covalently coupled to the surface of magnetic beads and these were incubated with the hybridized chromosomes. The human chromosomes were bound to the magnetic beads through the strong biotin-streptavidin complex and then rapidly separated from nonlabeled Chinese hamster chromosomes by a simple permanent magnet. The hybridization was visualized by additional binding of avidin-FITC (fluorescein) to the unoccupied biotinylated human DNA bound to the human chromosomes. After magnetic separation, up to 98% of the individual chromosomes attached to magnetic beads were classified as human chromosomes by fluorescence microscopy

    Performance model for “Just-in-Time” problems in real-time multimedia applications

    Over the last few years, the use of large-scale multimedia data applications has been growing tremendously, and this growth is not likely to slow down in the near future. Many multimedia applications operate in a real-time environment (e.g., surveillance cameras, iris scans), which must meet strict time constraints, i.e. to analyze video frames at the same rate as a camera produces them. To meet this requirement, Grid computing is rapidly becoming indispensable. However, the variabilities of the software and the hardware in grid environment cause the strong burstiness in the transmission delay of video frames. Because the burstiness is unknown beforehand, it is difficult to determine the right sending moments of video frames. If the time interval between sending two sequential frames is too large, then the service utilization may be low. If use large buffer to guarantee the service utilization, then video frames may be outof- date because of the long waiting time at buffer in the server side. This problem is referred to as “Just-in-time” problem. To solve this problem, it is essential to determine the right sending moments of video frames, properly dealing with the trade-off between the service utilization and the “up-to-date” of video frames. Motivated by this, in this paper we develop an adaptive control method that react to the continuously changing circumstances in grid system so as to obtain the highest service utilization on the one hand and to keep the video frame up-to-date on the other hand. Extensive experimental validation in our DAS-3 testbed and the trace-driven simulation show that our method is indeed highly effective
