    Advocating for the inclusion of operatic collaborative piano curricula in higher education: Informing curricula through lived experience

    The purpose of this article was to examine how the lived experiences of five professional collaborative pianists during the 35th International Hans Gabor Belvedere Singing Competition, that was held in Cape Town in 2016, could inform the inclusion of new curricula for specialised operatic collaborative piano modules in tertiary institutions. Based on the literature reviewed and themes that emerged from the data, a curriculum could include the following aspects: a knowledge of the rules of lyric diction in foreign languages; excellent sight-reading skills; knowledge of the art of orchestral reduction and repertoire; the ability to transcribe and reduce full opera scores. One personal skill that is not often developed and should also be included is the ability of a pianist to evince empathy when working with singers

    Valuing the Expertise of Mobile Herders in Arid South Africa--A Photographic Essay

    Pastoralism has been practiced for millennia in numerous ecosystems across the world. Throughout history, pastoral systems and in particular herding have been replaced by the commercialization of livestock production, which essentially made herders redundant in many sub-Sahara countries resulting in job losses. Herding is however still practiced in many countries, but the essential role of the herder is in many instances snubbed upon and disregarded as trivial work, hence their low social standing within the community. Literature however indicates that herding does offer many benefits over commercial paddock systems e.g. improving rural livelihoods, reviving customary practice, reducing stock theft, reducing predation and improving biodiversity management. The contribution of herders is often underestimated, even within livestock-keeping communities, but is in fact much more complicated and they do far more than meets the eye. This paper (in the form of a photo essay) is an attempt not only to give a human face to herders through a series of photographic images, but to highlight particular activities of herders in the semi-arid Namaqualand region of South Africa. The images show the complexity of herding, which is in fact an artisanal task. It depicts herders in their daily activities as botanists with an in depth botanical knowledge including, taxonomy, phytochemistry, nutritional value, toxicity, cultural, and ritual value. It also depicts herders as midwives during lambing season, practicing nursing, and shows the interaction with their guarding dogs. Furthermore, the images illustrate the gender identity with shepherdesses functioning in an extremely harsh semi-arid landscape. It also shows the interaction between herders at the water point which acts as the gathering point for the exchange of knowledge between these artisans. These results show that the value of herders in dryland farming systems deserve more recognition and they should be key players in policy development

    Postoperative laryngeal symptoms in a general surgery setting. Clinical study.

    INTRODUCTION: Vocal cord injuries (VI), postoperative hoarseness (PH), dysphonia (DN), dysphagia (DG) and sore throat (ST) are common complications after general anesthesia; there is actually a lack of consensus to support the proper timing for post-operative laryngoscopy that is reliable to support the diagnosis of laryngeal or vocal fold lesions after surgery and there are no valid studies about the entity of laryngeal trauma in oro-tracheal intubation. Aim of our study is to evaluate the statistical relation between anatomic, anesthesiological and surgical variables in the case of PH, DG or impaired voice register. MATERIALS AND METHODS: 50 patients (30 thyroidectomies, 8 videolaparoscopic cholecistectomies, 2 right emicolectomies, 2 left emicolectomies, 1 gastrectomy, 1 hemorrhoidectomy, 1 nefrectomy, 1 diagnostic videothoracoscopy, 1 superior right lung lobectomy, 1 appendicectomy, 1 incisional hernia repair, 1 low anterior rectal resection, 1 radical hysterectomy) underwent clinical evaluation and direct laryngoscopy before surgery, within 6 hours, after 72 hours and after 30 days, to evaluate motility and breathing space, phonatory motility, true and false vocal folds and arytenoids oedema. We evaluated also mean age (56.6 ± 3.6 years), male:female ratio (1:1.5), cigarette smoke (20%), atopic comorbidity (17/50 = 34%), Mallampati class (32% 1, 38% 2, 26% 3, 2% 4), mean duration of intubation (159 minutes, range 50 - 405 minutes), Cormack-Lehane score (34% 1, 22% 2, 22% 3, 2% 4), difficult intubation in 9 cases (18%). No complication during the laryngoscopy were registered. We investigated the statistic relationship between pre and intraoperative variables and laryngeal symptoms and lesions. RESULTS: In our experience, statistically significant relations were found in prevalence of vocal folds oedema in smokers (p < 0.005), self limiting DG and DN in younger patients (p < 0.005) and in thyroidectomy (p < 0.01), DG after thyroidectomy (p < 0.01). The short preoperative use of steroids and antihistaminic to prevent allergic reactions appears not related to reduction or prevention of DN, DG, PH and ST. No statistical relation in incidence of postoperative complications was found for the prolonged intubation, gastro-esophageal reflux, BURP manoeuvre (backward upward right sided pressure) and Mallampati and Cormack-Lehane class more than 2, maintenance with sevoflurane 2% and use of stilet. CONCLUSIONS: Direct laryngoscopy is essential for the detection of arytenoid lesions after orotracheal intubation for general anesthesia. In our opinion, a part of temporary post-operative DN or PH is due to monolateral or bilateral arytenoids oedema, secondary to prolonged or difficult orotracheal intubation, valuable with laryngoscopy 72 hours after surgery. Is necessary to adjunct these complications in the surgical informed consensus scheme

    Nutritional Quality of Available Forages for Small Stock during a Drought in an Arid Pastoral Landscape in South Africa

    This study aimed to assess the nutritional quality of the available forage species during a drought in an arid pastoral system in South Africa. Forage biomass was collected during the wet and dry seasons whilst following livestock herds consisting of boer goats, swakara sheep and mixed breed sheep, in both the summer and winter rainfall regions of the pastoral system. Mineral nutrient content in the plant species revealed that the forages utilized by the livestock generally contained adequate concentrations of Mg, Ca, Na, and K to meet the dietary requirements of the small stock in both the winter and summer rainfall areas. Zinc concentrations in more than half of the forages sampled in the summer rainfall area, during both wet and dry seasons, however, were below the required concentrations for small stock. When considering all plant species utilised, the diets were generally adequate in all mineral nutrients. However, none of the forage species contained sufficiently high concentrations of protein to meet the minimum requirements for small stock. These findings therefore show that pastoralists have to deal with chronic low levels of protein during droughts, and their inability to purchase supplementary feed, or to cultivate fodder crops, or temporary emigrate out of the system puts their livelihoods at high risk to climate change

    Observation of a New Type of Low Frequency Waves at Comet 67P/Churyumov-Gerasimenko

    We report on magnetic field measurements made in the innermost coma of 67P/Churyumov-Gerasimenko in its low activity state. Quasi-coherent, large-amplitude (δB/B1\delta B/B \sim 1), compressional magnetic field oscillations at \sim 40 mHz dominate the immediate plasma environment of the nucleus. This differs from previously studied comet-interaction regions where waves at the cometary ion gyro-frequencies are the main feature. Thus classical pick-up ion driven instabilities are unable to explain the observations. We propose a cross-field current instability associated with newborn cometary ion currents as a possible source mechanism.Comment: 6 pages, 3 Figure

    Swallowing disorders after thyroidectomy: What we know and where we are. A systematic review

    Introduction Dysphagia and hoarseness are possible complications that can be observed in patients undergoing thyroidectomy or other neck surgery procedures. These complaints are usually related to superior and inferior laryngeal nerves dysfunction, but these can appear even after uncomplicated surgical procedure. Methods We reviewed the current literature available on MEDLINE database, concerning the swallowing disorders appearing after the thyroidectomy. The articles included in the review reported pathophysiology and diagnostic concerns. Results Twenty articles were selected for inclusion in the review. Depends on the possible causes of the difficulty swallowing (related to nerve damage or appearing after uncomplicated thyroidectomy), different types of diagnostic procedures could be used to study patient discomfort, as well as intraoperative nerve monitoring, fiber optic laryngoscopy, endoscopy, pH monitoring, esophageal manometry and videofluorography. Among all these procedures, videofluorography is considered the gold standard to evaluate the entire swallowing process, since that allows a real-time study of all the three phases of swallowing: oral phase, pharyngeal phase and esophageal phase. Conclusion The diagnostic procedures described can help to identify the mechanisms involved in swallowing disorders, with the aim to choose the best therapeutic option. More studies are needed for understanding the causes of the dysphagia appearing after thyroidectomy

    The Diversity of Poisonous Plant Patches in the Arid Rangelands of Namaqualand, South Africa

    The Namaqualand region in South Africa is part of the richest arid biodiversity hotspot in the world. Plant distribution and diversity here are impacted by various biophysical and anthropogenic factors. In these landscapes, poisonous plant patches, which pose serious threats to livestock, are widespread but their contribution to the regions biodiversity are not fully understood. This study assessed their plant diversity and compared its matrix. This study was conducted in the semi-arid to arid Steinkopf pastoral area located in Namaqualand where livestock is still herded daily. Twenty-five paired sites were selected based on the dominance of poisonous plants within the genera Tylecodon, Euphorbia and Adromischus. Within these sites, the number and abundance of different plant species were recorded and categorised into different plant functional types. Results showed a significant difference in Shannon Wiener plant diversity where poisonous plant patches displayed a greater diversity compared to sites sampled in the matrix. We interpret these findings as a consequence of herding in the region, where herders do not allow their animals to graze on or near poisonous plant patches. As such, palatable plants, which are absent or low in abundance in the surrounding landscape have a refuge where they can survive and set seed. This study provides evidence that the ethnobotanical knowledge of herders and palette of livestock are also major contributors to the spatial distribution and diversity of plant species in the arid biodiversity hotspot
