    Clinical and biochemical response to neridronate treatment in a patient with osteoporosis-pseudoglioma syndrome (OPPG)

    Osteoporosis-pseudoglioma syndrome (OPPG) is a rare autosomal recessive syndrome characterized by juvenile-onset osteoporosis and ocular abnormalities due to a low-density lipoprotein receptor-related protein 5 (LRP5) gene mutation. Treatment with bisphosphonates, particularly with pamidronate and risedronate, has been reported to be of some efficacy in this condition. We report on a patient with OPPG due to an LRP5 gene mutation, who showed an encouraging response after a 36-month period of neridronate therapy. We report a case of a patient treated with bisphosphonates. Bisphosphonates should be administered in OPPG patients as a first-line therapy during early childhood

    Gut microbiota related to Giardia duodenalis, Entamoeba spp. and Blastocystis hominis infections in humans from CĂ´te d'Ivoire.

    INTRODUCTION: Literature data provide little information about protozoa infections and gut microbiota compositional shifts in humans. This preliminary study aimed to describe the fecal bacterial community composition of people from CĂ´te d'Ivoire harboring Giardia duodenalis, Entamoeba spp., and Blastocystis hominis, in trying to discover possible alterations in their fecal microbiota structure related to the presence of such parasites. METHODOLOGY: Twenty fecal samples were collected from people inhabiting three different localities of CĂ´te d'Ivoire for copromicroscopic analysis and molecular identification of G. duodenalis, Entamoeba spp., and B. hominis. Temporal temperature gradient gel electrophoresis (TTGE) was used to obtain a fingerprint of the overall bacterial community; quantitative polymerase chain reaction (qPCR) was used to define the relative abundances of selected bacterial species/group, and multivariate statistical analyses were employed to correlate all data. RESULTS: Cluster analysis revealed a significant separation of TTGE profiles into four clusters (p < 0.0001), with a marked difference for G. duodenalis-positive samples in relation to the others (p = 5.4Ă—10-6). Interestingly, qPCR data showed how G. duodenalis-positive samples were related to a dysbiotic condition that favors potentially harmful species (such as Escherichia coli), while Entamoeba spp./B. hominis-positive subjects were linked to a eubiotic condition, as shown by a significantly higher Faecalibacterium prausnitzii-Escherichia coli ratio. CONCLUSIONS: This preliminary investigation demonstrates a differential fecal microbiota structure in subjects infected with G. duodenalis or Entamoeba spp./B. hominis, paving the way for using further next-generation DNA technologies to better understand host-parasite-bacteria interactions, aimed at identifying potential indicators of microbiota changes

    A proxy cost model for tramway services

    In this paper, we build a proxy cost model for tramway services. we estimate separately: (i) transport services production costs; (ii) infrastructure costs; (iii) maintenance costs; (iv) administrative and general costs and (v) the cost of capital. we apply the proposed methodology to estimate the standard cost of italian tramway services. detailed data about costs, technical and environmental characteristics were collected by means of questionnaires sent to italian companies providing 100% of tramway services in 2012. we perform a simulation study in order to highlight the marginal impact of efficiency gains obtained by manipulating cost-driving variables both under the control of the operators (trains and drivers productivity) and of the local authority who assigns the service (number of train revenue kilometers (trK) assigned within the service contract, average fleet age). the simulations show how the local authority should allocate extra resources if it wants to increase the quality-quantity mix of tramway services. our results might help the decision-maker to define the maximum economic compensation (auction base) in competitive tendering procedures or a benchmark for the bargaining with the local monopolist

    A Geometric Index Reduction Method for Implicit Systems of Differential Algebraic Equations

    This paper deals with the index reduction problem for the class of quasi-regular DAE systems. It is shown that any of these systems can be transformed to a generically equivalent first order DAE system consisting of a single purely algebraic (polynomial) equation plus an under-determined ODE (a differential Kronecker representation) in as many variables as the order of the input system.     Development of a Moderated Online Intervention to Treat Social Anxiety in First-Episode Psychosis.

    Background: It is well established that social anxiety disorder (SAD) is a significant clinical problem for individuals with a psychotic disorder. Comorbid social anxiety in individuals with psychosis has been associated with poorer premorbid functioning, increased depression, and a reduced quality of life. Cognitive behavior therapy (CBT) is recommended for people with psychosis as a first-line psychological treatment; however, its focus and evaluation primarily revolves around reducing psychotic symptoms and not necessarily targeting comorbid social anxiety symptoms. We developed a novel online social cognitive behavioral intervention (entitled EMBRACE) specifically designed to treat social anxiety symptoms in first episode psychosis (FEP). Methods: The key clinical and engagement features of the intervention were established through integrating evidence-based material derived from 1) CBT-based treatment models for SAD, 2) relevant literature findings related to psychosis and its clinical correlates (e.g., shame, social rank, and its relationship with social anxiety and paranoia), 3) feedback from youth focus groups in order to inform a user-centered intervention design, and 4) a highly multidisciplinary collaborative development approach to design therapy comics. Results: A detailed description of the final version of the 12-week online social intervention to treat social anxiety in FEP is presented. Conclusion: The EMBRACE intervention was designed to provide young people with the necessary skills and confidence to overcome social anxiety within a supportive, safe online space. By design, it allows young people the opportunity to practice their newly learnt skills to connect with others and in doing so, learn to embrace their true authentic selves

    Among the known causative genes of familial ALS, SOD1 mutation is one of the most common. It encodes for the ubiquitous detoxifying copper/zinc binding SOD1 enzyme, whose mutations selectively cause motor neuron death, although the mechanisms are not as yet clear. What is known is that mutant-mediated toxicity is not caused by loss of its detoxifying activity but by a gain-of-function.     Solution conformation and dynamics of the ion pairs originating from the reaction of B(C6F5)(3) with bisindenyl dimethyl zirconium complexes

    The two ion pairs [(4,7-Me(2)indenyl)(2)ZrMe](+)[MeB(C6F5)(3)](-) (1b) and [(indenyl)(2)ZrMe](+) [MeB(C6F5)(3)](-) (2b) have been generated in situ by reaction of stoichiometric B(C6F5)(3) with the corresponding dimethyl zirconocenes. It has been shown that molecular mechanics computations, guided by experimental H-1/H-1 NOE correlations, can provide information on the conformers present in solution. The dynamics of the ion pairs has also been investigated, showing the occurrence of both the processes previously characterized for this class of compounds, namely the B(C6F5)(3) migration between the two methyl groups and dissociation-recombination of the whole [MeB(C6F5)(3)](-) anion, the latter process being much faster than the first one (about three order of magnitude). Moreover, it has been shown that in certain conditions intermolecular processes can occur, which mimic the above-mentioned dissociative exchanges. In particular, the presence of species containing loosely bound [MeB(C6F5)(3)](-) anion fastens the exchange of this anion, while the presence of free B(C6F5)(3) accelerates its exchange between the two methyl sites
