Algoritmos generales para simuladores de cirugía laparoscópica
Recent advances in fields such as modeling of deformable objects, haptic technologies, immersive technologies,
computation capacity and virtual environments have created the conditions to offer novel and suitable training tools and learning methods
in the medical area. One of these training tools is the virtual surgical simulator, which has no limitations of time or risk, unlike conventional
methods of training. Moreover, these simulators allow for the quantitative evaluation of the surgeon performance, giving the possibility to
create performance standards in order to define if the surgeon is well prepared to execute a determined surgical procedure on a real patient.
This paper describes the development of a virtual simulator for laparoscopic surgery. The simulator allows the multimodal
interaction between the surgeon and the surgical virtual environment using visual and haptic feedback devices. To make the
experience of the surgeon closer to the real surgical environment a specific user interface was developed. Additionally in this paper
we describe some implementations carried out to face typical challenges presented in surgical simulators related to the tradeoff
between real-time performance and high realism; for instance, the deformation of soft tissues are simulated using a GPU (Graphics
Processor Unit) -based implementation of the mass-spring model. In this case, we explain the algorithms developed taking into
account the particular case of a cholecystectomy procedure in laparoscopic surgery.Recientes avances en áreas tales como modelación computacional de objetos deformables, tecnologías hápticas, tecnologías
inmersivas, capacidad de procesamiento y ambiente virtuales han proporcionado las bases para el desarrollo de herramientas y métodos de
aprendizaje confiables en el entrenamiento médico. Una de estas herramientas de entrenamiento son los simuladores quirúrgicos virtuales,
los cuales no tienen limitaciones de tiempo o riesgos a diferencia de los métodos convencionales de entrenamiento. Además, dichos
simuladores permiten una evaluación cuantitativa del desempeño del cirujano, dando la posibilidad de crear estándares de desempeño con
el fin de definir en qué momento un cirujano está preparado para realizar un determinado procedimiento quirúrgico sobre un paciente.
Este artículo describe el desarrollo de un simulador virtual para cirugía laparoscópica. Este simulador permite la interacción
multimodal entre el cirujano y el ambiente virtual quirúrgico usando dispositivos de retroalimentación visual y háptica. Para hacer
la experiencia del cirujano más cercana a la de una ambiente quirúrgico real se desarrolló una interfaz cirujano-simulador especial.
Adicionalmente en este artículo se describen algunas implementaciones que solucionan los problemas típicos cuando se desarrolla un
simulador quirúrgico, principalmente relacionados con lograr un desempeño en tiempo real mientras se sacrifica el nivel de realismo
de la simulación: por ejemplo, la deformación de los tejidos blandos simulados usando una implementación del modelo masa-resorte
en la unidad de procesamiento gráfico. En este caso se describen los algoritmos desarrollados tomando en cuenta la simulación de un
procedimiento laparoscópico llamado colecistectomía
Detección, rastreo y reconstrucción tridimensional de marcadores pasivos para análisis de movimiento humano. Cinemed III
Uno de los desafíos más importantes cuando se desarrolla un sistema de análisis de movimiento es la reconstrucción
tridimensional, ya que este proceso involucra una serie de algoritmos que tienen que trabajar de forma sinérgica con el fin de
reconstruir el movimiento correctamente y aminorando el número de intervenciones del usuario. Por esta razón en este artículo,
se realiza una descripción detallada de un esquema para la reconstrucción tridimensional del movimiento humano mediante la
integración de diferentes algoritmos que a su vez minimizan la intervención del usuario durante el proceso (Sistema de análisis
de movimiento CineMED III). Para probar el desempeño del esquema propuesto se realizaron tres pruebas experimentales; un
marcador estático, barra con dos marcadores en movimiento y dispositivo de medición de errores. Obteniendo finalmente que
CineMED III posee un desempeño cercano al de los sistemas comerciales, mostrando en las pruebas realizadas con la barra un
error absoluto medio menor respecto al sistema Vicon, pero mayor con respecto al sistema Elite y en las pruebas con el dispositivo
un valor de RMS mayor respecto al sistema Ariel, pero un error máximo menor respecto a este ultimo sistema, sin embargo la
aproximación propuesta presenta problemas cuando se ocluyen marcadores por largos periodos de tiempo y cuando se cruza la
trayectoria de dos marcadores. Para resolver dichos problemas se propone adicionar algunas cámaras al sistema y así obtener
información redundante de la captura del movimiento.One of the most important challenges when a movement analysis system is been developed is the three-dimensional
reconstruction. This process involves several algorithms that must work synergistically in order to reconstruct the movement
correctly and decreasing the user interventions. For this reason this paper describes in detail a new methodology to carry out the
three-dimensional reconstruction of the human movement through the join of different algorithms decreasing the user input during
the process (movement analysis system CineMED III). To analyze and evaluate the performance achieved using the methodology
proposed, three experimental tests were carried out; a static marker test, bar with two markers in motion and using an error
measurement device. CineMED III finally has a performance close to that of commercial systems, showing in the tests performed
with the bar a lower average absolute error for the Vicon system, but higher with respect to the Elite system and testing the device
with a value of RMS more about the Ariel system, but a smaller maximum error on the latter system., However, the proposed
approach presents problems when markers are occluded for long periods of time and when the paths of two markers intersect. To
solve these problems, it is proposed to add some cameras into the system to obtain redundant information from the motion capture

Descripción de un dispositivo destinado al análisis de la marcha en dos dimensiones, CineMED
Este artículo presenta un dispositivo para el análisis cinemático de la marcha humana. El dispositivo propuesto
se basa en el uso de técnicas de videogrametría y procesamiento de imágenes para registrar el movimiento
humano. Con este fin, se utilizó una cámara de video digital, ubicada a 4 metros del paciente, capturando su
marcha en el plano sagital. También se desarrolló un modelo cinemático de los miembros inferiores que, a partir
del desplazamiento de cierto número de puntos anatómicos, determina los ángulos articulares de la cadera, rodilla
y tobillo, además de la inclinación de la pelvis durante un ciclo completo de la marcha. La identificación de
dichos puntos fue facilitada mediante el uso de marcadores reflectivos, ubicados en siete posiciones anatómicas
diferentes. Después, se diseñó y desarrolló un programa para el registro y procesamiento de los datos obtenidos,
por medio de la cámara en el lenguaje de programación C# .NET. La aplicación desarrollada captura y preprocesa
el video, detecta cada uno de los marcadores en la imagen, aplica el modelo cinemático y visualiza a modo de
gráfica el valor de los ángulos articulares de los miembros inferiores, a lo largo del ciclo de la marcha. Por fin, se
diseñó un protocolo y se realizó una prueba experimental con un sujeto, para comparar las curvas entregadas por
el software y las curvas teóricas encontradas en la literatura. A pesar de las limitaciones del sistema referentes al
análisis bidimensional de la marcha, se lograron obtener curvas articulares muy similares a las teóricas, obtenidas
con métodos comerciales para el análisis del movimiento tridimensional.This paper introduces a device for the gait kinematic analysis. It is based on videogrametry methods
and image processing in order to record the human movement. It was used a digital camcorder, located
to a distance of four meters from the person, capturing its gait in the sagital plane. Also, a kinematic model
was developed of lower limbs; this model takes the position data of the bony landmarks and calculates the
joint angle of the hip, knee and ankle, besides the pelvic tilt during gait. The identification and tracking of
the points was carried out by a special reflective coated in the superficial markers. Later, it was designed and
developed an algorithm in order to record and process the data gathered, through of the camcorder in the
develop language C# .NET. The application developed captures and processes the video, tracking every
marker in the frame, applies the kinematic model and shows the graph of the joint angle during gait. Finally,
a protocol was designed and a test was carried out with a person in order to compare the data gathered
for the software and the theoretical data described in the literature. In spite of the limitations of the system
related to gait bidimensional analysis, articular curves very similar to those theoretical were obtained with
commercial methods for the tridimensional movement analysis

Descripción de un sistema para la medición de las presiones plantares por medio del procesamiento de imágenes. Fase I
La medida de la presión plantar es fundamental para conocer la distribución de las fuerzas en la planta del pie. Estos valores son importantes para el diagnóstico y tratamiento de diferentes patologías como úlceras diabéticas en la planta del pie y presencia de deformidades en extremidades inferiores, entre otras. En el presente artículo se describe el desarrollo de un podobarógrafo para el análisis de la huella plantar, cuyo desarrollo incluyó la fabricación de un podoscopio, la selección de un material transductor adecuado y el desarrollo del software para el procesamiento de las imágenes capturadas por una cámara de video. El dispositivo propuesto PodoMED permite realizar un análisis dinámico y estático de la huella, pues aparte de mostrar cómo es la distribución de presiones en la planta del pie durante la marcha, por medio de imágenes en seudocolor, tiene la opción de emitir una valoración de la huella, donde se muestran medidas como el índice de valgo, el ángulo de Clarke, el índice de Hernández Corvo, entre otros. Para el desarrollo de este proyecto se realizaron ensayos experimentales para minimizar el ruido de la imagen, lo cual se logró con la utilización de un filtro físico y otro para la imagen por medio de software. Para una etapa futura, se planea la calibración y validación del sistema, con lo cual se pretende brindar mayor confiabilidad y repetibilidad.The measurement of the plantar pressure is basic to know the force distribution in the sole. These values are important to diagnosis and treat different pathologies as diabetic ulcerate in the sole, bone, and muscular deformations in the lower limb, etc. This paper describes the development of a pedobarograph in order to analyze the sole. The development can be classified in three stages, building of the Podoscopio, selection of the transducer material and development of the software in order to processes the picture captured by the camcorder. The device PodoMED makes a dynamic and static analysis of the sole, using pseudocolor images for walking analysis and podological indexes as valgo index, Clarke angle, Hernandez Corvo index, etc. for static analysis. In the development of this project we carried out experimental test to minimize the noise present in the image. It was achieved by using a physical filter and image processing filter. In a future phase, it plans the system calibration and validation, which expects to offer more reliability itself

Effects of pre-operative isolation on postoperative pulmonary complications after elective surgery: an international prospective cohort study an international prospective cohort study
We aimed to determine the impact of pre-operative isolation on postoperative pulmonary complications after elective surgery during the global SARS-CoV-2 pandemic. We performed an international prospective cohort study including patients undergoing elective surgery in October 2020. Isolation was defined as the period before surgery during which patients did not leave their house or receive visitors from outside their household. The primary outcome was postoperative pulmonary complications, adjusted in multivariable models for measured confounders. Pre-defined sub-group analyses were performed for the primary outcome. A total of 96,454 patients from 114 countries were included and overall, 26,948 (27.9%) patients isolated before surgery. Postoperative pulmonary complications were recorded in 1947 (2.0%) patients of which 227 (11.7%) were associated with SARS-CoV-2 infection. Patients who isolated pre-operatively were older, had more respiratory comorbidities and were more commonly from areas of high SARS-CoV-2 incidence and high-income countries. Although the overall rates of postoperative pulmonary complications were similar in those that isolated and those that did not (2.1% vs 2.0%, respectively), isolation was associated with higher rates of postoperative pulmonary complications after adjustment (adjusted OR 1.20, 95%CI 1.05–1.36, p = 0.005). Sensitivity analyses revealed no further differences when patients were categorised by: pre-operative testing; use of COVID-19-free pathways; or community SARS-CoV-2 prevalence. The rate of postoperative pulmonary complications increased with periods of isolation longer than 3 days, with an OR (95%CI) at 4–7 days or ≥ 8 days of 1.25 (1.04–1.48), p = 0.015 and 1.31 (1.11–1.55), p = 0.001, respectively. Isolation before elective surgery might be associated with a small but clinically important increased risk of postoperative pulmonary complications. Longer periods of isolation showed no reduction in the risk of postoperative pulmonary complications. These findings have significant implications for global provision of elective surgical care.
