    Malnutrition According to GLIM Criteria Is Associated with Mortality and Hospitalizations in Rehabilitation Patients with Stable Chronic Obstructive Pulmonary Disease

    Malnutrition has a negative impact on patients with chronic pulmonary obstructive disease (COPD). The purpose of this study was to assess the prevalence of malnutrition, defined by the Global Leadership Initiative for Malnutrition (GLIM), in stable COPD patients referred to pulmonary rehabilitation, and to explore potential associations of malnutrition according to GLIM, and its components, with increased risk of mortality and hospitalizations in 2 years. In a post-hoc analysis of a prospective cohort of 200 rehabilitation patients with stable COPD, main outcome variables were hospital admissions, length of stay, and mortality during a 2-year follow-up. Covariates were malnutrition according to GLIM and its phenotypic criteria: unintentional weight loss, low body mass index (BMI), and low fat-free mass (FFM). Univariate and multivariate analysis were performed using logistic and proportional hazard Cox regression. Malnutrition according to GLIM showed 45% prevalence and was associated with increased mortality risk. Low age-related BMI and FFM were independently associated with mortality, which persisted after adjustment for age and lung function. Malnutrition and low BMI were also associated with increased risk of hospitalization. Malnutrition according to GLIM criteria was highly prevalent in rehabilitation patients with COPD and was associated with nearly 3 times greater mortality and hospitalization risk

    Esta tesis aborda la malnutrición y la sarcopenia en pacientes con enfermedades crónicas, centrándose en la enfermedad pulmonar obstructiva crónica (EPOC) y la enfermedad renal crónica. Utilizando los criterios de la Global Leadership Initiative for Malnutrition (GLIM) y los criterios EWGSOP2, se evaluó la prevalencia de la malnutrición en pacientes con EPOC estable y candidatos a trasplante de riñón, así como la utilidad de Redes Neuronales Artificiales (ANN) para identificar factores pronósticos en receptores de trasplante pulmonar (TP). En el estudio de pacientes con EPOC (1), se encontró una prevalencia del 45% de malnutrición según GLIM, asociada a un riesgo significativamente mayor de mortalidad y hospitalización. En el estudio de trasplante de riñón (2), la malnutrición se asoció con un mayor riesgo de hospitalizaciones durante el primer año en la lista de espera. En receptores de TP (3), el 41.4% experimentó una alta tasa de pérdida de datos, siendo el bajo índice de masa corporal el factor más influyente, seguido por la indicación del trasplante, baja calidad de vida y fuerza de agarre reducida. Estos hallazgos destacan la importancia de abordar la malnutrición y la sarcopenia en pacientes con enfermedades crónicas, identificando factores de riesgo y proponiendo estrategias de intervención.This thesis addresses malnutrition and sarcopenia in patients with chronic diseases, focusing on chronic obstructive pulmonary disease (COPD) and chronic kidney disease. Using the criteria from the Global Leadership Initiative for Malnutrition (GLIM) and EWGSOP2 criteria, the prevalence of malnutrition was assessed in stable COPD patients and kidney transplant candidates, along with the utility of Artificial Neural Networks (ANN) in identifying prognostic factors in lung transplant recipients (TP). In the COPD patient study (1), a 45% prevalence of malnutrition according to GLIM was found, associated with a significantly higher risk of mortality and hospitalization. In the kidney transplant study (2), malnutrition was associated with an increased risk of hospitalizations during the first year on the waiting list. In TP recipients (3), 41.4% experienced a high data loss rate, with a low body mass index being the most influential factor, followed by transplant indication, low quality of life, and reduced grip strength. These findings underscore the importance of addressing malnutrition and sarcopenia in patients with chronic diseases, identifying risk factors, and proposing intervention strategies.Programa de Doctorat en Biomedicin

Whilst sarcopenia is a nutrition-related disease, malnutrition and cachexia are nutritional disorders sharing the common feature of low fat-free mass. However, they have differential characteristics and etiologies, as well as specific therapeutic approaches. Applying the current definitions in clinical practice is still a challenge for health professionals and the potential for misdiagnosis is high. This is of special concern in the subgroup of older people with cancer, in which sarcopenia, malnutrition, and cancer cachexia are highly prevalent and can overlap or occur separately. The purpose of this review is to provide an updated overview of the latest research and consensus definitions of sarcopenia, malnutrition, and cachexia and to discuss their implications for clinical practice in older patients with cancer. The overall aim is to improve the quality of nutritional care in light of the latest findings

    The feasibility and reliability of measuring forearm muscle thickness by ultrasound in a geriatric inpatient setting : a cross-sectional pilot study

    Given the potential benefits of introducing ultrasound in the clinical assessment of muscle disorders, this study aimed to assess the feasibility and reliability of measuring forearm muscle thickness by ultrasound in a geriatric clinical setting. Cross-sectional pilot study in 25 participants (12 patients aged ≥ 70 years in an acute geriatric ward and 13 healthy volunteers aged 25-50 years), assessed by three raters. Muscle thickness measurement was estimated as the distance between the subcutaneous adipose tissue-muscle interface and muscle-bone interface of the radius at 30% proximal of the distance between the styloid process and distal insertion of the biceps brachii muscle of the dominant forearm. Examinations were repeated three times by each rater and intra- and inter-rater reliability was calculated. Feasibility analysis included consideration of technological, economic, legal, operational, and scheduling (TELOS) components. Mean muscle-thickness measurement difference between groups was 4.4 mm (95% confidence interval [CI] 2.4 mm to 6.3 mm], p < 0.001). Intra-rater reliability of muscle-thickness assessment was excellent, with intraclass correlation coefficient (ICC) of 0.947 (95%CI 0.902 to 0.974), 0.969 (95%CI 0.942 to 0.985), and 0.950 (95%CI 0.907 to 0.975) for observer A, B, and C, respectively. Inter-rater comparison showed good agreement (ICC of 0.873 [95%CI 0.73 to 0.94]). Four of the 17 TELOS components considered led to specific recommendations to improve the procedure's feasibility in clinical practice. Our findings suggest that US is a feasible tool to assess the thickness of the forearm muscles with good inter-rater and excellent intra-rater reliability in a sample of hospitalized geriatric patients, making it a promising option for use in clinical practice

    Frailty among chronic kidney disease patients on the kidney transplant waiting list : the sex-frailty paradox

    Frailty is defined as decreased physiologic reserve and resistance to stressors that predisposes patients towards poor health results. Its prevalence in chronic kidney disease (CKD) patients who are kidney transplant (KT) candidates is high. Frailty is associated with a higher rate of complications and mortality after transplant. It is unknown whether frailty phenotype differs depending on sex in this population. This was a prospective longitudinal study of 455 KT candidates evaluated for frailty by physical frailty phenotype at the time of inclusion on the KT waiting list. Pre-frailty was defined as the presence of two criteria and frailty as three or more criteria. Univariate and multivariate analyses searched for associations of frailty status, frailty components and gender differences. Thirty percent of the total cohort resulted to be pre-frail (20%) or frail (10.3%), but disparities were observed between sexes, with 22.5% of men and 47.2% of women falling into one of these categories. Among frailty criteria, women presented with a higher percentage of exhaustion (39.6% versus 17%) and slowness (22.2% versus 9.6%) compared with men. Comorbidity burden was higher among frail men, whereas social factors were poorer between frail women. Disability was common among those patients who were frail, both men and women. Frailty is twice as frequent in advanced CKD women as men. Frailty criteria distribution and phenotype seem to differ among sexes, which might have implications in terms of specific and individualized interventions to improve their status before transplantation
